When a patient talks about their sexual dysfunction, I always ask: what's going on with the person they're having sex with? You'd be surprised how many providers don't.Most people don't think to ask what's happening on both sides of a sexual relationship. But sexual dysfunction doesn't happen in a vacuum—it's a two-way street. In this episode, I'm joined by Dr. Jessica Yih, a urologist who treats both men and women, and we get real about the overlap of issues that show up in the bedroom. We dive into Peyronie's disease and why it's been overlooked for so long, how injections can break down plaques, and why those curves aren't just uncomfortable—they can lead to serious problems for both partners. We also break down erectile dysfunction beyond the “just pop a blue pill” myth. Spoiler: Viagra doesn't work for everyone.Jessica brings a powerful perspective to the conversation, especially around what it's like treating both members of a couple, sometimes in back-to-back appointments. She's also helping train the next generation of sexual health urologists through her fellowship at UC Irvine.Sex is more than individual dysfunction—it's communication, treatment access, aging, and understanding both bodies in the room. If you're a provider, a patient, or just curious about how these dynamics work together, this episode is for you.Highlights:The truth about Peyronie's disease, painful sex, and treatment optionsWhy Viagra isn't a one-size-fits-all fix for erectile dysfunctionHow injectable medications and implants are changing the gameDyadic sexuality and the importance of treating both partnersJessica Yih's unique path to sexual medicine and her work training new fellowsIf this episode got you thinking, please hit that follow button, leave a comment, and give us a review on Apple Podcasts or Spotify. Imagine that your genitals were in a constant, uncontrollable state of arousal. You might be thinking that doesn't sound too bad, but you'd be mistaken. Persistent genital arousal disorder (PGAD) is a highly distressing condition. People who have it live with unwanted arousal that doesn’t bring pleasure, and there's almost no relief from it. This makes it difficult, if not impossible, to concentrate, work, and sleep. In this episode, we're going to dive into what we know about PGAD, including how common it is, what causes it, and how it's treated. I am joined once again by Sue W. Goldstein, an AASECT certified Sexuality Educator and Clinical Research Manager at San Diego Sexual Medicine. Ms. Goldstein co-authored the book When Sex Isn't Good to provide education and empowerment to women with sexual dysfunction. Some of the questions we explore in this episode include: What does PGAD look like in everyday life? What kind of effects does this have on people? How long have we known about PGAD, and how common is it? What causes uncontrollable genital arousal? Are there any treatments that work? Antidepressants are notorious for their sexual side effects, including diminished sexual desire and difficulties with arousal and orgasm. It was long thought that these side effects were temporary; however, a growing number of people are reporting persistent sexual difficulties after discontinuing the medication, known as Post-SSRI Sexual Dysfunction (PSSD). In today’s show, we're going to discuss what we know about PSSD, including how common it is, what causes it, and what we can do about it. I am joined by Sue W. Goldstein, an AASECT certified Sexuality Educator and Clinical Research Manager at San Diego Sexual Medicine. Ms. Goldstein co-authored the book When Sex Isn't Good to provide education and empowerment to women with sexual dysfunction. Some of the questions we explore in this episode include: How many people experience sexual side effects from antidepressants? What are the most common symptoms of PSSD? Is PSSD a formal medical diagnosis? Why is PSSD controversial? Are there any gender differences in PSSD? Why might SSRIs can cause long-term sexual dysfunction? Dr. Irwin Goldstein, the founder of field of sexual medicine, joins me in the conversation about female sexual pain. He drives the talk with tons of fascinating information about sexual pain, including what are the different categories, common causes, and treatment options. The prevalence of female sexual pain Within the last month, 1/3rd of women reported experiencing sexual pain or some form of discomfort during sex, while only 2% to 7% of men reported sexual dysfunction or secondary pain. He urges women to ensure they find the correct medical professional and find answers to their questions as he has found many women go untreated due to misdiagnosis. Dr. Goldstein best categorizes various kinds of sexual pain by the area it originates. The pain in the vulva is diagnosed as vulvodynia. However the vestibule is often overlooked as the source of pain, and more than 90% of the time is misdiagnosed as vulvodynia. Hormonally Mediated Vestibulodynia Dr. Goldstein warns against birth control pills as they have harmful side effects that can eventually affect your sex life. He urges women to consider other birth control methods like Long-acting reversible contraceptives (LARC) – IUDs, Nexplanon and Implanon contraceptive implants, and progesterone. He further informs that The American College of Obstetricians and Gynecologists and The American Academy of Pediatrics no longer consider birth control pills as the leading method of contraception. Causes in Older Women and Treatment Options For older women over 40, the hormonal challenges of menopause are a leading cause of pain. He mentions that women go through two stages of menopause, where the first one causes low testosterone levels and the latter causes low estrogen levels. He shares available treatment options for this. Other Common Causes and Treatment Options Among other causes, Dr. Goldstein talks about Neuroproliferative vestibulodynia, a condition where women suffer from life-long pain. Monistat is the number one medicine women use that causes neuroproliferative vestibulodynia. The only treatment option available is surgically removing the vestibule, which has an 80% cure rate and is completely non-disfiguring. Tune in for valuable advice that can