Rotary or reciprocating files — which should you actually be using? Is one safer than the other? Does reciprocation really reduce file separation? Are you choosing your system because it suits the canal anatomy, or because it is simply the one you were taught? Endodontic file systems can feel like a maze of brands, tapers, alloys, motions and marketing claims. But beneath all that noise, the real question is much more practical: what is your file doing inside the canal, and what compromise are you accepting? In this episode, Dr Samuel Johnson returns to unpack the Endo Showdown: rotary versus reciprocating files. We cover file motion, glide paths, shaping philosophy, NiTi metallurgy, cyclic fatigue, torsional fatigue, and why no system is perfect. https://youtu.be/HfWDBbNgjsA Watch PDP270 on YouTube Protrusive Dental Pearl A palliative root canal can be useful for an unrestorable tooth if disinfecting the canal allows infection to heal and natural bone to recover before extraction and future implant planning. ⚠️ Do not dismiss root canal treatment purely because the tooth is not a long-term functional restoration. ✅ Where appropriate, consider whether endodontic disinfection could improve the future implant site by allowing natural bone healing. Key Takeaways The purpose of shaping is not simply to scrape canal walls; it is to create space for irrigant flow. Irrigation is the most important part of root canal disinfection. Rotary files move in a continuous 360-degree rotation. Reciprocating files cut in one direction and reverse before excessive stress builds up. Modern reciprocation is designed to cut, release and gradually progress apically. File choice is not just about motion; metallurgy, taper, design and operator experience all matter. NiTi hand files with strong shape memory may be problematic in curved canals because they want to straighten. Martensitic heat-treated files are more flexible and can better follow canal curvature. Unwinding flutes are a warning sign that a file may be close to separation. Inspect files regularly during treatment, especially in curved, calcified or difficult canals. A glide path is essential before introducing larger rotary or reciprocating files. Without a glide path, a shaping file may create its own path, risking ledging, transportation or perforation. “Grabby” files pull themselves into the canal; this can be useful in experienced hands but risky if forced. Reciprocating systems can feel simpler and safer, but they are not foolproof. Cyclic fatigue happens when a file repeatedly bends around a curve until microcracks form. Torsional fatigue happens when part of the file binds while the motor continues to turn. Dentists have been attempting to treat cracked teeth for decades, with the goal of alleviating patient pain and conserving teeth through restorative treatment. But as Dr. David Alleman, DDS, describes in this episode, “it's a coin toss of success.” Methods such as retention to hold the crack together or bonding over the crack with adhesion are unpredictable. Sometimes the treatment works, and sometimes it doesn't, leaving patients and practitioners frustrated. The solution: treat cracked teeth like engineers have been treating cracks for over a century. The challenge: applying these techniques to a biological environment and accounting for bacteria, dentin hydration and dental adhesives.Dr. David Alleman, DDS, discusses the history and treatment protocols for predictable cracked tooth treatment in this episode of the Six Lessons Approach podcast.Articles referenced in this episode:Brannstrom M. In this episode of Endo Voices, Dr. Marcus D. Johnson sits down with Dr. Pierre Wohlgemuth for a thoughtful and collegial examination of two defining challenges in contemporary endodontics: the elusive cemental tear and the disciplined art of soft tissue management in surgical care.Cemental tears—seldom discussed yet likely underdiagnosed—often masquerade as endodontic or periodontal pathology. With a limited but growing body of evidence, accurate identification demands clinical vigilance. Key red flags include vital teeth presenting with periapical radiolucencies and persistent disease following technically adequate root canal therapy. The discussion explores the roles of occlusal trauma, CBCT interpretation, and histologic confirmation, reinforcing the importance of expanding the differential diagnosis before defaulting to retreatment or extraction. Decisions surrounding tooth preservation versus implant placement are framed not as reflexive, but as biologically and prognostically driven.A central theme resonates