Summary In today's episode, we delve into the emerging role of ketamine in managing cancer and chronic pain. Our discussion is anchored around a comprehensive review article titled "Ketamine Use for Cancer and Chronic Pain Management," published in Frontiers in Pharmacology on February 1, 2021. This review, authored by Clayton Culp, Hee Kee Kim, and Salahadin Abdi, explores ketamine's potential as an analgesic in chronic pain conditions, particularly cancer-related neuropathic pain. Key Points from the Review Article: - Mechanism of Action: Ketamine functions as an N-methyl-D-aspartate receptor antagonist, providing analgesic effects at sub-anesthetic doses. Its ability to counteract central nervous system sensitization makes it effective in opioid-induced hyperalgesia. - Clinical Efficacy: Recent studies highlight ketamine's potential to reduce pain scores and opioid consumption, offering a promising alternative for patients with refractory pain. - Safety Profile: At lower doses used for analgesia, ketamine's safety and adverse event profile are significantly improved compared to its use as an anesthetic. - Pharmacogenomics and Interactions: The article discusses how genetic variations can affect ketamine metabolism and highlights potential drug interactions that clinicians should be aware of. Daniel Ritz stumbled upon motorcycling a dozen years ago while working as a newspaper editor in Southern California. “There was a small shop up the street from where I was living,” he tells “Driven to Ride” host Mark Long. “I started looking at Triumphs, and I saw the Scrambler as a good mix: heavy enough for big trips but light enough to still move around a bit.”For three years, a Matte Khaki Green Scrambler (“To this day, it's still the most beautiful bike I've ever seen”) was Ritz's sole transportation. “I just really committed to being as light-footed as I could, to being mobile,” he says, noting that he interacted with more people commuting and running errands on his motorcycle than when driving his pickup truck. Now living in Idaho, the conservation editor for “Swing The Fly” and founder of “Jack's Experience Trading Co.” has traded Pacific Coast Highway for Forest Service roads. “Wild people enjoy wild places and wildlife,” says Ritz. “I feel very lucky to have access to a pretty remarkable landscape that is well-built, and sort of curated, for motorcycling.” Connect with Us:Website: www.driventoridepodcast.comInstagram: www.Instagram.com/driventoridepodcastFacebook: www.facebook.com/driventorideEmail:hello@driventoridepodcast.com

Dr. Rosenblum reviews the benefits of Scrambler Therapy for CRPS and Neuropathic Pain State. What is Scrambler Therapy? Efficacy of Scrambler Therapy for Neuropathic Pain Mechanism of action of Scrambler Therapy Regenerative Pain Management Course PainExam Board Prep NRAP Academy Private Tutorials for Ultrasound Guidance and Regenerative Medicine ST was introduced as a chronic pain relief method in 2003. That same year, Giuseppe Marineo published findings from a small clinical trial involving 11 terminal cancer patients suffering from drug-resistant chronic visceral pain, with all participants showing positive responses and significant reductions in pain scores. In a subsequent trial involving 226 patients with neuropathic pain, 80% reported a 50% reduction in pain. Since then, numerous case reports and studies have documented the use of ST for various pain types. Evidence from these reports suggests that ST is effective for managing both acute and chronic pain from different causes. For instance, a child with acute mixed pain, resistant to pharmacological treatment, experienced significant relief after four ST sessions, with pain levels dropping from 5/10 to 0/10. Additionally, a 52-year-old woman with burning pain from her foot to knee, stemming from a right medullary acute hemorrhage and suffering for 12 years, reported immediate relief after ST. Her pain score decreased from 9/10 to 3/10 on the first day, and to 0/10 by the second day, remaining below 1 on the Visual Analog Scale (VAS) throughout the 10-day treatment period. In terms of chronic pain, literature includes a case where a patient with shoulder joint pain and limited range of motion saw significant pain reduction and increased mobility after 10 sessions of ST. ST has shown considerable promise in treating severe pain conditions that are typically difficult to manage, such as complex regional pain syndrome and pain related to HIV. Despite the encouraging results from these case studies, higher-quality evidence is necessary to establish the efficacy of ST, which could be obtained through extensive clinical trials, particularly focusing on chronic pain. Besides the aforementioned studies by Marineo and Sabato et al, additional trials have indicated that ST is an effective treatment for various chronic pain conditions, including low back pain, postherpetic pain, and neuropathic pain. For instance, a prospective study on chronic low back pain patients showed a significant decrease in VAS scores from 8.12 to 3.63 after six treatment days. Another trial involving 10 patients with postherpetic pain reported a drop in the average Numeric Rating Scale (NRS-11) score from 7.64 to 1.46 at baseline and 0.42 to 0.89 after one month, with benefits persisting at two and three months. ST has also demonstrated significant potential in treating neuropathic pain. In a prospective study of 45 patients with neuropathic pain lasting over three months, 28 experienced a decrease in Douleur Neuropathique en 4 questions (DN4) pain scores, with four patients stopping treatment early due to complete pain resolution. The mean baseline DN4 score dropped from 5.67 to 2.82 by the end of treatment. A pilot randomized trial involving 52 patients found that 21 out of 26 in the intervention group achieved complete pain relief. While the findings from these studies, along with others that have been systematically analyzed, suggest strong evidence for the efficacy of ST, a definitive conclusion regarding its effectiveness has not yet been reached. A systematic review by Majithia et al concluded that while studies generally indicate ST results in pain reduction with lasting benefits, there are still gaps in the evidence. This article aims to evaluate the research needs surrounding ST for cancer pain management. While Majithia et al focused on chronic pain across various conditions and noted specific evidence limitations, this study will concentrate on the effectiveness of ST for cancer-related pain. The objective is to identify gaps in the existing literature and provide recommendations for future research through a systematic review. We will specifically analyze the types and levels of evidence supporting the use of ST in managing cancer pain and determine what studies are necessary to enhance the evidence base. Field Trip Fails: Cadavers, Carcasses, and The Scrambler by 102.9 The Hog
He oído esto muchas veces.

