    Dr. Rachel S. Rubin is a board certified urologist with fellowship training in sexual medicine. She is an assistant clinical professor in urology at Georgetown University and owns her own practice in Washington DC. Dr. Rubin provides comprehensive care to all genders.     Understanding the Stellate Ganglion Block with John Vogel DO

    Stellate Ganglion Block for PTSDIndividuals may develop PTSD (also called Post Traumatic Stress Injury) from a single or repeated traumatic events. When trauma causes a persistent injury pattern to the nervous system, it can become locked into a permanent protective mode, even in the absence of on-going threat. This can cause a variety of symptoms including:Being "on-guard" all the timeConstant anxiety and fearIntolerance of being around groups of peopleNightmaresIsolation from others physically and emotionallyDifficulty progressing in therapy because talking about the trauma makes you too anxiousWhat is a stellate ganglion block?The sympathetic nervous system (the "fight or flight" part of the nervous system) is organized in front of the spinal column. Along the spine there are nerve bundles referred to as ganglion. Several ganglia are located in the neck, including one in the lower neck called the stellate ganglion. These nerves are special for their connection to brain regions which store and control emotional memories. Injection of local anesthetic around these nerve bundles interrupts the nervous system's constant exaggerated response to emotional trauma. You can think of it like rebooting your computer.How is a stellate ganglion block done?For decades, the procedure was done by feeling for a particular bone in the front of the spine, pushing the carotid artery toward the middle with one hand, and inserting a needle until it contacted bone prior to injection. Later, the procedure was improved by the use of x-ray to verify the correct level of the spine. However, use of x-ray does not allow visualization of important nerves and arteries in the neck.Ultrasound imaging and needle guidance is the gold standard for accuracy and safety, and the only technique Dr. Vogel has used over the last 7 years.     Context: This national needs assessment study explores the knowledge, attitude, beliefs, and practices (KABP) gaps related to vasomotor symptoms (VMS) associated with menopause among primary care and OB/GYN clinicians. VMS significantly impacts healthcare costs, workplace productivity, and patient psychosocial health, but a notable disconnect exists between healthcare providers and patients, with provider reticence and knowledge gaps playing a contributing role.Objectives: This study aims to identify and propose optimal educational strategies to address these barriers, with attention to health disparities affecting women of color.Methods: Methods employed include a multimodal approach of literature review, expert opinion, qualitative interviews, surveys, focus groups, and case studies, ensuring diverse clinician input. Data collection involved in-depth clinician interviews, a nationally disseminated clinician survey, and focus groups.Results: Results indicate a critical deficiency in healthcare providers' understanding and management of VMS, especially among OB/GYN residents, with 75 % showing limited knowledge. The study also highlights the disproportionate impact of knowledge gaps on women of color, emphasizing the need for a culturally informed approach in medical training and practice. There's a notable discrepancy between clinicians' current and desired abilities in managing VMS, indicating a need for ongoing professional development. Significant variability in approaches to diagnosing and treating VMS, as well as substantial knowledge gaps about treatment options, underscore the need for evidence-based protocols.Conclusions: Although VMS are a normal aspect of aging, they can significantly disrupt quality of life for many women, necessitating intervention. Beyond the immediate discomfort, VMS can impact quality of life and trigger insomnia and mood disturbances. This study exposes both new and previously recognized gaps in healthcare providers' knowledge and management skills concerning VMS treatment options, particularly regarding hormonal and nonhormonal therapies. Furthermore, our findings highlight the need for a deeper understanding of how VMS uniquely impacts women of diverse backgrounds. Research, including the Study of Women's Health Across the Nation (SWAN), suggests that the experience and severity of VMS may be influenced by socioeconomic status, race/ethnicity, body mass index (BMI), and smoking status. However, the complex interplay of these factors and their relative contributions remain unclear. Further investigation is crucial to facilitate equitable access to effective treatment for all women. To bridge these gaps, improved education starting as early as residency is essential. This education should address common misconceptions about VMS and its management.     A Contemporary View of Menopausal Hormone Therapy with Dr. Jim Simon MD

