In this episode I chat to Stephanie Ernst, founder of the TAPS Support Foundation, about monochorionic twins and the complications they may experience. Monochorionic twins, often referred to as identical twins, can experience more complications compared to dichorionic twins (fraternal), and Stephanie talks us through the chances of these twins experiencing conditions such as TTTS, TAPS and fetal growth restriction. We discuss the ways these babies may be monitored for these conditions, the pathways of care if they're diagnosed, and the potential outcomes. Stephanie Ernst facilitates conversations between parents and professionals about the unique needs of complicated twins and multiple birth families and their challenges. Founder of TAPS Support, and experienced twin parent, her passion is breaking down medical jargon and ensuring that people have quality information about multiple births from before birth, to beyond the NICU. Send us a Text Message.Welcome to the fourth episode of season four of Conversations in Fetal Medicine, in conversation with Stephanie Ernst, founder of the charity TAPS Support. Stephanie's bio: Stephanie Ernst: Twin mum, TAPS nerd, expert patient and Australian in the Netherlands (self identified as a clogaroo). After delivering monochorionic twins at 31 weeks,, Stephanie struggled to find quality information about Twin Anemia Polycythemia Sequence (TAPS), the diagnosis she had received for her daughters. This led to the creation of a Facebook community, and later grew to the charity TAPS Support, Her aim is to help continue research into the complications of monochorionic twins, through actively raising funds for research projects, supporting researchers of the future, and talking with professionals about the perspective of parents with complicated twins pregnancies, and the additional considerations they may need. (And promises she's not out to start fights or arguments, she just has a lot to say). She also has a passion for breaking down medical jargon, and helping families understand their diagnosis through giving good quality research in everyday terms. Patients should be an active part of their own care team, and need the resources and support to do this. She actively publishes research with two other TTTS mothers, and writes and speaks regularly on twin-related topics, as well as many other subjects, and works in marketing, specifically for companies dealing with clinical research, training and coaching.. In her spare time, she hangs out with the coolest twins ever (her daughters) and her long suffering, very patient husband, and drinks a lot of coffee (and doesn't mind the occasional G&T!).Find a portfolio of her work here: https://stephanieernst.nl/portfolio/For more information on TAPS Support, visit https://www.tapssupport.com/Additional resources: ICOMBO Declaration of rights & statement of needs for twins and higher order multiples: https://icombo.org/wp-content/uploads/2024/01/Declaration-of-Rights-2022.pdf European Standards of Care for Newborn Health: https://newborn-health-standards.org/NICE Guidelines for Twin and Triple Pregnancy: https://www.nice.org.uk/guidance/ng137Podcast information:We have not included any patient identifiable information, and this podcast is intended for professional education rather than patient information (although welcome anyone interested in the field to listen). Gianina joined us on the pod to talk about the hardest day of her life. Her story involves an incredible journey of resiliency, support from loved ones, and working closely with a team of excellent doctors and nurses. Gianina went through one of the most complicated pregnancies we have ever heard and we are impressed with her bravery, optimism, and her desire to bring awareness to what happened to her and her babies. Gianina was diagnosed with two conditions. TTTS, twin-to-twin transfusion syndrome, is an unequal sharing of volume of blood through the babies' shared placenta causing an amniotic fluid difference. sIUGR, selective intrauterine growth restriction, a form of TTTS, is an unequal placental share causing growth restriction in one of the babies. We love what she wrote here: “Everyday is still an emotional battle to honor Gianna and to also be present and happy for my baby girl Liliana who I love so much. Looking into Liliana's eyes everyday and watching her grow is a daily reminder that I was to have another beautiful girl just like her and that she was in the womb with her identical best friend. When she looks into my eyes deeply I feel her sister's presence and feel as though her soul is inside of Liliana. She's our angel

This show is focused so far on the unexpected elements of the emotional or physical toll of pregnancy and birth. Today that story is extended to include the emotional and physical impact of a pregnancy that didn't find its footing and ended in abortion. Abortion is obviously a topic most people have strong feelings about. And unfortunately, those strong feelings tend to flatten the conversation, when in reality like all elements of reproduction, thinking and talking about abortion requires nuance. Some people may seek out first trimester abortion early in their pregnancy and feel like this choice. This procedure is like any other medical procedure. For other women. It comes with more personal or cultural or religious baggage and it's in no way easy for them to make that choice. In today's episode, my guest has a second trimester abortion and her pregnancy which many complications arose. And her experience of working through the grief of this loss is impactful to hear and very much a story that I will carry with me for a long time. For extended show notes, see: https://warstoriesfromthewomb.com/
Daisha Johnson is a mother to 8 wonderful children, however, she never forgets that she also has 5 “angel babies” in heaven. It was so incredibly hard for Daisha and her husband Cole to lose these babies during pregnancy. Although they had other successful pregnancies, their hearts were broken each time they experienced a loss. Daisha's water broke at 14 weeks during two separate pregnancies, one of their identical twins sons passed away due to TTTS at 16 weeks and they had two other early miscarriages. Even though they had the support of loved ones around them it was still a difficult and lonely journey of grief and then trying to find peace and hope again. Daisha so desperately wanted to find support in connecting with others who had gone through similar losses and circumstances in hopes to not feel so confused and alone. In today's podcast episode, Daisha shares her journey of navigating those difficult times and the thoughts that helped her through. Twin-to-twin transfusion syndrome (TTTS) is a rare prenatal condition affecting identical twins. If left untreated, the survival rate for TTTS twins hovers between 10-15 percent. The decision to treat Keeley and Kambry while in utero was a given, but it came with a plethora of risks. At 17-weeks' gestation, Jade Ewoldt, underwent surgery at Cincinnati Children's Hospital to separate and seal the twins' anomalous vessels. The most evident threat associated with this surgery is that it often causes premature birth. Pushing the boundaries of medicine, and earning a Guinness World Record in the process, the twins arrived on 24 November 2018, at just 22 weeks and one day. Hear what it took from the entire University of Iowa Stead Family Children's Hospital Neonatal Intensive Care Unit (NICU) staff to get the twins into this world and how their entire community has embraced them as members of Keeley & Kambry's Tribe. Today, the twins live typical lives of average 3-year-olds and development is ahead of schedule. Jade continues to share the twins' story in hopes of saving more babies and supporting families going through similar struggles. Listen in to hear this story of pushing the boundaries of what is humanly possible and changing the future of humanity for the better.

Birth defects affect 1 in every 33 babies born in the U.S. each year, according to the Centers for Disease Control and Prevention. Before a baby is born, early intervention using fetal surgery can treat life-threatening birth defects and improve outcomes in some cases. Fetal surgeons at Mayo Clinic Children's Center treat many conditions, including: Spina bifida (myelomeningocele). Twin anemia-polycythemia sequence (TAPS). Twin reversed arterial perfusion (TRAP) sequence. Twin-twin transfusion syndrome (TTTS). Lower urinary tract obstruction (LUTO). On the Mayo Clinic Q&A podcast, Dr. Angela Mattke, a Mayo Clinic pediatrician and host of Ask The Mayo Mom, is joined by Dr. Mauro Schenone, a Mayo Clinic maternal fetal surgeon, to discuss advances in technology and treatments.
