BUFFALO, NY- January 22, 2024 – A new #research perspective was #published in Oncotarget's Volume 15 on January 16, 2024, entitled, “Bone marrow adipocytes provide early sign for progression from MGUS to multiple myeloma.” Multiple Myeloma (MM) is the second most common hematological malignancy and is characterized by clonal expansion of malignant plasma cells in the bone marrow. In spite of recent advances in the field of MM, the disease has remained incurable. MM is preceded by a premalignant state known as monoclonal gammopathy of undetermined significance (MGUS), with a risk of progression to MM of 1% per year. Establishing a scalable approach that refines the identification of MGUS patients at high risk of progression to MM can transform the clinical management of the disease, improve the patient's quality of life, and will have significant socioeconomic implications. In this new perspective, researchers Bilal M. El-Masri, Benedeta Leka, Fatima Mustapha, Michael Tveden Gundesen, Maja Hinge, Thomas Lund, Thomas L. Andersen, Marta Diaz-delCastillo, and Abbas Jafari from the Danish Spatial Imaging Consortium, University of Southern Denmark, University of Copenhagen, University of Aarhus, Odense University Hospital, and Lillebaelt Hospital provide evidence that changes in the bone marrow adipose tissue (BMAT) provide an early sign for progression from MGUS to MM. “We employed AI-assisted histological analysis of unstained bone marrow biopsies from MGUS subjects with or without progression to MM within 10 years (n = 24, n = 17 respectively).” Although the BMAT fraction was not different between the two groups, bone marrow adipocyte (BMAd) density was decreased in MGUS patients who developed MM, compared to non-progressing MGUS patients. Importantly, the distribution profile for BMAd size and roundness was significantly different between the two groups, indicating a shift toward increased BMAd size and roundness in MGUS patients who developed MM. Ali is a junior doctor who graduated from Cambridge University in 2018. Alongside his day job saving lives, he has a YouTube channel with over 200,000 subscribers and runs a business that helps students get into medical school. He also writes weekly articles about productivity, education and technology.3 Things I’ve learnedIgnore the whole legal and financial side when starting a company. You don’t actually need to worry about it until you have some amount of success and people start noticing you; by which time hopefully you are making enough revenue to deal with these issues anyway. Keep things simple and focus on the product when you get started. Start with low expectations that you can meet. Instead of getting carried away with seemingly unachievable ‘goals’, double down and focus on ’system’. ’System’ is your pipeline by which you churn out your content or get work done consistently. System is what will eventually help you reach your goals How to think about time and our use of it. It is never a case of ‘I don’t have time for this’, but rather ‘I don’t have enough motivation to do this’. We need to remember that fundamentally we all have control over our time, and it’s always a choice that we make to do what we most want to be doing at that point in time. This is incredibly empowering. We just need to be honest with ourself about our choices of how we spend our time and what matters most to us. I have been following Ali for quite some time, so it was a great time to have him on the podcast to talk about some things that I see Common Sense Medicine becoming throughout my journey in medicine. First, we talked about the video aspects of medical education. He thought it would be fun, and it would be a cool thing to start the podcast. We also talked about optimizing his productivity in medical school and tech, which he covers in his YouTube channel. We also talked about how the UK and US differ in their structure of medical school. He was able to use his passion for coding to start a business based on his score on the BMAT (the medical entrance exam in the UK).
