Now, if you haven't yet listened to the first part of this episode, that's PDP156. Jaz's Introduction:You should probably start there first because Dr. Devang Patel, our esteemed guest, he talks us through from the very beginning, like how do you assess the site that might be suitable for an implant and what to actually do if someone else's placed implant is coming to you for the restoration. And let me tell you, I learned so much. The episode is called an Idiot's Guide. I'm the idiot, right? So I was learning so much as getting along. He taught us about internal hex, external hex, conical, or butt joints, all these things I was learning about implants. Then we talk about impressions and scanning. And now we're going to be talking about what happens when the lab work comes back from the lab and you're going to assess it. You want to take a radiograph. You're going to actually screw the screw retain crown in. But what are you checking for? Do you have to give local anesthetic, for example? How much talk do you need to give at that point? How do you then restore the screw access hole? And what should be the follow up protocol going forward, as well as the all important troubleshooting? It's really important when you learn a new skill, that you learn about the troubleshooting. So any of the common complications that you can get ahead of it. Protrusive Dental Pearl: How I communicate a high RCT risk: For ordinary patients, I would always say “YOUR tooth decay (in YOUR tooth)/ YOUR filling was very deep.” But for some patients who you feel would make trouble, I would continue to add “...if we do nothing, then your tooth will eventually be in a worse situation. And you may lose your tooth. This could be a painful process as well. If I do something, then that involves drilling your tooth decay, drilling the soft bits of your tooth away and drilling away the old filling that's leaking. Drilling is not a nice thing. So by drilling, the drill is damaging your nerve. Your nerve may die and need a Root Canal." And today's episode is part of a three series. So this first part is about full mouth rehabilitation, adhesive dentistry, so an adhesive rehab, ie using some sort of composite resin, either direct or indirect, and how to transfer that from the wax up to the mouth. So we're gonna break it up into three different episodes with Dr. Dev Patel, who came on before, and he talked about private dentistry versus public dentistry and how we can be comprehensive in public dentistry. So in today's episode, we're looking at the mindset of the clinician who's doing full mouth rehab, because we can't just dive in talking about how to do these beautiful wax ups, and how to do all the big composites and indirect work as well, which we mentioned, that comes in episode three, actually. But we need to first start off on the right foot. And we need to know why you want to get involved with full mouth dentistry. What the mindset of the clinician is, how to diagnose, how to communicate, what to say, and how to say it at your treatment plan presentation appointment, to be able to then lead you with the correct patient, with the correct diagnosis to treat that patient in a full mouth manner. This series is titled 11 steps have a decent format rehabs and so really, this episode looks at appointment zero, ie your mindset, you as a dentist, and appointment one. And also when you bring the patient back and you speak to them. So those are the first couple appointments. The second part of the series coming probably in about four weeks time when we record is going to be appointments two to five, this is when you actually do, your mounting, your wax up, your transferring of the wax up to the mouth and you're assessing the phonetics and the aesthetics and you send them home to test drive. And then the final part getting to there are really meaty bits. Okay, but I think it's good for you to listen to these foundational parts one and two, before you come to that one, which will be around about six or seven weeks.

Infectious Diseases expert Devang Patel,MD joins Matt DeLaney, MD and Neda Frayha, MD for a conversation on CAP in general and the new guidelines in specific. Pearls: The latest guidelines for community acquired pneumonia now includes amoxicillin or doxycycline for 5-7 days as first-line treatment given the rising rates of macrolide resistance and less emphasis on coverage of atypical pneumonia pathogens.   Review of pathophysiology: Lower respiratory tract often preceded by an upper respiratory tract infection, that inhibits ability to clear mucus and pathogens invade the lungs Other risk factors: Smoking Elderly Immune compromise (ie: infection, steroids, cancer) Pathogens: Typical - strep pneumo, haemophilus, staph aureus Atypical (more common) - influenza, parainfluenza, mycoplasma, chlamydia pneumoniae, legionella, coccidioidomycosis (in the southwest) EPIC Study (2015) - study to determine pneumonia pathogens using all the tools we have available (culture, PCR) 62% no pathogen detected 22% viral - most were rhinovirus which does not cause lower respiratory tract infections but predisposes to pneumonia Strep pneumonia was the number one bacterial pathogen Bottomline: we still don’t know what causes most pneumonias but just that our patients get better with antibiotics Differentiating between typical v. atypical pneumonias - there’s no good way to know viral versus bacterial → default is to treat as bacterial pneumonia with antibiotics Diagnosis: Clinical features (cough, fever, sputum production, pleuritic chest pain, crackles) Guidelines recommend a chest x-ray For outpatient uncomplicated pneumonia, don’t get blood or sputum cultures For severe cases (those with risk factors for multidrug resistance, MRSA, or pseudomonas) you still want to get blood and sputum cultures Pearls: No more healthcare-associated pneumonia Emphasis on CURB-65 to assess severity of who does NOT need to be admitted Procalcitonin is NOT endorsed as a way to determine who gets antibiotics and who doesn’t Treatment: Increasing strep pneumo resistance to macrolides so no more monotherapy with macrolide (azithromycin) unless resistance is less than 20% in the area First-line in non-hospitalized adult is amoxicillin or doxycycline for 5-7 days Steroids recommended not use but may be considered in septic shock Commentary from Dr. Patel (ID specialist): Not a major change in practice other than to consider not covering atypicals in an otherwise healthy person   REFERENCES: Metlay JP, Waterer GW, Long AC, et al on behalf of the American Thoracic Society and Infectious Diseases Society of America.
