Dr. Adrian Soto-Mota brings a unique blend of expertise as a practicing clinician, data specialist, researcher, and educator at the National Autonomous University of Mexico (UNAM), the largest university in Latin America. The Yale Food Addiction Scale (YFAS) (https://sites.lsa.umich.edu/fastlab/yale-food-addiction-scale/) is the most widely used food addiction screening tool worldwide. Dr. Soto-Mota utilized another food addiction screening technique, CRAVED (https://the-chc.org/assets/uploads/CRAVED%20Introduction%20and%20Notes%20for%20the%20User_Update_240506.pdf), a shorter yet effective screening method, in his research. His study distinguishes between binge-eating disorder and food addiction, highlighting key differences. While binge-eating disorder is recognized in the American Psychiatric Association's diagnostic manual, food addiction as a substance use disorder is not—an important distinction.  Having experienced childhood obesity himself, Dr. Soto-Mota understands the personal impact of food addiction and is deeply committed to addressing the ongoing childhood obesity crisis in Mexico. In May 2024, he participated in the IFACC conference in London, where he signed a groundbreaking Consensus Statement (https://heyzine.com/flip-book/a00ee3aa6c.html). This agreement, developed by 40 experts over many months, recognizes ultra-processed food addiction as a substance use disorder. Motor vehicle collisions or road traffic collisions are a massive problem worldwide. Data from the World Health Organisation reports that there are around 1.2 million deaths every year and this is the leading cause of death internationally for children and young adults aged 5-29 years.  In the UK there are around 1,500 deaths annually and also around 60,000 patients with significant and life changing injuries, which is 7 patients every hour!! So anything we can do to improve patient care following an MVC is definitely a worthwhile venture. We've looked at Extrication here on the podcast before but we're back on it again because today the Faculty of Pre Hospital Care have released their Consensus Statement on Extrication Following a Motor Vehicle Collision.  The statement builds on the work from the EXIT project and the research that has helped inform our understanding of multiple factors of extrication. The statement will inform a change of practice for both clinicians and non-medical responders and in this episode we run through the statement with two of it's authors and discuss the practical applications. Dr. Margo Mountjoy received her medical education and her family medicine training at McMaster University, Canada and her sports medicine specialty degree in Ottawa, Canada. Margo received her PhD from the VU University in Amsterdam. Margo was the medical and scientific lead at the Health & Performance Centre at the University of Guelph where she focused her practice on promoting elite athlete care. In addition, Margo has acted as the national team physician for Synchro Canada for 20 years as well as for the National Endurance Training Centre Athletes (middle- and long-distance track athletes) and the National Triathlon & Wrestling team training centers.  Margo is a Clinical Professor in the Faculty of Family Medicine in the Michael G. DeGroote School of Medicine, McMaster University, Canada where she is the Dean of the Waterloo Regional Campus. Margo is the Chair of the ASOIF Medical Consultative Group and a member of the IOC Medical Commission Games Group. Well, Well, Well, this is very interesting. Back in May 10, 2023, we released an episode on "OSA (Obstructive Sleep Apnea) in Pregnancy: Time to Screen?". In that episode, we summarized the impressive data on OSA in pregnancy and its association with some adverse perinatal sequelae. Spring forward to July 6, 2023, the ACOG has now released a brand new Consensus Statement on OSA in pregnancy. This statement is a joint guideline from the Society of Anesthesia and Sleep Medicine and the Society for Obstetric Anesthesia and Perinatology. Seems that we were ahead of the curve on this one. So, should we screen for OSA in pregnancy? The answer is YES and NO. Is CPAP really safe in pregnancy? We'll explain in this episode.

In this episode of the IJGC podcast, Editor-in-Chief Dr. Pedro Ramirez is joined by Dr. Mario Preti to discuss the ESGO, ISSVD, ECSVD, and EFC consensus statements on pre-invasive vulvar lesions. Dr. Preti received his Specialization in Gynecology and Obstetrics and Specialization in Medical Oncology at the University of Torino. He is Associate Professor at the Department of Surgical Sciences, University of Torino, Italy, and the Immediate Past President of the ISSVD (International Society for the Study of Vulvo-vaginal disease). Dr. Preti is the author of 4 books on vulvo-vaginal diseases, and as of June 2022, he has published 108 papers indexed in Scopus/Pubmed, with 2,014 citations and an H-index of 23. Highlights: Epidemiological data show no decrease in vulvar invasive cancer incidence. This means that accurate diagnosis and treatment of pre-invasive vulvar disease must be improved: these are among the ESGO-ISSVD-ECSVD aims. The two carcinogenic pathways of vulvar squamous neoplasia recognize as precursors two vulvar intraepithelial neoplasia (VIN): HPV-associated squamous intraepithelial lesions (High Grade VIN) and HPV-independent VIN (mainly differentiated VIN). These precursors have different clinical approach, treatment and oncological risk. Their histologic features can be subtle, and the histological diagnosis may be further complicated by coexisting conditions. For differentiated VIN, an excisional procedure must always be adopted. Medical treatment (imiquimod or cidofovir) and ablative treatment can be considered for high grade VIN to preserve anatomy and function. These conservative approaches must be preceded by representative biopsies to exclude invasive disease. Follow up should be modulated according to the risk of recurrence (type of lesion, patient age and immunological conditions, other associated lower genital tract lesions). Pre-invasive vulvar lesions deserve specific attention because they affect not only functionality and body image, but also psychosexual factors.
