I have found that women often wonder what to expect during a pelvic floor assessment and have no idea what it all entails. Starting pelvic floor physical therapy can be scary and intimidating for anyone! But wondering what to expect during a pelvic floor assessment can make it seem much scarier! After working in pelvic floor physical therapy for many years, I've learned that there are other factors involved in order to get people better long term. This is where functional health for pelvic floor enters in. Functional health encompasses other aspects of our health that have an impact on pelvic floor conditions and pelvic health. These include stress, nutrition, gut health, vaginal health, and sleep. After incorporating these aspects of functional health for pelvic floor issues and seeing the results, I knew that this was important to include in order to have a more cohesive approach to treatment. The approach to training for female athletes is often the same as male athletes, with a lower capacity or resistance amount. However, training for female athletes is much more complex and there are several factors to consider. Rarely is the female anatomy differences, hormone differences, and lifestyle differences taken into account when developing training plans. In this episode, Dr. Brenda talks about the differences between male and female athletes. She discusses how anatomy, hormone fluctuations, and life stages can impact a female athlete's performance. The use of acupuncture for fertility and other women's health issues may not be as well known in mainstream healthcare, but has been shown to be very effective. Acupuncture, a form of Chinese or eastern medicine, can be used to address women's well-being during pregnancy, postpartum and beyond. And it can be used to treat a variety of issues such as hormone imbalance, dysmenorrhea, and pelvic pain, to name a few. Our guest on today's episode is Lisa Mussak, one of Milwaukee's only board-certified acupuncturists in fertility. Lisa specializes in reproductive health, menopause, pregnancy and postpartum, birth and menstrual support, and other women's health concerns. Lisa shares some insight into how she uses acupuncture for fertility, both as a stand-alone treatment as well as a complement to other western treatments. Like other aspects of women's health, fertility treatment can be an opportunity for a collaborative team of professionals to come together to address the issue. Over 50% of women have pain with sex. Yet very few women know about the real causes of their pain. And even fewer know how to get relief. In fact, most women are given bad advice like “relax more” or “drink more wine.” Or they're told that it is “all in their head,” which is neither true nor helpful. Women can experience pain with sex at any point in their life, but it is most common after having a baby and during premenopause. On this episode of The Revitalize Woman Podcast, Dr. Brenda goes into detail on why women have pain with sex, the different types of pain with sex and what you can do to get rid of pain. Plus, she shares tips you can start to implement right away to get relief from your pain. Show Notes [3:20] Pain with sex is common, but not normal. Many women suffer for years, thinking it's just something they need to deal with. Dr. Brenda talks about the excuses she hears and the misinformation women are been given by friends, healthcare providers, partners, etc. [5:04] Dr. Brenda discusses the two most common types of pain with sex. She also talks about scar tissue and overactive pelvic floor muscles, the two most prevalent causes of pain with sex. [10:34] It is important to discuss hormones and the role they play when it comes to pain with sex for women, specifically during the postpartum and premenopause phases. [17:00] So what can we do about painful intercourse? Dr. Brenda explains that manual breaking up of scar tissues and myofascial release of muscles are the two places to start. It is common for women to experience leaking with running, especially after having a baby. However, leaking is not normal and can be very limiting and frustrating for the women who experience it. On this episode, I had the opportunity to sit down and talk with Krista Ruehmer who is a mother, runner and one of my fabulous clients. We talk about how she dealt with leaking with running after having a baby. Eventually she decided enough was enough after completely soaking her running shorts. This inspiring interview is so relatable for many women. We talk about the inner battle of knowing something is “wrong” and trying to push through and deal with it. For Krista, she wishes she had listened to her intuition and found help sooner. It has been so much fun to work with Krista to help her overcome leaking with running and I am so appreciative of her willingness to share her story with you. Pelvic organ prolapse is an issue that is often not discussed. Many women find it difficult to get adequate prolapse support. Contrary to popular belief, prolapse can occur for any woman regardless of childbirth history or age. Lack of education and resources can make it difficult to find prolapse support for women who are diagnosed with this pelvic floor dysfunction. We are SUPER excited to share this podcast episode because we are talking all about our pregnancy research study, which has been a labor or love (no pun intended) for a long time! We are now enrolling participants into our pregnancy research study which will be looking at the effects of pelvic floor physical therapy during pregnancy and the outcomes on delivery, including the length of pushing, perineal tearing, and C-section. It has been a long process to design this prenatal research study and get the official approval in order to begin. Now that we can accept participants, we