On the history of the churches True Christian Religion 762. That four churches have existed on this earth since the creation of the world is in accordance with Divine order, which requires that there be a beginning and then its end before a new beginning starts in. Therefore every day begins with morning, progresses, ends in night, and then begins anew; also every year begins with spring, progresses through summer to autumn, closes in winter, and then begins again; and in order that these changes may take place the sun rises in the east progresses therefrom through the south to the west, and finishes its course in the north, after which it rises again. It is the same with churches; the first, which was the Most Ancient, was like morning, spring, and the east; the second or Ancient church was like day, summer, and the south; the third was like evening, autumn, and the west; and the fourth like night, winter and the north. From these orderly progressions the wise men of ancient times inferred four ages of the world, the first of which they called the golden age, the second the silver age, the third the copper age, and the fourth the iron age, by which metals, moreover, these churches are represented in Nebuchadnezzar's statue. And again, in the Lord's sight the church is seen as a single man, and this larger man must pass through his stages of life like an individual, that is to say, from infancy to youth, from this to manhood, and finally to old age; and then, when he dies, he will rise again. The Lord says: Except a grain of wheat fall into the earth and die, it abideth [alone]; but if it die, it beareth much fruit (John 12:24). This is evident also from the signification of "coming quickly," as being certainly, at hand, and coming to pass; that "quickly" signifies certainty may be seen also above (n. 7, 216). What this third woe that was to come quickly involves, can be seen from what follows, namely, that it involves the final state of the church, when there is no longer any truth or good, and the state of separation at that time of the evil from the good, and of the good from the evil, and at length the Last Judgement, which is effected by a full separation, and then the casting down of the evil into hell, and the elevation of the good into heaven. AC 2118. By the Last Judgment is meant the last time of a church, and also the last state of each person's life. As regards the last time of the church, it was the Last Judgment of the Most Ancient Church, which was before the flood, when their posterity perished; whose destruction is described by the flood. The Last Judgment of the Ancient Church, which was after the flood, was when almost all who were of that church became idolaters, and were dispersed. The Last Judgment of the Representative Church, which succeeded among the posterity of Jacob, was when the ten tribes were carried away into captivity, and were dispersed among the nations; and when later, after the Lord's coming, the Jews were driven out of the land of Canaan, and were scattered throughout the world. CardioNerds (Dr. Claire Cambron and Dr. Rawan Amir) join Dr. Ayan Purkayastha, Dr. David Song, and Dr. Justin Wang from NewYork-Presbyterian Queens for an afternoon of hot pot in downtown Flushing. They discuss a case of congenital heart disease presenting in adulthood. Expert commentary is provided by Dr. Su Yuan, and audio editing for this episode was performed by CardioNerds Intern, Julia Marques Fernandes. A 53-year-old woman with a past medical history of hypertension visiting from Guyana presented with 2 days of chest pain. EKG showed dominant R wave in V1 with precordial T wave inversions. Troponin levels were normal, however she was started on therapeutic heparin with plan for left heart catheterization. Her chest X-ray revealed dextrocardia and echocardiogram was suspicious for the systemic ventricle being the morphologic right ventricle with reduced systolic function and the pulmonic ventricle being the morphologic left ventricle. Patient underwent coronary CT angiography which confirmed diagnosis of congenitally corrected transposition of the great arteries (CCTGA) as well as minimal non-obstructive coronary artery disease. Her chest pain spontaneously improved and catheterization was deferred. Patient opted to follow with a congenital specialist back in her home country upon discharge. Pearls- A Case of Congenital Heart Disease Presenting in Adulthood Congenitally Corrected Transposition of the Great Arteries (CCTGA) is a rare and unique structural heart disease which presents as an isolated combination of atrioventricular and ventriculoarterial discordance resulting in physiologically corrected blood flow. CCTGA occurs due to L looping of the embryologic heart tube. As a result, the morphologic right ventricle outflows into the systemic circulation, and the morphologic left ventricle outflows into the pulmonary circulation. CCTGA is frequently associated with ventricular septal defects, pulmonic stenosis, tricuspid valve abnormalities and dextrocardia. CCTGA is often asymptomatic in childhood and can present later in adulthood with symptoms of morphologic right ventricular failure, tricuspid regurgitation, or cardiac arrhythmias. Systemic atrioventricular valve (SAVV) intervention can be a valuable option for treating right ventricular failure and degeneration of the morphologic tricuspid valve. notes- A Case of Congenital Heart Disease Presenting in Adulthood Notes were drafted by Ayan Purkayastha. What is the pathogenesis of Congenitally Corrected Transposition of the Great Arteries? Occurs due to disorders in the development of the primary cardiac tube Bulboventricular part of the primary heart forms a left-sided loop instead of right-sided loop, leading to the normally located atria being connected to morphologically incompatible ventricles This is accompanied by abnormal torsion of the aortopulmonary septum (transposition of the great vessels) As a result, there is ‘physiologic correction' of blood flow. Non-oxygenated blood flows into the right atrium and through the mitral valve into the morphologic left ventricle, which pumps blood into the pulmonary artery. Oxygenated blood from the pulmonary veins flows into the left atrium and through