Update from 2023: Heat exhaustion is less serious than heatstroke also sometimes called sunstroke. Symptoms of heat exhaustion can occur quickly or over a few days. Many patients will call complaining of leg cramping here is a list of symptoms: Heat exhaustion can lead to: - headaches - muscle cramping - nausea and vomiting - excessive sweating and cold clammy skin - on occasion a decreased pulse and feelings of weakness - dizziness and sometimes fainting - dark-colored urine can occur from dehydration It's important to note that heat exhaustion can be alleviated with hydration and moving to cooler temperatures indoors. Heatstroke is a life threatening medical emergency and your body's temperature can rise to 104 or more. It needs immediate treatment and we encourage you to call 911. Heat stroke affects the brain and nervous system which can lead to serious injury. It also affects your organs. Immediate attention is required. When a person has reached this level they usually have stopped sweating, as their body's ability to cool is now disabled they are more likely to faint as well as have the symptoms arriving from heat exhaustion. Instead of a weak pulse, the body responds with a racing heart beat. They may appear confused, agitated, slurred speach and appear if they are drunk if they had been drinking alcohol. Alcohol and heat do not mix and a serious event can be mistaken for a drunken fool instead of a heat stroke that can cause a person to loose consciousness and even coma. These are simple definitions to help you and your loved ones seek medical care. The key is to stay hydrated while out in the sun, use sunscreen and have shade available. Remember to keep an eye out on your fur children as pets can also suffer from these same consequences and if the pavement, sand or walking is area is hot on your feet it is also hot on all of our four legged friends. We should be on big happy family that communicate for the best interest of the patient. How is your relationship with your doctor/doctors and nurses. If you are having a procedure, surgery or being hospitalized, know your rights when a patient advocate is needed when you are a patient in the hospital and not being heard. Have a checklist before your procedure and be prepared. We will also discuss private practice, corporate doctoring and knowing the difference and deciding if you are in the right place. When you have to take prednisone, many will complain of having the munchies and are gaining weight. Let's learn today how to combat those hormonal changes and help explain why a burst of energy occurs when taking prednisone. With Children, you may see them more active than they had been all month...yes, these are normal effects. While Prednisone's side effects can be serious, used in the right amounts it is life altering and life saving. Many patients who are on long term physiological prednisone, actually do not loose weight, however their adrenal glands do not produce the hormone that makes cortisol, very similiar to prednisone and they need the medication to be able to function. If your baby or child looses their appetite, runs a fever there are several concerns. For babies, look for nasal flaring and if the lung muscles look retracted (like their working hard) these symptoms are serious and your child may need to be sent to an ER. Many children have RSV and suffer from mild symptoms, however it can turn serious quickly. If this should happen to you, be sure you ask for a nebulizer, with medication shortages we have seen inhalers prescribed to three year olds. So glad these parents call their doctor as we recommend a nebulizer, holding the mask up to the baby or child to breath in, It's okay if they are crying. Crying is great for the lungs and the medication can quickly get into the lungs to help the child breath better. The flu is around this year more than ever, like Covid and RSV it spreads easily, washing your hands continues to be important as well as covering your sneezes and coughs. During the pandemic public became wary of shortages of many things, including toilet paper. Today, the concern for medication is a real concern. What should you do if you receive a call from your doctor's office or pharmacy that your needed medication is no longer available? We will cover a few immediate options that you can take. 1. Consider different dosages, for example if you normally receive a 30 mg dose of Adderall, you may have to consider a 20 and a 10 mg or three 10 mg tablets as your dosage. The difficulty that arises from this change of prescription is that it is recorded in many states' Prescription Monitoring Program. Which means that instead of dispense 30 tablets you may have to receive 90 tablets of a smaller dosage. These numbers are all reported to the government monitoring program, which many patients are leery of. While we encourage natural supplements for our patients many may need ADD/ADHD medication to stay concentrated in school or their workplace. If you find you have encountered a shortage immediately ask if there is an alternative dosage your doctor can e-script in and immediately ask for the pharmacist to cancel the previous one, by doing so this helps protect you and your doctor's license. Other shortages have included: a few months ago, we found difficulty with a shortage of a popular type 2 diabetic medication, it was mainly caused by viral videos of weight loss a benefit of the medication for many patients used on label and off-label. Hypertensive medications have had to be changed due to re-calls. While many patients found these issues frustrating, we shared with each patient this is a new opportunity to find alternative methods of therapy and treatment and many patients were able to change there lifestyle and have weight loss, with the effect of no longer needing the medication. While patients with ADD/ADHD found new opportunities in their career that kept them focused and excited about their endeavors with a new drive in knowing that they were no longer dependent on the medication they took all through Middle School and High School. Every obstacle in medication, we find new solutions and new studies help us daily to be better doctors and better providers. We hope today's episode provides food for thought. If your mom, dad or elderly loved one is having memory issues, wether dementia, Alzheimer's small vessel disease that causes short term memory loss, when should you decide on memory care and is assisted living an option. Many with memory disorders can move to an assisted living facility, they have monitors on the floor so that if the patient tries to walk the front desk is alerted for immediate assistance. Even in assisted living or memory care, patients can wear a device that is similiar to a life alert that they can push for assistance. However, many individuals may forget how to ask for help and that is when it is best to have laser movement devices that can alert the front desk. Many patient families ask when it is best to move their parents many prefer to be by their friends and have a unique support system with family, church and friends that allows them to maintain their independence. Others, may become more difficult as they refuse to take medications that allows the family to help them. It is one situation that can be better handled with assisted and memory care alternatives. Patients come in all the time and share their last doctor told them they had normal tests of their thyroid and hormones. Well, that is not always true. Most thyroid tests have on between 0.5 and 4.5, however at 2.5-3.5 you most likely are already suffering from symptoms of thinning hair, loss of hair, constipation, dry skin, feeling cold all the time, gaining weight, and more.
