Leviticus 05.1-13 Bible Study - Sin Offering (Part 2)In this Bible study, we look at the 2nd half of the Sin Offering as outlined in Leviticus 5.1-13. The Israelites were required to perform a “sin offering” if they failed to do something that was required, such as failing to give a testimony, dealing properly with ritual uncleanness, or fulfilling an oath. We also look at the “sin offering” as the “purification offering” asking why a woman who just gave birth would be required to give a “sin offering?” A key verse in this passage is Leviticus 5.5 in which the text tells us that confession is key to the process. OUTLINE:01:45 - What is Iron Sheep Ministries? 07:04 - Outline of the talk: 5.1-6 Failure to do something that was required: - v1 - Failure to give testimony, which could prevent justice from being carried out. - v2-3 - Failure to deal properly with ritual uncleanness, which could lead to the Lord's tabernacle being defiled.- v4 - Failure to fulfill an oath, which would result in the Lord's name being profaned. In any of the above. Contributor: Jorge Chalit-Hernandez, OMS3 Educational Pearls: Acute mountain sickness (AMS) is the term given to what is otherwise colloquially known as altitude sickness High altitude cerebral edema (HACE) is a severe form of AMS marked by encephalopathic changes Symptoms begin at elevations as low as 6500 feet above sea level for people who ascend rapidly May develop more severe symptoms at higher altitudes The pathophysiology involves cerebral vasodilation Occurs in everyone ascending to high altitudes but is more pronounces in those that develop symptoms The reduced partial pressure of oxygen induces hypoxic vasodilation in the brain, which results in edema and, ultimately, HACE in some patients Symptomatic presentation Headache, nausea, and sleeping difficulties occur within 2-24 hours of arrival at altitude HACE may occur between 12-72 hours after AMS and presents with ataxia, confusion, irritability, and ultimately results in coma if left untreated Clinical presentation may be mistaken for simple exhaustion, so clinicians should maintain a high index of suspicion Notably, if symptoms occur more than 2 days after arrival at altitude, clinicians should seek an alternative diagnosis but maintain AMS/HACE on the differential Treatment and management AMS Adjunctive oxygen and descent to lower altitude Acetazolamide is used as a preventive measure but is not helpful in acute treatment +/- dexamethasone HACE Patients with HACE should receive dexamethasone to help reduce cerebral edema Immediate descent to a lower altitude References Burtscher M, Wille M, Menz V, Faulhaber M, Gatterer H. In this final Bible Study in Exodus, we see the Tabernacle erected, anointed, and consecrated. We then see the Glory of the Lord fill the Tabernacle (the Lord has come to abide with his people as promised in Ex 6.6-8). We take a deep dive into“the cloud” and the “Glory of the Lord” seeing how both play out throughout scripture. As we wrap up the book of Exodus we see God's ever-present narrative, woven throughout the Bible, of a loving God reuniting his creation to himself. Outline: 00:57 - Overview of Exodus 40 02:15 - Exodus 40.1-33 - the setting up of all the elements of the Tabernacle along with their anointing and consecration. 06:25 - Exodus 40.2 - “First day of the first month” - v17 adds “the tabernacle was set up on the first day of the first month in the 2nd year.” From this, we learn that they are entering the 2nd year of their desert time. Ex 12.2 is the description of Passover. Ex 12.10 we learn that it was to take place on the 10th day of the 1st month. (So they would be celebrating Passover 9 days after consecrating the Tabernacle. We also learn from Ex 19.1 that it took less than 6 months to build and put together all the elements of the Tabernacle. 08:21 - Exodus 40.9-16 - Anoint and consecrate the Tabernacle. “Aniont” occurs 10x in Exodus (5x in Ex 40). Māšaḥ (maw-shakh') מָשַׁח. “To smear, spread liquid, to consecrate.” Ex 30.22-33 gives us the recipe for the anointing oil. “Consecrate” occurs 16x in Exodus (4x in Ex 40). qāḏaš (Kaw-Dash) קָדַשׁ. “To consecrate, sanctify, prepare, dedicate, to be set apart.” 10:07 - Exodus 40.16 - “As the Lord Commanded” - 8x in Ex 40, and 10x in Ex 39 10:50 - Exodus 33 - The Tabernacle is fully set up. “And so Moses finished the work.” v33 12:59 - Exodus 40.34-38, the Glory of the Lord fills the Tabernacle 13:45 - “The cloud” - 49x in 44 verses. Occurs 11 times in Exodus and 14 times in Num as the text documents Israel's wanderings in the desert. The pillar of cloud was a guide for Israel. Another awesome instance of the Cloud of God can be found in the New Testament, the transfiguration. Matt 17, Mark 9, and Luke 9 document the story of Jesus, Peter, James, and John on a mountaintop. Jesus is praying and he is transformed into pure light before them, then they see Moses and Elijah standing with him. Then a cloud comes and covers all of them and Luke 9.35 tells us that “a voice came from the cloud saying, ‘this is my Son, whom I have chosen; listen to him.'” 17:57 - The “Glory of the Lord” in the Bible. It occurs 36 times in the NIV; 6 in Exodus; In 1 Kings ch 6-8 and 2 Chronicles ch 3-7. Ezekiel 8-11 is the retelling of a vision that Ezekiel had that was a warning for the Israelites in exile in Babylon; it tells of the Glory of the Lord leaving the temple due to Israel's idolatry. Eze 8.1, 4, 6; 9.3, 9; 10.4, 18-19; 11.23; 43.1-5. We see the Glory of the Lord return to earth in Luke 2.8-11 as well as in John 1.1, 14. No longer is the Glory in the temple, it is in Christ, walking amongst us! 31:49 - Final point. The Tabernacle and Temple were where God dwelt with his people. Where does he dwell today? Ex 6.6-8, Ex 40.34, 2 Chr 7.1-3, 1 Cor 3.16 and 6.19. 