Et tema som virkelig engasjerer blant våre medlemmer er spørsmålet om langvakter. Noen elsker det, mens andr ekan ikke utstå tanken på å være 12 timer i strekk på jobb. Derfor har vi i ukas podkast tatt en prat med to tillitsvalgte som har et ulikt syn på temaet. Kjør debatt!I ukas episode har vi snakket med Anniken Lien van Marion som leder yrkesseksjonen helse og sosial i Trondheim og Cecilie Almli som intill nylig var tilltisvalgt i Tysvær og Botn. Mens Annikken er skeptisk til langvakter, er Cecilie mer positiv, og i ukas episode får vi høre argumenter for og imot. Først snakker vi med Annikken før Cecilie kommer til orde.Det er ingen som tar til orde for å si nei til alle ønsker om langvakter, men det handler om i hvor stor grad det skal åpnes opp for nettopp dette på arbeidsplassene.Dette ble en episode der vi med innestemme og respekt snakker om et tema det er mange meninger omMen det de har til felles er at langvakter må være frivillig og at det ikke er alle det passer for. Na maanden van afwezigheid zijn Buitenhuis en Van Rijs weer terug! Wat was er aan de hand? Waarom hebben de luistervinken zo lang droog gestaan? Alles wordt duidelijk in deze langverwachte aflevering.Een nieuwe botbreuk, pijn, drugs, revalidatie. Kort samengevat is dat het (slappe) excuus van Van Rijs. Buitenhuis doet er niet voor onder qua gezondheidsproblemen want hij had last van een fikse mannengriep. Omdat er zoveel tijd is verstreken, nemen de Parlevinkers van de gelegenheid gebruik om elkaar (en de luistervinken) bij te praten over de afgelopen maanden. Van Rijs had een incidentje. Wat 's ochtends een taartje eten (verjaardag) had moeten worden, eindigde in een lange dag waarop hij iets te intiem werd met een glasbak. Na het opschepperige gedoe van Van Rijs over zijn sportschool resultaten, is Buitenhuis voor de 27.000e keer op zoek gegaan naar een sportschool. En met resultaat! Het werd een mooie kleinschalige, fysio-achtige locatie. Inmiddels is hij begonnen aan zijn winter body. Ook bezocht hij een comedyshow in Utrecht. Helaas kwam hij wel op een pijnlijk moment binnen. Tot grote verrassing van Van Rijs sluit Buitenhuis het verhaal af met een aankondiging waar hij spijt van gaat krijgen. Van Rijs heeft Buitenhuis in de vorige aflevering gedwongen tot het maken van een pijnlijke fout. Dankzij hem heeft hij nu iets goed te maken. Maar gelukkig gaat het al snel weer over leukere zaken. Buitenhuis is weer eens naar AJAX geweest én z'n ouders hadden een huwelijksjubileum, hoera! Van Rijs had weer een uitje: het was tijd voor een ledendag van ‘de patiëntenvereniging'. Buitenhuis kon er dit keer niet bij zijn, dus hij deelt met hem hoe deze dag eruit zag. Deze aflevering bellen de heren weer met Qusai, hij heeft een flinke reis en een groot avontuur meegemaakt. Waarbij hij natuurlijk ook weer onder de invloed van een nieuwe vrouw is geraakt én echte engelen met gloeiende oren heeft ontmoet. Van Rijs is weer eens (wel voor de laatste keer dit jaar hoor) op vakantie geweest, naar Valencia dit keer. De mooie stad die de Parlevinkers in den beginne (seizoen 1, aflevering 3) al eens samen bezocht hebben. Ook nu was het weer een mooie trip, alleen had ‘ie wel een klein foutje gemaakt. Buitenhuis onthult zijn nieuwe tatoeage en vertelt over de symboliek erachter. Helse pijnen heeft hij moeten doorstaan maar goed, dan heb je ook wat. Wat ook wel een beetje pijn deed was de trammelant die hij heeft gehad met zijn nieuwe handbike. Inmiddels zijn de problemen opgelost en fietst hij er weer lustig op los, maar hij moet er wel weer erg aan wennen. Het blijkt dat ie toch wat angstig en voorzichtig is geworden na de lange periode van niet fietsen. Van Rijs herkent zich hier wel in en trekt de conversatie meteen weer naar zichzelf toe. In Over smaak valt te twisten proeven de heren weer uit het broodtrommeltje van Buitenhuis. In eerste instantie wordt er opgelucht adem gehaald maar zoals wel vaker zit het venijn ‘m in de staart. Stop de show want Buitenhuis was ziek. Van Rijs moest het allemaal een plekje zien te geven maar gelukkig knallen beide heertjes deze week weer kerngezond je oortjes in! Er moeten wat dingetjes rechtgezet worden én Buitenhuis heeft een nieuwtje wat je hart zal doen verwarmen... Al een tijd is Van Rijs nu bezig met trainen bij zijn geliefde fysio, en inmiddels merkt hij nu toch duidelijk de resultaten van zijn noeste arbeid. En dus is een shoutout wel op zijn plek. Buitenhuis gaat nog altijd lekker op zijn werk en maakte dit jaar zijn eerste prinsjesdag als ambtenaar mee. Beleeft hij dit nu anders? En was hij er druk mee? Van Rijs is wezen kamperen. Althans, hij heeft z'n ouders van en naar de camping gereden. Twee vrije zaterdagen heeft hij (met alle liefde) als taxi gefungeerd en hij was verrast hoe leuk dat eigenlijk was. Buitenhuis heeft - zoals bekend - een nieuw huis en dat moet natuurlijk een slim huis worden. En dus werd Google Home geïnstalleerd; een belachelijk groot succes bij de kinderen. Sowieso begaf hij zich in de wondere wereld van AI, en dat gaf hem inspiratie voor een