throughout: clinicians do not rise to the level of their intentions—they fall to the level of their training.Episodes of Endo Voices may include opinion, speculation and other statements not verifiable in the scientific method and do not necessarily reflect the views of AAE or the sponsor(s). How can you tell if a root canal treatment is truly successful? Do you always need cuspal coverage after a root canal? Are hand files still relevant, or has rotary completely taken over? And does GP pumping really improve the effectiveness of irrigants like hypochlorite? Emma returns for another Protrusive Student Series episode as she heads into her final year of dental school. Together, we explore the fundamentals of endodontics - covering restoration choices, success criteria, instrumentation, and irrigation protocols. This episode breaks down the basics every student and young dentist should understand, while also tackling the common debates and real-world challenges of endo. https://youtu.be/DK1ZAEPE_E4 Watch PS017 on YouTube Key Takeaways Understanding the 'why' behind dental procedures is crucial for effective practice. Both hand files and rotary files have their place in endodontics, especially for beginners. Good irrigation techniques are essential for effective endodontic treatment. Rubber dam isolation is critical for safe and effective endodontic procedures. Learning to determine the master apical file size is a key skill in endodontics. The use of EDTA helps in removing the smear layer during root canal treatment. Endodontic specialists often use advanced techniques and tools for more efficient treatments. Success in endodontics is not just about radiographs, it is sometimes defined by patient comfort and healing. Cuspal coverage is often necessary after root canal treatment. Patient communication is key to managing expectations. Consent forms should be tailored to individual cases. Understanding proprioception is important for tooth preservation. Should we be doing more to save questionable teeth? What if you could buy more time — without compromising patient care? Dr. Omar Ikram returns for a powerful episode diving into the real-world decision-making between endodontics and implants. Together with Jaz, they explore tough scenarios — like teeth with nasty cracks or minimal remaining structure — and ask the critical question: when is it truly time to extract? They break down concepts like retained roots, root burial, amputation, and a new term Jaz introduces — palliative endodontics. Instead of saying, “I plan my dentistry to age 100,” say, “I want this to last well into your eighties or nineties.” This makes the conversation more personal and realistic, helping patients better connect with the concept of long-term outcomes. Key Takeaways Understanding the limitations of implants compared to natural teeth is vital. Medical history significantly impacts dental treatment decisions. Managing patient expectations is crucial for satisfaction. Palliative endodontics can provide temporary relief and management. Reading and interpreting CBCT scans requires skill and experience. If it's not that five millimeter defect, it's up to you. The second molar is a good one because often second molars can't be replaced with an implant. Retaining roots is definitely a good way to go. What's interesting about root resorption is that it utilizes our own soft tissue cells to work against us, initiates its attack against the tooth from the inside out and from the outside in. In this podcast, presented by Endodontist Dr. Phil Klein, we'll learn about this insidious process and hopefully we'll get a better understanding of the fundamentals behind the different types of root resorption. By doing so, we as clinicians, can sharpen our diagnostic skills and optimally manage these conditions on a case by case basis. What if there were a simple way to deliver an electronic charge into difficult-to-reach spaces, cauterizing soft tissue and making it easier to flush out? By doing so, we could effectively eliminate microbes hiding in constricted lateral and accessory canals. The good news is that with the introduction of a new technology we can do just that. This innovative device seamlessly integrates with the Root ZX3 apex locator, requiring no additional time or space—yet offering a major breakthrough in root canal therapy. Joining us today to discuss this technology is Dr. Chafic Safi. Dr. Safi completed his postgraduate residency in Endodontics at the University of Pennsylvania in 2015, where he also earned a Master of Science in Oral Biology. He lives and practices in Montreal, Canada.

