Approximately 10% of the population (which translates to around 500,000 people in BC) believes they have a penicillin allergy and are labelled as such in their patient records, but in reality, more that 90% of these patients do not have a true penicillin allergy. These false penicillin allergy labels can have serious consequences to both individual patients and the system; a false allergy label can lead to the use of an alternative antibiotic, which may have increased side effects, increased costs, and can lead to antibiotic resistance. Proactively ‘de-labelling' patients that have an inaccurate penicillin allergy label can help avoid these risks. Dr. Tiffany Wong is a pediatric allergist based at BC Women's and Children's Hospital and is an advocate for choosing the best medications for patients, which involves proving or refuting true drug allergies. Dr. Wong and her team have developed a mobile de-labelling tool to support healthcare providers to do this work in their own settings with their patients. This involves a risk assessment and follow-up processes to accurately diagnose patients deemed as high-risk in a way that is safe, standardized, and evidence based. She has also been working with patient partners to co-design patient resources, so patients and caregivers can be empowered to understand penicillin allergies. This work is also currently being adopted at the University Hospital of Northern BC, with support from the Spreading Quality Improvement Initiative, so the benefits of penicillin allergy de-labelling, and associated improvements to patient care, will be realized in the North. In this episode, we discuss the utility of veno-arterial extra-corporeal membrane oxygenation (VA-ECMO) for the temporary management of biventricular failure and cardiogenic shock requiring full cardiopulmonary support. Here, we define the types of ECMO and describe the unique physiology of this mechanical circulatory support platform, as well as review the potential complications and management strategies. Most notably, we highlight indications for and contraindications to the use of VA-ECMO and review the importance of patient selection. Lastly, we discuss de-escalation and de-cannulation strategies for patients on VA-ECMO as a bridge to recovery. The ECMO circuit is a narcissist, i.e. cannulas are named in reference to the circuit and not the patient (“inflow” vs “outflow”). The decision to utilize ECMO should be made by a multidisciplinary shock team and patient selection is KEY! ECMO physiology rule #1: VA-ECMO increases LV afterload Patients on VA-ECMO should be monitored with a PA catheter and an arterial line in the right arm Show notes - Biventricular Failure and the Use of VA-ECMO Notes drafted by Dr. Megan Burke. 1. What is ECMO and what are the different types? Extracorporeal membrane oxygenation (ECMO) is a temporary form of mechanical life support that comes in two flavors: veno-arterial, or “VA” and veno-venous, or “VV.” VV-ECMO supports extracorporeal gas exchange in the setting of acute respiratory failure VA-ECMO provides full circulatory support in addition to gas exchange, doing the work of both the heart and lungs. 2. What are the components and “anatomy” of the VA-ECMO circuit? The circuit is made up of the following major components: Venous (inflow) cannula Centrifugal Pump Oxygenator (also responsible for CO2 removal) Arterial (outflow) cannula The cannulas are named in reference to the ECMO circuit, not the patient. Dr. Gage suggests that we think of the ECMO circuit (and mechanical circulatory support in general) as narcissistic, i.e. flow is always in reference to the device. Gas exchange happens in the oxygenator. In the oxygenator blood flows through thin filaments that allow for diffusion of oxygen and carbon dioxide. Gas flows in the opposite direction of blood flow to maximize diffusion through the countercurrent effect. Oxygenation is determined by rate of blood flow through the oxygenator and FiO2 delivered. Carbon dioxide removal is determined by rate of countercurrent gas flow,

Megan Burke is a certified feng shui consultant based out of Brooklyn, New York, who has taken a special interest in home offices and started WFH Feng Shui. Megan has been a girl scout since Kindergarten. She is currently in the 10th grade. She has completed her Bronze and Silver awards. Megan is currently on the service team for her chapter counsel as a girl member. Megan has lots of interests outside of school. You will always see Megan in a theater production of competing in speech and debate. Her area she competes in is Original Oratory. Last year she spoke about perfectionism. Megan is part of the link crew at school which welcomes new students to the high school. She is also part of the high school theater company and will be playing the part of Nigel in the upcoming production of Matilda. She loves to read and journals when she can find the time. She has enjoyed being on her dance studios dance team for about 6 years and competed in group and solo competitions. She has also been part of the National Charity League, a mother daughter charity organization since the 7th grade. Harvey is from San Diego, and was charged as a test case by San Diego District Attorney Bonnie Dumanis using a law that had never been used before. It said someone could be charged for conspiracy for gang shootings, even if that person had nothing to do with the shootings at all. That was the case for Harvey. He was charged because he was in social media pictures wearing gang colors and making gang signs. A judge dismissed the charges against him, but not before he spent seven months in jail. Now, Harvey has done something that when he was in jail seemed like an impossible dream: graduating from UC Berkeley. This KPBS Investigates episode was reported and written by Claire Trageser. In When Time Warps: The Lived Experience of Gender, Race, and Sexual Violence (University of Minnesota Press, 2019), Megan Burke considers the relationship of sexual violence to lived time by reexamining and building upon the work of Simone de Beauvoir, and in conversation with Judith Butler, María Lugones, Maurice Merleau-Ponty, and many others. Through developing a feminist phenomenology of time, Burke allows us to consider how racialized colonial sexual domination structures feminine subjectivity. By focusing our attention on temporality, Burke deepens our understanding of how the past haunts the present, giving rise to sexual domination, as well as how we can actualize latent possibilities to lay those ghosts to rest.
