Contributor: Aaron Lessen, MD Educational Pearls: Pediatric patients frequently have vital signs considered abnormal for age at discharge Large multicenter study recently evaluated if pediatric patients discharged with abnormal vital signs have worse outcomes  97,824 pediatric discharges were included in the study 18.1% were discharged with vitals considered abnormal for age No significant difference in readmission rates at 48 hours (2.28% in abnormal cohort vs. 2.45% in normal cohort) No significant adverse outcomes in those discharged with abnormal vital signs (4 total PICU admissions with no deaths, CPR, or intubations) When considering discharging pediatric patients, it is important to evaluate how the patient looks rather than just relying on vital signs Consider leaving the child attached to a monitor, leaving the room, and then reevaluating them if they could be agitated by the presence of healthcare providers References Kazmierczak M, Thompson AD, DePiero AD, Selbst SM. After debuting with a self-titled effort issued on the Home Sweet Home label, Green exited the line-up, and was replaced by vocalist Scott Douglas for 1984's Vital Signs; another roster swap exchanging Dann Huff for guitarist Gordon Kennedy again shook up the Whiteheart sound just prior to 1985's Hotline. Craig Rosenberg has worked with hundreds, if not thousands of B2B SaaS companies as the co-founder of TOPO, Distinguished analyst at Gartner, and now as Chief Platform Officer at Scale Venture Partners.Across Craig's roles, he was able to take an expensive view across each stage of a SaaS company's growth including strategy, people, process, technology, tactics, and over time METRICS!Craig highlighted that the best companies in the world were/are "metrics" driven, and as Craig started to work with larger, enterprise-class companies beyond SaaS being "metrics and data" driven was even more critical to decision-making."End to End" Customer Journey is an often discussed subject, but what is it really? Craig's perspective is most customer journey mapping is too generic and needs to be very focused on how the customer buys starting with using third-party internet activity to marketing interactions to Sales Development to Sales and then ending at "Closed-Won". Going beyond Closed-Won to include customer engagement, retention, and expansion,Going beyond mapping and understanding the entire customer journey including acquisition, retention, and expansion, companies need to "SEGMENT" the metrics by customer cohort, such as SMB vs Mid-Market vs Enterprise. Another view should be based upon "HOW" the prospect/customer came into the customer lifecycle process, such as lead source and/or lead channel.When I asked "who" in a company should map the customer lifecycle, Craig's response was quite pragmatic: "whoever is best at mapping the customer lifecycle in your company". Craig added that Revenue Operations is a perfect organization to take the lead on customer journey mapping, and building a "coalition" across Marketing, Sales, and Customer Success. An important caveat is that without the support and involvement of the CEO it becomes less significant and strategic.Another topic we discussed, was if a company should involve customers in the "journey mapping" process. Craig said of course, but you only need to include a few customers in the process as talking with more than 10 customers will provide diminishing returns.Next, I asked Craig about what metrics are priorities to measure the efficiency of the customer lifecycle across acquisition, retention, and expansion. Craig started with the Four Vital Signs Framework to track in a SaaS company:- Growth- Efficiency- Churn- BurnNext, we discussed if any of the Vital Signs are more important at each stage of a company's evolution. Craig's first recommendation was to instrument and begin capturing metrics for all four vital signs early in the journey. Certain metrics like churn/Customer Retention will become more important as a company grows beyond the first and second renewal cycles, but identifying and instrumenting for metrics should begin earlier than most companies believe are required.No matter what stage of growth your SaaS company is currently in, this discussion with Craig Rosenberg provides many interesting, insightful perspectives on the importance and priority of metrics across the customer lifecycle.

This week we will discuss the annual medical checkup or annual physical exam.  We will not have a guest on this episode but hope you will learn just as much from the discussion based on the research we have done for this episode. ​ Annual Physical Exam: The Basics The physical exam is an essential part of any doctor's visit. Surprisingly, though, there are no absolutes in a routine physical. A good doctor may be thorough or brief, but they will spend time listening to your concerns and providing counseling for your particular complaints and risk factors. Annual exams usually check your: History. This is your chance to mention any complaints or concerns about your health. Your doctor will also likely quiz you about lifestyle behaviors like smoking, excessive alcohol use, sexual health, diet, and exercise. The doctor will also check on your vaccination status and update your personal and family medical history. Vital Signs. These are some vital signs checked by your doctor: Blood pressure: Less than 120 over less than 80 is a normal blood pressure. Doctors define high blood pressure (hypertension) as 130 over 80 or higher. Heart rate: Values between 60 and 100 are considered normal. Many healthy people have heart rates slower than 60, however. Respiration rate: From 12 to 16 breaths per minute is normal for a healthy adult. Breathing more than 20 times per minute can suggest heart or lung problems. Temperature: 98.6 degrees Fahrenheit is the average, but healthy people can have resting temperatures slightly higher or lower. ​ General Appearance. Your doctor gathers a large amount of information about you and your health just by watching and talking to you. How is your memory and mental quickness? Does your skin appear healthy? Can you easily stand and walk? ​ Heart Exam. Listening to your heart with a stethoscope, a doctor might detect an irregular heartbeat, a heart murmur, or other clues to heart disease. Lung Exam. Using a stethoscope, a doctor listens for crackles, wheezes, or decreased breath sounds. These and other sounds are clues to the presence of heart or lung disease. Head and Neck Exam. Opening up and saying "ah" shows off your throat and tonsils. The quality of your teeth and gums also provides information about your overall health. Ears, nose, sinuses, eyes, lymph nodes, thyroid, and carotid arteries may also be examined. ​ Abdominal Exam. Your doctor can use a range of examination techniques including tapping your abdomen