Send us Fan Mail El Niño warnings are in the news every day, with some declaring a super or even Godzilla event is coming, and then someone else will say it won't be that bad, while others declare that even if it isn't a record event, an El Niño landing on an already overheated planet (it was just announced we had the hottest May since 2024) is not good news, regardless. It's confusing, isn't it? But the even bigger story, which is often reported separately, is there's a double impact on our horizon, and this is where it becomes a polycrisis. We are moving into even hotter conditions at a time when infrastructure, energy systems, food systems, trade routes, and political systems are all under strain. It speaks to the essential truth that climate risk and geopolitical risk do not sit in separate boxes; they interact and compound. Yet the media keeps reporting it all separately, so how can we prepare if we do not know? The war in Iran will have a profound impact on our food supply. Farmers are either not planting crops because they can't afford fertilizer or are paying much higher costs, which means we – the consumer - will experience significantly higher food prices and inflation. However, with extreme temperatures and extreme weather events (like extreme flooding) coming, how much food will even make it to market? Just rice, which more than 50% of humanity relies on as a staple, doesn't grow above 40°C, and we've seen months above that recently in Asia – where most of the world's rice is grown! It's getting serious. We are very happy to welcome our very special guest, Jan Umsonst, the Earth System Nerd, and he's promised he'll join us this time! Israel attacks on Lebanon escalated, threatening UNESCO World Heritage sites, and then a ceasefire was announced, but will Hezbollah stop? Russia scaled up its attacks on Ukraine, with murmurs suggesting internal support in Russia is waning. And on the other side of the world, Pauline Hanson in Australia is moving up the electoral charts, leaving us wondering, really? That's the direction Australia thinks is sensible right now? Extreme heat events continue around the world, with India releasing estimates of excess deaths during heatwaves at 3,400 a day, and up to 30,000 over a five-day heatwave - and that's just India. Weather professor Jeff Berardelli said the forecast strength of this El Niño is nothing short of astonishing, deserving a “Supercalifragilistic expialidocious!” rating. However, later that day, he shared another view with a different data set, suggesting it won't be that catastrophic – we hope so! This week on the Disaster Podcast Dr. Joe Holley comes on the show for a clinical update episode on Heat-Related Medical Emergencies. As we head into the summer months here in the Northern Hemisphere, the likelihood of encountering patients with heat emergencies increases. For disaster responders in the summertime, the risks are even greater as they wear gear and carry equipment that adds to their load while working in hot weather. Co-Hosts Sam Bradley and Jamie Davis join Joe on the discussion. Types of heat emergencies Heat Cramps – The symptoms of heat cramps include painful involuntary muscle spasms brought on by loss of sodium through excessive sweating. Most often seen in people exerting themselves in extreme heat situations without rehydrating themselves or taking adequate breaks to allow the body cool down. Heat Exhaustion – Next on the spectrum of heat emergencies is heat exhaustion which progresses from heat cramps as the body starts to lose the ability to compensate for lack of adequate cooling. Symptoms include early signs of shock like dizziness, nausea, cool, pale, and clammy skin. Heat Stroke – The most severe level of heat emergency is heat stroke which is a true medical emergency that may result in permanent disability or death. Hot, dry skin, loss of consciousness, and complete failure of the body’s cooling mechanisms are key signs. Heat Management in Disaster Situations Joe explained that preparing for heat management in disaster situations is crucial, especially for first responders working in austere environments with limited support systems. He highlighted the increased physical and mental demands of search and rescue operations in debris-filled areas following adverse weather conditions. When asked about immediate actions for heat stroke emergencies, Joe indicated that it is a life-threatening situation requiring immediate attention, though the specific response was not fully detailed in the transcript. In case you've been living under a rock, Europe's heatwave took centre stage in the world's news, which unleashed climate deniers across social media, meanwhile - not gaining anywhere near the level of attention - India, Pakistan, Bangladesh, Vietnam and beyond, are seeing extreme wet bulb temperatures creeping towards 50°C, with grids straining to keep citizens cool, especially