I've learned so much from other people's questions! I have two of my own. 1. Regarding your past podcast on the importance of broccoli and garlic. I travel a lot for work. When I travel and can't get broccoli or garlic, would it be ok to take a broccoli and garlic supplement. ? 2. I recently purchased the equililife mushroom supplement. Do you recommend taking it all year round or just in the winter. Thanks so much! Will   Carol: Dr Cabral, I would like to start by thanking you for all the information you freely give. I've been listening to your podcast almost since it started and I have been able to make positive changes in my life and my family's. I am a healthy, active 62 year old woman. I walk 5 to 10 miles most days. I do strength training 3 to 5 times a week plus cardio a couple days a week. Through diet and exercise I have been able to maintain my weight most of my adult life. If my weight started to go up I would make adjustments in my diet (which was usually from getting sloppy with my diet). Since 2017 I've done your detox at least 3 times a year (I just haven't been able to swing 4), and they usually help me lose the few pounds I gained and put me back on track with my eating. For the last few years, however, my weight has been creeping up higher and nothing (not even the detoxes) have helped me lose the added weight. I've tried everything I could think of, but my weight continues to go up instead of down. I went through menopause 10 years ago. I'm at a loss at what to do, but I definitely don't want to continue to gain weight. Do you have any thoughts or suggestions? Grateful for all that you do. My question is regarding a sort of pinch like feeling on my left side (near heart). On and off randomly.. did blood work and EKG (normal findings). Ran minerals and Metals.. on the higher side for K & N.. Mg green and Calcium (leaning towards high). Other minerals were low, except phosphorus (slightly high) Noticing bloating after meals.. could leaky gut be related to the occasional pinch feeling I get… it's either that or cortisol.. Which lab should I run (can only do 1).. Taking Omega3 support, DNS, Magnesium and exercising twice a week.. I'm a 27 year old man. Noticing new onset fatigue, bloating.. A bit worried about my heart.. thanks.. What can help, proteolytic enzymes, Apple cider Vinegar before meals. Or B vitamins. Thoughts?    Cheryl: Morning, My 73 year old dad has type 2 diabetes. He is otherwise in good health, an active golfer, is about 165lbs and walks daily. My parents are old school and believe everything the dr says. Recently, his dr just uped his metformin to 2x a day from 1x and put him on a pill for his A1C. I am annoyed that the meds are just increased instead of looking at the root cause. They recently saw a dietician who said it is not reversable which I know is not at all true. My mom cooks healthy meals but my dad does have a sweet tooth. When he wants something sweet it is often sugar/free which is terrible and full of chemicals. I do not agree with all of the sugar free stuff/sweetners and try go get them to choose different things-monkfruit/coconut sugar but the dietician recommended the splenda type stuff. would love to help my dad reverse this. My 80 year old mother has been experiencing consistent burning mouth syndrome for the past 12 years. She had tried all the conventional methods, gabapentin, CT scan etc and nothing has helped. Nicola and Jamie describe what a living review is and discuss the steps involved in carrying out a living systematic review, from screening and extraction to data analysis and dissemination. They outline how the methods differ from traditional systematic reviews. They highlight the value of the living review process for the fast-moving topic of vape research. This approach means that the author team are constantly up-to-date with the literature and able to input into policy and to respond to press or research queries. Thank you for joining us for our 2nd Cabral HouseCall of the weekend!   I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks…   Tricia: Hi Dr Cabral - I'm writing in for my brother. He is a 73-year-old male who has been healthy his entire life. No major illnesses. I noticed a few years ago his head would slightly bob along with his hands. So slight that he doesn't notice. I think he could have a vitamin deficiency. Unfortunately, I can't get him to do any of your labs or detox, but I do have him on your DNS, veggie blend, renewal system, fish oil, eye, magnesium, and Vit d. Do you have any ideas what this might be? If I could get him to test, which test would you recommend? My grandmother did shake    Alicja: Hello Dr.Cabral, I hope you're having a great day. My question is about heart rate. I wear smart ring that detects high heart rate during periods of time when I seat a lot and the only activity would be: getting up occasionally from my desk to walk to kitchen or bathroom. I do 10k steps per day, every day and I exercise every other day for 30-50 min alternating between: hot yoga, pilates, jogging and resistance training. Most of my walking and exercising is done in the first half of the day. During walking or exercising my heart rate is pretty steady, not very high, usually below 120 (except for running) but often the highest heart rate of my day I get in the second half of the day when I don't do any physical activity. It doesn't make sense to me. Why would my heart rate jump to 135 when I don't exercise or do any big activity? I'm 42 and I have low blood pressure. (if that matters)   Tricia: Hi Dr. Cabral - Do you mind telling me if I'm taking all of your supplements below correctly? Honestly, I would love to work with an IHP but affording all the supplements is all I