Summary In today's episode, we delve into the emerging role of ketamine in managing cancer and chronic pain. Our discussion is anchored around a comprehensive review article titled "Ketamine Use for Cancer and Chronic Pain Management," published in Frontiers in Pharmacology on February 1, 2021. This review, authored by Clayton Culp, Hee Kee Kim, and Salahadin Abdi, explores ketamine's potential as an analgesic in chronic pain conditions, particularly cancer-related neuropathic pain. Key Points from the Review Article: - Mechanism of Action: Ketamine functions as an N-methyl-D-aspartate receptor antagonist, providing analgesic effects at sub-anesthetic doses. Its ability to counteract central nervous system sensitization makes it effective in opioid-induced hyperalgesia. - Clinical Efficacy: Recent studies highlight ketamine's potential to reduce pain scores and opioid consumption, offering a promising alternative for patients with refractory pain. - Safety Profile: At lower doses used for analgesia, ketamine's safety and adverse event profile are significantly improved compared to its use as an anesthetic. - Pharmacogenomics and Interactions: The article discusses how genetic variations can affect ketamine metabolism and highlights potential drug interactions that clinicians should be aware of. Reference Culp, Clayton, Hee Kee Kim, and Salahadin Abdi. "Ketamine use for cancer and chronic pain management." Frontiers in Pharmacology 11 (2021): 599721.

Ohad Arazi, CEO of Clarius, a medical imaging startup with a mission to bring accurate, easy-to-use, AI-assisted, and affordable hand-held ultrasound tools to a broad range of medical professionals. Traditional ultrasound is costly, stationary, and requires specialized training. The portable Clarius device communicates with an iPad or an Android or iOS phone. The ultrasound produces highly nuanced grayscale real-time images that, with the help of artificial intelligence, can be used safely in a wider point-of-care environment to make informed decisions. Ohad explains, "We're all about bringing together high-performance ultrasound imaging, cloud data, and artificial intelligence into this powerful ecosystem that improves patient care enabled by medical imaging in every setting. The key differentiation is that handheld ultrasound is not about replacing legacy systems in places where they're already well entrenched, like radiology or cardiology. It is rather about bringing medical imaging to new care settings and putting it in the hands of new practitioners that may not have had access to an ultrasound before." "We can even extend that outside of the walls of the hospital into the ambulance. Powered by AI, we have a feature called Auto Preset AI, which, as the EMT roam through the patient's body, looking for trauma, our AI will automatically adapt the view of that image to optimize it for the organ the EMT is looking at. This lowers that threshold for knowledge to enable them to make a more informed decision. For example, to see that there might be stenosis in the carotid and they should radio the hospital and get the thrombectomy lab ready because there's a stroke patient on hand." #Clarius #MedTech #Ultrasound #MedicalImaging #AI #DigitalHealth #HandheldUltrasound clarius.com clarius.com/2023-state-of-ultrasound-report/ Listen to the podcast here

In this episode you will discover: What the difference is in working for a corporation versus a startup when it comes to innovationHow Ohad transitioned into the founder journey What the benefits are to using AI in diagnosticsWhat it means to be problem-oriented rather than solution-oriented Key Takeaways: People often have an inaccurate view of what it's like to lead a small company if they're coming from a big company. In a big company, the CEO is the coach, they're on the sidelines guiding an army of people while in a small company, the CEO is a player-coach, someone who leads while also in the field. Successfully transitioning to becoming a founder and hiring a CEO takes a lot of humility, trust, and transparency. It's incredibly difficult to admit when the company needs a better fit for the leadership role, but sometimes it is necessary to take that step if