to detect liver size and presence of abdominal fluid, listening for bowel sounds with a stethoscope, and palpating for tenderness. Neurological Exam. Nerves, muscle strength, reflexes, balance, and mental state may be assessed. Dermatological Exam. Skin and nail findings could indicate a dermatological problem or disease somewhere else in the body. Extremities Exam. Your doctor will look for physical and sensory changes. Pulses can be checked in your arms and legs. Examining joints can assess for abnormalities. ​ Male Physical Exam An annual physical exam for men might also include: Testicular exam: A doctor can check each testicle for lumps, tenderness, or changes in size. Most men with testicular cancer notice a growth before seeing a doctor. Hernia exam: The famous "turn your head and cough" checks for a weakness in the abdominal wall between the intestines and scrotum. Penis exam: A doctor might notice evidence of sexually transmitted infections such as warts or ulcers on the penis. Prostate exam: Inserting a finger in the rectum lets a doctor feel the prostate for its size and any suspicious areas. Female Physical Exam A woman's annual exam might include: Breast exam. Feeling for abnormal lumps may detect breast cancer or benign breast conditions. The doctor will also check the lymph nodes in the underarm area and look for visual abnormalities of the breasts and nipples. Pelvic exam: The pelvic exam allows examination of the vulva, vagina, cervix, uterus and ovaries. Routine checks for sexually transmitted infections are often done. Life expectancy for women in the United States in 2021 was 79 years; for men, it was 73 years. This is the largest gap between the two since 1996. The impact of COVID-19 has marginally lowered both. Historically, men have been the main breadwinners and so it was argued they suffered more “life-shortening” stress. Yet the number of working women now rivals—and even surpasses—the number of working men, so shouldn't their lifespans also be coming into alignment? Acts 2:42-47 | Andrew Murch | Thus far, the first two chapters of the book of Acts have been quite the adventure. Jesus promises the Holy Spirit, then he floated up into the clouds (who saw that one coming?!). Then there's the whole scene on the day of Pentecost. And then the last thing we read is that God added more than 3,000 souls to the church. Much of this book has read more like a thriller than biblical narrative. But then we come to this week's text. Six verses that, at face value, may seem like the most “normal” part of the story so far. But is it? As we dig in, we will find something quite extraordinary in the midst of the ordinary. More than that, we'll find a blueprint for following Jesus in community together. While some might look at this text as an idealistic way of life that would never work in today's day and age, it's here that we find many core principles of the faith.

Today's guest is Dr. Mark Lewis, executive director of NDIA's Emerging Technologies Institute (NDIA ETI), a non-partisan think tank focused on technologies that are critical to the future of national defense. ETI provides research and analyses to inform the development and integration of emerging technologies into the defense industrial base.   We will discuss the Emerging Technologies Institute's Vital Signs report, which is an evaluation of the readiness and health of the defense industrial base. Prior to his role at the Emerging Technologies Institute, Mark was the Director of Defense Research & Engineering in the Department of Defense, overseeing technology modernization for all military services and DoD Agencies, as well as the acting Deputy Under Secretary of Defense for Research & Engineering.  In this role he was the Pentagon's senior-most scientist, providing management oversight and leadership for DARPA, the Missile Defense Agency, the Defense Innovation Unit, the Space Development Agency, Federally Funded Research and Development Centers, and the DoD's basic and applied research portfolio. At the Department of Defense, Mark worked closely with Mike Griffin, who appeared on episodes 23 and 134 of STEM-Talk. In today's interview with Mark, we will again discuss hypersonics and other emerging technologies and modernization priorities that are critical to our national defense. Mark is also the former longest-serving and is perhaps best known for his work in hypersonics. In addition to these important defense-related roles, Mark is also a professor emeritus at the University of Maryland. He spent 25 years as a faculty member at Maryland, conducting basic and applied research in hypersonic aerodynamics, advanced propulsion, and space-vehicle design. Show notes: [00:03:27] Dawn opens the interview asking where Mark grew up and what he was like as a kid. [00:04:29] When Dawn asks Mark when he first became interested in science, Mark tells a funny story form his time as president of the American Institute of Aeronautics and Astronautics? [00:06:21] Ken asks Mark how he ended up at MIT after high school. [00:07:46] Mark talks about taking a job as an assistant professor of aerospace engineering at the University of Maryland after earning his Ph.D. in aeronautics and astronautics at MIT. [00:09:34] Dawn mentions that from 2002 to 2004, Mark was the director of the Space Vehicle Technology Institute. She asks Mark to give an overview of the Institute and the kind of work that goes on there. [00:12:45] Ken mentions that in 2004, Mark became Chief Scientist of the U.S. Air Force, going on to become the longest-serving Chief Scientist in Air Force history. Ken asks Mark to explain the role of the chief scientist, and what he focused on during his time in the position. [00:17:37] Dawn explains that in 2012, Mark became the director of the Science and Technology Policy Institute, which worked with the executive office of the President and other Executive Branch agencies. Mark talks about the kind of work the Science and Technology Policy Institute does. [00:20:23] Dawn mentions that during Mark's 25 years as a faculty member at the University of Maryland, he conducted basic and applied research in a variety of fields, such as hypersonic aerodynamics, space vehicle design, and advanced propulsion.  She point out that Mark, however, is best known for his work in hypersonics. She asks Mark what led him to focus on hypersonics. [00:22:46] Ken asks Mark to explain why he decided to work under Mike Griffin (episodes 23 and 134) in the Pentagon as the Director of Defense Research and Engineering, and what that experience was like. [00:28:19] Dawn mentions that during Mike Griffin's time as Under Secretary of Defense for Research and Engineering, he made hypersonics the department's number one priority. Dawn asks Mark to explain the importance of hypersonics in terms of our national defense.
