Their noninvasive platform changes the protocol for assessing a suspicious mole from visual review and surgically removing tissue to genomic analysis and using broader objective data before treatment is determined.  John elaborates, "Genomics is fundamentally changing the way medicine is practiced, and it is particularly changing cancer care and how cancer is detected and treated. We're really bringing that revolution to the dermatologist. We do that with our noninvasive genomics platform. We have something called the Smart Sticker, which is a special adhesive patch that can collect the sample of the skin noninvasively. And it feels a lot like scotch tape, you just press it on the skin, and you lift it off. And from that skin sample, the tissue we've collected, we can run a variety of genomic analyses, and that allows us to determine whether or not cancer might be present in a mole, for example." "Our first product in the market is specifically for melanoma skin cancer. That's the most deadly of skin cancers. It's the most concerning skin cancer. It's a real problem. Doctors don't want to miss melanoma because it can lead to higher mortality in a patient. But melanoma skin cancer can also be cured very effectively if it's found early. Our program is affiliated with UCLA, and it's sponsored by Clinica Sierra Vista, Let Us Be Your Healthcare Home.What is onychomycosis?-Onychomycosis is a fungal infection that resides in the finger and toenails. The nails become discolored, have onycholysis (painless separation of nail bed), splitting of nail bed, thickened. There are various causes of onychomycosis and examples include the following: dermatophytes, yeast, non-dermatophyte molds.-Onychomycosis occurs in 10% of the general population. Microbiology:Dermatophytes such as Tinea rubrum, account for most onychomycosis infections (~60-70%) while candida account for most of yeast causes of onychomycosis. Non-dermatophyte molds include fusarium, aspergillus, acremonium, scytalidium, Scopulariopsis brevicaulis. The type of organism involved has an association for the type of infection it causes. Yeast infects fingernails preferentially while the dermatophytes prefer to infect toenails. Diagnostic testing including culture, KOH preparation and PAS staining can help with confirming fungal infection, but culture not required for empiric treatment with oral terbinafine. Severity of onychomycosis:-Mild-moderate: ≤50 percent involvement of the nail and sparing the matrix/lunula-Moderate-severe: involving >50 percent of the nail or involving the matrix or lunula, including further spread throughout nail.-It's common to have multiple nails affected at the same time. Toenails and fingernails can both be affected. Remember to check all nails in your patients. Nails can show signs of local but also systemic diseases. Risk factors:-Health conditions: Diabetes, immune suppression, venous insufficiency, peripheral artery disease, or even just having slow growth of the nails. This makes sense because there is decreased blood flow to those areas resulting in decreased immune surveillance of that area. Patient s with PAD are at risk for onychomycosis. Nails normally grow slower in male. Hormones play a role in that growth.-Exposure: smoking, trauma to the nail, sports, wearing sweaty shoes, being barefoot in communal areas such as swimming pools, college showers, jail house showers, and gyms.-Dermatological diseases: tinea pedis (athletes' foot), excessively sweaty hands (hyperhidrosis), psoriasis-Other factors: old age, having family members whom the patient shares a living space with, bunion (hallux valgus). Effects on mental healthUnfortunately, the infection takes a toll on the patient because the infection is unsightly it results in psychosocial disturbances. The patients may not want to wear sandals, get pedicures, or shower during gym class if they are school age. These types of feelings can cause patients to not want to go to work or do things they enjoy due to feelings of embarrassment. ManagementTreatment of dermatophyte onychomycosis is guided by causative organism, severity, treatment availability, and cost.Oral agents-Oral treatment is generally the gold-standard for onychomycosis due to shorter course and greater efficacy compared to topical. -Oral terbinafine is the preferred oral agent. Itraconazole can be used in patients not able to tolerate/respond to terbinafine.-Terbinafine and itraconazole both work by blocking important enzymes in fungal synthesis.-A randomized double-blind trial showed that terbinafine is more effective outcomes and better long-term cure rates than itraconazole.-Adult dosing of terbinafine: fingernail onychomycosis =250 mg per day for 6 weeks. Toenail onychomycosis= 250 mg per day for 12 weeks.-Some side effects of oral terbinafine include headache, dermatitis, GI distress, taste disturbances, and liver enzyme abnormalities. Adverse effects of Itraconazole include headache, GI disturbances, liver enzyme abnormalities.