Seeing Clearly: A Pre-Clerkship's Guide to All Things Ophthalmology

In this episode of Seeing Clearly, Emaan Chaudry and Danielle Solish talk to Dr. Sapna Sharan, a pediatric and adult strabismus ophthalmologist at the Western University.  She currently holds the position of Associate Professor at the Western University and was the Director of the Undergraduate Medical Education Curriculum for Ophthalmology for 9 years. She is the mentorship lead in the Department of Ophthalmology at Schulich School of Medicine, and a part of the accreditation process, and policy-subcommittee at Schulich. Dr Sharan has undergone broad training. She graduated from medical school at Jawaharlal Institute of Postgraduate Medical Education and Research, Pondicherry University, India.  After finishing Ophthalmology residency with the National Academy of Medical Sciences, New Delhi, she fellowship trained in Pediatric Ophthalmology and Strabismus at the Save Sight Institute/Sydney Eye Hospital, University of Sydney and then at the IWK Health Centre, Dalhousie University, Halifax. Dr. Sharan is also trained in glaucoma with fellowship training at Toronto Western Hospital, University of Toronto.She has a keen interest in teaching and a special research interest in Pediatric Glaucomas. Langley, MD, MSc, FRCPCProfessor of Pediatrics and Community Health and EpidemiologyGSK-CIHR Chair in Pediatric Vaccinology, Dalhousie UniversityHead, Division of Pediatric Infectious Diseases, IWK HealthHalifax, Nova ScotiaBiography: Dr. Langley is a pediatric infectious disease physician at the IWK Health Centre in Halifax, a professor in the Faculty of Medicine at Dalhousie University, and Director of the Evaluation Group at the Canadian Center for Vaccinology. She conducts studies on the prevention of infectious diseases using vaccines, from phase 1 (first in humans) through to efficacy trials (phase 3) and post-marketing studies of how well vaccines work when they are used in immunization programs (phase 4). These studies are done with collaborators in public health, industry, universities, and non-governmental organizations. Dr. Langley has a particular interest in prevention of respiratory infections such as Respiratory Syncytial Virus and influenza. Her work also focuses on vaccine policy and evidence-based decision making in immunization programs. She is a member of the COVID-19 Science Expert Panel, the Expert Group on Health Systems of the Chief Science Advisor of Canada, a former member of the Canadian Task Force on Preventive Health Care and Canada's National Advisory Committee on Immunization (Chair, 2007-2011), and serves as an advisor on several immunization decision making expert groups. She's a maternal and women's health researcher and practicing Registered Nurse. She currently works as a travel nurse on various acute medical-surgical units across British Columbia and Nova Scotia and is also employed as a nurse on the women's surgical unit at the IWK Health Centre. She has also completed nursing work in both Zambia and Tanzania. Some of her previous awards include being named a Vanier Scholar and being recognized as one of Optimyz magazine's Top 100 Health Care Leaders for 2021. In March of this year, she completed her PhD in Nursing at Dalhousie University, where her research focused on exploring sexual health after birth using feminist poststructuralism. Rachel and I dove into what her research is all about and some pretty significant findings. Many factors contribute to children's pain being under estimated, under-recognized and inadequately treated. Kathryn Birnie, PhD shares her work identifying the top priorities essential for improvement in pediatric pain management. Join me in this episode as she shares her patient-oriented research which includes them in priority setting and recognizing the areas where we need to focus on.  Her work is grounded in empowering the disempowered; the pediatric pain patients and their caregivers, working alongside and in concert with the clinicians, making the small and big shifts in doing so especially when there is a power differential.  Dr Birnie discusses the top 10 priorities identified in pediatric pain management, how they went about doing so, especially having to distill them down to 10 from >500 identified at the start. She shares which one or ones of them should be prioritized to the top of the line, how that is relevant to each one of us caring for children, and what surprising elements, perspectives or shifts they encountered in the course of this work. For more information, check out www.birthbabysleep.ca Katherine is a Health Promotion Specialist for Child Safety Link – a children's injury prevention centre at the IWK Health Centre in Halifax, Nova Scotia. With unintentional injury as the leading cause of death in Canadian children, with injuries to children in motor vehicle collisions being at the top of this list...Katherine's work focuses on child passenger safety.  