35:37 - conclusion. It's a common physiological process that happens to many people, especially after meals. Occurs less than twice a week. Associated with mild and temporary symptoms such as heartburn or regurgitation. Gastroesophageal reflux disease (GERD): a chronic and more severe form of GER. It occurs when acid reflux happens frequently, typically more than twice a week, and/or causes esophageal injury/complications. -Non-erosive reflux disease (NERD)= GER without evidence of esophageal injury on endoscopy. -Erosive reflux disease (ERD)= GER with evidence of esophageal injury on endoscopy.AFP Journal, January 2024: “Nonerosive GERD does not increase the likelihood of esophageal cancer. However, erosive GERD is associated with a doubled, but still low, risk of developing cancer, with the likelihood increasing over time.”Pathophysiology:The main pathophysiology behind GERD is lower esophageal sphincter (LES) dysfunction which can occur due to the following:-LES Pressure: The LES is a muscular ring at the junction of the esophagus and stomach. It normally maintains a high-pressure zone to prevent reflux. In GERD, the intragastric pressure is higher than the pressure created by the LES. The tone of the LES can be reduced by caffeine, nitroglycerin, and scleroderma. -Transient LES Relaxations (TLESRs): These are normal relaxations of the LES that occur independently of swallowing. In GERD, these relaxations are more frequent or prolonged, allowing acid to reflux into the esophagus.-Anatomic abnormalities: A hiatal hernia occurs when a portion of the stomach protrudes through the diaphragm into the chest cavity. This disrupts the normal anatomy of the gastroesophageal junction, reducing the pressure barrier and promoting reflux.Epidemiology: It affects 10-20% of adults in Western cultures and less than 5% in Asia. Prevalence in the US ranges from 18.1% to 27.8% with a slightly higher rate in men. Risk factors: -Obesity, pregnancy, scleroderma, hiatal hernia; smoking, caffeine, alcohol, stress, fatty/fried/spicy foods. Spicy foods can be a challenge in some cultures (e.g. Mexican and Indian.) Sometimes, patients may ask for “something” to stop GERD but all they may need is dietary modification. -Medications: -aspirin, ibuprofen, clindamycin, tetracycline, bisphosphonates (irritate the esophagus and cause heartburn pain similar to GERD) -anticholinergics, TCA's, CCB's, ACEi, statins, benzodiazepines, theophylline, opioids, progesterone (increase acid reflux and worsen GERD)Clinical features: Typical symptoms: -heartburn (burning retrosternal pain) -regurgitation (acidic stomach contents)Atypical symptoms: -chest pain (can mimic angina pectoris, squeezing/burning substernal, radiates to back/neck/jaw/arm) -water brash (hypersalivation)-globus sensation (lump in throat)-nausea -belching-bloating Alarm features in GERD: -dysphagia-odynophagia (pain with swallowing)-new onset of dyspepsia in ≥60yo -weight loss-GI bleeding-vomiting-anemia Diagnosis: -There is no gold standard test -Patient with typical symptoms: diagnosis can be based on clinical symptoms alone -Patient with atypical symptoms: these symptoms can be seen in GERD but are not sufficient for diagnosis of GERD in the absence of typical symptoms. Need to rule out other disorders before associating the symptoms with GERD. (ex: chest pain r/o other causes such as MI with ECG) -Patient with alarm features: refer to GI for upper GI endoscopy. Complications: -Esophagitis: Erosive reflux disease (ERD) = GER with evidence of esophageal distal injury on endoscopy; in untreated GERD 30% have esophagitis. -Iron deficiency anemia: due to mucosal ulcerations -> chronic bleeding.-Esophageal stricture: narrowing near GE junction, solid food dysphagia.-Barrett Esophagus: intestinal metaplasia of esophagus due to chronic GERD (stratified squamous epithelium replaced by columnar epithelium)-Risk factors: GERD for 5-10 years, >50yo, males, obesity, Caucasian, Tobacco use, family history -Predisposes to esophageal adenocarcinoma Role of H. pylori.Sometimes we tend to think that H. pylori is the cause of GERD. “H. pylori infection appears to protect the esophagus from gastroesophageal reflux disease, Barrett's esophagus, dysplasia in Barrett's esophagus, and esophageal adenocarcinoma, perhaps by causing chronic gastritis that interferes with acid production.”It is unclear whether long-term use of PPIs heightens the risk of atrophic gastritis in patients with H. pylori. Consequently, routine screening for H. pylori infection and empiric eradication of H. pylori are NOT advised for patients with GERD. However, if H. pylori is diagnosed in the setting of GERD, eradication of H. pylori has been associated with an improvement of symptoms in patients with antral-predominant gastritis. Treatment: Two categories: Mild/intermittent symptoms (