the tricuspid valve to the morphologic right ventricle, which pumps blood to the aorta. Compared with standard anatomy, the flow of blood is appropriate, but it is going through the incorrect ventricle on both sides. In this episode, I use my friend Tim's pickleball injury as a jumping-off point to explore what the evidence actually says about ice, rest, NSAIDs, stretching, and more.When Tim skipped his warm-up and pulled a calf muscle, it raised a question many of us face: was it avoidable? While ancient wisdom and modern influencers often shout conflicting advice, this episode sorts through the noise to uncover what's evidence-backed, what's outdated, and what might actually delay healing. For pain, yes, ice works—cooling slows nerve conduction and can help with comfort, as seen in this study of ankle injuries. But does it reduce inflammation in a helpful way? Possibly not. Some research suggests that vasoconstriction may hinder the delivery of reparative cells and removal of waste, as noted in this trial.The evolution from RICE to PEACE to MEAT and even PEACE & LOVE reflects our shifting understanding. A meta-analysis of 22 randomized trials found no conclusive benefit of ice when added to compression or elevation. As for NSAIDs like ibuprofen, the Cochrane Review revealed no significant advantage over acetaminophen in pain relief or swelling reduction—and no clear evidence they speed up recovery.What about rest? Surprisingly, prolonged rest may do more harm than good. The Deyo study and later NEJM data show that continued normal activity (within pain tolerance) results in faster recovery than either bed rest or structured exercises, at least for acute low back pain—offering insights that might extend to other strains or sprains.Can you prevent injuries altogether? Static stretching (think toe touches) doesn't show strong support in RCT reviews, and while a recent meta-analysis found a small reduction in muscle injuries, the impact was modest. Dynamic stretching remains inconclusive according to current evidence.The takeaway? When treatments or prevention strategies are studied over and over yet results remain ambiguous, it likely means any real benefit is small—a principle I call “Dr. Bobby's Law of Many Studies.” Compare that with fall prevention in older adults: 66 RCTs involving 47,000 people showed strength and balance training significantly reduces falls by 20–30%. When something works, it tends to show up clearly and consistently.Takeaways: If you're injured, ice and NSAIDs can ease discomfort—but don't count on them to speed up healing. Resting too much may slow recovery; try gentle movement instead. Stretching might help a bit with prevention, but don't expect miracles. Evidence

Leviticus 05.1-13 Bible Study - Sin Offering (Part 2)In this Bible study, we look at the 2nd half of the Sin Offering as outlined in Leviticus 5.1-13. The Israelites were required to perform a “sin offering” if they failed to do something that was required, such as failing to give a testimony, dealing properly with ritual uncleanness, or fulfilling an oath. We also look at the “sin offering” as the “purification offering” asking why a woman who just gave birth would be required to give a “sin offering?” A key verse in this passage is Leviticus 5.5 in which the text tells us that confession is key to the process. OUTLINE:01:45 - What is Iron Sheep Ministries? 07:04 - Outline of the talk: 5.1-6 Failure to do something that was required: - v1 - Failure to give testimony, which could prevent justice from being carried out. - v2-3 - Failure to deal properly with ritual uncleanness, which could lead to the Lord's tabernacle being defiled.- v4 - Failure to fulfill an oath, which would result in the Lord's name being profaned. Contributor: Jorge Chalit-Hernandez, OMS3 Educational Pearls: Acute mountain sickness (AMS) is the term given to what is otherwise colloquially known as altitude sickness High altitude cerebral edema (HACE) is a severe form of AMS marked by encephalopathic changes Symptoms begin at elevations as low as 6500 feet above sea level for people who ascend rapidly May develop more severe symptoms at higher altitudes The pathophysiology involves cerebral vasodilation Occurs in everyone ascending to high altitudes but is more pronounces in those that develop symptoms The reduced partial pressure of oxygen induces hypoxic vasodilation in the brain, which results in edema and, ultimately, HACE in some patients Symptomatic presentation Headache, nausea, and sleeping difficulties occur within 2-24 hours of arrival at altitude HACE may occur between 12-72 hours after AMS and presents with ataxia, confusion, irritability, and ultimately results in coma if left untreated Clinical presentation may be mistaken for simple exhaustion, so clinicians should maintain a high index of suspicion Notably, if symptoms occur more than 2 days after arrival at altitude, clinicians should seek an alternative diagnosis but maintain AMS/HACE on the differential Treatment and management AMS Adjunctive oxygen and descent to lower altitude Acetazolamide is used as a preventive measure but is not helpful in acute treatment +/- dexamethasone HACE Patients with HACE should receive dexamethasone to help reduce cerebral edema Immediate descent to a lower altitude References Burtscher M, Wille M, Menz V, Faulhaber M, Gatterer H. Hence it is that no miracles are wrought at this day. That they would be hurtful, can be seen from what has been said; for they drive men to believe, and fix their ideas in the external man that the case is so; and if the internal man afterward denies that which the miracles have confirmed, there results an opposition and collision of the internal and external man; and finally when the ideas derived from miracles are dissipated, there is effected a conjunction of falsity and truth, and thus a profanation. From this it is evident how injurious at the present day are miracles in a church in which the inward things of worship have been disclosed. These moreover are the things signified by the Lord's words to Thomas: Because thou hast seen Me, Thomas, thou hast believed; blessed are they who see not, and believe (John 20:29); thus they are blessed who do not believe through miracles.
