I am excited today to connect with my friend and colleague, Jackie Piasta. Jackie is Vanderbilt-trained and has been board-certified as a women's and gender health nurse practitioner. She has been practicing since 2010. She serves on medical advisory committees for several foundations and co-hosts the justASK Podcast.  In our conversation, we dive into the challenges and complexities surrounding menopausal hormone replacement therapy and FDA-approved formulations, looking at the minute percentage of women currently on menopausal hormone replacement therapy. We discuss supraphysiologic-dosing of hormones and lab work strategies regarding prescribing lifestyle, and Jackie clarifies how she differentiates hypoactive sexual disorder from low libido and dysfunctional uterine bleeding from early menopause. We explore IUDs, ablations, and other long-term bleeding therapy options, explaining how endometriosis can impact the onset of menopause. Jackie also shares her approach to managing patients who have had ablations or are using IUD therapies and not getting regular menstrual cycles, helping them to determine when they transition into menopause. This conversation is full of valuable insights, and we look forward to having Jackie back for a further deep dive into this pivotal stage of women's health. Her perspective on the systemic, normalized abuse and mistreatment of childbearing people is aligned with advocacy workers, interdisciplinary scientists, and all interested and affected parties working to end obstetric violence and achieve birth equity.   Dr. Brie Thumm is an Assistant Professor at the University of Colorado College of Nursing. She has been practicing midwifery domestically and internationally since 2001 when she completed her Masters in the Science of Nursing at Yale University. She obtained her MBA in Healthcare Administration at Baruch College in New York City and her PhD in health systems research at University of Colorado College of Nursing. Her area of research is perinatal workforce development to address disparities in maternal health outcomes and improve the well-being of health care professionals. Prior to her current position, Brie provided care at Planned Parenthood of New York City, served as the Assistant Director of the Sexual Assault Response Team for the Manhattan public hospitals, conducted mental and behavioral health research at the Rocky Mountain Regional Veteran's Affairs Medical Center, and led the clinical and research arms of the Maternal Mortality Prevention Program at the Colorado Department of Public Health and Environment. Anne Judge, DNP, WHNP-BC, ARNP is a Nurse Practitioner at Seattle Reproductive Medicine (SRM). Anne has worked at SRM since 2007 and has both a personal and professional connection to the field of reproductive medicine. She has a Bachelor's degree in Human Biology from Stanford University, and a Bachelor's degree in Nursing from the University of Washington, where she completed an honors research project on paternal infant attachment. Passionate about women's health, Anne initially worked as a labor and delivery nurse, until her personal experiences with infertility inspired her to make a career change into reproductive medicine. Anne obtained her graduate degrees from Frontier Nursing University. She obtained a Master's and then Doctor of Nursing Practice degrees as a Women's Health Nurse Practitioner in 2016/2017, with a doctoral project on fertility care for patients with obesity. Her other professional interests include the psychological components of infertility and male factor infertility. Anne also supports outreach efforts to increase community awareness of fertility assessment and fertility preservation. She is one of the creators and co-hosts of The Whole Pineapple Podcast, which focuses on wellness and fertility. Ruby Booras is a board-certified Family Nurse Practitioner at Seattle Reproductive Medicine. She received her BSN from Montana State University and her DNP from the University of Washington. Ruby is a board-certified American College of Lifestyle Medicine Diplomat and is the creator/co-host of The Whole Pineapple podcast, which focuses on fertility and whole-person wellness. One thing that makes Ketura'h special is that in the past year, Ketura'h trained to become a birth doula and is now serving clients in her community with Accompany Doula Care, a local doula agency that has developed a model of care where their doulas are reimbursed by the client's health insurance, which is helping address health disparities.Ketura'h  has served as a women's health nurse practitioner at the Suffolk county jail, a Primary Care clinical care manager, OB-GYN nurse and even a pediatric nurse. She also has experience providing acute treatment for patients experiencing substance use disorders and is a Sexual Assault Nurse Examiner for the Massachusetts Department of Public Health.During today's conversation, Ketura'h explains what a nurse practitioner is and how their expertise and scope of practice differs from other clinicians and she describes her path to becoming an NP.  She talks about what she loves about being a nurse practitioner and what aspects of OB/GYN care she does and doesn't do.We talked with Ketura'h about her her training in breastfeeding and infant feeding and what led her to become a birth doula (and how she manages being a clinician while attending births as a doula).We end our discussion with her providing listeners with several final words of wisdom for pregnant listeners.
