So I guess, make sure when you're charging to factor in that there's a degree of fudge factor. And by adding in this fudge factor, you're not going to have anxiety about doing the remakes, about the financial aspect of it as well. So the lesson there is, even if you're doing like a review appointment, make sure you charge for a review appointment. Because if it's your opinion, and your expertise that the patient's paying for, and the overheads of the building, that is really important to consider. You guys might know this already if you've been a listener for a long time. We moved to the UK, to Singapore, to practice dentistry, but also travel and stuff, which was, which is great. And dentistry in Singapore was a nice chapter that we look back at when we reflect on our time there. It was a beautiful time. And of course, the very first episode of Protrusive was all about our time in Singapore. But while we were there, Sim, my wife is a practicing dentist and me and her, we worked. In the same corporate, so shout out to Q&M and the kind of working hours we had there was strange. It's like nine to nine. That's right. It was 9:00 AM to 9:00 PM. But in some countries this, like in the Western countries, like to have that kind of a working schedule sounds absolutely insane, but hear me out, right? Because we didn't have kids and the pace of work wasn't crazy. Like I'd see a patient. I'd have a coffee, I'd see a patient, I'd have some noodles. I'd see a patient, I'd have some dimsum, couple hours of a break kind of thing. Have a siesta, it was a chilled way of working.

And the more seasoned clinicians who are into TMD, I think this one, you're going to be really, really finding this one very stimulating in terms of the different facets the conversation goes in and both the beauty and the frustration of having so many different treatment philosophies.

Click below for full episode transcript: Jaz's Introduction: Have you ever got a crown or an onlay back from the lab and tried it in the mouth and thought, whoa, the occlusion is just way off. Jaz's Introduction:Either it's in infra occlusion, it's not even in the bite at all, or it's in supra occlusion. It's going to need a lot of time to adjust to get it happy, both in the static, the tap, tap, tap, and in the dynamic. i.e. the lateral excursions. And sometimes it's a painstaking process where we try and get this crown perfect and happy in the patient's bite and then the patient's perception comes into it and there's other protocols that we talk about in other YouTube videos and episodes that I've done about checking the occlusion. But this episode is about how can we prevent this scenario from happening. Well, you have to first think about when does this scenario often happen? Well, it's more likely to happen in funky occlusions. And guess what? Loads of our patients have funky occlusions. That's AOBs, cross bites, class 2 division 1s, and super, super deep bites, right? These are classified as funky occlusions, but they're actually very common in the real world. And it's because these occlusions are not your standard class one, that the articulators that we have, whether analog or digital, they struggle to recreate the patient's jaw movement. So this is why the problem exists, and it's more often in these funny occlusions. Well, Protruserati, my name is Jaz Gulati, and in this episode, we're going to talk about the Functionally Generated Path Technique, whereby we can use this little simple trick to get our indirect work happy in the occlusion even in the funkiest of occlusions. You think it looks a little bit worse for wear, so you plan an indirect restoration, let's say an onlay. Jaz's Introduction:And everything goes smoothly, you did it under rubber dam, and it looks beautiful, and he followed all the adhesive principles, but unfortunately this asymptomatic tooth became very symptomatic and now you're looking at a pulpitis and the patient needs a root canal. Now sometimes the patient is upset. Does this sound familiar? Well if this doesn't sound familiar then you probably haven't done enough dentistry because this is something that we face. It's a real world problem. It's no one's fault that this happened. It's just biology. But attached to this scenario are emotions and feelings of the patient which can sometimes be quite bitter. So today we tackle the clinical part of the scenario in terms of what do we do, how do we prevent this from happening, what kind of communication should be happening with the patient to preempt this kind of scenario, and medically, legally, how can we protect ourselves. So in a similar vein, I have so many examples of rubber dam isolation, different teeth, different tricky scenarios, the coronoid process in the way, the clamp doesn't fit or