wanted to share more about the pregnancy research study for those who are interested and answer some of the common questions we have been asked. When you think of sex education, your mind probably goes back to a middle school health class or awkward teen sex education talk. But the truth is, ongoing sex education is important for women at all stages of life. On today's episode, I chatted with Ellen Barnard, MSSW, who is a sex educator, sex counselor and owner of A Woman's Touch which is a sexuality resource center in nearby Madison, WI. Ellen is “interested in increasing the total quantity of pleasure in the world” and is so knowledgeable about all things related to sex education and issues with intercourse, desire, and sexuality. She is passionate about teaching people that sexual health and sexual pleasure can coexist. There is literally something for everyone in this episode. We talk in-depth about which types of lubricants are best, how to address common issues during intercourse, how to rejuvenate vaginal tissues, what to do about low libido, and more. Prenatal fitness can be confusing and scary, especially for those who aren't sure what is safe to do, what to expect, or where to start. Then, after having a baby, they face the same challenges with a vast array of misconceptions on the internet about postpartum exercise. Coupled with the societal pressure to "bounce back” quickly, prenatal fitness and postpartum exercise can be downright daunting! On today's episode of The Revitalize Woman Podcast, I had the chance to sit down with Molly Powell, certified personal trainer and owner of Birthfit Milwaukee. Molly specializes in prenatal fitness and postpartum exercise. She works extensively with this population of women and also teaches Birthfit classes focused on prenatal fitness and postpartum recovery. We had so much fun talking about safe exercise for these women and the misconceptions about exercise after baby, including the pressure to “get your body back." This is often a stress reliever for most people and it is hard to find other alternatives during this stage [37:13] We talk about the newest research on returning to running with the new recommendation of no running prior to 12 weeks postpartum. The guideline also states that women should do a progression of exercises leading up to running as well as no pelvic floor dysfunction [39:00] With the increased pressure on women to lose weight quickly after having a baby, it can be challenging to be patient. We discuss that the initial goal postpartum is to restore function but this is difficult for women. Lactation resources are crucial for any new mom and baby and this episode gives you loads of information! Molly Peterson is an International Board-Certified Lactation Consultant and the owner of Peterson Lactation Services here in Milwaukee and the Professional Development Coordinator for the United States Lactation Consultant Association. It was so much fun to talk about ways a lactation consultant can help support moms and babies by providing lactation resources. We also talk about positioning, ways to help your posture, pain with breastfeeding and how it impacts your hormones. In addition, we talk about the new milk bank that provides donor milk to anyone that needs it. Show Notes: [2:50] Molly Peterson, BS, IBCLC, RLC, shares her background and how she ended up pursuing a career as a lactation consultant after studying speech pathology in college. She now works out of Authentic Birth Center in Wauwatosa, WI. [5:18] We discuss the model of collaborative care in women's health found at Authentic Birth Center - Wellness Collective, where many services for moms and babies can be found under one roof. Dr. Brenda is adding functional medicine component to the Revitalize clinic to fill in a missing piece of this model of care.[6:35] Molly explains what a lactation consultant does, what it takes to become an IBCLC, and how she works with mothers and babies to meet their individual feeding goals, whether they want to breastfeed or bottle feed.[8:30] We talk about some of the misconceptions people have about lactation consultants, and Molly shares her personal philosophy that she isn't aiming to tell parents what to do but rather to educate them based on that family's unique needs.[10:08] Dr. Brenda shares her personal experiences with both unsupportive and supportive lactation consultants in the hospital after she chose to exclusively pump.[11:50] Dr. Brenda asks Molly what her best advice is for moms who are exclusively pumping and she shares some helpful tips, including one about how the number of hours between pumps is not as important as the number of pumps per day.[13:40] Dr. Brenda often hears moms complain about pain while breastfeeding. Molly explains whether or not breastfeeding should ever be painful and says pain is the number one reason moms come to see her.[16:24] The next most common issues Molly hears are low milk supply (or perceived low milk supply), going back to work and transitioning to pumping, and recommendation from a pediatrician related to baby's weight or age.[18:20] We talk about the worries of whether or not baby is taking in enough milk, and Molly explains the best ways parents can gauge how much milk their child is getting.[20:18] We talk about mom's posture when breastfeeding, noting that Dr. Brenda frequently sees moms coming with complaints of neck or back pain related to how they are positioned while nursing. Molly suggests certain positions to help alleviate posture issues.