Skin cancer is the most common form of cancer, and melanomas are the most serious form of this disease. “And we project an increase every year of 20 to 30 percent more cases,” said Merve Hasanov, MD, a James skin cancer and melanoma expert whose research focuses on how melanomas metastasize and spread to the brain. In this episode, Hasanov describes how exposure to ultraviolet light from the sun is the leading cause of all the different types of skin cancer, and that family history is another indicator. “Sun exposure creates a cumulative risk over a lifetime and some people, with a fair complexion, are at higher risk,” she said. Precautions that reduce risk “are decreasing your exposure to the sun, using sun block and reapplying every 50 to 90 minutes and avoiding tanning beds, which use UV rays,” Hasanov said. “Wear a hat or long sleeves, and, even when it's cloudy or during the winter the UV rays are coming from the sun.” Advances in treatment have led to better outcomes for patients, especially when melanomas are detected in the earlier stages, before they have metastasized and spread to other parts of the body. Because of this, “it's now recommended that you should, once a year, get a thorough dermatological exam,” Hasanov said, and she also explained the A-B-C-D-E method of detecting skin cancer: asymmetry, border, color, diameter and evolving. Immunotherapy has been a big breakthrough for the treatment of melanomas. “Melanomas have a lot of molecular and genetic changes that can be recognized by the immune system,” she explained. “But cancer cells are smart and can shut down the immune system. But immune checkpoint inhibitors [in immunotherapy drugs] take the brakes off the immune system so the T cells better recognize and kill cancer cells.” Hasanov said that the melanomas in 10 to 12 percent of patients diagnosed in the early stages of their disease will eventually metastasize and travel to the brain. “My research focuses on detecting this earlier when we have better treatment options and survival rates,” she said. Hasanov and her lab have developed a scoring system to determine which early-stage melanoma patients are most likely to develop brain metastasizes. These patients can then be screened on a regular basis. “We're hoping that this information will lead to better guidelines and trying to spread this information to more physicians and oncologists.”

"Ja runājām konkrētāk, ļoti daudzi apzinās, ka saules aizsardzība ir vajadzīga, daudzi apzinās, ka saulē nevajadzētu būt dienas vidū. Tās ir aksiomas, ko atkārtojam. Bet ir jāiet tālāk, ir arī jāizglīto iedzīvotāji, kas ir saules ultravioletais indekss, kuram vajadzētu sekot līdzi. Varbūt arī mediji laika ziņās šajās saulainajās dienās varētu informēt, pasakot: "Šodien saules ultravioletais indekss ir augsts." Tālāk mums būtu jāizglīto, ko nozīmē augsts, ko nozīmē vidējs un kāda būtu darbība katrā no šīm situācijām," norāda Raimonds Karls. Ārsts arī norāda, ka mēs dzīvojam nelielā ilūzijā par to, ka saules pie mums nav, ka esam bāli.  "Jāsaprot, ka dienas vidū arī Latvijā saules ir daudz. Paskatieties, mēs visur esam salikuši saules kolektorus un baterijas un iegūstam gana daudz saules šajos mēnešos," norāda Raimonds Karls. Nevajadzētu plānot tagad doties uz siltajām zemēm un pavadīt laiku pludmalē no rīta līdz vakaram, un ja es nebūšu apdedzis, mans atvaļinājums nebūs izdevies. "Vēl viens liels mīts, kas mums ir, ka saule un D vitamīns ir ļoti vienāds. D vitamīna ražošana un iedegums ir pretēji proporcionāli - D vitamīns mūsu ādā veidojas, kamēr mēs vēl neesam ieguvuši iedegumu. Kad esam iedeguši, šī stacija veras ciet, jo dabīgi āda ir iekārtota tā, ka D vitamīna ražotne samazinās. Nevajag arī šo mītu izmantot kā advokātu, kāpēc es tik daudz laika pavadu saulē," skaidro Raimonds Karls. Arī jebkura darbošanās saulē - zāles pļaušana, sportošana - ir saules staru uzkrāšana.

QIMR Berghofer researchers have found that nearly a third of melanomas picked up during routine skin checks may not be harmful.The findings highlight the need for further research to determine why some melanomas appear to stay dormant and undetected for long period of time.It also may lead to more accurate diagnostic tools which might spare some people the anxiety associated with a melanoma diagnosis. Professor David Whiteman, who led the study, said the results suggest up to 29 per cent of melanomas detected during skin checks may never have come to light if that person wasn't screened.

