Before the Change Healthcare cyberattack, hospitals were already grappling with insurers' tactics of delaying and denying payments for patient care. The cyberattack only amplified the challenges providers face—not just in delivering care, but also in getting reimbursed for that care. Despite the crisis, insurers continued to use these tactics. Now, six months later, Matt Szaflarski, a director and revenue cycle intelligence leader at Kodiak Solutions, and his team have uncovered something alarming: a surge in insurers' initial Request for Information (RFI) claim denials.Kodiak's latest report, “Death By A Thousand Requests,” highlights the growing trend of payors denying initial claims due to RFIs, creating an enormous administrative burden on hospitals and providers. In 2024 alone, these tactics are projected to cost hospitals $4.6 billion. Szaflarski returns to the show to explain the impact of these denials on the hospital revenue cycle, which ultimately impacts the hospital's ability to provide care.In this episode, Szaflarski discusses:Updates on the Change Healthcare cyberattack;Rising trends in claim delays and denials, particularly RFI denials;Revenue cycle data insights;Medicare Advantage, the two-midnight rule, and observation stay challenges; andRecommendations for improving processes between insurers and providers.

Today's video is the last in The Beyond Politics Video Series on The Unlearning Playground by Chetan Narang & Rocco Jarman. Well, last in the first iteration (we hope!). This is my first standalone video in this series, and what I talk about is a summary of the entire series, key takeaways everyone can have from it and practical advices for everyone listening to actually make an impact in our society & in our shared future together. When Rocco & I brainstormed the idea of Beyond Politics, we could see that in the current situation of the global political atmosphere, in the USA or in India or in various other societies throughout the world, polarisation is increasingly becoming the norm. The ideological divide among people is deepening and this growing rift is not only impacting political discourse but also hindering our ability to address pressing societal issues effectively - issues for which even if we already have the necessary tools already, our political framings block us from cooperating and actually solving them. -------------- The entire series is available via this playlist - https://www.youtube.com/playlist?list=PLGTAFgEsIneXcoBEzsYfc0wNB5H0Rssot You may also watch short clips from the series here in this playlist - https://www.youtube.com/playlist?list=PLGTAFgEsIneWwqiZ_y8JgNUshNx0Jh3q8 -------------- The key insights I share today (and what we discuss in the entire Beyond Politics Series) are applicable not just to the political climate in India or the United States of America, but is fundamental to the way we humans work and coordinate (or at least try to coordinate!) ==== Outline ==== (00:00) - Coming-Up Next (00:33) - A brief summary of Beyond Politics by Chetan Narang & Rocco Jarman (05:06) - A Personal Story (Polarisation in Our Everyday Life) (09:24) - Our Fears are the same (Recent Trends in Modern Politics) (11:40) - IF you want to be a part of the solution (17:09) - The Cone Example and Politics Today (20:37) - Key Point 1 - The Two Golden Rules (26:07) - Key Point 2 - Two Questions to Always Consider (27:50) - Key Point 3 - The Tim Urban Levels of Thinking Paradigm (29:37) - Key Point 4 - The Human Consciousness Pyramid (34:46) - Understanding the Current Human Condition & What lies AHEAD (37:47) - How to use Agile to Solve Politics (42:44) - Concluding Thoughts & Invitations -------------- The polarisation we see all around us is indeed a sad reality of modern life, and it's about time someone acts like the adult in the room and helps us get out of the mess we have created for ourselves - as a society and also as individuals. HYBRID POLICY SEMINAR Globalization of the Bioeconomy: Recent Trends and Drivers of Bioeconomy Programs and Policies Co-organized by IFPRI, International Advisory Council on Global Bioeconomy (IACGB) and CGIAR MAY 7, 2024 - 9:00 TO 10:30AM EDT The bioeconomy approach to sustainable development holds great promise in reducing dependence on fossil fuels, addressing climate change, and promoting resource-use efficiency, thereby stimulating economic growth, enabling innovation, and improving food security. The bioeconomy is the production, utilization, conservation, and regeneration of biological resources, including related knowledge, science, technology, and innovation, to provide sustainable solutions (information, products, processes, and services) within and across all economic sectors and enable a transformation to a sustainable economy. Multilateral organizations have intensified their engagement in, and for, the bioeconomy. Under India's lead in 2023, the G20 drew attention to the bioeconomy and, in 2024, Brazil put the bioeconomy prominently on the G20 agenda. The Food and Agriculture Organization of the UN (FAO) included bioeconomy in its most recent science strategy. At the same time, national bioeconomy strategies are emerging to shape multisectoral approaches to climate neutrality, food and nutrition security, improved health, economic growth, and other objectives aligned with the Sustainable Development Goals. In April 2024, the International Advisory Council on Global Bioeconomy (IACGB) released a new policy review in preparation for the Global Bioeconomy Summit in October 2024. The new report analyzes bioeconomy policy trends and their determinants, and highlights the growing importance of the bioeconomy as a key enabler and solution provider to global sustainability challenges across various sectors and dimensions of society. Importantly, the report identifies international and multilateral cooperation as a key building block. The report—and the growing body of research on the bioeconomy—emphasizes the opportunities to advance innovation and facilitate the rise of a bio-based industry and manufacturing, sustainable and regenerative