can do right now. I've been doing this since June of 2025. Upon waking - 2 proteolytic enzymes, 30 minutes later 3 of the sinus and immune support (only during cold season) and daily probiotic. Roughly 2 Hours later right before my morning DNS smoothie (am Metavolve, 2 eye vitamins, 2 cell boost, collagen & D3K2 drops in smoothie). With Lunch 2 hair vitamins and 3 inflamma soothe. Dinner (2 pm Metavolve, 2 fish oil) An hour before bed 2 Full Spectrum Magnesium. I started this protocol to help with lowering inflammation, joint stiffness and pain and it has been a game changer. I exercise daily and eat your Med diet recommendation and literally you gave me my life back so I thank you so very much. My health journey is still a work in progress, but I will get there! I just started Metavolve two months ago. I really like it. I haven't been able to give up all my berries and sweet potatoes but I'm going to try again Jan 1! P.S. I love your new book!   Scott: Hello Stephen, happy holidays!! I'm writing in about my mother. She's 69 years old with coronary artery disease. 2 years ago she had 2 separate procedures to remove considerable blockage. Then, this past September, she had it done again. All together 4 stints, 2 in her widow maker. She hasn't drank alcohol in 10 yrs and quit smoking for over 2 years, although her previous lifestyle was lacking in many areas which I'm sure built up to her CAD. There's heart issues within the extended family, but, I know its not just genetics. She has done EXCELLENT zeroing in on her diet after this last round of surgery and she has felt the inflammation in her body pretty much disappear. She still, once in awhile, gets winded, which I can understand her being weary to go too long on the upright bike. She has researched online and food seems really restricted like salt intake, sugar and even really healthy things like Xtra virgin olive oil, which says only 2 tablespoons a day....that seems odd. She's on a certain type blood thinner but her heart doctor gave the okay for omega 3's and was really happy when she read the ingredients on the DNS bottle. I have her on EquiLife supplements since the first surgery; Omega 3's, Vitamin C., Vitamin D3, DNS, B-Complex, Cal-Mag, Balanced Zinc, the new Pea Protein, and collagen from another company (but it's clean) As an IHP level 1 and starting level 2, I'm trying to help her the very best I can. I suggested lime w/ a pinch of sea salt for an electrolyte morning drink but she's too cautious of the salt intake. I'd love for her to try a 21 day detox, she doesn't think she can do all the fasting days except maybe the first 2 days. Even the 14 day detox would be good too, but, would these be too hard on her with her condition, especially when CAD patients are restricted from eating even the healthiest of foods like olive oil? The literature says CAD patients do not have a long time once they have that condition, but, I'm trying to my best to extend her years, she is our family's rock. “In view of all this, make every effort to respond to God’s promises. Supplement your faith with a generous provision of moral excellence, and moral excellence with knowledge, and knowledge with self-control, and self-control with patient endurance, and patient endurance with godliness, and godliness with brotherly affection, and brotherly affection with love for everyone. The more you grow like this, the more productive and useful you will be in your knowledge of our Lord Jesus Christ. But those who fail to develop in this way are shortsighted or blind, forgetting that they have been cleansed from their old sins.” (2 Peter 1:5–9 NLT) Many people make resolutions this time of the year. Some are going to swear off alcohol or smoking or social media or unhealthy eating. Others are going to try to change destructive habits like gossip or worry or losing their temper. Many people will try to start new habits like exercising. Health clubs love the beginning of the year because people love to commit to going to the gym and working out three to five times a week. The problem is, most people never follow through on their resolutions because they weren’t serious in the first place. There are other, more important, resolutions for you to focus on as we begin 2026. There are godly habits to develop and ungodly habits to break. Horace Mann once said, “Habit is a cable; we weave a thread each day, and at last we cannot break it.” That can be a good thing or a bad thing, depending on the habit. This New Year, make a concerted effort to develop godly habits—habits that cannot be broken and that will build others up in their love for the Lord. The words of the apostle Peter in 2 Peter 1:5–9 offer a good starting point. “In view of all this, make every effort to respond to God’s promises. Peter emphasizes that difference-making spiritual growth is a process. It doesn’t involve reaching certain plateaus according to a specific timetable. The important thing is that we prioritize spiritual growth in our life, that we make consistent progress as we grow in our relationship with Christ. That’s the kind of habit we should pursue. How Much Do I Need to Retire? Why That Question Is Misleading The classic commercial asked, “What's your number?” People walked around carrying a big orange figure that supposedly represented what they needed to retire. Here's the problem: That number assumes: A set rate of return A set withdrawal rate No major disruptions And that you won't touch your principal But real life is not a straight-line projection. When you ask how much do I need to retire, you're usually really asking: “How can I have enough cash flow for as long as I'm alive, without living in fear?” The issue is not just how much you have—it's