Kas ir melanoma, kā iespējamo saslimšanu var pamanīt un kur vērsties pēc palīdzības skaidro Rīgas Stradiņa universitātes asociētā profesore, onkoloģe, ķīmijterapeite un imunoloģe Rīgas Austrumu klīniskās universitātes slimnīcā Simona Doniņa un dermatologs, biedrības "Dermatologi pret ādas vēzi" valdes priekšsēdētājs Raimonds Karls. Simona Doniņa norāda, ka noteikti jāpievērš uzmanība jebkuram ādas veidojumam, kas mainās vai parādās no jauna. Diemžēl Latvijā trūkst dermatologu, lai vienkārši profilakses pēc vērstos pie ārsta. Latvijā tāpat kā pasaulē pieaug melanomas pacientu skaits un pieaug arī to cilvēku skaits, kas mirst no ādas vēža. Tā kā viens no veidiem, kā iegūt melanomu ir saules, Raimonds Karls aicina lietot saules aizsarglīdzekļu lietošana. "Ja izvēlamies līdzekļus, ko esam iegādājušies aptiekās, tie ir droši un nekaitīgi. Saules aizsardzībai ir ļoti liela nozīme, sākot no bērna vecuma, pieaugušajiem un arī senioriem," atzīst Raimonds Karls. "Nopirkt ir pirmais solis, lietot ir otrais solis, trešais – jālieto pareizi. Jāatceras, ja no rīta, atnākot uz pludmali, uzklāju saules aizsargkrēmu, tas nenozīmē, ka esmu pasargāts visu dienu." Ārsts arī atgādina, ka arī mērķtiecīgi nesauļojoties, bet dienas vidū darbojoties ārā – dārzā vai sportojot, ir jālieto saules aizsargkrēms. Protams, var izvēlēties tieši dienas vidū neiet saulē, bet Latvijas iedzīvotāji bieži neseko šim ieteikumam. "Mēs paši sev esam radījuši mītu, ka mums ir īsa vasara, ka mums nekas no vasaras nav, mums ir zaļā ziema, bet, ja mēs noliekam kaut vai avīzi uz palodzes, mēs redzam, kas ar to notiek jau vakarā. Ja mēs saulē ejam 50, 60, 70 gadus, iedomājieties, ko āda cenšas izdarīt, lai to visu atjaunotu un šos bojājumus novērstu," vērtē Raimonds Karls. Volume 11 Issue 25 of @Oncotarget reported that this study aimed to define the mutation profile of SUM in Caucasians. Next-generation sequencing-based genomic analysis was used to identify frequently mutated loci in 50 cancer-related genes in 31 SUM primary tumors. The most abundant mutations in SUM were found in KIT – in 13% of cases and NRAS – also in 13%, while BRAF - only in 3% of cases. The authors' findings confirmed a high frequency of KIT and NRAS mutations in SUM, as well as a low incidence of BRAF mutations. They reported novel KRAS, CTNNB1, TP53, ERBB2, and SMAD4 mutations in SUM. Dr. Aneta Borkowska from The Maria Sklodowska-Curie National Research Institute of Oncology said "Across all human cancers, cutaneous malignant melanoma (MM) genome has one of the highest prevalence of somatic mutations." At the same time, NRAS mutations are detected in approximately 20% of MM and are more commonly reported in melanomas developing in the skin with chronic sun exposure. WHO Classification of Skin Tumours recognizes the most common acral melanoma histotype is acral lentiginous melanoma, followed by nodular cutaneous melanoma and superficial spreading melanoma. Cutaneous MM located on the acral part of extremities - hand and foot melanoma - comprises a rare group within all melanomas in Caucasians. Whole-genome sequencing study shown that structural changes and mutational signature of acral melanomas were dominated by different than other MMs sites. SUM seems to be not related to sun exposure, however, in Australian Melanoma Genome Project UVR signatures on acral melanomas occurred most frequently in subungual parts. The Borkowska Research Team concluded in their Oncotarget Research Paper, "Our study offers new insights into the genetics SUM subtype, for understanding pathogenesis and providing potential biomarkers for future studies. Molecular testing is now widely used in patients with advanced melanoma in the process of therapeutic decisions. Mutations reported in melanoma cells provide starting points for the development of the rational design of novel therapies, including immunotherapy agents. They also may provide to find the molecular pathogenesis and natural history of subtypes of this heterogeneous disease. We confirmed that SUM have different than other cutaneous melanomas genetic profile, which due to its rareness and lack of studies should be subjected to further analyzes in multicenter studies." Katru gadu vairākiem simtiem Latvijas iedzīvotāju tiek atklāts ļaundabīgs ādas audzējs – melanoma. Šīs slimības pacientu atbalsta biedrība „Soli priekšā melanomai” rīko izglītojošu kampaņu "Balts arī ir skaisti". Melanomas pacientu atbalsta biedrības "Soli priekšā melanomai" dibinātāja Sondra Zaļupe norāda, ka maijs ir izsludināts par melanomas mēnesi un biedrība kopā ar dermatolgiem informēs, ka cilvēkiem sevi pasargāt. Biedrības mājaslapā pieejams jaunās kampaņas buklets, kā arī būs vēl papildus informācija, kā sevi pasargāt šajā laikā, kad daudz laika pavadām brīvā dabā, dārzā vai pastaigās. Sondra Zaļupe mudina katram pašam sekot līdzi veidojumiem, kas parādās uz ādas, sargāt sevi no saules, neuzturēties tiešos saules staros, izvēlēties apģērbu ar garām piedurknēm vai starām, nesāt cepuri un saulesbrilles, kā arī lietot saules aizsargkrēmus. Dermatologs Raimonds Karls norāda, ka melanoma uz ādas var attīstīties arī tādās vietās, kas sauli nemaz nav redzējušas. „Nedzīvosim ilūzijā, ka Latvijā nav saules. Latvijā saule ir, ir gana daudz saules. Tas, ka ir vēss, tas nav ekvivalents ultraviolētam starojumam, tas, kas ir vēss, nenozīmē, ka nav ultravioletā starojuma. Nevajag likt vienādības zīmi starp gaisa tempereratūru un ultraviolēto indeksu," skaidro Raimonds Karls. Tāpat ārsts norāda, ka arī dūmakainā laikā ir saule un var just ultraviolēto staru ietekmi.

