I'm Stacey Simms and every other Friday I bring you a short episode with the top diabetes stories and headlines happening now. XX New research in type 1 diabetes prevention launches. Nektar Therapeutics and TrialNet will evaluate a drug currently used for exzema for patients with new onset stage 3 type 1 diabetes The drug is RezPeg – that's a shortened version of the name (rezpegaldesleukin) This will be a study of about 70 adults and children and will launch this year. The new study will use a mixed meal tolerance test (MMTT) to measure the efficacy of rezpegaldesleukin or placebo for preserving C-peptide area under the curve over a 12-month duration comprised of a 6-month treatment period and a 6-month follow-up. Secondary objectives include pharmacokinetics, pharmacodynamics, and additional disease assessments including HbA1c levels and patient insulin requirements. Rezpegaldesleukin is being developed as a self-administered injection for a number of autoimmune and inflammatory diseases. https://www.prnewswire.com/news-releases/nektar-announces-clinical-trial-agreement-to-evaluate-rezpegaldesleukin-in-patients-with-new-onset-type-1-diabetes-mellitus-302383052.html XX NLS Pharmaceutics (NLSP) and Kadimastem announced successful completion of a pre-IND meeting with the FDA for iTOL-102, a potential diabetes treatment. iTOL-102 combines Kadimastem's IsletRx cells (stem cell-derived pancreatic islets) with iTolerance's immunomodulator iTOL-100, aiming to cure Type 1 Diabetes without requiring life-long immune suppression. The treatment was evaluated at the Diabetes Research Institute at the University of Miami School of Medicine, where it demonstrated functional insulin release and disease reversal in animal models. Based on FDA feedback, the companies are updating plans for safety toxicology studies and First-in-Human clinical trials. IsletRx is a clinical-grade product comprising human pancreatic islet-like cells capable of secreting insulin, offering a scalable source of insulin-producing cells to address donor islet shortages. The technology can detect glucose levels and produce required amounts of insulin and glucagon. XX FDA has signed off on a rapid-acting insulin biosimilar for the first time. The agency has given a thumbs up to Sanofi's Merilog (insulin-aspart-szjj) as the first biosimilar to Novo Nordisk's NovoLog for patients with diabetes. Merilog will be provided by prefilled pen in a 3 mL dose or in a multiple-dose 10 mL vial. It is for adults and pediatric patients age 6 and older. NovoLog and Novo Nordisk's other rapid-acting insulin follow-on Fiasp are among the drugs subject to government price negotiations under the Inflation Reduction Act. The new prices will be enacted at the start of next year. In July 2021, the FDA approved Biocon and Viatris' Semglee (insulin glargine-yfgn) as the first biosimilar to Lantus. Five months later, the U.S. regulator endorsed Eli Lilly's version of the drug, called Rezvoglar (insulin glargine-aglr). https://www.fiercepharma.com/pharma/fda-signs-sanofis-biosimilar-first-novo-nordisks-rapid-acting-novolog XX Eli Lilly said Tuesday that it will offer more doses of its obesity drug Zepbound in vials and lower the prices of the doses it already sells, as the pharma giant seeks to draw patients away from cheap, compounded copies of weight loss medications. The company launched 7.5 mg and 10 mg vials of tirzepatide, sold under the brand name Zepbound, which typically cost $599 and $699, respectively. However, they are now available for $499 per month for patients paying without insurance. This applies to the first fill and all refills that are delivered every 45 days. Additionally, the company lowered the prices of the 2.5 mg and 5 mg vials to $349 and $499 per month, respectively. The company, which has seen a significant boost in profits from Zepbound and Mounjaro – essentially the same drug with different FDA-approved uses – announced that the new vials and pricing are exclusively available through the company's self-pay pharmacy, LillyDirect Self Pay Pharmacy Solutions. https://www.bloomberg.com/news/articles/2025-02-25/zepbound-cost-for-vials-cut-to-battle-cheaper-copycats XX Medicare spending on 10 diabetes drugs, including popular GLP-1s, more than quadrupled over a five-year period and could reach $102 billion next year, an analysis by Health and Human Services' inspector general found. The findings also come as the Trump administration weighs the fate of a Biden administration proposal that would require Medicare and Medicaid to cover GLP-1s for weight loss. The biggest spikes in usage were for Rybelsus, Novo Nordisk's once-daily GLP-1 tablet, and for the company's weekly injectable Ozempic, whose