Surgery is so incredible. The things we can do today were dreams a generation ago. As a nurse, I love being part of it. I love seeing lives and function restored. Of course, surgery carries risks - some foreseeable, others just bad luck. For good outcomes you need a good surgeon, obviously, but also so much more. As we struggle with increasing need and limited resources, the pressure to optimize and help people recover as fully as possible, as quickly as possible, only grows. New technologies clearly play a role, but so do foundational principles. Here, I take a look at these principles through the lens of Enhanced Recover After Surgery (or ERAS), a set of guidelines used around the world, across many specialties. I am joined by researcher Emily Pearsall, general surgeon Anand Govindarajan, and anesthesiologist Stuart McCluskey. Recorded separately, their narratives weave together and paint a picture of Enhanced Recovery: what it is, what it is not, and what can be learned from years of trying to bring its recommendations to life in the complex reality of caring for patients. This has been a topic that we have wanted to highlight for a long time so we are grateful to our guests for coming on the show and giving us an understanding of what is happening, why the numbers are going up, the heart-breaking disparities for black and brown mothers, and the innovative and equitable solutions that are being implemented at ATLANTIC HEALTH OVERLOOK HOSPITAL and across the country.  Although this is a difficult topic to discuss, the good news is that the more awareness we can bring to the issues, the more we can all work towards a long term solve for this.  About our GuestsDr. Megan Rosenstein is the Associate Chief Medical Officer and Director of Obstetric Anesthesia at Atlantic Health System's Overlook Medical Center in Summit New Jersey.   Dr. Rosenstein came to Overlook from New York Presbyterian Weill Cornell Medical Center, where she completed her training in Anesthesiology with a focus on the care of high risk perinatal patients.  In her time at Overlook, Dr. Rosenstein has actively participated in Atlantic Health's system-wide Maternal Collaborative, Overlook's Perinatal Safety and Quality Committee and spearheaded projects such as the Massive Transfusion Protocol, Opioid Reduction Task Force, protocoled response to Amniotic Fluid Embolism, and Enhanced Recovery after Cesarean Birth.  Dr. Rosenstein is the New Jersey state representative for the Society for Obstetric Anesthesia and Perinatology, which recognized Overlook as one of the first designated centers of excellence in Obstetric Anesthesia care.  Dr. Rosenstein is a wife and mother to three sons and enjoys weekends on the sidelines and enjoying time with her family.  She is an avid supporter of equitable access to women's healthcare in New Jersey and beyond.  Isabel Pierre-Louis, BSN, RNC-OB, is a career-long advocate of equitable, accessible, high-quality healthcare for all. Ann & Robert H. Lurie Children's Hospital of Chicago and Northwestern University are leading an innovative study to evaluate ways to improve recovery and outcomes in children undergoing elective gastrointestinal surgeries. Such protocols have been found to decrease hospital length of stay and lower costs and complications among adult surgical populations. But evidence for pediatric populations is lacking. This episode features Mehul Raval, MD, MS, FAAP, FACS, a pediatric surgeon and the principal investigator on a $3.5 million National Institutes of Health-funded ENRICH-US study (Enhanced Recovery In Children Undergoing Surgery). The study includes 18 U.S. pediatric hospitals around the country. Dr. Raval is an expert in general pediatrics surgery and surgical oncology at Lurie Children's and discusses the study as well as some of its preliminary findings.

