As promised, here is the now unlocked first bonus episode of our podcast. Building on our conversation earlier in Season 2 about Jojo's Bizarre Adventure and Shonen Anime in general, we decided to cover all of the parts of Stardust Crusaders that we didn't get to touch on in that episode. So if you were anxious to hear us talk about your favorite JoBro or a particular stand battle, now's your chance. At one point Ian starts talking about Moby Dick for a while too. These unlocked bonus episodes are looser and less scripted than our "in season" episodes. We're not going to bother with re-caps at the start of episodes and our research is going to be more off the cuff. Mark Mirchandani is hanging around the studio as well, bringing some cool things of the week and helping with the question of the week! Informatica provides data managing products that offer complete solutions focusing on metadata management, integration, governance, security, data quality, and discoverability. Bill’s job at Informatica is to ensure these products really take advantage of the strengths of Google Cloud Platform. One such example is a product that allows customers to design in Informatica and push their projects to Cloud Dataproc. Informatica also offers similar capabilities in BigQuery. When moving data from on-prem to the cloud, customers can use Informatica and Google Cloud together for a seamless transition, cost savings, and easier data control. Together, Informatica and Google Cloud can also facilitate the acquisition of high quality data. To have better, more trustworthy output, data inputed needs to be safe to access, have few or no duplicates and null values, and be complete. To achieve this, developers usually use a combination of the Informatica tools Intelligent Cloud Services, Enterprise Data Catalog, and Big Data Management, and the Google tools BigQuery, Cloud Storage, Analytics, Dataproc, and Pub/Sub. Bill’s closing advice for companies comes in three parts: take stock of the data you’ve got, set goals, and develop a well-rounded team. Amal Mattu talks about ACS presentations, workup, and management in older adults, and why the atypical is typical! For the full shownotes and references, and to leave a comment, see: https://gempodcast.com Chest pain is one of the most common reasons why people present to the ED. The chief complaint of Chest Pain typically triggers an automatic EKG, and potentially a workup for acute coronary syndrome. However, many patients who are having ACS do not present with chest pain. Instead, they may have dyspnea, diaphoresis, nausea, vomiting, abdominal pain, or other non-specific symptoms. Which patients are most likely to present this way? Older adults. And the older the patient, the more likely they are to be chest-pain free when they present with an NSTEMI or STEMI. So it is up to the medical provider to be vigilant, consider possible angina equivalents, and order the right workup. Dr. Mattu talks us through some of the statistics of how often MIs occur without chest pain with age, how EKG interpretation may differ, and how management should differ vs how it does differ. Tim Platts-Mills shares his pearls about pain management for older adults in the ED. One major goal of acute care is diagnosing the cause of the pain, but another is helping relieve the suffering associated with pain. In older adults, some of the risks of pain management with opioids are amplified, such as the risk of sedation and falls. With NSAIDs, there is a higher risk of acute renal insufficiency and electrolyte abnormalities, as well as cardiovascular risks with longer treatment. How should we approach acute pain management in the ED, and on discharge in older patients? In this podcast episode, Tim Platts-Mills, an expert and researcher on pain in older adults talks us through some ideas for non-opiates, opiates, and other adjuncts. There are many medications that we commonly prescribe in the ED that can have potentially deadly side effects when combined with other meds that a patient is already on. “Is there no limit to the power of these children?" John Wyndham’s 1957 novel “The Midwich Cuckoos” is the foundation for Wolf Rilla’s 1960 film “Village of the Damned.” It’s hard to say which title may actually be worse, but the story in both seems to work for most people. It’s a decent sci-fi story that, while dated, creates an interesting scenario with all the women in a village being impregnated at the same time by some alien force. Even with that interesting scenario, however, for every element that works in favor of the film, there seems to be another working against it. At least with today’s eyes. Join us — Pete Wright and Andy Nelson — as we continue our Naughty Children series with Rilla’s 1960 film “Village of the Damned.” We talk about what works and what doesn’t, as well as why some of that matters more when it comes to actual enjoyment of the film (at least for Andy) and what they could have done to make it better. We chat about the performances, notably George Sanders, Barbara Shelley and Martin Stephens, and what they bring to the table. When it comes to Shelley, we also chat about the fact that her part is largely dismissed and why it bothers us so much. We discuss the look of the film with Rilla’s documentary approach as well as cinematographer Geoffrey Faithfull’s overall look, which worked nicely for us. And we look at MGM’s history with the film and touch on why they filmed it in England instead of Hollywood. It’s a fun film that, if you think too much about, you’ll find tons of problems; Pete sees the problems while Andy sees the fun. Check out the movie then tune in! She calls the PCP. She gets palliative care on the line. She advocates for the patient to help make sure their wishes are understood and honored. There comes a time when you go from prolonging life to prolonging death. Knowing when that point is can be hard. Listen to hear her thoughts in this post from 10/2015. There are many models for how palliative care can work in an ED. We can provide it ourselves to a certain extent, and in some cases, can consult palliative care services to help with end-of-life decisions. But we should do something to make sure we consider the patient’s wishes before performing aggressive measures that could leave the patient with a quality of life that would not be meaningful for them.