make a huge difference in your life. Background Dr. Goldstein has been involved with sexual dysfunction research since the late 1970s. He has authored more than 350 publications as well as multiple book chapters and edited 6 textbooks in the field. His interests include penile microvascular bypass surgery, surgery for dyspareunia, sexual health management post-cancer treatment, genital dysesthesia/persistent genital arousal disorder, physiologic investigation of sexual function in men and women, and diagnosis and treatment of sexual dysfunction in men and women. Sexual pain, postpardum, perimenopause and menopause care, post-cancer survivorship. These are all powerful areas that impact so many women's lives, and yet we don't have spaces of support in the medical industry or society at large. Dr. Heather Quaile aims to change that. Sexual health is an area that needs to opened up on multiple levels, from healthcare to legislation, and this is where Heather does her work. Viewing sexual pain from a bio/psycho/social approach, she believes in trauma-informed care that comes from a multitude of providers, including allopathic doctors and functional practitioners. We talk about specific instances of pain, including in menopause and with vulvodynia, and how we need to take a multi-pronged approach is supporting healing. We also chat about how important it is to educate girls on their bodies to change the whole paradigm, so that when they are women, they will experience less pain. Learn about Heather's work in legislation around women's health happening in Georgia (which mirrors a lot of what is going on nationwide in the US), and well as educating about trans rights. We also dive into the $150 billion sex trafficking industry, what Heather is doing to fight it, and how each of us can, too. This interview covers SO MUCH of what is important in women's health today. It'll connect some major dots for you in your own sexual health journey. Gospel Scripture: (all NIV) Romans 3:23 “for all have sinned and fall short of the glory of God,” Romans 3:24 “and are justified freely by his grace through the redemption that came by Christ Jesus.” Romans 3:25 (a) “God presented him as a sacrifice of atonement, through faith in his blood.” Hebrews 9:22 (b) “without the shedding of blood there is no forgiveness.” Romans 5:8 “But God demonstrates his own love for us in this: While we were still sinners, Christ died for us.” Romans 5:11 “Not only is this so, but we also rejoice in God through our Lord Jesus Christ, through whom we have now received reconciliation.” John 3:16 “For God so loved the world that he gave his one and only Son, that whoever believes in him shall not perish but have eternal life.” Romans 10:9 “That if you confess with your mouth, “Jesus is Lord,” and believe in your heart that God raised him from the dead, you will be saved.” Luke 15:10 says “In the same way, I tell you, there is rejoicing in the presence of the angels of God over one sinner who repents.” Romans 8:1 “Therefore, there is now no condemnation for those who are in Christ Jesus” Ephesians 1:13–14 “And you also were included in Christ when you heard the word of truth, the gospel of your salvation. Having believed, you were marked in him with a seal, the promised Holy Spirit, who is a deposit guaranteeing our inheritance until the redemption of those who are God's possession- to the praise of his glory.” Ephesians 1:15–23 “For this reason, ever since I heard about your faith in the Lord Jesus and your love for all the saints, I have not stopped giving thanks for you, remembering you in my prayers. I keep asking that the God of our Lord Jesus Christ, the glorious Father, may give you the spirit of wisdom and revelation, so that you may know him better. I pray also that the eyes of your heart may be enlightened in order that you may know the hope to which he has called you, the riches of his glorious inheritance in the saints, and his incomparably great power for us who believe. That power is like the working of his mighty strength, which he exerted in Christ when he raised him from the dead and seated him at his right hand in the heavenly realms, far above all rule and authority, power and dominion, and every title that can be given, not only in the present age but also in the one to come. And God placed all things under his feet and appointed him to be head over everything for the church, which is his body, the fullness of him who fills everything in every way.” Ephesians 2:8–10 “For it is by grace you have been saved, through faith – and this not from yourselves, it is the gift of God – not by works, so that no one can boast. For we are God‘s workmanship, created in Christ Jesus to do good works, which God prepared in advance for us to do.“ Ephesians 2:13 “But now in Christ Jesus you who once were far away have been brought near through the blood of Christ.“ Philippians 1:6 “being confident of this, that he who began a good work in you will carry it on to completion until the day of Christ Jesus.”

You're woken up every single day by your man's erection. Does this mean that he is more aroused in the morning?Sharing a bed with your man can be great... that is until your daily wake-up call comes from between your sheets and not the alarm on your nightstand. You may think your guy just had a very intimate sexual fantasy in his sleep and is looking for a way to have it satisfied. This is not always the case.Did you know that morning wood has nothing to do with sexual desire?Actually, it happens when your man is in his REM (rapid eye movement) sleep cycle. During REM, your body turns off some neurotransmitters that regulate body functions. One of these neurotransmitters is in charge of stopping erections.REM decreases norepinephrine levels, and testosterone is free to pump as much blood into the penis, causing an erection. This doesn't mean that men aren't aroused immediately when they wake up. Each person is different; some men and women may be aroused in the morning while others get randy later in the day.Director of San Diego Sexual Medicine, Irwin Goldstein, MD, joins Melanie Cole, MS, to explain why morning wood happens and if it means men are more sexually aroused than women.