Melanoma ir ļaundabīgs ādas audzējs, kurš Latvijā ik gadus skar aptuveni 200 cilvēku. Par tā izplatības tendencēm, jaunākajām diagnostikas un ārstēšanās iespējām, kā arī profilaksi runājam raidījumā Kā labāk dzīvot. Raidījuma viesi: dermatologs, Latvijas Dermatoskopijas biedrības priekšsēdētājs Raimonds Karls, ķirurgs onkologs Latvijas Onkologu asociācijas valdes loceklis Guntis Ancāns, medicīnas doktore onkoloģe imunoloģe Simona Doniņa un melanomas pacientes - Sondra Zaļupe un Olga Valciņa. Sondra Zaļupe un Olga Valciņa ir nodibinājušas melanomas pacientu biedrību. Šobrīd ir izzveidota mājas lapa “Soli priekšā melanomai”, kurā aicina anonīmi aizpildīt anketu, kas palīdzētu saprast, cik ir saslimušo un ko kopā var paveikt. Biedrība vēlas veidot tematiskus informatīvus pasākumus kopā ar profesionāļiem, kas var palīdzēt cilvēkiem, kas saslimuši ar melanomu, kas var skaidrot situāciju pacientu līdzcilvēkiem. Biedrības darbības pamatprincipi būs – pacients ir pirmajā vietā, risinājumi, nevis problēmas, dati un fakti, nevis viedokļi un kurš tad, ja ne es. Ar tiem plašāk var iepazīsties arī mājaslapā. Speciālisti atgādina: Latvijā pēdējos 5-7 gadus melanomas slimnieku skaists ir virs diviem simtiem. Problēma ir aktuāla, jo valstī melanomu parasti atklāj vēlīnā stadijā, tas nozīmē sliktāku prognozi ārstēšanai. Galvenie riska faktori: gaiša āda un esam pakļauti ultravioletajam starojumam, gan dabīgam, gan mākslīgam. Āda jāpārbauda agrīni un savlaicīgi. Jo agrāk atklāj ļaundabīgu veidojumu, jo labāk. Viena no pazīmēm, ko min, kas varētu liecināt par melanomu, ir atipiskas dzimumzīmes. Raimonds Karls norāda, ja cilvēkam ir daudz pigmentēto veidojumu uz ādas, tie jāpārbauda, tipisks vai atipisks, lai ar to nodarbojas ārsts. Ādas vēža attīstībai ir divi scenāriji: iepriekš ir bijusi dzimumzīme, kas sākusi mainīties, bet tie ir 15-20% visu gadījumu. Visbiežāk audzējs sākas kā jauns veidojums, kas ar laiku pārvēršas, jo ilgāks laiks paiet, jo aktīvāk audzējs aug. Ārsti reģistrē ap 220 melanomas pacientu gadā, diemžēl zaudē katru gadu ap 90 cilvēku. Bieži melanomu diagnosticē samērā jauniem cilvēkiem. Viņi kūtri apmeklē ārstu, veselībai pievērš mazāk uzmanības. Slimību galvenokārt iegūst dzīves laikā, pārmantojamība ir reta.

Eleven uveal melanomas were analyzed using comparative genomic hybridization (CGH). The most abundant genetic changes were loss of chromosome 3, overrepresentation of 6p, loss of 6q, and multiplication of 8q. The smallest overrepresented regions on 6p and 8q were 6pterp21 and 8q24qter, respectively. Several additional gains and losses of chromosome segments were repeatedly observed, the most frequent one being loss of 9p (three cases). Monosomy 3 appeared to be a marker for ciliary body involvement. CGH data were compared with the results of chromosome banding. Some alterations, e.g., gains of 6p and losses of 6q, were observed with higher frequencies after CGH, while others, e.g., 9p deletions, were detected only by CGH. The data suggest some similarities of cytogenetic alterations between cutaneous and uveal melanoma. In particular, the 9p deletions are of interest due to recent reports about the location of a putative tumor-suppressor gene for cutaneous malignant melanoma in this region.