-In patients receiving continuous therapy, monitoring of transaminase levels is typically performed at baseline and repeated at six weeks if therapy will continue beyond six weeks. A medication interaction check is recommended before starting treatment with oral agents. -Mycotic cure rates of 76% for terbinafine, 63% for itraconazole with pulse dosing, 59% for itraconazole with continuous dosing, and 48% for fluconazole, topical cure rate is about 40%.-Recurrence of infection ranges 10-50% (reinfection or persistent infection). Patients need to wait for up to 1 year to see full effect of treatment. Treatment is highly recommended in patients with diabetes, treatment in other patients is cosmetic.CompliancePatient compliance is difficult because while taking oral medications, you cannot drink alcohol, and this becomes a problem due to the length of the treatment.Topical agents-Efinaconazole, Amorolfine, Tavaborole, Ciclopirox-Patients who have contraindications to systemic antifungal therapy, who are at risk for drug-drug interactions with systemic antifungal drugs, or who prefer to avoid systemic treatment can be treated with topical therapy. Similarly, to oral agents, these medications work by blocking important processes in fungal synthesis. These agents come in solutions or nail lacquer. Possible side effects include local skin irritation or ingrown nails.Alternatives-Less common therapeutic interventions for onychomycosis include oral antifungal agents other than terbinafine and itraconazole, laser therapy, photodynamic therapy, and surgical nail removal.-Patients with pain or discomfort from infected nails may benefit from removal of hyperkeratotic nail debris. Application of topical urea under occlusion can help with debridement of the nail and symptom improvement.-Recurrence after treatment of onychomycosis is common.Prevention:Now that we have gone over a lot of material about onychomycosis, we should discuss how we can prevent these types of infections from occurring.  Having good “foot hygiene” can help reduce the of infection and re-infection.Wash your hands and feet frequently, especially after encountering someone who is infected.  Clip nails straight across and file afterward making sure to sterilize clippers before and after each use.  Do not share nail clippers with others.If you have a history of sweaty feet, consider using sweat absorbing socks or wearing “breathable shoes” to prevent sweat from accumulating.Throw out old shoes or disinfect them using antifungal powders.Wear sandals in communal shower areas and at the pool.Pay attention to the cleanliness of your nail salon.Joke: Do you want to know how a person with toenail fungus feels? In this episode, Ashley and I are so excited because we have been wanting to do this sunscreen podcast for a while now. We wanted to take our time because we wanted to try out all these sunscreens and we bought a lot of them. Protect yourself and your children from the sun, especially when at the beach, pool, or playground for several hours with these sunscreens. However, not all sunscreens are created equal. Remember, your skin is your largest organ and whatever you put on, your skin will absorb it into your body and your children's body.   Learn more about sunscreen with Lindsey and Ashley! Enjoy the show! They share….. Recommended sunscreens What ingredients to avoid and why we chose these based on research.  Applying sunscreen on skin And many more! Check out these Highlights: First and foremost, we should all be wearing sunscreen as well as reapplying it every 2 to 3 hours. If you are worried about not getting enough Vitamin D (you need this for strong bones and protect against cancer by providing a strong immune system) then eat a diet rich in Vitamin D as well as take a supplement. A general rule of thumb is if the sun ray's shadow is shorter than you are, seek shade.  2 types of Sunscreen: Physical blocker (they reflects the sun rays and usually have zinc oxide or titanium dioxide) and  a Chemical blocker (contain blockers to absorb the sun rays). When choosing a sunscreen flip over the back and look at the ingredients.  Ingredients you will typically see are avobenzone, octisalate, octocrylene, and oxybenzone. There are some concerns because these ingredients may act as a hormone disruptor and have endocrine-disrupting effects. *Ashley and I tested out products that were approved by EWG. Environmental Working Group (EWG)* The products tested on both ourselves and Ashley's children were mineral sunscreens: Blue Lizard SPF 50, Blue Lizard SPF 30 (Ashley's  absolute favorite.  It goes on smooth and creamy and does not leave a white cast. Her kids loved it too. )   Thinksport SPF 50+ for kids,  Thinksport SPF 30 body and face sunscreen stick,    Babo Clear Zinc Sunscreen SPF 30 (worked well – it wasn't as creamy as Blue Lizard but still a good choice.)   Badger SPF 30 clear zinc,  Badger SPF 35 face stick.     