[23:43] We discuss hands-free, closed system, discreet pumps and whether or not they are as effective as popular pumps like Spectra and Medela. Molly mentions the Haaka breastpump as a good option to collect excess milk. Women's healthcare includes a wide variety of topics but it can be overwhelming to deal with some of these issues. Often times, women have so many issues about what is normal, what is important to talk to their doctor about and what are some of their options. On today's episode, I chat with Dr. Amy Rider, board-certified obstetrician gynecologist, about all things related to women's healthcare. We cover all the issues including pregnancy, postpartum, and menopause. Listen to some of the reasoning behind why things are done a certain way in the medical model and what is “normal” and what is not. We talk about common postpartum issues such as diastasis recti and prolapse and why these may not always be checked by your OBGYN. Then we discuss some of the hot topics regarding hormone replacement therapy and what the research really says about the risk for cancer and how to use it. It's also really important to understand the symptoms of menopause and how the changing levels of estrogen impact your body. This episode is jam-packed with some really great information so you don't want to miss it! Show Notes: [2:50] Dr. Rider sees a wide variety of women's health issues within her practice but the biggest reason women come in to see her are for irregular cycles and painful periods. Other reasons that she sees women are for birth control and family planning, pregnancy, painful intercourse, pelvic pain and post-menopausal complaints.[4:03] Hear what Dr. Rider assesses for at the 6-week postpartum visit and why she may not be as focused on musculoskeletal issues such as diastasis recti and prolapse at that time due to the medical follow up that needs to be done at that time.[6:08] Women often times ask us “why didn't my OBGYN check my diastasis recti” so we asked about this and also talk about the differences in training in regards to this area between physical therapists and physicians. [7:22] When women get assessed for issues such as diastasis recti and prolapse, there are sometimes discrepancies between providers in women's healthcare so I talk about the importance of realizing there are going to be slight differences between how something is assessed.[10:40] Is it harder to recover from a C-section or a vaginal delivery? Hear Dr. Rider's explanation of recovery for both and what goes into recovery.[12:50] Whenever there is an injury to tissue, the strength of that tissue does not return to 100%. We discuss this in regards to VBAC deliveries and what the concerns are with it.[15:34] Is it true that practitioners are prescribing estrogen cream as a way to break up scar tissue? Dr. Rider discusses how lower levels of estrogen can cause vaginal dryness and pain and when it's helpful to prescribe estrogen cream.[17:39] Learn about common premenopause and menopause symptoms like insomnia, hot flashes, and weight gain. [18:50] Dr. Rider explains how decreased estrogen levels affect the body and cases where hormone replacement therapy is beneficial and the symptoms it helps the most.[26:56] We talk again about estrogen cream and hormone replacement therapy, specifically with regard to potential risk for cancer. Navigating the waters after baby for fitness for moms can be so frustrating and tricky! Not only is there so much information out there on what you should and shouldn't do, but there is alot of pressure on moms to regain their fitness and meet societal expectations. When she opened Authentic Birth Center and Wellness Collective, she had a vision to empower women to experience physiological birth in a quiet, peaceful setting and wanted to create a place that women are treated with warmth, patience and respect. Exercising with Pelvic Floor Dysfunction - the Good, the Bad, and the Starting Point Exercising with pelvic floor dysfunction can make it so confusing to know which exercises are good, which ones you should avoid and where to even start. The most common pelvic floor dysfunctions that leave women feeling confused and frustrated with exercise are incontinence, prolapse and diastasis recti. With so many different types of exercise available, it can be hard to know what to believe nowadays. How do your changing hormones impact pelvic floor health and function? Women tell me all the time that they didn't realize so many of their symptoms and issues that they had dealt with for years, stemmed from changing hormones. When estrogen levels begin to decrease in the perimenopause stage, as early as age 35-40 years old, it has a big effect on all of the systems within your body. Many women don't realize that their changing hormones play such a big role and most have no idea that it can also affect your pelvic floor! I hope that this episode today will educate women on this topic so they know what will happen in the future or have a better understanding if they are dealing with this currently. On this episode, I talk about the effect of decreasing estrogen levels and how some common pelvic floor dysfunctions such as incontinence, prolpase and painful intercourse, can occur or be exacerbated because of changing hormones. When to do kegels and when NOT to Have you ever been confused about when to do kegels and when to avoid them? On this episode, I will explain everything there is to know about kegels. Women ask me all the time at the office if they really need to be doing kegels regularly and how do they know if they are doing them correctly. And I feel that the “advice” that is given out regularly to women for fixing their issues is to “do more kegels.” However, for some women, kegels could actually be making their issues worse. Or, if they aren't doing them properly, they could be creating more problems for themselves. In this episode, I help clarify when to do kegels, how to do them properly, why they are important, and when you should NOT do kegels. Show Notes: [1:15] What is a kegel?