agriculture, human health, and circular bio-based economies. This seminar will spotlight key findings from the IACBG report and explore the role of the bioeconomy in addressing food security, nutrition and diets, and poverty reduction in low- and middle-income countries. Please join us on May 7, 2024, at the International Food Policy Research Institute (in-person or online) for an exciting seminar on the globalization of the bioeconomy. Welcome Remarks Johan Swinnen, Managing Director, Systems Transformation, CGIAR; Director General, IFPRI Keynote Speaker Joachim von Braun, Distinguished Professor for Economic and Technological Change, Center for Development Research (ZEF), Bonn University Panel Discussion Julius Ecuru, Principal Scientist and Manager, Research Innovation Coordination Units, BioInnovate Africa Programme, International Centre of Insect Physiology and Ecology Ismahane Elouafi, Executive Managing Director, CGIAR (Video Remarks) Mary E. In this second episode of the series, they talk about mistake number two: Do you project recent trends indefinitely into the future?LEARNING: Hyper-diversify and rebalance your portfolio. “You cannot run away from risks; you can only choose which risk you're going to take. Hyper-diversify on as many different unique risks as you can, stay the cause, and rebalance.”Larry Swedroe In today's episode, Andrew continues discussing with Larry Swedroe, head of financial and economic research at Buckingham Wealth Partners. You can learn more about Larry's Worst Investment Ever story on Ep645: Beware of Idiosyncratic Risks.Larry deeply understands the world of academic research and investing, especially risk. Today Andrew and Larry discuss a chapter of Larry's book Investment Mistakes Even Smart Investors Make and How to Avoid Them. In this second series, they talk about mistake number two: Do you project recent trends indefinitely into the future?Missed out on mistake number one? Check it out: ISMS 8: Investment Mistake No.1: Are You Overconfident in Your Skills?Recency bias explainedAccording to Larry, most investors suffer from recency bias. Recency bias is that we tend to overweight whatever has happened in the most recent past, whether it's months or years, and ignore long-term evidence. Say you're watching a stock and go back to 1995 and notice that technology stocks in ‘96, ‘97, and ‘98 performed well. So you think the same performance will prevail, and now you buy tech stocks based on that recent trend. If you buy things that have done well in the last few years, and now you think it's safe, what you've done is bought high. You didn't get those great returns but paid high prices. High prices generally mean you'll get low expected returns.Larry reminds investors that knowing your history is the best way to overcome recency bias. History tells us that all risk assets, gold, real estate, US stocks, small stocks, value stocks, high-yield bonds, etc., go through very long periods of poor performance. That means you don't want to be subject to recency bias because you think three, five, or even ten years is a long time to judge performance. It's not; otherwise, there would be no risk for an investor with a 10-year horizon. So you just have to wait it out.An excellent example of that problem is when the S&P underperformed T bills for at least 13 years for three periods, from 1929 to 1943, from 1966 to 1982, and then again from 2000 to 2012. Of course, the stocks did great in the other half of that period, but you don't get those returns if you're subject to recency bias. The never-ending game of buying high and selling lowThe message that Larry tries to give investors is that there are no clear crystal balls. So don't be subject to recency bias because you'll forever chase and buy high and sell low. This is not a prescription for success. You cannot run away from risks; you can only choose which risk you'll take. And if you don't have a clear crystal ball, there's only one logical answer; you should hyper-diversify on as...

Show Interview with Ranah Esmaili What are some of the general examination practices you’re seeing right now? What are some of the key areas where you’re seeing this play out in examinations today? In the areas of cyber and information security, where do you see exam teams focusing their efforts? Is the SEC focusing on the issue of off channel communications during examinations and what are they looking at? How can firms enhance their electronic communications policies, procedures and practices in anticipation for an exam? What is the focus in examinations in the private fund space? What is the focus in examinations in the mutual fund space? Now that the compliance date for the new Marketing Rule has passed, what’s the SEC doing on the exam front? What are some things firms can be doing post-Compliance date to ensure good marketing rule hygiene? Tips for firms that looking to prepare for exams Quotes 10:07 - “We’re talking about environmental, social and government investing, so ESG. The SEC has been scrutinizing ESG investing out of a concern for green washing. And that term ‘green washing’ is basically the practice of making investments look more E, S, and G friendly than they really might be.”- Ranah Esmaili 13:00 - “Compliance failures alone can really carry serious consequences. So when the SEC is focused on ESG compliance, policies, and procedures, that’s really not just an academic exercise. It can carry really serious consequences.”- Ranah Esmaili 24:04 - “Firms can send out periodic compliance reminders, just so that the issue is always at the top of employees’ minds. They can conduct training. They can require employees to certify compliance with the firm’s policies and procedures. And they can conduct electronic surveillance, looking for indicators of unauthorized communications. To the extent that the firms are identifying employee violations of their [recordkeeping] policies, they should have a thoughtful approach for dealing with those violations, particularly where they find employees that have repeat violations.” - Ranah Esmaili