whatever, various different scenarios, different challenges when it comes to rubber dam isolation. I've created the Rubber Dam Bank. So this is, as voted by you on our telegram group and on Facebook, an uncut version. Click below for full episode transcript: Jaz's Introduction: When I qualified from dental school, I was really into restorative, but the part of restorative I was most into was actually endodontics. In fact, I actually did quite a lot of root canals when I was a dental student. Jaz's Introduction:And the funny story is that because of orthodontics, I actually lost vitality of my four lower incisors.I had root canals from specialists for all my lower four incisors. So I actually experienced root canal treatment as a patient while I was a dental student. And also having really inspirational tutor and someone called Dr. Stephen Godfrey at dental school meant that I was actually really looking at these postgraduate options for specializing in endodontics as one of my serious career considerations. I went on to win the Tom Pitt-Ford prize, which is given to like one student per dental school every year. And I also won like this runner up prize in the Julian Webber Harley Street prize. And I was basically about submitting a case. And I did an MTA case back when I was a DCT in Sheffield. That was pretty cool. Using a scope, learning how to do simple retreatments. So I was on the path to really consider endodontics. Now fast forward 10 years. And my favorite type of endodontics is the referral pad. Okay, I'm joking but I'm kind of not joking because it's not my favorite thing to do anymore. I do TMD, I do restorative, I do occlusal stuff, tooth wear, and I get a lot of joy from that. Root canals is low down in my pecking order and one of the reasons I'm so keen to refer to my specialist colleagues is how long it takes me and I'm thinking if this tooth is going to take two hours with me and it's going to take one hour and 15 minutes with the specialist and they'll probably do a better job than I will. Then it just makes sense to see a specialist, right? If the patient can afford it, great. And also, in terms of an hourly rate, like GDPs doing endodontics that are particularly tricky and pushing boundaries and spending longer on it, it just doesn't make sense as a financial point of view. Because if your patient is paying for your time, then they may end up paying you more than the endodontist. And that just is bizarre. That should never happen, right? So I'm more than happy to rely on my endodontic colleagues. The way that this ties in to the superstar guest that we have today, Dr. Omar Ikram, who I'm sure many of you know of. He's very prolific on social media. He puts out such good stuff to the world in terms of being helpful in our endodontic treatments. He's very giving with his time and knowledge. So it's a great pleasure to have on Dr. Omar Ikram today. And I was thinking, what could I ask him? There's so many different topics that we could ask him. There's some topics I've already covered on the show when it comes to endodontics. And I thought it'd be a really good one about how to make root canals more efficient.

Now, he was recommended to me by Rupert Monkhouse, who we all know and love from the previous episodes he's done. [Jaz]The man behind Impression Club, of course. So just a little hat tip to Rupert for recommending Dr. Andreas, because according to Rupert, Andreas taught him everything he knows. And I was blown away by the knowledge, but also some of the gems that he gave that are really going to save us time. Like, I was really debating what to title this episode because it had so much breadth and actually when I was recording with him, I was supposed to, by the script, asking so many more questions about different types of dentures and partial dentures and overlay dentures. But we just had such a great time geeking out on complete dentures that the whole focus of this episode came about complete dentures, but in a way that no matter which experience level you're at, you're going to take something away. For example, we discuss everything from checking the border extensions to the vertical dimension to discussing the base materials that are available. And simple things like ensuring a comfortable fit at the day of delivery. The biggest takeaway for me, as someone who's really into occlusion, is the Protrusive Dental Pearl for today. So, hello Protruserati, I'm Jaz Gulati, in every episode I'll give you a Protrusive Dental Pearl, and today's pearl is from the main episode. And it's kind of surprising, like, I spend a lot of time when I'm doing complete dentures, which nowadays is not as many as I used to, just because the population that you treat and the different demographics, etc. So I used to