spending about doubled every year under review. https://www.axios.com/2025/02/25/medicare-spending-surge-diabetes-drugs XX Tandem Diabetes Care has secured a new FDA clearance for its insulin dose-calculating algorithm, opening up Control IQ for use in adults with Type 2 diabetes. The expanded label was based on data from a pivotal, randomized trial of more than 300 people with Type 2 diabetes, comparing its use to manual multiple daily injections. Tandem said it plans to present the study's results at the annual Advanced Technologies & Treatments for Diabetes meeting scheduled for next month in Amsterdam. https://www.fiercebiotech.com/medtech/tandem-diabetes-care-insulin-dosing-algorithm-nets-fda-clearance-type-2-diabetes XX Drugs approved for diabetes and obesity might be useful for the treatment of cognitive and mental health disorders, according to a new paper published in Nature Mental Health. The study reviewed and integrated data from both preclinical and clinical studies to gather evidence on the possible effects of these drugs GLP1s and semaglutide in conditions such as dementia, substance use disorders, psychotic disorders, mood and anxiety disorders, and eating disorders. The study found promising but still preliminary evidence that GLP-1RAs could be beneficial over a range of cognitive and mental health disorders. These drugs have shown potential in improving cognition, reducing addictive behavior, and alleviating depression and anxiety. More data from robustly designed studies (i.e., randomized controlled trials) are needed to better understand GLP-1RAs' prospective efficacy and safety profile, especially with long-term use. https://medicalxpress.com/news/2025-02-diabetes-drugs-mental-health-treatment.html XX The FDA issued draft guidance that includes recommendations to support the development and marketing of safe and effective AI-enabled The guidance, if finalized, would be the first guidance to provide comprehensive recommendations for AI-enabled devices throughout the total product lifecycle, providing developers with an accessible set of considerations that tie together design, development, maintenance, and documentation recommendations to help ensure the safety and effectiveness of AI-enabled devices. FDA is requesting public comment on this draft guidance by April 7. Although he was enjoying an exciting, rewarding career at IBM, he took a leave of absence and entered the School of General Studies at Columbia University to complete the prerequisites for medical school. With the support of his family, Mihale was accepted at the University of Miami School of Medicine. Clearly a non-traditional applicant at the age of 35, he was grateful for the school's confidence in his potential. Mihale soon proved himself to his colleagues and teachers. While in medical school, Mihale applied his background in engineering to design a flexible knee orthotic through a project sponsored by NASA. For his post-graduate training, he returned to his native New York to do a surgical internship and an orthopedic residency. He then took a year off to work at the IBM Research Center to develop an engineering model for robotic surgery. Under his direction, the project was completed, and the robot is currently used for hip replacement surgeries at the University of California at Davis. In 1990, Dr. Mihale continued his professional development and entered the Executive MBA Program at the University of South Florida. He subsequently helped to launch a health maintenance organization, and in January 1994, he started St. Augustine Health Care, a Medicaid and commercial HMO. The business prospered and ultimately achieved $30 million in annual revenues. St. Augustine was recently acquired by AvMed, and Dr. Mihale retired from that venture. Today, Dr. Mihale maintains a strong connection to the medical school and provides financial support, sponsors regional alumni meetings, and serves on the Dean's Leader-ship Cabinet. Hazle Smith Konerding, 76, entered eternal rest on June 17, 2024. She was born in Fairbanks, Alaska on October 18, 1947, where her parents, Dewey S. and Evalyn M. Smith were civilian employees of the Army during the early Cold War years, developing Ladd Army Airfield. In 1955 the family moved to Fort Walton Beach, Florida, where her father continued his civil engineer profession building more runways, this time at Eglin Air Force Base. In this episode, we pass the mic to Dee Jones to discuss pivoting during your law career and preparing to become a judge. Dionne "Dee" M. Jones is the daughter of Command Sergeant Major (Ret.) Levester Jones, Sr. and the late Liza Dudley Jones. She is an alumna of the University of Florida, having earned