Today we're hearing from attorney Bart Stichman, Co-founder of National Veterans Legal Services Program, regarding the case “Soto vs United States” and how a Marine Corps combat veteran (and 9,000 other vets) had their benefit claims denied, and disputed by our government. The petition was filed on behalf of USMC vet, Mr. Simon Soto who was initially approved for Combat Related Special Compensation (CRSC), but later denied the full amount after government lawyers claimed a six-year ceiling on retroactive benefits. We open with the facts behind this case, and examine the PTSD he suffered after serving as Mortuary Affairs in Iraq, where he processed the remains of our fallen Marines and Soldiers. Stichman further explains: Why the government fights against veterans filing CRSC claims The arguments that will be heard by The United States Supreme Court in April We'll continue to follow this story as it develops. As many of our listeners know, the Departments of Veterans Affairs and Defense (VA/DoD) Clinical Practice Guideline (CPG) for posttraumatic stress disorder (PTSD) and acute stress disorder (ASD) was released in June of 2023. The CPG, designed to assist clinical decision-making, provides recommendations that, in essence, give a clinician a fighting chance of identifying treatments that the research suggests should help their clients. But for many busy providers, it is a lot to unpack and digest! Join us as our (fearless) Director, Dr. David Riggs, who served on the CPG workgroup, walks us through important aspects of the guideline. We'll discuss the why behind first and second-line treatments, updates from the last CPG, and actionable intel to support you in treating trauma. As with every episode this season, we invited Dave into the EBP confessional, and you won't want to miss his story of a time when things didn't quite go according to plan with a client AND how he recovered. Buckle up. It's a roller coaster of a tale! (https://www.healthquality.va.gov/guidelines/MH/mdd/VADoDMDDCPGFinal508.pdf) clearly state there is insufficient evidence to support this activity and testing. This is mainly because of low quality evidence and concern of bias given commercially funded studies. And then a use of fewer medications that have a potential gene-drug interaction which from what I can find was a ‘theoretical' interaction not an actual increase in adverse events. When patients were told they were getting placebo for back pain and it helped their back pain! The current VA/DoD guidelines clearly state, “For patients who cannot tolerate a statin, we suggest a washout period followed by a rechallenge with the same or a different statin or lower dose, and if that fails, a trial of intermittent (nondaily) dosing”. https://www.healthquality.va.gov/guidelines/CD/lipids/VADoDDyslipidemiaCPG5087212020.pdf (full disclosure and bias I helped write them) N-of-1 Trial of a Statin, Placebo, or No Treatment to Assess Side Effects | NEJM was published in the NEJM and then a few weeks later Statin treatment and muscle symptoms: series of randomised, placebo controlled n-of-1 trials (bmj.com) was published in the BMJ. Around that same time a publication in JAMA https://jamanetwork.com/journals/jamanetworkopen/fullarticle/2773490 showed that using pharmacogenetic testing resulted in no worse LDL levels (soft endpoint) at one year but also no better

We stuck purely to the evidence. There was no evidence panel or committee to vote on the evidence like is often done by the American college of rheumatology and no expert opinion statements where their only citation is themselves like can be seen with the ACC. This was purely 100% evidence recommendations. So lets start try to make it quick because guidelines on a podcast a boring, I want to hit the high points and get out of here This guideline does not cover 1) adults < age 40 years old o 2) patients with ejection fraction

There are bills before Congress to use drones along our border for mass surveillance and provisions to capturing all manner of biometric data when you travel. How will that data be used and who will have access? We break it all down for you and tell you how to voice your opinions. India McKinney and Adam Schwartz from the EFF are with me this week to explain several ways that your privacy rights are under serious attack at the U.S. border. Your favorite PDF viewer is probably riddled with security vulnerabilities. I cover some recent news about Foxit PDF Reader and help you find safer alternatives with my Tip of the Week. India McKinney is a Legislative Analyst with the Electronic Frontier Foundation, primarily focusing on privacy and surveillance issues in upcoming legislation. Prior to joining EFF, India spent over 10 years in Washington, DC as a legislative staffer to three members of Congress from California. Her work there largely focused on the appropriations process, specifically analyzing and funding programs in the Departments of Veterans Affairs, Homeland Security, and Justice. Her biggest legislative accomplishment was authorizing, funding and then naming a new outpatient VA/DoD clinic that will serve over 80,000 people. Adam Schwartz is a Senior Staff Attorney at the Electronic Frontier Foundation. Adam works to ensure that new technologies expand instead of shrink our privacy, freedom of speech, and other civil liberties. Before joining EFF, Adam worked as a Senior Staff Attorney at the American Civil Liberties Union of Illinois. Adam graduated in 1995 from the Howard University School of Law.