spend a bit of time with balanced articulation, making sure I follow the BULL rule, adjust the buccal of the uppers and lingual of the lowers to get a lingualized occlusion and occluding scheme. Protrusive Dental PearlBut the pearl I have for you is that actually, Andreas just goes for a group function occlusion on his dentures, and you'll see why he does that. If you're spending too much time on the occlusion side, then maybe you are wasting your time. And there are other aspects of complete dentures where we should be focusing our time to get better outcomes for our patients. Oh, and just as Andreas says later in this episode, enter bolus, exit balance. Because ultimately when there is a bolus between the complete denture set, then all those dots and lines you work on are kind of irrelevant. I've got my rubber dam isolation and I just enter this flow state. It really is a fun procedure for me because I feel that every prep and something I'll guess today, Dr. Ashley Chung, aka @dr.ashlifts, also says later in this episode that she feels like no two of her preps ever look the same. Jaz's Introduction:And there is a beauty in that. You really have to think before you prep. It's not like making a one millimeter shoulder all day round. There's decisions to make which cusps are in compression, which cusps are in tension, which cusps can you preserve? And that's why all of our onlay preps look different for different teeth, for different scenarios. And it's a big part of why I've been doing this for about six years now. And I just love it. I still love it. I still tell my nurse, Zoe. We have an onlay prep today. I'm going to be in the zone. We'll put on some classical music. I'll have rubber dam on and I just enjoy it. It's usually either classical music or like rock music. So those are my two-treatment music. I used to do hip hop, but then the swear words and curse words, I didn't think that was a good vibe in my patients. So think like, Red Hot Chili Peppers, Foo Fighters, that kind of stuff. Always classic FM, right? It's one of the two. Now in this episode with Ash, who by the way, is just the most humble dentist ever. She's so sweet, she's so humble, she's so caring, so giving. All those good things that we need in dentistry, right? There's too much fake stuff in dentistry. There's too much showing off. There's too much, look at me, I'm awesome. I've got millions of followers and I've got a Ferrari and this kind of watch. Where she is just as far away from that as you can go. And this is why she's attracted such a large audience on Instagram, and I think she deserves all the followers and all the applause that goes with it for all her work that she does. So we need more people like Ash in dentistry, so it was great to have her on the podcast. We hear from Emma Yates, who is a nurse turned into a treatment coordinator. And a treatment coordinator, if you haven't heard of this term before, is someone who is kind of in between the reception team and the dentist, right? They've got enough clinical knowledge to discuss the pros and cons of different treatment and help the patient in their journey to find out what's the best treatment for them and also help them to pay for their treatment, how to finance it and answer those niggling queries they sometimes have and sometimes patients have to wait some time to hear from the dentist. But that treatment coordinator is a friendly phrase, right? They're not the scary dentist. They're a friendly face. They're an advocate for the patient. Today's Protrusive Pearl relates to cramp operations, and in particular, getting better impressions or scans. Look, once you've placed the retraction cord, and I know many people like to use things like Expasyl or Traxodent, which are like the pace systems. I quite like using retraction chords and also in combination with PTFE tape, especially as I do lots of vertical preparations or VertiPreps. Now I have got lots of webinars planned in October. As part of the live series I'm doing for the Protruserati, the premium subscribers, it's going to be VertiPrep for Plonkers. So that's coming soon. And one of the strategies I use to be able to scan subgingivally is once I've got my triple zero cord in place, I will put some PTFE tape over that. But sometimes what can happen is that the gingiva it sulks, right? It sulks on to the PTFE and sometimes even contacts the prep. So when I scan it because the tissues because the gums are touching the preparation it creates a nightmare scenario for my technician who wants that gingiva well out of the way So at that point the pearl the tip i'm giving you is if you've got a laser, fine. Great. Use a laser. If you've got any fancy burrs, use them. But the cheapest thing you can buy is something called a Thermocut Bur.