how that wealth behaves under stress and how it converts into dependable income. Retirement Cash Flow vs Nest Egg: What You Really Need Traditional planning focuses on accumulation: “If I can just get to $X million, I'll be fine.” But what you actually live on is cash flow, not the size of your account statement. You need to know: How much income do I need in retirement each month? Which part of that income is guaranteed and which part is variable How that income will behave if markets drop or inflation spikes If you have $2 million but no idea how to turn that into reliable, sustainable cash flow, you will feel fragile. If you have a mix of guaranteed income in retirement plus flexible cash flowing assets, even a smaller nest egg can feel much more secure. The question isn't just how much money do I need to retire, but how do I design cash flow that will last? Sequence of Return Risk in Retirement: Why Timing Matters More Than Averages The industry loves to tell you that “the market averages 10% over time.” That's nice trivia—but it's not how your life works. If you're accumulating, you can ride out the ups and downs.If you're retired and pulling money out, the sequence of returns can make or break you. Here's a simple illustration: Start with $100,000 Year 1: -20% → now you have $80,000 Year 2: +20% → now you have $96,000 The average return is 0% (-20 + 20 / 2).But your actual money is down $4,000. Now imagine that on top of the losses, you're pulling out 4–6% per year to live. Suddenly, the portfolio has to recover the market loss and everything you withdrew. That's sequence of return risk explained with examples—and why relying solely on averages is dangerous. Building a Retirement Buffer Account to Protect Your Portfolio One of the most powerful ways to address sequence of return risk in retirement is using a retirement buffer account. The idea is simple: When markets are down, you do not take distributions from your volatile assets. Instead, you live off a separate, safe buffer of liquid capital. This buffer could be: Cash in the bank CDs or other stable vehicles Cash value in a well-designed whole life insurance policy How a buffer account protects your retirement portfolio: It gives your market-based assets time to recover It reduces the risk of selling low during downturns It lowers emotional stress when headlines scream “market crash” You're no longer forced to sell when everything is on sale. The LIFE Acronym for Retirement Planning: Liquid, Income, Flexible, Estate To make this practical, we often walk clients through the LIFE acronym for retirement planning: L – LiquidHow much “15-minute money” do you need to feel comfortable? This is money you can access quickly for emergencies or peace of mind—not dependent on your cash flow plan. I – IncomeHow much income do you need each month? How much of that would you like guaranteed? This is where retirement income planning really happens. F – FlexibleThis is liquid money that's not earmarked for emergencies or core living expenses. It's for things like trips, special projects, and helping kids or grandkids. It's the “I can do this without stress” bucket. E – EstateHow much do you want to leave behind, and in what form? This is where how to make your retirement plan serve future generations becomes part of the design. A well-designed mix of cash, whole life insurance, and other assets can touch every part of LIFE: Liquid, Income, Flexible, and Estate. Problems With Traditional Retirement Planning and the 4 Percent Rule Traditional planning often rests on: A withdrawal rule (4% or 5%) Market-based portfolios Historical averages and Monte Carlo simulations But as Bruce mentioned: A 100-year average doesn't matter if you're retired for 20 years Inflation erodes real purchasing power Market volatility plus withdrawals increase fragility Focusing only on accumulation creates emotional anxiety This is why cash flow vs accumulation in retirement planning is such an important shift. When you're not dependent on markets going up every year just so you can eat, your whole experience of retirement changes. Redefining Retirement: Gradual Retirement vs Traditional “Out of Service” Nelson Nash used to remind us: Retirement, by definition, means “taken out of service.” Most of us don't want to be taken out of service; we want to stay useful, engaged, and purposeful. Instead of a hard stop at 65, consider redefining retirement as a gradual retirement vs traditional retirement: Negotiating part-time work or consulting Reducing hours instead of walking away completely Staying in the game mentally, physically, and relationally We've seen engineers move to 10 hours a week, seasoned professionals mentor younger staff, and business owners step back from daily operations while still contributing. Purposeful work, even part-time, can: Supplement your retirement income Reduce pressure on your portfolio Keep you sharp and connected Retirement doesn't have to mean being benched. Cash-Flowing Assets and Alternative Investments for Retirement Cash Flow Another powerful way to support retirement is shifting some focus from growth-only assets to cash flowing assets for retirement. Examples include: Dividend-paying stocks Real estate (direct ownership or funds) Private lending Certain alternative investments for retirement For accredited investors, there are a variety of alternative investments for retirement cash flow: Multifamily apartment funds Industrial and distribution center funds Certain energy or infrastructure programs Technology and telecom infrastructure (like tower or data assets) These are not guaranteed and require careful due diligence, but they're often backed by real underlying assets and designed with yield in mind.