the founder wants the company to grow further. AI can be used as a tool to help doctors become more efficient in their diagnostics and practice by providing assistance with things that are measurable, quantifiable, routine, and mundane. However, it can't be used as a replacement for medically-trained and experienced professionals. Be problem-oriented. Innovators tend to think of a solution first and then find somebody who could use that. You always have to be in tune with what problem you are solving. “Medicine is a mix of art and science. An AI can really help with science - things that are measurable and quantifiable. But you're not going to lose the art, the human in the loop that's helping to make a better-informed decision or to complex tasks that require not just routine pattern recognition, but a more experienced set of eyes.” - Ohad AraziResources: Website: https://clarius.com/ LinkedIn: https://www.linkedin.com/in/ohad-arazi-44492293/ | https://www.linkedin.com/company/clarius-mobile-heath-corp- Facebook: https://www.facebook.com/clariusmhealth/ Instagram: https://www.instagram.com/clariusmhealth/ Twitter: https://twitter.com/clariusmhealth Learn more from Carrie and Rebecca: Healthcare Insights (a monthly deep dive into specific topics for innovators): https://nixongwiltlaw.com/healthcare-insightsTelehealth/Virtual Care Mgmt Update (a biweekly LinkedIn newsletter tracking legal and policy changes across the nation): https://www.linkedin.com/newsletters/telehealth-virtual-care-update-6901900767960068096/ Website: https://nixongwiltlaw.com/ Carrie on LinkedIn: https://www.linkedin.com/in/carrienixon/ Rebecca on LinkedIn: https://www.linkedin.com/in/rebeccagwilt/NGL on LinkedIn: https://www.linkedin.com/company/nixon-gwilt-law/

Starting a career in the Israeli army, becoming a lawyer, and finally transforming the field of medical imaging. In a very schematic way, this is what might define Ohad Arazi's path, who is now spearheading the field of handheld ultrasound as the CEO of Clarius. His curiosity for medical imaging - coupled with a very product-oriented mindset and an unrelenting desire to understand the problems that healthcare faces - leads him to integrate different structures, including the Fortune 500 giant McKesson, the Canadian telecommunication company Telus, and the Israeli startup Zebra Medical Vision, where he quickly occupies leadership roles with great responsibility. At Clarius (this “true gem” as he calls it in his own words) he draws on his expertise in the field of medical imaging to embed artificial intelligence into the most compact high quality ultrasound imaging platform on the market, developed by its brilliant founder Laurent Pelissier. The goal? Allowing any healthcare professional to perform an ultrasound scan, with a system as intuitive, qualitative, and compact as an iPhone. Perhaps those will even find their place in patient's homes at some point! In light of the current boom in digital health and the radical developments in AI, Ohad shares his perspective on the role that these technologies will have on certain medical specialities and notably radiology, where he believes that they will rather augment them without replacing them. He also explains why he thinks that most technological capabilities in digital health are outstripping the value they can create, and reflects on his career path where his transitions from huge corporates to human-sized startups shaped his evolution and leadership. A surprising dive into the future of medical imaging, that we invite you to discover in this new episode! Lion Electric was attracted to Joliet for its robust logistics industry, labor force and the Illinois Climate Change Policy which aligns well with the electric vehicle market. When the project started, the team knew it would have a big economic impact due to the amount of power and parking needed to support the plant. There was a tremendous amount of effort to take a speculative shell building and adapt it for producing zero emission vehicles.  Hear how the team pivoted in the middle of construction to add in over 100,000 square feet of office space, cranes, paint booths and reconfigured loading docks in our latest episode.