Ashley and I also tested out products that were not approved by the EWG but as an adult, we wanted a light professional look.    Something to note: I have oily skin and Ashley has dry skin. So some of the below are absolutely loved as they worked well for my skin type. So keep that in mind, when choosing your sunscreen.    These products are chemical sunscreens: We tried this for our FACES only.  Glo Skin Beauty for the face oil free SPF 40 – we both love – goes on smooth and light and feels like a very light moisturizer.  Circadia light day sunscreen for the face spf 37 – I really enjoyed this one as well. Has the same feel as Glo Skin Beauty – it goes on very light.  Elta MD spf 46 – very light – easily absorbed – no white cast. GREAT for acneic skin. Koola spf 50 – Lindsey and I both loved this one – easily absorbed/no white cast.  This is a little thick but it is absorbed.  Glow Stick Sunscreen – This is Lindsey's favorite. It goes on clear and glides easily. It is seriously so easy and the glide is incredible.  One very important thing is that wearing sunscreen can cause a build-up on your face. So it is very important to really wash your face well in the evening. Lindsey suggests doing a  DOUBLE CLEANSE. Use a gentle cleanser - depending on your skin- gel for oily and cream for dry. Then rinse. Followed by a hydrating toner. Then serum. Then moisturizer. Try to avoid these because they can go into your bloodstream and have been known to have harmful effects. Oxybenzone / Benzophenone-3. (It acts like estrogen making it a hormone disruptor - can give you low sperm count and may contribute to endometriosis) Octinoxate - hormone disruptor and can be linked to breast cancer and bleaches the coral reef (Heads up - this ingredient can be found in hair coloring, lipsticks, shampoo, lotion) Octocrylene that is not Prop-65 compliant (can produce oxygen radicals that can be damaging to cells as well as cause cell mutations) Fragrances (I always try to avoid this ingredient. It can disguise nasty chemicals. For example, synthetic scents are known to cause cancer, birth defects, reproductive issues, and allergic reactions.) Avobenzone and Benzophenone (skin irritation, a hormone disruptor, and becomes toxic when in chlorine) Phthalates (used to smell good and soften plastics. esp bad for males - blocks male hormones. For females - increased risk of breast cancer and early breast development. Also found in nail polish, lotion perfumes, deodorants, moisturizers, and hair sprays - can be DISGUISED AS FRAGRANCE!) Background: Europeans represent the majority of international travellers and clinicians encountering returned patients have an essential role in recognizing, and communicating travel-associated public health risks. Methods: To investigate the morbidity of travel associated infectious diseases in European travellers, we analysed diagnoses with demographic, clinical and travel-related predictors of disease, in 6957 ill returned travellers who presented in 2008 to EuroTravNet centres with a presumed travel associated condition. Results: Gastro-intestinal (GI) diseases accounted for 33% of illnesses, followed by febrile systemic illnesses (20%), dermatological conditions (12%) and respiratory illnesses (8%). There were 3 deaths recorded; a sepsis caused by Escherichia coli pyelonephritis, a dengue shock syndrome and a Plasmodium falciparum malaria. GI conditions included bacterial acute diarrhea (6.9%), as well as giardiasis and amebasis (2.3%). Among febrile systemic illnesses with identified pathogens, malaria (5.4%) accounted for most cases followed by dengue (1.9%) and others including chikungunya, rickettsial diseases, leptospirosis, brucellosis, Epstein Barr virus infections, tick-borne encephalitis (TBE) and viral hepatitis. Dermatological conditions were dominated by bacterial infections, arthropod bites, cutaneous larva migrans and animal bites requiring rabies post-exposure prophylaxis and also leishmaniasis, myasis, tungiasis and one case of leprosy. Respiratory illness included 112 cases of tuberculosis including cases of multi-drug resistant or extensively drug resistant tuberculosis, 104 cases of influenza like illness, and 5 cases of Legionnaires disease. Sexually transmitted infections (STI) accounted for 0.6% of total diagnoses and included HIV infection and syphilis. A total of 165 cases of potentially vaccine preventable diseases were reported. Purpose of travel and destination specific risk factors was identified for several diagnoses such as Chagas disease in immigrant travellers from South America and P. falciparum malaria in immigrants from sub-Saharan Africa. Travel within Europe was also associated with health risks with distinctive profiles for Eastern and Western Europe. Conclusions: In 2008, a broad spectrum of travel associated diseases were diagnosed at EuroTravNet core sites. Diagnoses varied according to regions visited by ill travellers. The spectrum of travel associated morbidity also shows that there is a need to dispel the misconception that travel, close to home, in Europe, is without significant health risk.

