Dr. Suzanne Crowe studied in TCD and graduated in Medicine in 1995. She spent the summer of 1994 as a medical student working as a researcher with the Department of Surgery in the Meath Hospital, and following entry into postgraduate training with the College of Anaesthetists in Ireland, she continued her research and writing interests. After completion of specialist training in Anaesthesia and Intensive Care Medicine, Dr. Crowe worked as a Fellow in the Royal Children's Hospital in Melbourne. She returned to Dublin in 2005, taking up a clinical post as Consultant Anaesthetist with a special interest in Paediatrics in Tallaght Hospital. The following year she became Clinical Lecturer in Surgery in TCD and contributed to the teaching of TCD medical students. In 2014 Dr. Crowe changed consultant post, becoming a Consultant Paediatric Intensivist and Anaesthetist in Our Lady's Children's Hospital Crumlin, and subsequently also Senior Clinical Lecturer in Paediatrics TCD. During her long association with TCD, Dr. Crowe has authored and co-authored more than 50 peer-reviewed papers and book chapters. She is President Elect of the Irish Medical Council for the term 2021-2025, the youngest incoming President in the IMC.

The conversation features key figures from CPOC, including: David Selwyn, Acting Chief Executive at Sherwood Forest Hospitals NHS Foundation Trust, appointed as the inaugural Director of CPOC in May 2019, Denny Levitt, Professor in Perioperative Medicine and Critical Care at the University of Southampton and a Consultant in Perioperative Medicine at Southampton University Hospital NHS Foundation trust (UHS), Scarlett McNally, Consultant Orthopaedic Surgeon in Eastbourne DGH, UK since 2002, Deputy Director at the COPC (2020-26) and an honorary clinical professor at Brighton and Sussex Medical School, Jugdeep Dhesi, Deputy Director for CPOC, Clinical lead for the innovative and award winning POPS (Perioperative medicine for Older People undergoing Surgery) service, Honorary Reader at King's College London and Associate Professor at University College London, and Lawrence Mudford, who transitioned from a healthcare professional to a patient, following a cancer diagnosis. Following his successful treatment, he took up his current role as a patient representative for CPOC. The panel explore the creation and evolution of CPOC since its inception in 2019, highlighting the collaborative efforts to improve surgical pathways by integrating patient and multidisciplinary voices. They discuss various initiatives, guidelines, and successful practices aimed at enhancing patient-centered care, managing conditions like frailty, diabetes, and anaemia, and promoting shared decision-making. The episode also delves into future goals, such as addressing the implementation gap, fostering digital transformation, and operationalizing educational curricula for healthcare professionals to further support perioperative care.

Topics include historical usage, the dangers of high-dose opioids, and alternative pain management techniques such as opioid-free anesthesia and multimodal analgesia. The discussion also highlights the importance of ongoing research, patient education, and evidence-based practices to minimize the side effects of opioids while ensuring effective pain management. Presented by Tim Miller, Professor of Anesthesiology at Duke University Medical Center. Tim completed his training in Nottingham, UK followed by a fellowship in cardiothoracic anaesthesia at Glenfield Hospital, Leicester, UK. He is a fellow of the Royal College of Anaesthetists.

In this piece we discuss breaking trial results with Chris Bain, a consultant anaesthetist and researcher, and Luke Perry, a trainee anaesthetist and PhD student*, both from Melbourne, Australia. We explore the effects of tissue trauma on inflammation and immune responses, and new results about the modifying effect of genetic expression with glucocorticoids. Then we delve into a pilot study on pulmonary artery catheters and cardiac surgery: one of the big questions in our specialty. The pilot study showed that a large randomised trial is feasible and has equipoise. Professional organisations: the ebb and flow of ERAS | EBPOM World Congress

Recorded at Evidence Based Perioperative Medicine (EBPOM) in London last year this presentation is the second of a series of four sessions under the heading: “Implementation – what works and what doesn't?”. In this presentation we cover the historical development and implementation of Enhanced Recovery After Surgery (ERAS); key milestones from Henrik Kellett's pioneering work in the late 1990s, to the formation of the ERAS Study Group and subsequent international efforts. Also, the differences in implementation between countries, particularly the coordinated efforts in the UK through the NHS and the more fragmented approach in the USA. There's an emphasis on the importance of protocol adherence, auditing, and feedback and barriers to implementation such as lack of agreement with evidence, organizational challenges, and institutional hurdles. Presented by Timothy Miller, Professor of Anesthesiology at Duke University Medical Center. Timothy completed his anesthesia training in Nottingham, UK followed by a fellowship in cardiothoracic anaesthesia at Glenfield Hospital, Leicester, UK. He is a fellow of the Royal College of Anaesthetists. He then joined the Department of Anesthesiology at Duke University Medical Center. His clinical interest is management of the high-risk surgical patient undergoing non-cardiac surgery. His practice is varied and involves general, vascular, major urology and thoracic anesthesia, as well as anaesthesia for transplantation. He founded and is co- director of the Enhanced Recovery After Surgery (ERAS) program at Duke.
