In this episode, Dr. Mark Piehl and Dr. Peter Antevy discuss why EMS must adopt a more thoughtful approach to fluid resuscitation for critically ill or injured patients.  Specifically, patients in shock. It turns out that EMS needs to take a more of a proactive approach to recognizing the need for and administering fluids for certain patients. Dr. Piehl, and Dr. Antevy are here to put a spotlight on the value of fluid resuscitation in the pre-hospital setting, walk us through the data, and discuss the how EMS can provide the right amount of fluid in the right amount of time to make a big impact for our patients.  Dr. Peter Antevy is a Pediatric Emergency Medicine Physician, as well as an EMS Medical Director for several fire-based EMS agencies in South Florida.  Dr. Mark Piehl is a Pediatric Intensivist at WakeMed Health & Hospitals.  He is also the Assistant Medical Director for WakeMed Mobile Critical Care, and an Associate Professor of Pediatrics at the University of North Carolina School of Medicine. Dr. Peter Antevy, innovator of the Handtevy Pediatric Resuscitation System, is a nationally recognized lecturer and expert in the field of pre-hospital pediatrics. He is extremely passionate about improving both adult and pediatric pre-hospital care and evaluating and improving upon currently accepted medical practice.  Dr. Antevy received his medical degree from the University of Miami School of Medicine in 1999 and then continued on to complete his pediatric training at the Children's Hospital of Los Angeles. He then achieved a subspecialty degree in Pediatric Emergency Medicine at the Children's Hospital of Pittsburgh before returning back to Florida where he has been a Pediatric Emergency Medicine Physician at Joe DiMaggio Children's Hospital since 2005. Dr. Antevy also currently serves as the EMS Medical Director for Davie Fire-Rescue, Coral Springs Fire Department, Southwest Ranches Fire Rescue and American Ambulance, and is the Associate Medical Director for Palm Beach County Fire Rescue, Miramar Fire Rescue and Seminole Tribe Fire Rescue. Peter currently serves as the President of the Greater Broward EMS Medical Directors' Association, an organization whose providers serve the 1.5 million residents of Broward County, Florida. Ryan is a dad to two, a Pediatric Emergency Medicine Physician, and the Director of Emergency Medicine at East Tennessee Children’s Hospital. He first joined us on the pod back in 2019 for episode #15 (Raising Kids as an ER Doc.), and later joined us at the start of the pandemic for episode #34, titled, “Coronavirus - Cutting Through the Bullshit with Dr. Ryan Redman”. Ryan came back (mainly because he wanted to be the first 3-Peat in the show’s history) to talk about the current state of Covid, specifically to share his and hear our thoughts on vaccines for our kiddos, the impact it has had on our families, and much more.We really enjoyed hearing Ryan’s take on it all., and hope it helps to support anyone who is on the fence about the vaccine or who has concerns about getting their kids vaccinated. Pediatric patients require special considerations compared to adults when receiving medical care, and that remains true for traumatic injuries dealt with in the ED and pre-hospital settings. Dr. Christine Darr, Pediatric Emergency Medicine Physician, discusses a range of traumatic injuries in pediatric patients and how to appropriately perform a physical exam, order radiographic diagnostics to further assess and identify injuries and key steps for management. She reviews growth plate injuries as well as considerations at different developmental stages that can mask the presence of more serious injuries like blunt internal organ trauma without rib fractures and SCIWORA (spinal cord injury without radiographic abnormality). Common injuries associated with abuse at different ages are also addressed to help you identify concerning signs of maltreatment of pediatric patients. As such, it's important to be careful with the documentation of these patients to maintain accuracy and avoid inclusion of common normal findings that may populate in "normal patient" macros that are inappropriate for pediatric patients. Dr. Karen Woolf, Pediatric Emergency Medicine Physician, shares some key areas to be wary of in your documentation for pediatric patients. Some examples include the description of vital signs and whether they fall within normal ranges, general appearance descriptors as well as developmental stage specific findings i.e AAOx3, normal gait and fontanelle description etc. Listen for a rundown of the key pointers for keeping pediatric documentation accurate and informative.

In this episode, we are going to talk to the Telehealth team at Children’s Hospital Colorado and share how primary care providers could operationalize the delivery of care through telehealth quickly to help navigate the COVID-19 pandemic and looking to the future post-pandemic.   For all the reasons that COVID-19 has turned our personal and professional lives upside down, it has positively accelerated the delivery of care through telehealth. Until a few weeks ago, most of our ambulatory providers administered care through telehealth as a secondary, less preferred option in cases where distance between provider and patient was too far to be together in person. In this episode we are joined by Christina Olson, MD; Fred Thomas, PhD and Alison Brent, MD.