Disease Incidences in Dogs – Dependency on Age, Sex, Size and Breed A Retrospective Study Based upon all In-Patient Dogs at the First Clinic for Internal Veterinary Medicine in Munich 1991 – 1997 This retrospective study is based upon the case files of all in-patient dogs treated at the First Clinic for Internal Veterinary Medicine of the Ludwig - Maximillians University in Munich within the period from 1991 and 1997. Data from 6005 patients, totalling 14056 diagnoses is listed and evaluated. The sex ratio (male : female) is 1.1 : 1 (i. e. almost balanced). 10.3 % of the male and 35.3 % of the female dogs were castrated. The population was composed of 164 breeds, which were classified into 15 breed groups for the analysis. The average age of the patients increased during the period investigated. Correlations between age and sex and height and breeds, respectively, have been observed. The multimorbidity increased significantly with the age. Additionally, the height and the breed of the patients also affected it. The multimorbidity of big dogs was lower than that of small dogs. Poodles, Dachshunds and Spaniels showed the highest multimorbidity, Labradors/Retrievers the lowest one. The following observations were made: Age-related tendencies: Infectious diseases (especially virus infections) were observed predominantly in young animals as well as (endo-) parasites and inflammatory respiratory tract diseases. Young adult dogs commonly experienced poisoning and injuries. Diseases of the heart, the liver, the pancreas, the endocrine and the reproductive system, diseases of the kidneys and the eyes, variations in body weight and tumors were typical in older animals. Dermatological diseases, diseases of the ears, the nervous system, the digestive tract, the blood and the hematopoetic system as well as diseases of the lymphatic organs had a higher prevalence with increasing age. Nevertheless there are variations within special disease groups regarding the age-relationship. For example, ear parasites and the disease complex “encephalitis/meningitis/myelitis" affected predominantly young adult dogs. The age-predilection of vaginitis and balanoposthitis did not correspond to its disease group (vaginitis: decreasing incidence with increasing age; balanoposthitis: incidence decreased after age of nine). Gastroenteritis occurred less often in older dogs. Hypothyroidism was most prevalent in middle-aged dogs (six to nine years). Sex predilections: Female dogs were predisposed to variations in body weight, poisoning, endocrine diseases and also – but only slightly - to diseases of the respiratory tract. Male dogs appeared to be at increased risk for heart diseases, diseases of the nervous and the digestive system, for malformations and injuries. Infectious diseases, parasites, diseases of the reproductive and the immune system were more often diagnosed in intact than in castrated dogs. Diseases of the skin, the liver, the nervous and the metabolic system, the pancreas, tumors, variations in body weight and injuries occurred predominantly in castrated animals. Height-specific predilections: Diseases of the eyes, the liver, the heart, the digestive tract, the pancreas and endocrine diseases and deformities were more common in smaller dogs. Big dogs were prone to infectious diseases, skin, ear and circulation diseases as well as to gynecologic/andrologic, blood and hematologic diseases, diseases of the lymphatic and the immune system. Parasites and variations in body weight were also more often diagnosed in bigger dogs. Renal diseases showed no height disposition, but they were often diagnosed in giant breeds. Diseases of the nervous system occurred rarely in giant breeds. Tumors were strikingly more common in bigger than in smaller dogs. Breed predilections: The disposition of big dogs for infectious diseases and parasites correlated with the breed predilection (Greyhounds, Retrievers/hunting dogs). Pomerians, Boxers and the German shepherd were prone to dermatological diseases. Pendulous-eared dogs and Rottweilers were predisposed to ear diseases. Spaniels, Poodles and Terriers had ophthalmologic problems more often than other breeds. Bernese Mountain Dogs showed a high incidence for diseases of the kidneys and the lower urinary tract. Cocker Spaniels and Boxers developed tumors much more frequently. Poodles, Terriers, Spaniels and Dachshunds were predisposed to diseases of the pancreas as well as to diseases of the heart and the endocrine system. Terriers and Pinchers/Schnauzers often revealed diseases of the liver and the gallbladder. Hunting dogs (Irish Setters) suffered more commonly from gynecologic and andrologic diseases. Diseases of the nervous system affected especially Cocker Spaniels, West Highland White Terriers and Boxers. Brachycephalic breeds had a high incidence in deformities.