So, with an insurance pool, we put all the good dentists, so to speak, in one pool. And let's call them the distressed dentists, paying 30, 40, 50, 80, 100 grand in another pool. So, there's different risk factors and different prices. So, if something happens in the other pool, it's not having a knock-on effect on yours, so your premium doesn't go up. Whereas with a mutual everyone's in the same pool. So that's why you can be saying, I haven't had a complaint. Why is my indemnity going up, insurance going up every year? Now, obviously some of that's expenses, cost of living, but it's disproportionately high, but it's because you're paying for other people's errors and emissions. Jaz's Introduction:Indemnity vs Insurance. I don't know if it's like a UK thing. I'm kind of sure it is. Although when I was in Singapore, I was with dental protection. So let's give you a bit of background first, right? Indemnity is a bit like the big organizations, mutuals like DDU and dental protection, whereas Insurance are kind of like a newcomer in the last 10 years. There's these different insurance products. So what's the need for these? Well, when I was in Singapore, when I moved from the UK to Singapore, I was paying so much lower to be part of dental protection at the time, like, compared to UK fees, Singapore, it was so much easier to get protected as a clinician. This is because in Singapore at the time, dentists were getting sued way less than they are in the UK, which is nice, and also taxes are lower, which is really cool. Anyway, came back to the UK, and now that I've been paying way less in Singapore, to have to pay what I was getting quoted for with DDU at the time, or Dental Protection, or whoever, It was a bitter pill to swallow. So already I was thinking surely there must be a better way. Why am I a non-implant dentist who hadn't had any complaints at that point? Why am I paying so much so that it covers the high-risk dentist, right? Cause that's the way it works. These mutuals, they work because they've divided all the costs between all the members. And this is why your premium goes up year by year by year, even though you didn't really use their services. That's when insurances came my radar and my friend, Dr. Neel Jaiswal, who's a fantastic conscientious clinician. He started PDI, which is Professional Dental Indemnity. So, I had a chat with him, I went to their evening, I met the underwriters, the lawyers there, and I was very impressed with what he was setting up. Now, if you haven't yet listened to the first part of this episode, that's PDP156. Jaz's Introduction:You should probably start there first because Dr. Devang Patel, our esteemed guest, he talks us through from the very beginning, like how do you assess the site that might be suitable for an implant and what to actually do if someone else's placed implant is coming to you for the restoration. And let me tell you, I learned so much. The episode is called an Idiot's Guide. I'm the idiot, right? So I was learning so much as getting along. He taught us about internal hex, external hex, conical, or butt joints, all these things I was learning about implants. Then we talk about impressions and scanning. And now we're going to be talking about what happens when the lab work comes back from the lab and you're going to assess it. You want to take a radiograph. You're going to actually screw the screw retain crown in. But what are you checking for? Do you have to give local anesthetic, for example? How much talk do you need to give at that point? How do you then restore the screw access hole? And what should be the follow up protocol going forward, as well as the all important troubleshooting? It's really important when you learn a new skill, that you learn about the troubleshooting. So any of the common complications that you can get ahead of it. It is not a replacement for a tooth, because teeth are still the best, but the next best thing for a missing space at least, are IMPLANTS. Now, implants can get very complicated, different stages, surgical, restorative, but the very initial stages is all to do with ossteointegration, which is essentially, in a crude way to describe it, would be the fusing of the implant to a patient's bone. Jaz's Introduction:Or as I like to call it, how long it takes for the implant to cook, so it's ready to accept load. Today on the show, I've got Dr. Pav Khaira on again, he's our resident implant expert and a titanium nerd, and he's going to cover four techniques he uses, and most of these are very quick to act on, very simple, and something that you can apply straight away. One of them does involve you to buy some extra kit, but the reasoning is really solid, and it's something I hadn't heard of before, so it's four ways to BOSST your OSSEOINTEGRATION. That's the success rate overall, and the quality of your osseointegration itself. Like Pav taught me in this episode that most implants, when they're placed, about 56% of the implant is covered by bone, or rather 56% of the implant is actually directly contacting bone. And with his techniques, he's getting that much higher, which he believes results in a longer lasting, more successful implant. So he calls this all super osseointegration.
