In this enlightening conversation, Dr. Jennifer Reid speaks with Dr. Jordan Grumet, hospice physician, writer, and podcast host, as he shares his journey. At age 7, Dr. Grumet lost his father, an oncologist, to a brain aneurysm, and he became determined to become a doctor himself. However, despite achieving his goal, Dr. Grumet experienced burnout when he realized that becoming a doctor couldn't "cosmically make up" for his father's death.Drawing from his experiences as a hospice physician, he offers profound insights on distinguishing between meaning and purpose, overcoming burnout, and creating a life that genuinely "lights you up."Key Points* Meaning vs. Purpose:* Meaning: How we cognitively understand our past; the stories we tell ourselves* Purpose: Actions in the present and future that "light us up"* Finding Your Purpose: Look for "purpose anchors" - activities and experiences that energize and fulfill you* The Art of Subtraction: Remove elements from your life that don't bring joy to discover what remains that does* Generational Growth: Breaking cycles of expectation to model authenticity for the next generation* Achieving Impact: Dr. Grumet found that pursuing what truly energized him (writing and podcasting) ultimately allowed him to help more people than his medical practiceNotable Quotes"Purpose is about the present and future, and it's about actions. All it is is about things that light you up. If you're trying to get more out of purpose than that, then you really don't have a purpose problem. You have a meaning problem.""Happy people tend to tell themselves heroic stories about their past. They see their traumas, they see their difficulties, and they see that they heroically got through them.""You can't purpose your way to enough. That's a meaning problem. That's a going back and dealing with your childhood problem.""All you have control over, the one measly thing we can control is what activities are we involved with as time passes.""I've really spent a lot of time thinking about what winning the game when it comes to life looks like... winning the game is as you get older, from the day you're born to the day you die, basically you want to be looking at your calendar... and you want to add in as many purposeful, joyful things that light you up into those time slots of your life.""When people see you doing things authentic to you, you are giving them permission."About Dr. Jordan GrumetDr. Jordan Grumet is a physician, writer, and host of the popular Earn and Invest podcast. He currently serves as the Associate Medical Director at Unity Hospice. Following the discussion on ECLS in AMI and cardiogenic shock, we go on to discuss eCPR for cardiac arrest specifically. This episode was recorded live at the Critical Care Canada Forum 2024 as part of our special series on cardiac intensive care. Our guest is Dr. Darryl Abrams, Associate Medical Director and Director of Research for the Medical ECMO Program at New York-Presbyterian/Columbia University. Dr. Abrams joins us for an in-depth discussion on the current state and future direction of extracorporeal cardiopulmonary resuscitation, or eCPR.We dive into the complex world of eCPR in refractory cardiac arrest, starting with a breakdown of the three landmark trials that have shaped the field: the ARREST trial, the Prague OHCA trial, and the INCEPTION trial. Each study offers a unique perspective, from the dramatic early findings of ARREST to the pragmatic design of Prague OHCA and the sobering multicenter outcomes of INCEPTION. A major theme throughout the episode is the role of system design. Dr. Abrams emphasizes the importance of minimizing low-flow time, rapid cannulation, and consistent team expertise—factors that can make or break the success of eCPR. We also explore the ethical and practical considerations that come with rolling out such a resource-intensive intervention, including the balance between innovation and equity. Is it fair that access to eCPR may depend on geography or institutional resources? And how do we make meaningful improvements in survival when only a few centers can offer this advanced care? The episode closes with a practical lens: how should clinicians approach building an ECMO program? What are the essential pieces that need to be in place before considering eCPR? And how do you select patients in a way that balances risk, benefit, and system capacity?Chapters:Introduction and guest welcomeSetting the scene: What is eCPR and why now?The ARREST trial: Small study, big impactThe Prague OHCA trial: Early randomization, broader populationThe INCEPTION trial: Multicenter reality and negative resultsComparing the evidence: Why do outcomes differ?Low-flow time and speed of cannulationThe role of meta-analyses and what they do (and don't) tell usOpportunity cost: What are we giving up to fund eCPR?Duration of support: How long is too long? Will there be another trial? Over the last few decades, there have been many incredible medical advances in newborn care. At Children's Hospital Colorado, we're launching an exciting new initiative to take another step forward in care for our tiniest patients. For newborns who need to be hospitalized, getting home as soon as possible is critical for their successful development. A common barrier for discharge that can keep babies in the neonatal intensive care unit (NICU) for additional days or weeks is the need for a nasogastric (NG) tube. Ensuring an infant is receiving proper nutrition is essential to securing a safe discharge. What if we could deliver NG tube feeds at home and provide all the necessary support? That's the game changing idea we explore in this episode with our neonatology experts. To dive into this topic, we are joined by three neonatologists practicing at Children's Colorado. Stephanie Bourque, MD, Kathleen Hannan, MD, and Danielle Smith, MD, tell us about a new at-home NG tube feed program that the team at Children's Colorado is currently developing. Dr. Smith is the Medical Director of the NICU, Dr. Bourque is the Associate Medical Director of the NICU, and all three of our guests are faculty at the University of Colorado School of Medicine. Some highlights from this episode include: What the need has been historically for NG tubes Model for this at-home program What's required to make this at-home model a reality The role of the primary care provider in this partnership The impact a change like this could have on patient outcomes and family satisfaction For more information on Children's Colorado, visit: childrenscolorado.org.