The opioid epidemic continues to be problematic across the country with increases in deaths from addiction leading to overdose. In an effort to minimize use of opioids postoperatively, the ERAS society developed guidelines and protocols pre-,intra-, and postoperatively to enhance recovery from surgical procedures while limiting use of opioids. Pharmacists play an integral part in the development of individual  hospital protocols for ERAS, monitoring patients postoperatively, and preparing appropriate and individualized regimens for pain management on discharge. Episode 9: Role of Regional Anesthesia and Analgesia in Enhanced Recovery After Colorectal Surgery

In surgical care worldwide, there has been a growing trend towards enhanced recovery with the emphasis on early mobility, avoidance of unnecessary procedures and drug-induced side effects, and faster return of normal gastrointestinal function, with the end result of shorter hospital stay. It's natural to assume that regional anesthesiology and acute pain medicine would play a central role in enhanced recovery protocols for all of the well-described benefits. However, the base evidence is continually evolving as both anesthetic and surgical practices change. In this month's RAPM Focus, Associate Editor Edward Mariano, MD, joins Kariem El-Boghdadly, MBBS, BSc, FRCA, EDRA, lead author of an international collaboration in the regional anesthesiology and enhanced recovery world, “Role of regional anesthesia and analgesia in enhanced recovery after colorectal surgery: a systematic review of randomized controlled trials,” first published in March 2022 (https://rapm.bmj.com/content/47/5/282). Investigators carefully evaluated thirteen published randomized controlled trial studies involving 1,170 participants and concluded that regional analgesia may only have modest evidence to support its inclusion in enhanced recovery protocols involving colorectal surgery patients. Dr. Kariem El-Boghdadly is a consultant anesthetist at Guy's and St. Thomas' NHS Trust in London, and an editor of the journal Anesthesia. Dr. El-Boghdadly completed his undergraduate training at University College of London in 2007 and proceeded to undertake his anesthetic training in Southeast School of Anesthesia. He was awarded the Nuffield Prize for the fellowship of the Royal College of Anaesthetists and completed fellowships in prehospital medicine in Australia, followed by regional anesthesia in Toronto, Canada. Dr. El-Boghdadly is an honorary senior lecturer at King's College London, and his research interests include regional anesthesia, airway management, and research delivery. He is a brilliant researcher and writer and is well-known for his work in the COVIDSurg Collaborative during the ongoing pandemic, and the ASRA-ESRA regional anesthesia nomenclature project. *The purpose of this podcast is to educate and to inform. Dr Mike Scott  MB ChB FRCP FRCA FFICM Professor of Anesthesiology and Critical Care Medicine at the Hospital of the University of PennsylvaniaDivision Chief Surgical & Neuroscience Critical Care Medicine Medical Director, PENN E-LERT Telemedicine ICU Program University of Pennsylvania Biography Dr. Mike Scott undertook his undergraduate medical training in the UK and Postgraduate training in the UK and Australia. He trained in internal medicine before doing anesthesiology and critical care medicine and has Fellowships with the Royal College of Physician's, Royal College of Anaesthetists and Faculty for Intensive Care Medicine. He was one of the first clinicians to adopt Enhanced Recovery after Surgery (ERAS) principles in 2001 at the Royal Surrey County Hospital in Guildford UK and has championed it ever since. He was appointed a National Clinical Advisor in Enhanced Recovery for NHS Improvement in 2012. Mike has a keen research interest and has been investigator / Chief Investigator for multiple studies in analgesia, haemodynamic monitoring, fluid therapy and the stress response for surgery within an ERAS Protocol. He advocates opioid sparing analgesia and hemodynamic monitoring including bedside ultrasound as a standard of care. He was lead author of the Perioperative Quality Initiative for opioid sparing analgesia and recently co-author of the international multidisciplinary consensus statement on the prevention of opioid-related harm in adult surgical patients. Dr. Scott is President of ERAS USA and co-author for some of the ERAS Society Consensus Guidelines, Chair of the ERAS Education Committee and is on the ERAS Society Executive Committee. Mike has lectured extensively around the world and has published clinical trials, editorials and review papers. He is Senior Editor for Anesthesia and Analgesia. Dr. Scott moved to the USA from the UK at the end of 2016 to take up the position of Medical Director for Clinical Effectiveness and Division Chief of Critical Care medicine at Virginia Commonwealth University Health System, Richmond, Virginia. He moved to take up the position of Division Chief at the University of Pennsylvania at the beginning of 2020 just in time for the COVID pandemic! On this episode of the Painless Pain Management Podcast, I interview Wyatt Scott, muscle activation technique specialist, certified strength and conditioning specialist, and corrective exercise specialist and has a degree in Kinesiology from James Madison University. According to Muscle Activation Techniques, Inc, MAT goes through 77 Range of Motion Exams and 214 Muscle Tests to help practitioners identify which muscles are not “firing” (receiving and/or acting on the brain's signal). Once the cause for the pain and impaired mobility is identified, practitioners begin a series of muscle tests in an established process and work to “reactivate” the affected muscles. Other methods attempt to relax the muscle to lengthen or change it by stretching, heating, kneading, or foam rolling. In contrast, MAT activates your muscle so that it will become better prepared to handle the force applied to it through everyday movements and exercise.