In this insightful episode of Endo Voices, Dr. Allen Ali Nasseh explores the evolution and clinical applications of bioceramics in endodontics, highlighting how material innovation has simplified obturation and improved patient outcomes. He shares the origin story of bioceramics, including his role in developing the EndoSequence product line and shifting the focus from technique to chemistry through hydraulic condensation. Dr. Nasseh emphasizes proper use to avoid extrusion, explaining that while bioceramics are biocompatible, sealer should remain within the canal. He critiques the extremes of minimally invasive endodontics, advocating instead for an approach that prioritizes effective disinfection and accessible, patient-centered care. Looking to the future, he discusses how AI will enhance consistency and streamline workflows without replacing the clinician, revealing his own efforts in developing an AI-powered consultation tool. Reflecting on his retirement, Dr. Nasseh encourages the next generation to continue innovating while staying grounded in the purpose of delivering better care. In this episode of the Very Dental Student Podcast, Mo talks with Kevin and Zach to discuss endodontic procedures, comparing dental school protocols with real-world practices. They delve into a step-by-step walkthrough of an endo procedure on a premolar, highlighting key differences and offering valuable insights for dental students and new dentists. Key Takeaways: CBCT is Your Friend: While PAs and bitewings are helpful, a CBCT scan provides a 3D view of the canal system, acting as a "GPS" for navigation. This is especially crucial for molars and complex premolars. Begin with the End in Mind: Always consider the final restoration (likely a crown). Take a pre-op impression before starting the endo to simplify temporary crown fabrication. Quadrant Isolation: Isolate more than just the tooth being treated with your rubber dam. This provides better orientation and access for both endo and crown prep. Flat Top that Tooth: Reduce the occlusion and flatten the tooth before accessing the canals. This improves visibility, access, and post-op comfort. Lubricate Early and Often: Use RC Prep on your files before entering the canals to prevent pushing debris apically and to aid in lubrication and chelation. Hypochlorite Timing: Introduce sodium hypochlorite after establishing working length to avoid pushing debris and potentially blocking the canal. Secondary Rinses: Use EDTA or a similar product (like Smear Off) to remove the smear layer created by sodium hypochlorite before obturation. Sealer Coating Technique: Coat the canal walls with sealer by running your files in reverse. This ensures a more thorough and even distribution. Post-Op Comfort: Even if a crown is planned, taking the tooth out of occlusion can improve patient comfort and reduce post-op complications. Posts - Controversial but Sometimes Necessary: While posts are less common today, they can be valuable in certain situations, particularly for premolars and anterior teeth. Crown Prep Under Rubber Dam: While unconventional, prepping crowns under a rubber dam can be done effectively with practice and proper technique. Our final podcast for 2024! All of us who are doing endo have one main goal in mind, and that's to save the tooth. But in many cases, it's impossible to reach lateral, accessory and small constricted canals with our disinfectant solution, such as sodium hypochlorite. This means we often leave active microbes in these areas that can eventually lead to an endodontic failure. So the question is, is there something we can do in our procedure that can solve this problem? The answer is yes! It has to do with a new technology that is relatively inexpensive and very easy to use. This technology is based on high frequency conduction and it is extremely effective in helping to disinfect tiny accessory canals that would otherwise go untreated. To tell us all about is our guest Dr. Bruno Azevedo. Dr. Azevedo is a board certified oral and maxillofacial radiologist, Endodontist and arguably the best teacher and educator on the topic of CBCT in dentistry.
The latest Endo Voices podcast drops some serious financial wisdom as Drs. Marcus Johnson, Andrew Chong, and Janatha Withanachchi aka "Dr. Jonny Walker" tackle the money matters that most dentists avoid but desperately need to master. Dr. Walker reveals how shifting from active real estate to passive investments freed him to focus on family and life, while Dr. Chung shares how trial-and-error investing inspired him to build an equities fund for dentists with modest capital. Their mantra? Align your financial moves with your career stage and long-term goals.Borrowing from Robert Kiyosaki's The Cashflow Quadrant, they encourage endodontists to transition to the “right side” of wealth—business owner and investor. Walker's “Walk of Wealth” platform empowers dentists to navigate passive investing smartly, offering resources like mastermind groups and real estate funds while warning of pitfalls like Ponzi schemes targeting the unwary. Meanwhile, Chung uncovers a stark reality: most endodontists feel financially unprepared but crave the knowledge to change that.Episodes of Endo Voices may include opinion, speculation and other statements not verifiable in the scientific method and do not necessarily reflect the views of AAE or the sponsor(s).