She is also the president of the Child Passenger Safety Association of Canada, a national not-for-profit that focuses on training and advocacy.I'm fortunate to have had Katherine as my trainer when becoming a Certified Child Passenger Safety Technician (aka “car seat techs”) - we are volunteers that can help you safely install your child's car seat. I've since had the pleasure of speaking with Katherine about car seat safety for children and we dove into a very important topic that comes up a lot with babies - namely how long is too long to be in a car seat and what should you do (and not do!) if your baby falls asleep in said car seat. Because we know that happens. Sometimes we go for a drive or a walk with a baby in the car seat for the sole purpose of getting them to sleep, so I totally get the struggle here… but there are some safety risks to be mindful of and we're going to dive into that a little bit. ...Mental Health MinuteI DO NOT want you to feel like you can NEVER allow your baby to sleep in the car seat. Going down this rabbit hole will only make you feel less than a great parent, but you are a great parent! We're all going to make decisions based on our own experiences, knowledge and cultural or environmental influences. As always, I want you to have all the facts so that you can make the best informed decisions you can when necessary.So, what are the FACTS?Well, The safest position for babies to sleep in is on their backs on a firm, flat surface. For babies under 1 year of age, you should always place your baby on his/her back for sleep.Babies MUST be in a certified car seat that is approved by the country of origin and they must be in the car seat at all times when travelling in a motorized vehicle. So now, how do we make decisions based on this information? Here are just a few options to give you some examples:If you are planning on going for a walk with baby with the hopes of he/she squeezing in a nap…Can you use a carrier instead?Or if you have the equipment, some stroller/seat combos have a bassinet that lays flat instead of the traditional car seat. Historically children’s pain has been poorly understood and treated, replete with misconceptions that though improving, still persist. Children deserve to be tended to with proper treatment and care. In order for them to grow healthy, responsible and happy citizens of the world, they need to be in an environment that fosters growth, learning and protects them from unnecessary suffering and illness. Children are not small adults. In this episode, I’m joined with Dr. Finley, a pediatric anesthesiologist and pain physician, to discuss what’s unique about pediatric pain and why we can’t just treat children as small adults especially when it comes to pain management.   Dr. Finley is a trailblazer in pediatric pain medicine. He is the co-leader of the ChildKind International Initiative, and has worked for over 20 years in pain research and management. He has published over 100 papers in peer-reviewed journals and has lectured widely, with more than 230 invited presentations on six continents. His own research and educational projects have recently taken him to Jordan, Thailand, China, Brazil, and elsewhere.    His main interest is pain service development and advocacy for improved pain care for children around the world, and he is co-leader of the ChildKind International Initiative.

Dr. Souvik Mitra offers insight for patient's families and caregivers as to how recent research has impacted our understanding of PDAs and what the future might hold for treatment options.Dr Souvik Mitra is a neonatologist and researcher based in Halifax, Nova Scotia. His primary research interest is improving outcomes of preterm babies with a patent ductus arteriosus (PDA), both through systemic review of current evidence and generation of new evidence through clinical research. He is the Principal Investigator of an ongoing CIHR-funded pan-Canadian study comparing the different medications used for treating a PDA, as well as the lead on several Cochrane Neonate systematic reviews on this topic. She is the Scientific Director of a national knowledge mobilization network, Solutions for Kids in Pain (SKIP). Headquartered at Dalhousie, SKIP’s mission is to improve children's pain management by mobilizing evidence-based solutions through coordination and collaboration. She is also the incoming scientific director of the Institute of Human Development, Child and Youth Health (IHDCYH) at the Canadian Institutes of Health Research (CIHR). Dr. Chambers’ research is based in the Centre for Pediatric Pain Research at the IWK Health Centre and is aimed at improving the assessment and management of children’s pain. She has published over 150 articles in peer reviewed scientific journals and was identified as one of the top 10 most productive women clinical psychology professors in Canada. Mayor Savage and his wife Darlene have two children, Emma and Conor, and continue to make their home in Dartmouth where Mayor Savage grew up and attended school before graduating from Dalhousie University
