Juvenile obesity and its association with utilisation and costs of pharmaceuticals - results from the KiGGS study

Background: According to a national reference, 15% of German children and adolescents are overweight (including obese) and 6.3% are obese. An earlier study analysed the impact of childhood overweight and obesity on different components of direct medical costs (physician, hospital and therapists). To complement the existing literature for Germany, this study aims to explore the association of body mass index (BMI) with utilisation of pharmaceuticals and related costs in German children and adolescents. Methods: Based on data from 14, 836 respondents aged 3-17 years in the German Interview and Examination Survey for Children and Adolescents (KiGGS), drug intake and associated costs were estimated using a bottom-up approach. To investigate the association of BMI with utilisation and costs, univariate analyses and multivariate generalised mixed models were conducted. Results: There was no significant difference between BMI groups regarding the probability of drug utilisation. However, the number of pharmaceuticals used was significantly higher (14%) for obese children than for normal weight children. Furthermore, there was a trend for more physician-prescribed medication in obese children and adolescents. Among children with pharmaceutical intake, estimated costs were 24% higher for obese children compared with the normal weight group. Conclusions: This is the first study to estimate excess drug costs for obesity based on a representative cross-sectional sample of the child and adolescent population in Germany. The results suggest that obese children should be classified as a priority group for prevention. This study complements the existing literature and provides important information concerning the relevance of childhood obesity as a health problem.

Risk factors for obesity: further evidence for stronger effects on overweight children and adolescents compared to normal-weight subjects.

Background: We recently showed that in preschoolers risk factors for overweight show stronger associations with BMI in children with high BMI values. However, it is unclear whether these findings might also pertain to adolescents. Methods: We extracted data on 3–10 year-old (n = 7,237) and 11–17 year-old (n = 5,986) children from a representative cross-sectional German health survey (KiGGS) conducted between 2003 and 2006 and calculated quantile regression models for each age group. We used z-scores of children's body mass index (BMI) as outcome variable and maternal BMI, maternal smoking in pregnancy, low parental socioeconomic status, exclusive formula-feeding and high TV viewing time as explanatory variables. Results: In both age groups, the estimated effects of all risk factors except formula-feeding on BMI z-score were greatest for children with the highest BMI z-score. The median BMI z-score of 11–17 year-old children with high TV viewing time, for example, was 0.11 [95% CI: 0.03, 0.19] units higher than the median BMI z-score of teenage children with low TV viewing time. This risk factor was associated with an average difference of 0.18 [0.06, 0.30] units at the 90th percentile of BMI z-score and of 0.20 [0.07, 0.33] units at the 97th percentile. Conclusions: We confirmed that risk factors for childhood overweight are associated with greater shifts in the upper parts of the children's BMI distribution than in the middle and lower parts. These findings pertain also to teenagers and might possibly help to explain the secular shift in the upper BMI percentiles in children and adolescents.

Einführung: Das Münchner-Auxologie-Projekt (MAP) (auxo= Wachstum ; logos=Sprache) wurde Anfang des Jahres 2003 unter Federführung von Prof. Dr. D. Kunze und PD Dr. H. Schmidt begonnen. Primäres Ziel von MAP ist es, ein generelles Screening für Wachstumsstörungen im Rahmen der Kindervorsorgeuntersuchungen zu etablieren, das dem praktisch tätigen Pädiater ein diagnostisches Instrumentarium zur Verfügung stellen soll, optimale Rahmenbedingungen für die Überwachung der Entwicklung des Kindes zu ermöglichen. Hierzu wurden in der MAP-Zentrale die Daten der einzelnen Befundbögen in ein speziell entwickeltes, der Problematik angepasstes EDV-basiertes Programm eingelesen und die Parameter Körperlänge (L) , Körpergewicht (KG) , Body-Mass-Index (BMI) und Kopfumfang mit Referenzdaten aus der Literatur verglichen. Material: Es wurden bis zum IV. Quartal 2007 insgesamt 13.864 Kinder untersucht; valide Datensätze lagen von 7074 Jungen und 6434 Mädchen vor, die in der hier vorgelegten Zwischenauswertung analysiert wurden. Die MAP-Daten für L wurden mit denen von KIGGS (2007) [KI], Prader (1989) [Pr] und Kromeyer-Hauschild (2001) [K-H] verglichen, die für KG und BMI nach [K-H] und [KI]. Ergebnisse: Bezüglich L zeigte sich für MAP und [KI] eine weitgehende Übereinstimmung der Konfidenzintervalle der Altersklassen 0 bis 17 Jahre für Jungen und Mädchen. Nach [Pr] bzw. [K-H] lagen 79% bzw. 80% der Jungen und 72% bzw. 80% der Mädchen im Normalbereich, Auffällig hochwüchsig fand sich bei 7,1% bzw. 5,9% der Jungen und bei 9,5% bzw. 5,6% der Mädchen und Hochwuchs bei 3,8% bzw. 2,5% der Jungen und 7,2% bzw. 2,6% der Mädchen. Bezüglich KG fand sich für MAP und [KI] eine weitgehende Übereinstimmung der Confidenzintervalle der Altersklassen 0-17 Jahre bei beiden Geschlechtern. Bezüglich BMI lagen nach [K-H] rund 10% der MAP-Jungen und Mädchen über der 90. Percentile! 6,5% bzw. 3,5% der Jungen und 6,6% bzw. 3,4% der Mädchen waren übergewichtig oder. adipös, wobei dieser Effekt altersabhängig war. Eindrucksvoll war der Einfluss des Migrationshintergrundes: Es zeigte sich, dass Jungen und Mädchen deutscher Eltern bereits zu einem Prozentsatz von 6,04% und 5,67% übergewichtig bzw. von 2,55% und 2,52% adipös sind. Bei einem deutschen Elternteil erhöhte sich der Anteil übergewichtiger und adipöser Mädchen auf 6,74 % und 4,21%. Die höchsten Anteile übergewichtiger und adipöser Kinder fanden sich, wenn beide Elternteile nicht deutscher Herkunft waren; die Prozentanteile Übergewichtiger liegen für Jungen und Mädchen bei 7,85% bzw. 5,71% und Adipöser bei 8,39% bzw. 4,71%. Schlussfolgerung: Hieraus folgt – in Übereinstimmung mit der Literatur - dass der Anteil übergewichtiger sowie adipöser Kinder bei einem vorliegenden Migrationshintergrund bei den Jungen um den Faktor 3,3 und bei den Mädchen um den Faktor 1,87 im Vergleich zu den Daten der Referenzgruppe ohne Migrationshintergrund zunimmt. Die Ergebnisse der Zwischenauswertung zeigen, dass eine Datenbank wie MAP für den Pädiater ein geeignetes Instrument zur Früherkennung von Wachstumsstörungen im Kindesalter ist. Die statistische Auswertung bisher verfügbarer Daten zeigt, dass der Münchener Raum in der Bundesrepublik Deutschland keine Ausnahmestellung einnimmt, denn auch hier wird die Prävalenz von Übergewicht und Adipositas im Kindesalter - insbesondere bei Kindern mit Migrationshintergrund – erschreckend deutlich und verlangt geeignete Interventionsmaßnahmen.
