Annually on October 10th, the World Health Organization (WHO) commemorates World Mental Health Day with the aim of raising awareness about mental health priorities. Not only are the microbes invisible to the human eye, in many places, they're invisible because people simply are not looking for them systematically.Doctors often do not know what infections their patients have and treat them based on best guesses, which allows for ineffective treatments and exacerbates drug resistance. Policymakers don't know which infections are most common among populations and cannot make informed decisions about needed treatments or vaccines.This is a major problem across Africa and a new report shows just how complex the problem is.The Mapping AMR and Antimicrobial use Partnership (MAAP), which included the One Health Trust as well as the African Society for Laboratory Medicine; Africa CDC; the East, Central, and Southern Africa Health Community; Innovative Support to Emergencies, Diseases, and Disasters, a nonprofit focused on technology and communication;  the clinical research group IQVIA; and the West African Health Organization, collected data from laboratories from 14 countries in Africa (Burkina Faso, Cameroon, Gabon, Ghana, Kenya, Eswatini, Malawi, Nigeria, Senegal, Sierra Leone, Tanzania, Uganda, Zambia, and Zimbabwe). “The study revealed significant gaps in bacteriology testing capacities,” the group, whose work was paid for by the UK government's Fleming Fund, wrote in PLoS Medicine.It's the biggest survey yet of testing for antimicrobial resistance – AMR or drug resistance – in Africa.Among the gaps: too little testing overall, a lack of laboratory capacities, and poor coordination and analysis of records. Many records were kept only on paper, which made them almost impossible to access.None of this surprises Dr. Sabiha Essack, South African Research Chair in Antibiotic Resistance and One Health and Professor in Pharmaceutical Sciences at the University of KwaZulu-Natal. In an ideal world, she says, a doctor, nurse or other professional should see a patient, test them to see what specific microbe is causing an infection, check to see which drugs will successfully fight that germ, and then treat the patient accordingly.Cheap point-of-care tests should be available everywhere and the results of those tests should be entered into systems that officials can use to make policy decisions, she says. What happens when you throw out rigid timelines and arbitrary benchmarks, allowing children to learn at their own pace in ways that honor their unique personalities? Kristi's 11-year homeschooling journey with her three daughters reveals the transformative power of personalized education.When her introverted third-grader with learning difficulties asked to homeschool, Kristi—despite having no prior knowledge of homeschooling—took the leap. Within two years, she brought all three daughters home, discovering that the full benefits came when the entire family shared the same educational approach. This shift opened extraordinary possibilities: four-day school weeks, regular RV camping trips, skiing adventures that started on Thursdays, and meaningful participation in the family's citrus farm.Most remarkably, Kristi's approach to her oldest daughter's learning challenges demonstrates the power of patience and personalization. Working "one to two years behind typical grade level for a very long time" and even repeating ninth grade at the student's request, this daughter eventually caught up naturally. When a child loved horses, they studied equine science for a year. Quite a few of the episodes here on What Was That Like involve emergencies. Today we're talking about a different kind of emergency.   If you're new to the podcast, this is definitely not a typical episode. You might want to look through some of the past episode titles and pick one out that looks interesting.   But today is a little different. I'm in Dallas this week, hanging out with a bunch of my podcasting friends, so for this episode we're kind of taking a break from the normal interview format.   A little bit of background. About 10 years ago, I was creating and hosting a different podcast. It was called Ultrafinishers. Each episode, I would interview a runner who had just finished their first ultramarathon. An ultramarathon is any race longer than a marathon, which is 26.2 miles. So an ultra usually ranges from 50k, which is a little more than 30 miles, up to 100 miles. Some are even longer than that.   But for one of those episodes, I asked people to call in and tell their story of the nightmare that just about every runner has had to deal with – you're out on a run, and you have to poop, and there's no bathroom in sight. It's not a fun thing when it's happening – but as it turns out, it's pretty entertaining to hear about when it happens to other people.   So you're about to hear a bunch of my ultra running friends from 10 years ago, telling their poop stories. And all of these were called in on a phone line, so the audio quality is not quite what you'd expect on a modern podcast.
