When our bodies are physically changed because of breast cancer, it has the real potential of affecting sexual health. There are multiple layers to look at when a person presents with sexual dysfunction. In other words, the natural process of the act of sex has been disrupted due to treatment, surgery, the emotional impact of cancer, and other considerations. On this episode of the Urology Care Podcast, we sat down with two guests on the topic of female sexual dysfunction. Our first guest is Dr. Suzette Sutherland, the director of female urology at the University of Washington in Seattle. She has focused her 20 year career on women's pelvic health to include urinary incontinence, bladder dysfunction, pelvic prolapse surgery and female sexual health. Thanks for tuning in to the Armor Men's Health Hour Podcast today, where we bring you the latest and greatest in urology care and the best urology humor out there.In this segment, Dr. Mistry and Donna Lee discuss Votiva, an FDA-approved device that uses radio frequency (RF) energy waves to heat and stimulate the cells of the vagina and vulva (external skin), causing micro trauma. This increases collagen and elastin fiber production. As a result, vaginal moisture, blood flow and elasticity increases. This episode also addresses female sexual dysfunction, which can be caused by a number of things that all result in women avoiding sexual intimacy. The four main issues experienced by women with female sexual dysfunction are issues of interest, arousal, orgasm, and pain. While the first three of these issues can be overcome by "faking it until you make it," the fourth--pain--is a real intimacy killer that can be difficult to get past for both women and their partners. Increasing the health of the vagina can go a long way towards addressing each of the four issues mentioned above, many of which can impact each other, as when vaginal dryness (caused by lack of arousal) increases vaginal pain and vice-versa. Apply principles of history and physical examination to the woman with sexual dysfunction. 3. Characterize the consensus paradigms for identification of sexual problems in women, hypoactive sexual desire disorder, and persistent genital arousal disorder developed by the International Society for the Study of Women’s Sexual Health. 4. Diagnose and treat woman with sexual dysfunction presenting in a community setting with FDA-approved and off label medical therapies including estrogen (and progesterone) and androgen therapies as well as centrally-acting drugs. 5. Identify when sexual pain should be treated surgically.