News Brendan rambles on about the Clarius wireless veterinary ultrasound machine. Main Topic: Pro and Prebiotic use in Rabbits It's a controversial topic this week: the value and use of probiotics and prebiotics in rabbits. Strap in, listen in.. and send us an email with your thoughts and comments to VetGurus@gmail.com Definitions from Wikipedia: Prebiotics are compounds in food that induce the growth or activity of beneficial microorganisms such as bacteria and fungi.[1] The most common example is in the gastrointestinal tract, where prebiotics can alter the composition of organisms in the gut microbiome. Probiotics are live microorganisms promoted with claims that they provide health benefits when consumed, generally by improving or restoring the gut flora. Probiotics are considered generally safe to consume, but may cause bacteria-host interactions and unwanted side effects in rare cases. There is some evidence that probiotics are beneficial for some conditions, but there is little evidence for many of the health benefits claimed for them. A flight crew springs to action to help a French Bulldog. How smart are birds? - we find out - sort of; and a discussion of wild animals helping us in the garden are news items this week. Our final news item is about an incursion of the varroa honey bee mite into Melbourne, Australia. This weeks main topic? Here it is: Illegal pets. Brendan and mark discuss the complex factors at play when dealing with illegal pets - from the massive money involved in the trade of these animals, the threat they are to ecosystems - both from zoonoses and competing with native species, and the decisions to make when presented with these animals at a veterinary clinic. Our product review this week is the Clarius C7 wireless portable veterinary ultrasound. Brendan had the opportunity to trial this ultrasound in his practice recently and he found it very user-friendly, in fact a joy to use compared to traditional ultrasounds. Brendan checks out another Clarius ultrasound soon and he will score these products in a later podcast. Today’s guest shares his expertise from years of working in the ultrasound industry. Their unique handheld ultrasound device can link directly to a smartphone or tablet. Dave Willis, the co-founder of Clarius, tells us how their mobile device can change the way doctors work, how they created the device, and their unique sales strategy. 1:00 Dave introduces his company Clarius and his product, which is a groundbreaking ultrasound device that connects to any tablet or smartphone running iOS or Android. 1:30 He was inspired to create the device when he visited a hospital department and heard how the number of consultants was much greater then available ultrasound ma-chines. 2:58 Dave’s company has removed typical hardware that ultrasound machines use and has enabled consultants to literally work with whatever mobile phone device they have in their pockets. 3:48 At the moment, the device is limited to Android and iOS as the vast majority of physicians are using devices with those systems. 4:42 Dave’s background is in ultrasound and working directly with physicians. 5:45 He feels the big advantage of his device is accessibility and affordability. 6:53 In the past, anything sub $10,000 with good image quality was difficult to find. Image quality is extremely important to new users. 8:00 The device doesn’t remove the need for training. That is still required, but it makes a range of diagnosis and treatments much more accessible to physicians. 9:20 Dave feels that this technology could be a good fit in quite a number of developing nations. 9:48 A good friend of Dave’s had developed his own ultrasound product previously, so when Dave relayed the earlier story about the hospital department with limited ultrasound equipment, they began working to create a device. A three-year window followed with development of an open source wireless system to talk to any operating system. 10:57 The product was primarily funded internally. 11:39 Dave discusses acquiring FDA and CE approval. A key tip is not to make any outrageous claims about your product in this process. The process of approval is typically six months from starting tests to final submissions based on feedback from the authorities. 13:30 Dave recommends newcomers to engage a third-party reviewer to help them with getting a product ready for the approval review. 14:23 Dave discusses their sales strategy which involves direct sales to end-users which is atypical of the industry. This can allow a dramatic cost reduction for the customer. 16:19 Clarius’s focus is point-of-care. They don’t plan to replace the big ultrasound ma-chines in the radiology department - getting smaller machines into point-of-care departments is where the biggest growth will occur in next few years. 17:45 Dave shares his personal experiences to show how ultrasound can improve accuracy for simple procedures. 18:36 The best point of contact for Dave is www.clarius.me where there is information and testimonials about the product. I hope you enjoyed the conversation today - Please leave a review and subscribe on iTunes. It really does help the show! Thanks for listening! Read on to see how Dave answered my final question: Vinay: What advice would you give to aspiring healthcare entrepreneurs and innovators? Dave: I think a couple of things. One is certainly a clear understanding of the end user. I've seen a lot of devices get on the market and fail because they miss the fundamental use model. Either they were too expensive or the image quality wasn't good enough. I think it's imperative to really understand the marketplace that you're trying to address.