during an energy crisis. We saw the first Enhanced Games break one record (just), the backlash against Ferrari's first EV, Alberta has decided to move ahead with plans for independence from Canada, more than 90 people died in a coal mine explosion in China, and at least 500 children have died of measles in Bangladesh. In Saudi Arabia, the Hajj goes ahead with extreme heat a massive risk, apparently 1.2 billion people are living with mental disorders globally, scientists are screaming the alarm as Thwaites (aka the Doomsday Glacier) is facing imminent collapse, and of course, Ebola is raging and experts are becoming increasingly concerned. War is still up in the air, with the possibility of peace in Iran, although it's hard to know if you're only following President Trump's Truth Social posts. Can someone take his phone off him? Meanwhile, the stock markets are up and down, with energy and food experts screaming disaster is on the doorstep, but you wouldn't believe that if you only followed Wall Street, right? In Karachi, Pakistan, at least 10 people have died of heatstroke, while India continues to experience shocking temperatures, and finally, the great man, Sir David Attenborough turns 100 this week. We'll be discussing all of that and more, but first, we're going to look at the state of the insurance industry and what the actuaries are saying. We'll discuss this within the context of the Nine Planetary Boundaries, and assess the current state of the insurance sector, including how far until it collapses, why it will collapse, and what that means to all of us. Because if it does go down, the global economy is next, and well, that won't be pretty. It's a big, wide discussion, and we are delighted to welcome Praveen Gupta to share his wisdom. Praveen started in insurance back in the late 70s with The New India Assurance Co, followed by CGU, Allianz, Bajaj Allianz General Insurance, and finished his illustrious career as Managing Director and CEO of Raheja QBE General Insurance Company. Today he is a blogger, writer and thinker, with articles regularly featured in Illuminem, and he is also an Advisory Board Member for Sanctuary Asia. Broadcast from KSQD, Santa Cruz on 4-09-2026: Dr. Dawn shares a follow-up from an emailer in Switzerland providing seven functional medicine practitioner addresses near Zurich and Aargau, noting that Switzerland uses different terminology but is actually an "epicenter of functional medicine." Dr. Dawn calls for support of the bipartisan Break Up Big Medicine Act, modeled on Glass-Steagall, which would prohibit common ownership of medical providers with insurers, pharmacy benefit managers, or drug wholesalers. She explains how vertical integration by companies like UnitedHealth, CVS/Aetna, and Cigna allows them to game medical loss ratio requirements through self-dealing while driving up costs. A European clinical trial implanted 2mm x 2mm light sensors beneath the retinas of 38 people with advanced macular degeneration, with 80% gaining clinically meaningful improvement (two lines on the vision chart) after one year. The device bypasses damaged rods and cones, sending camera images from glasses directly to the optic nerve. Dr. Dawn explains that air temperature warnings are measured in shade, but direct sunlight can add 20°C to heat exposure. Heat stroke triggers gut permeability, releasing lipopolysaccharides that cause cytokine storms and organ failure. She advises fans over air conditioning when possible, shade, hydration, and loose natural-fabric clothing. An emailer asks if low-dose oral strontium supplementation has the same problem as pharmaceutical strontium. Dr. Dawn confirms it improves bone density scores without reducing fracture risk, and recommends telopeptide testing to monitor actual bone loss after discontinuing. An emailer's doctor wants to prescribe high-dose dexamethasone for low platelets. Dr. Dawn advises against rushing to steroids since platelets of 40 are adequate for clotting, recommending a hematology consultation and repeat testing with citrated blood. Dr. Dawn reviews fiber types: wheat dextrin (Benefiber) is fermentable but technically gluten-free; guar fiber (Sunfiber) ferments slowly and works for low-FODMAP diets; inulin feeds bifidobacteria and produces anti-inflammatory short-chain fatty acids; methylcellulose (Citrucel) is non-fermentable; and psyllium (Metamucil) is facing a class action lawsuit over undisclosed lead contamination. An emailer with varicose veins reports recurring superficial blood clots. Dr. Dawn explains these don't travel to lungs like deep vein clots, but repeated clotting suggests possible thrombophilia requiring workup. She recommends consulting a vascular surgeon about superficial venous ligation under local anesthesia. Analysis of 25,000 wearable users found that three daily "exercise snacks" of just 1-2 minutes of vigorous activity (stairs, running for a bus) reduced all-cause mortality by 38-40%. Benefits plateau around 7,500 steps daily, and simply standing up every couple of hours dramatically reduces sedentary risks.