This week, Editors Don Williams and Nic de Sena discuss the new Triumph Scrambler. This model uses the 1200 twin engine from the Bonneville range, and it in two distinct flavors, the X and the XE. Previous versions of the Scrambler were not very off-road focused, however Triumph has changed all that, and Nic had the opportunity to do a lot of pretty hard-core desert riding in his quest to find out how capable the Scrambler truly is once it goes off-pavement. In the second segment this week, I chat with the recently appointed Head of MV Agusta North America, Luca Dovesi. Luca started his career with Ducati in Thailand, and he then moved on to KTM Singapore where he was head of After Sales and Network Development. Three and a half years later, in January 2023, he was brought in by KTM from Singapore, to head up the re-organization and re-emergence of MV in the US. As Head of the premier brand of Italian performance motorcycles, Luca's energy and enthusiasm came across very clearly as we talked about the progress made so far, and what KTM's plans are for the storied Italian marque. Welcome back to another episode of the True Fiction Project Podcast. This week we have Taylor Koekkoek on the show, who is the author of the short stories series, Thrillville USA. Taylor talks to us about the inspiration behind the short stories and how pieces of the stories are pulled from his life. He talks about how he included Oregon's opioid crisis into the story and why he felt it was relevant. Then we learn what is next for Taylor as far as future projects he is working on. At the end of the episode, he reads us an excerpt from the first story of the Thrillville USA series, where we follow a character finishing his shift at the amusement park and the scandalous scene he stumbles upon behind the back side of the Scrambler. IN THIS EPISODE:[3:18] What is Thrillville USA?[4:40] Why did Taylor decide to name his book series after Thrillville?[6:29] What was the inspiration behind Thrillville USA?[7:51] How Taylor tackles the subject of Oregon's opioid crisis and is it discussed in all the short stories?[11:12] Taylor tells us about the excerpt he is going to read to us.[13:28] What future works will we see from Taylor?[15:55] An excerpt from the first book of the Thrillville USA series by Taylor Koekkoek KEY TAKEAWAYS:[3:19] Thrillville used to be a little roadside amusement park outside of Oregon that felt like a little bit of a death trap with rides that bucked in ways that didn't seem natural. Many rumors about fatalities and bodily harm were told about the amusement park, but they are believed to be false. [6:29] Thrillville USA was inspired by an amusement park that Taylor and his brothers visited from time to time when they were younger. The characters are not based on people he knew directly during his childhood, but instead pieced together from stories he heard from other people that he met. [10:03] There is an unintentional metaphor between the amusement park and drug use, being a dangerous activity that is also ominous, fun and exciting. After every holiday Jill feels like she has ridden the Scrambler ride too many times at the fair. She is overcome by sadness and anger as the flurry of happy FACEBOOK friends shares their FUN-FILLED LIVES. Moments that remind Jill that in her life those moments come at a cost. Intense planning, high anxiety and later complete low cortisol exhaustion. The scrambler ride effect buries Jill in sadness and wondering why her life is NEVER EASY. Why can't she live the life she planned? A widow at 24 and pregnant with her son only to come into an emotionally abusive marriage that destroyed her self-worth and amid everything diagnosed with ADRENAL INSUFFICIENCY. A disease she knew of all her life as her father died of an adrenal crisis at 56 years. A few days a "POOR" Jill and she knows she needs to see WHAT IS not might have been. So many WONDERFUL things in her life to celebrate. Maybe life for her is not elaborate parties and functions...maybe is it quiet excitement on her terms with the bright lights in her life. And she has a lot of BRIGHT LIGHTS. She took a BIG step today and wore a Halloween costume to work. She did it without the fear of ridicule from voices in the past. She did it with self-love and allowing herself to live her best life. Because she deserves it. Today she moved forward with ADRENAL INSUFFICIENCY.

This week on our motorcycle podcast Liza makes a big announcement. She bought a new bike, and it's a Scrambler 900. What a cool bike it is too. Emma shares a history on the Scrambler, it's racing roots, and how the scramblers we know today became a style different from the originals. Emma also introduces us to her orphans, Will and James, who she literally picked up on the side of the highway. These Canadians were heading to Mexico for a big adventure with little planning, and for one of them the adventure stops here. Listen and find out what happened to them on their trip. The last week or so in college football has been like taking a ride on the classic amusement park ride "The Scrambler"... with an insane NIL deal gone bad at Florida, Jim Harbaugh openly flirting with the NFL again but deciding to stay, Lane Kiffin loading up on quarterbacks at Ole Miss and much more. Keep your head on a swivel if you are a college football fan these days and count on JC and Morgan, presented by Blue Delta Jeans, to keep things in perspective for you.
