Lifting the face and neck without surgery is the holy grail of aesthetic medicine. While there's no meaningful substitute currently available that can give the degree of laxity correction in the face, neck and jawline as the facelift, there are good non-surgical skin lifting and tightening treatments for mild to moderate signs of aging. Non-surgical skin treatments started with various smoothing treatments such as chemical peels. As doctors noticed the more aggressive use of these treatments created some tightening as well, the idea of developing a standalone skin tightening treatment in and of itself piqued their interest. In the 1990's, there were attempts to use lasers and other infrared light sources to tighten skin, but it wasn't until the early 2000's with the introduction of Thermage and fractional laser treatments that the modern era of non-invasive skin treatments emerged. Through trial and error, doctors discovered that it's safer and more effective to treat with these energy devices over time rather than do one really aggressive treatment. More recently, the FDA has cleared ultrasound treatments Ultherapy and Sofwave for lifting and tightening the skin. Although relatively small, the eyes are the first area of the face to show changes, and have a big impact on one's appearance.  When considering which procedure to have for the eyes, first identify your specific concerns: is it volume loss, crepiness underneath the eyes, crow's feet, low brow position, or fat bulging causing bags beneath the eyes? There's no meaningful substitute for eyelid surgery, but non-surgical treatments are useful for certain concerns around the eyes. Botox and fillers, for example, can raise the brow and treat crow's feet and frown lines, and chemical peels can treat crepiness around the eyes. You know you are not ready for a facelift yet but your face is not looking as youthful as you want. There are many non-surgical options and minimally invasive options that are increasingly important to maintaining appearance if a facelift might be in your future in the next decade. If you are  counting down “T-Minus 10 Years” to a facelift or necklift, listen to Dr. Bass review your options to look your best until you are ready to launch a surgical project and to help extend the time until surgery is necessary.   Thirty years ago, the usual age for a facelift was around 50. Today it's 60 or even later thanks to the non-surgical technologies and treatments available including injectables, Ultherapy, radiofrequency microneedling, threadlifts, and myEllevate. Depending on how close you are to needing a face lift, you can take small steps, or do a combination of treatments to optimize appearance and delay surgery.    If you already have significant skin laxity, Dr. Bass cautions that there's no meaningful substitute for facelift and necklift and nothing on the horizon to replace them. If you're avoiding the plastic surgeon's office for fear of being sold something you might not need, know that it is much easier and more affordable to prevent your face from aging now than it will be to fix it later. Dr. Bass outlines how planning for the future and a great relationship with the right aesthetic provider will give you much more success throughout the years.   The first step is finding a provider who you enjoy and trust. This person should be a good listener and have a refined approach, informed by excellent training and years of experience. To avoid being shoehorned into a treatment or surgery that isn't right for your age or concern, a wide range of non-surgical and surgical options should be available. Once you find the right provider, the second step is to have a plan for the future that accounts for your age and your skin type. As you might expect, this plan begins with proper skin care with products that protect your skin from UV damage and keep it healthy with the right formulations for your skin type.  Energy-based treatments like laser resurfacing or Ultherapy, Botox, and fillers are the next stage of intervention and with a minimum amount of time, money and intervention can delay the need for face lift surgery by a decade or more.  Where should you start?