Visit your primary care provider for additional medical advice. Definition:Heat stroke represents the most severe form of heat-related illness, characterized by a core body temperature exceeding 40°C (104°F) accompanied by central nervous system (CNS) dysfunction. Arreaza: Key element is the body temperature and altered mental status. Jacob: This life-threatening condition arises from the body's failure to dissipate heat effectively, often in the context of excessive environmental heat load or strenuous physical activity. Arreaza: You mentioned, it is a spectrum. What is the difference between heat exhaustion and heat stroke? Jacob: Unlike milder heat illnesses such as heat exhaustion, heat stroke involves multisystem organ dysfunction driven by direct thermal injury, systemic inflammation, and cytokine release. You can think of it as the body's thermostat breaking under extreme stress — leading to rapid, cascading failures if not addressed immediately. Arreaza: Tell us what you found out about the pathophysiology of heat stroke?Jacob: Pathophysiology: Under normal conditions, the body keeps its core temperature tightly controlled through sweating, vasodilation of skin blood vessels, and behavioral responses like seeking shade or drinking water. But in extreme heat or prolonged exertion, those mechanisms get overwhelmed.Once core temperature rises above about 40°C (104°F), the hypothalamus—the brain's thermostat—can't keep up. The body shifts from controlled thermoregulation to uncontrolled, passive heating. Heat stroke isn't just someone getting too hot—it's a full-blown failure of the body's heat-regulating system. Arreaza: So, it's interesting. the cell functions get affected at this point, several dangerous processes start happening at the same time.Jacob: Yes: Cellular Heat InjuryHigh temperatures disrupt proteins, enzymes, and cell membranes. Mitochondria start to fail, ATP production drops, and cells become leaky. This leads to direct tissue injury in vital organs like the brain, liver, kidneys, and heart.Arreaza: Yikes. Cytokines play a big role in the pathophysiology of heat stroke too. Jacob: Systemic Inflammatory ResponseHeat damages the gut barrier, allowing endotoxins to enter the bloodstream. This triggers a massive cytokine release—similar to sepsis. The result is widespread inflammation, endothelial injury, and microvascular collapse.Arreaza: What other systems are affected?Coagulation AbnormalitiesEndothelial damage activates the clotting cascade. Patients may develop a DIC-like picture: microthrombi forming in some areas while clotting factors get consumed in others. This contributes to organ dysfunction and bleeding.Circulatory CollapseAs the body shunts blood to the skin for cooling, perfusion to vital organs drops. Combine that with dehydration from sweating and fluid loss, and you get hypotension, decreased cardiac output, and worsening ischemia.Arreaza: And one of the key features is neurologic dysfunction.Jacob: Neurologic DysfunctionThe brain is extremely sensitive to heat. Encephalopathy, confusion, seizures, and coma occur because neurons malfunction at high temperatures. This is why altered mental status is the hallmark of true heat stroke.Arreaza: Cell injury, inflammation, coagulopathy, circulatory collapse and neurologic dysfunction. Jacob: Ultimately, heat stroke is a multisystem catastrophic event—a combination of thermal injury, inflammatory storm, coagulopathy, and circulatory collapse. Without rapid cooling and aggressive supportive care, these processes spiral into irreversible organ failure.Background and Types:Arreaza: Heat stroke is part of a spectrum of heat-related disorders—it is a true medical emergency. Mortality rate reaches 30%, even with optimal treatment. This mortality correlates directly with the duration of core hyperthermia. I'm reminded of the first time I heard about heat stroke in a baby who was left inside a car in the summer 2005. Jacob: There are two primary types: -nonexertional (classic) heat stroke, which develops insidiously over days and predominantly affects vulnerable populations like children, the elderly, and those with chronic illnesses during heat waves; -exertional heat stroke, which strikes rapidly in young, otherwise healthy individuals, often during intense exercise in hot, humid conditions. Arreaza: In our community, farm workers are especially at risk of heat stroke, but any person living in the Central Valley is basically at risk.Jacob: Risk factors amplify vulnerability across both types, including dehydration, cardiovascular disease, medications that impair sweating (e.g., anticholinergics), and acclimatization deficits. Notably, anhidrosis (lack of sweating) is common but not required for diagnosis. Hot, dry