Victoria: Hello Dr. Cabral, My 7 year old son was recently diagnosed with Henoch Schonlien Purpura (HSP). His blood work did not "prove", so to speak, a true diagnosis nor was he sick prior to coming down with symptoms of HSP. He has had GI issues, swollen lymph nodes, eczema, and allergies in the past. He did take a steriod to help him kick the HSP. Is there an at home test or protocol that you would recommend to see what the root cause is? I have the classic symptoms. They put me on medication for it, and it is controlling the diarrhea, but I am wondering if I will have to take medication the rest of my life or is there a cure? Melissa: I trying to adopt a more plant based diet but am concerned about lectins and oxalates. I reading a lot about how they are damaging and consuming a lot of them on a plant based diet can have bad health effects. Can you advise if this is a real issue and how to handle, or if this something not to worry about? Dionna: Hey, Dr. Cabral. First, a sincere thank you from the bottom of my heart for doing what you do. After a year and a half of being so sick, your Big 5, plus the gut health lab have changed my life! Quarterly detoxes for 1 year with a 21-day, heavy metal detox, just started the CBO protocol. Tin: I am 15 weeks pregnant and I am taking my time to learn more about prenatal and maternity health. I’m curious what are your thoughts about Oral Glucose Tolerance Test? It usually contains 75g of sugar in the solution and taken after fasting. I'm a bit concerned about this much sugar plus the colorings added to that. My OB said this is required to test for gestational diabetes but heard that test can come out false and inaccurate. It is what we as nurses are most likely to encounter in our practice.” Timestamp (TS) 03:29 “TTP can show up in cancer and also as the result of chemotherapy and medications. They say it's either a system of malignancy or a consequence of treatment, and it has also shown up in patients who are post allogenic stem cell transplants.” TS 07:05 “Once you suspect that TTP is into play, you would immediately start the patient on plasma exchange or plasma pheresis, daily or twice daily, and also on high-dose steroids. You do that until you start to see improvement in platelet counts and some of the other hemolysis markers, like LDH.” TS 11:26 “Once patients come out on the other side, nursing care involves understanding how to triage their disease: If they call you with concerns, how would you address those concerns? In this episode, we talk all about our first hematologic emergency: immune thrombocytopenic purpura (ITP).Immune thrombocytopenic purpura (ITP):Be sure to check out episode 009 on thrombocytopenia for a general approach and differential!Specific instances where there may be close to undetectable platelet count: * Lab artifact (clumping)* Very severe DIC* Thrombotic thrombocytopenic purpura - though usually higher platelets in these cases * Heparin induced thrombocytopenia (in very severe cases) - though usually higher platelets in these cases * ITP ITP: Diagnosis of exclusion How to confirm it is ITP?* Post-transfusion CBC - a repeat CBC 30-60 mins after a platelet transfusion. In ITP, the platelet count will likely not budge. (Not perfect test!)* Immature platelet fraction (if available) - this will be elevated if mature platelets are being destroyed.
