In this piece we discuss breaking trial results with Chris Bain, a consultant anaesthetist and researcher, and Luke Perry, a trainee anaesthetist and PhD student*, both from Melbourne, Australia. We explore the effects of tissue trauma on inflammation and immune responses, and new results about the modifying effect of genetic expression with glucocorticoids. Then we delve into a pilot study on pulmonary artery catheters and cardiac surgery: one of the big questions in our specialty. The pilot study showed that a large randomised trial is feasible and has equipoise. Recorded at Evidence Based Perioperative Medicine (EBPOM) in London last year this presentation is the second of a series of four sessions under the heading: “Implementation – what works and what doesn't?”. In this presentation we cover the historical development and implementation of Enhanced Recovery After Surgery (ERAS); key milestones from Henrik Kellett's pioneering work in the late 1990s, to the formation of the ERAS Study Group and subsequent international efforts. Also, the differences in implementation between countries, particularly the coordinated efforts in the UK through the NHS and the more fragmented approach in the USA. There's an emphasis on the importance of protocol adherence, auditing, and feedback and barriers to implementation such as lack of agreement with evidence, organizational challenges, and institutional hurdles. Presented by Timothy Miller, Professor of Anesthesiology at Duke University Medical Center. Timothy completed his anesthesia training in Nottingham, UK followed by a fellowship in cardiothoracic anaesthesia at Glenfield Hospital, Leicester, UK. He is a fellow of the Royal College of Anaesthetists. He then joined the Department of Anesthesiology at Duke University Medical Center. His clinical interest is management of the high-risk surgical patient undergoing non-cardiac surgery. His practice is varied and involves general, vascular, major urology and thoracic anesthesia, as well as anaesthesia for transplantation. He founded and is co- director of the Enhanced Recovery After Surgery (ERAS) program at Duke.

This webinar will address the why and how of the planned Post-Graduate Diploma in Health Care management. Santhosh trained as an Internal Medicine specialist and worked part time as an Internal Medicine Consultant in Duncan Hospital, India, a 225 bedded hospital near the Nepal border. He is the training coordinator for ICMDA developing an online learning platform. He has pioneered HIV & TB healthcare programmes in marginalised communities and is involved in hospital leadership roles across North India. Whether you love them or loathe them, there's no denying that snakes are an inevitable part of life in regional Australia - particularly if you live in or around our beautiful bushlands. But what would YOU do if you - or someone you were with - were to suffer a suspected snake bite? And what happens if you're hundreds of kilometers from help or emergency advice? Over a one-year period (as part of a larger, three-year study) the RFDS (Western Operations) found that 85 people from regional, rural, remote and very remote Western Australia were flown by Royal Flying Doctor Service to hospital for suspected or confirmed snake bites. While only five of these patients (6%) ultimately received a toxicologist's diagnosis of envenomation, the research showed just how challenging a remote or regional 'suspected snake bite' scenario can be. Dr Jo Sinclair is an Anaesthetist with a career-long interest in doctors' health and wellbeing. In early 2020 she was appointed the Senior Medical Officer Wellbeing Lead at Te Whatu Ora Counties Manukau, and in that role has been instrumental in bringing Schwartz Rounds to the organisation and developing a Stress First Aid training programme. She was also the hospital lead for the Health Roundtable Workforce Wellbeing Improvement Group. In 2022, Jo was seconded into the role of Interim National Clinical Lead for Employee Wellbeing for Te Whatu Ora Health NZ. She works collaboratively with colleagues in the People and Communications Team to begin shifting the organisational culture to one that supports healthcare workers to thrive at work. In 2023, she attended the Stanford Chief Wellbeing Officer Course in San Francisco to further her capability in this area. Jo has undertaken additional training in coaching and mentoring, critical incident stress management, leadership and communication.