muzikale ode aan Van Rijs. In zijn oude hometown Zwolle heeft Van Rijs eens lekker de bloemetjes buiten gezet. In Het Urkerland van maandag 26 augustus vertelt hij daarover en Albert Brouwer en Jelle Bakker (die voor de krant met Tony ging kuieren) verbazen zich daarover in hun nieuwste podcast. Ook vragen de podcastmakers zich af in hoeverre er een taboe bestaat bij de Urkers om naar de serie ‘Urk!' te kijken… Verder zorgt de enorme toename van toeristen op Urk en met name de vele Belgische bezoekers voor verbazing. Nu van Rijs weer zo goed als volledig hersteld is, betekent dit dat hij weer steeds meer dingen kan ondernemen. En zo deed een klein familiebezoekje hem ineens bewust genieten van een autorit. Voor Buitenhuis was het daarentegen niet allemaal koek en ei. Zo had hij vanwege een dwarse beveiliger op zijn werk moeite om überhaupt het pand in te komen. En ook thuis waren er strubbelingen: hij bevond zich onverwacht tegenover een werk-ontwijkende bouwvakker, waarbij hij zichzelf ook ineens moest verdedigen tegen het denkbeeld dat het toch best wel zwaar is om voor ‘iemand zoals hem' te kiezen in het leven. Pearls and Quotes - Cardiomyopathies The presence of an S4 and a rapid y-descent in the jugular venous pulsation on exam should clue you to the presence of a restrictive filling pattern. Restrictive filling doesn't necessarily mean restrictive cardiomyopathy and is more commonly due to dilated or ischemic cardiomyopathy. The five main topics of counseling that every hypertrophic cardiomyopathy (HCM) patient should understand: (1) Prognosis, (2) Family Screening, (3) Risk of Sudden Death, (4) Treatments, and (5) Physical Activity. Remember 1/3: In clinical trials of cardiac myosin inhibitors for HCM (mavacamten), about a third of patients had a tremendous improvement in symptoms, another third had some improvement, and the final third had no improvement or had to discontinue the drug due to negative inotropy. When counseling patients about septal reduction therapy, consider the patient's age. For younger patients, surgical myectomy at an experienced center offers a higher success rate and greater durability with lower rates of pacemaker placement when compared to alcohol septal ablation. Historically, the conclusion that it was higher risk to be an athlete with HCM was unfortunately generalized to mean that it was high risk to exercise for patients with HCM. “And we turned a generation of HCM patients into HCM cardiometabolic syndrome patients, which is actually a worse combination.” Notes - Cardiomyopathies What is the initial approach to evaluating a patient with new or suspected cardiomyopathy, including hypertrophic cardiomyopathy (HCM)? A history and physical exam, including a thorough past medical and family history, is always the first step and helps determine the patient's risk for potential underlying etiologies, including genetic cardiomyopathies, hypertrophic cardiomyopathy, or those related to treatments of previous cancer. In terms of ECG findings, pay attention to QRS voltage (high or low) and the presence of any arrhythmias. TTE should be obtained in all patients and is often sufficient to diagnose many underlying cardiomyopathies, including HCM. Cardiac MRI (CMR) is helpful as an adjunct when TTE alone is inconclusive or imaging quality is poor. CMR can help provide a better idea of chamber sizes and wall thickness, and late gadolinium contrast enhancement (LGE) can also be helpful if present in a specific pattern, though often HCM patients may have non-specific patterns of LGE. Invasive hemodynamics assessment is reserved for patients with discordance between non-invasive testing and the clinical impression. Als we aan ‘kampen' in Nederland tijdens de oorlog denken, denken we in eerste instantie aan Westerbork, het doorgangskamp waar de Nederlands joden naartoe gebracht werden voordat ze werden afgevoerd naar de vernietigingskampen. Of aan Kamp Amersfoort, waar naast joden, ook verzetsmensen en Russische krijgsgevangenen zaten. Maar niet zo gauw aan Ommen en het daar gevestigde Kamp Erika, een kleiner kamp, dat in eerste instantie niet bedoeld was voor joden of verzetsmensen.  Over dat min of meer vergeten kamp is een boek verschenen, getiteld: Kamp Erika, het oorlogsverhaal van mijn opa en het Nederlandse kamp dat verzwegen werd, geschreven door Hester den Boer. De auteur is te gast.

Rinus de Rijke was van juli 1942 tot juni 1943 kampbewaarder in strafkamp Erika in Ommen. ‘De beul van Ommen', zoals de man later in de media werd genoemd, was zelf gevangene in het kamp, maar hij ‘klom op' en kwam aan het hoofd van een groepje medegevangenen te staan. In dienst van de nazi's zou hij in het kamp oorlogsmisdaden en misdrijven tegen de menselijkheid hebben gepleegd. Zeker twee, maar mogelijk drie of meer Joden zouden als gevolg van zijn handelen de dood zijn ingejaagd. Beschuldigingen die De Rijke ontkende. 44 jaar later werd hij bij de grensovergang Denekamp aangehouden toen hij vanuit Duitsland op weg was naar familie in Nederland, hij was toen 68.
