In this presentation we cover the historical development and implementation of Enhanced Recovery After Surgery (ERAS); key milestones from Henrik Kellett's pioneering work in the late 1990s, to the formation of the ERAS Study Group and subsequent international efforts. Also, the differences in implementation between countries, particularly the coordinated efforts in the UK through the NHS and the more fragmented approach in the USA. There's an emphasis on the importance of protocol adherence, auditing, and feedback and barriers to implementation such as lack of agreement with evidence, organizational challenges, and institutional hurdles. Presented by Timothy Miller, Professor of Anesthesiology at Duke University Medical Center. Timothy completed his anesthesia training in Nottingham, UK followed by a fellowship in cardiothoracic anaesthesia at Glenfield Hospital, Leicester, UK. He is a fellow of the Royal College of Anaesthetists. He then joined the Department of Anesthesiology at Duke University Medical Center. His clinical interest is management of the high-risk surgical patient undergoing non-cardiac surgery. His practice is varied and involves general, vascular, major urology and thoracic anesthesia, as well as anaesthesia for transplantation. He founded and is co- director of the Enhanced Recovery After Surgery (ERAS) program at Duke.

In our sixth and final episode of the Fire in the Southwest Series—sponsored by the Southwest Fire Science Consortium and the Arizona Wildfire Initiative—we explore the complex, multicultural fire histories and management dynamics in New Mexico, with State Forester and Tribal Liaison Lindsey Quam.New Mexico's recent relationship with fire has been fraught with distrust in the aftermath of the 2022 Calf Canyon Hermits Peak Fire, which started from an escaped prescribed fire and an escaped pile burn. Lindsey's career has been bookended by such events, having started his career in Los Alamos, NM in the aftermath of the 2000 Cerro Grande Fire, which also started as an escaped prescribed fire. This has allowed him to witness first hand how public trust has ebbed and flowed around the use of prescribed fire. It's also presented opportunities for him to help shape and better understand the many mixed emotions New Mexicans have around this topic.Lindsey spoke to how the intersecting cultures and management values across New Mexico—including Indigenous peoples, the Hispanic population and, well, white people—presents challenges but also opportunities in trying to extoll the merits of prescribed fire."There's no dispute amongst native New Mexicans who live off the land—there is a recognition that fire is important and necessary, but there's also a fear," Lindsey, who is himself a member of the Zuni Pueblo, said.Lindsey also shared how his agency is scaling up forest treatments through collaboration, and establishing priority landscapes to implement landscape-scale resilience projects.If you'd like to learn more about acequias, which are mentioned in this episode, I can't recommend Patrick Lohmann's reporting enough. They interview two experts on the region. Scott Dawson, whose family traces its roots back to the 1600s on Hatteras, has devoted countless hours to researching the language and culture of the Croatoan people and their early encounters with English settlers. An amateur archaeologist and historian, he has identified artifacts and produced word lists of the Carolina Algonquian spoken on Hatteras Island when the first English settlers arrived in the 16th century. He credits the efforts of English scientist and polymath Thomas Harriot (Hariot) working with Croatoans Manteo and Wanchese for much of what's known today about the indigenous residents, their culture and language. Linguistically, sociolinguist Prof. Walt Wolfram, William C. Friday Distinguished University Professor at N.C. State, places the dialects spoken on the Outer Banks islands as part of the Tidewater diaspora. For example, the brogue spoken on Ocracoke, he says, is closer to that of Tangier Island in the Chesapeake Bay than to that of nearby mainland communities. A scholar of language variation, Wolfram contrasts the vibrancy of the distinctive English dialect of the Lumbee in Robeson County with the rapidly declining use of Ocracoke's traditional dialect. He is also the director of the North Carolina Language and Life Project which documents dialects and speech variations. The changes in language use and dialect over 400 years in these coastal communities first tells the story of English settlement and its impact on indigenous communities. Here's our source for this week: The Ebb and Flow of Relationship When you love someone, you do not love them all the time, in exactly the same way, from moment to moment. It is an impossibility. It is even a lie to pretend to. And yet this is exactly what most of us demand. We have so little faith in the ebb and flow of life, of love, of relationships. We leap at the flow of the tide and resist in terror its ebb. We are afraid it will never return. We insist on permanency, on duration, on continuity; when the only continuity possible, in life as in love, is in growth, in fluidity - in freedom, in the sense that the dancers are free, barely touching as they pass, but partners in the same pattern. The only real security is not in owning or possessing, not in demanding or expecting, not in hoping, even. Security in a relationship lies neither in looking back to what was in nostalgia, nor forward to what it might be in dread or anticipation, but living in the present relationship and accepting it as it is now. Relationships must be like islands, one must accept them for what they are here and now, within their limits - islands, surrounded and interrupted by the sea, and continually visited and abandoned by the tides.