her Bachelor of Arts in Political Science with a minor in education. Following her passion for advocacy and the law, Dionne earned a Juris Doctorate from the University of Florida Levin College of Law, where she was a Council on Legal Education Opportunity Fellow. Dionne has been an attorney for 28 years, having practiced both civil and criminal law and tried a plethora of cases to verdict. Within the civil domain, she has handled auto liability as well as workers' compensation cases as well as family law and employment law cases. Under the criminal law umbrella, Dionne has provided oversight for dependency law cases and was a felony-level prosecutor for several years, where she served focused on domestic violence, aggravated battery and aggravated assault cases. Dionne earned a PhD in Higher Education Leadership in 2013. Upon graduation, she served as an Assistant Professor, the Co-Director of the Law and Medicine Scholarly Concentration, as well as the Co-Director of the Coaching program at the College of Medicine. Her research agenda centered upon physician leadership as well as the intersectionality of law and medicine. Currently, Dionne is a senior trial attorney for Allstate Insurance Company and simultaneously serves in a quasi-judicial role as a Hearing Officer for Hillsborough County. People with high uric acid levels have an increased risk of dementia and Alzheimer's. There's a relationship between these diseases and uric acid. Uric acid directly leads to insulin resistance. [19:30] Your Body Is Retaining Uric Acid As A Survival Mechanism We tend to retain uric acid. Our uric acid levels can be higher as a survival mechanism, thanks to our primate ancestors. We don't hibernate for six months. Therefore, we don't need to be storing this fat for the winter that never comes. 88% of American adults are metabolically unhealthy. Getting Covid is associated with a 40 to 60% increased risk of type two diabetes. Individuals with higher uric acid levels have a higher risk of dying from Covid. [25:10] Your Body Can't Handle More Than Five Grams of Fructose At A Time Your body can handle some fructose, about five grams at a time. When your body is presented with high levels of fructose, it overwhelms what the body can do. When fructose is delivered in the context of fiber, it slows the absorption. So, it's better to eat an apple than drink apple juice. Vitamin C aids in uric acid excretion. The most common way to overdo fructose consumption is by drinking fruit juice. A 12-ounce glass of orange juice might have 30 grams of sugar. There's no way the body will be able to deal with that appropriately. [35:30] Tart Cherries As A Remedy For Gout and Lowering Uric Acid Levels Tart cherries have been researched for decades. Originally, tart cherries have been looked upon as a remedy for gout. It has been noted that people who ate cherries would have fewer gout flare-ups. Tart cherries are pretty darn effective in lowering uric acid levels. [36:40] The Health Benefits of Drinking Coffee Drinking coffee is associated with a dramatically lowered risk of Alzheimer's. Coffee is also associated with lower uric acid levels. Plus, coffee reduces inflammation, upregulates antioxidant function, and improves detoxification in the body. Also, coffee helps us grow new brain cells, no matter our age. [41:45] The Relationship Between Animal-Based Protein and The Role of Uric Acid Fiber is so essential for human health. Most of Dr. Perlmutter's food is plant-based. Regarding our uric acid story, certain meats or animal products are worse than others. Organ meats, sardines, anchovies, and scallops can raise your uric acid levels. The most significant issues for uric acid are fructose, purines, and alcohol. [48:20] Should I Follow A Ketogenic Lifestyle If I Have Gout Attacks? The most significant player in gout is fructose. When you research how to fix gout, there will be no talk about removing sugar or fructose from your diet. The human requirement for sugar in our diets is zero grams per day. We subsidize the growth of corn from which high fructose corn syrup comes from. We are supporting the diseases that are sucking up our healthcare resources. Diseases are significant money makers for the healthcare industry. It's time we start thinking about prevention. [52:25] What Role Does Mental-Emotional Stress Play With Uric Acid Levels? When we are experiencing stress, our ability to make rational, forward-thinking decisions is compromised. When your decision-making is compromised, you won't be making good lifestyle choices. Therefore, under stress, you will eat more foods that have sugar. Eventually, you will raise uric acid levels and feed the flame of poor metabolic health.