skin can signal the shift from heat exhaustion to stroke. Arreaza: What other conditions look like heat stroke?Differential Diagnosis:Jacob: Presenting with altered mental status and hyperthermia, heat stroke demands a broad differential to avoid missing mimics. -Environmental: heat exhaustion, syncope, or cramps. -Infectious etiologies like sepsis or meningitis must be ruled out. -Endocrine emergencies such as thyroid storm, pheochromocytoma, or diabetic ketoacidosis (DKA) can overlap. -Neurologic insults include cerebrovascular accident (CVA), hypothalamic lesions (bleeding or infarct), or status epilepticus. -Toxicologic culprits are plentiful—sympathomimetic or anticholinergic toxidromes, salicylate poisoning, serotonin syndrome, malignant hyperthermia, neuroleptic malignant syndrome (NMS), or even alcohol/benzodiazepine withdrawal. When it comes to differentials, it is always best to cast a wide net and think about what we could be missing if this is not heat stroke. Arreaza: Let's say we have a patient with hyperthermia and we have to assess him in the ER. What should we do to diagnose it?Jacob: Workup:Diagnosis is primarily clinical, hinging on documented hyperthermia (>40°C) plus CNS changes (e.g., confusion, delirium, seizures, coma) in a hot environment. Arreaza: No single lab confirms it, but targeted testing allows us to detect complications and rule out alternative diagnosis. Jacob: -Start with ECG to assess for dysrhythmias or ischemic changes (sinus tachycardia is classic; ST depressions or T-wave inversions may hint at myocardial strain). -Labs include complete blood count (CBC), comprehensive metabolic panel (electrolytes, renal function, liver enzymes), glucose, arterial blood gas, lactate (elevated in shock), coagulation studies (for disseminated intravascular coagulation, or DIC), creatine kinase (CK) and myoglobin (for rhabdomyolysis), and urinalysis. Toxicology screen if history suggests. Arreaza: I can imagine doing all this while trying to cool down the patient. What about imaging?-Imaging: chest X-ray for pulmonary issues, non-contrast head CT if neurologic concerns suggest edema or bleed (consider lumbar puncture if infection suspected). It is important to note that continuous core temperature monitoring—via rectal, esophageal, or bladder probe—is essential, not just peripheral skin checks. Arreaza: TreatmentManagement:Time is tissue here—initiate cooling en route, if possible, as delays skyrocket morbidity. ABCs first: secure airway (intubate if needed, favoring rocuronium over succinylcholine to avoid hyperkalemia risk), support breathing, and stabilize circulation. -Remove the patient from the heat source, strip clothing, and launch aggressive cooling to target 38-39°C (102-102°F) before halting to prevent rebound hypothermia. -For exertional cases, ice-water immersion reigns supreme—it's the fastest method, with immersion in cold water resulting in near-100% survival if started within 30 minutes. -Nonexertional benefits from evaporative cooling: mist with tepid water (15-25°C) plus fans for convective airflow. -Adjuncts include ice packs to neck, axillae, and groin; -room-temperature IV fluids (avoid cold initially to prevent shivering); -refractory cases, invasive options like peritoneal lavage, endovascular cooling catheters, or even ECMO. -Fluid resuscitation with lactated Ringer's or normal saline (250-500 mL boluses) protects kidneys and counters rhabdomyolysis—aim for urine output of 2-3 mL/kg/hour. Arreaza: What about medications?Jacob: Benzodiazepines (e.g., lorazepam) control agitation, seizures, or shivering; propofol or fentanyl if intubated. Avoid antipyretics like acetaminophen. For intubation, etomidate or ketamine as induction agents. Hypotension often resolves with cooling and fluids; if not, use dopamine or dobutamine over norepinephrine to avoid vasoconstriction. Jacob: What IV fluid is recommended/best for patients with heat stroke?Both lactated Ringer's solution and normal saline are recommended as initial IV fluids for rehydration, but balanced crystalloids such as LR are increasingly favored due to their lower risk of hyperchloremic metabolic acidosis and AKI. However, direct evidence comparing the two specifically in the setting of heat stroke is limited. Arreaza: Are cold IV fluids better/preferred over room temperature fluids?Cold IV fluids are recommended as an adjunctive therapy to help lower core temperature in heat stroke, but they should not delay or replace primary cooling methods such as cold-water immersion. Cold IV fluids can decrease core temperature more rapidly than room temperature fluids. For example, 30mL/kg bolus of chilled isotonic fluids at 4 degrees Celsius over 30 minutes can decrease core temperature by about 1 degree Celsius, compared to 0.5 degree Celsius with room temperature fluids. Arreaza: Getting cold IV sounds uncomfortable but necessary for those patients.