    Enthusiasm for the use of hormones to ameliorate symptoms of perimenopause and menopause has waxed and waned over the years. Both treatment for symptoms and training of women's health care practitioners in the management of menopause have sharply declined since publication of the Women's Health Initiative initial results in 2002. Findings from that trial, which treated a population of older, asymptomatic patients, have been extrapolated over the past 21 years to all estrogen products, all menopausal women, and all delivery mechanisms. Our patients deserve a more nuanced, individualized approach. Conjugated equine estrogens and medroxyprogesterone acetate are no longer the predominant medications or medications of choice available for management of menopausal symptoms. All hormones are not equivalent any more than all antiseizure medications or all antihypertensives are equivalent; they have different pharmacodynamics, duration of action, and affinity for receptors, among other things, all of which translate to different risks and benefits. Consideration of treatment with the right formulation, at the right dose and time, and for the right patient will allow us to recommend safe, effective, and appropriate treatment for people with menopausal symptoms.Dr. James A.     In this episode we talk about the bias and stigma around women and sexual health and particular what his looks like for Muslim  women.  We talk about religious, culture and family views as well as societal taboos around sexual health.Dr. Rahman is a board-certified OB/GYN who has tailored her practice to meet the needs of women for gynecologic and cosmetic services. After years of practicing obstetrics and gynecology in a variety of large academic settings and locations, Dr. Rahman started her own practice to focus on her special areas of interest to ensure patients have their best quality of life. Through her research and dedication, she is one of the few physicians nationally to receive the designation of ISSWSH (International Society for the Study of Women's Sexual Health ) Fellow (IF) and is actively involved with cutting edge approaches to sexual pain and sexual dysfunction. She also is an active member of the IPPS, International Pelvic Pain Society, and specializes in a multidisciplinary approach to chronic pelvic pain. For midlife women, Dr. Rahman is also a specialist in menopause management with the designation of Certified Menopause Physician through the North American Menopause Society (NCMP).Dr. Rahman was raised in the south in North Carolina and as you will see from her rapport, she understands what good ole southern hospitality means. She attended Duke University for her B.S. degree in Biology where she also specialized in Women's Studies.     A Discussion of Testosterone Use During Midlife

    Testosterone therapy (TTh) has been shown to improve libido in women with sexual dysfunction, but its utilization has been limited due to concern for cardiovascular events and past studies reporting highly variable results.the goal was to assess the association of TTh in women with major adverse cardiac events (MACEs), including heart attack, stroke, or death, using a large database.The results contribute to safety data on TTh, a therapy for sexual dysfunction in women.     Desire Discrepancy in Long Term Relationships

    Low sexual desire in women partnered with men is typically presumed to be a problem—one that exists in women and encourages a research agenda on causation and treatment targeting women. In this paper, we present a distinct way forward for research on low sexual desire in women partnered with men that attends to a more structural explanation: heteronormativity. A heteronormative worldview assumes that relationships and structures are heterosexual, gender (usually conflated with sex) is binary and complementary, and gender roles fit within narrow bounds including nurturant labor for women. We propose the heteronormativity theory of low sexual desire in women partnered with men, arguing that heteronormative gender inequities are contributing factors. We outline four hypotheses and their predictions related to: inequitable divisions of household labor, blurring of partner and mother roles, objectification of women, and gender norms surrounding sexual initiation. We discuss some mechanisms—social, physiological, and otherwise—for the heteronormativity theory, especially related to stress, objectification, and nurturance.     Discussion : Weight loss response to semaglutide in postmenopausal women with and without hormone therapy use

    We took some time to discuss this recent article which is very timely and it compared weight loss response and changes in cardiometabolic risk markers in postmenopausal women using semaglutide with and without menopause hormone therapy (HT) use. In postmenopausal women with overweight or obesity treated with semaglutide, HT use was associated with an improved weight loss response. It was a small study size of 60 women but nonetheless good data.     Menopause deserves care and attention! Just because it is a normal process of aging doesn't mean that women should be forced to grin and bear the multitude of symptoms that are associated with menopause not to mention the significant health consequences that are associated with the hormone decline of aging.     Your Queen Bees are doing a deep dive today to discuss what is like to navigate as patient and trying to advocate for yourself to get your hormones or at least have an evidence based, shared decision discussion with your provider.  Today we interviewed Heather Maurer, the CEO of NPWH (The National Association of Nurse Practitioners in Women's Health), a leading organization for providers who care for women and gender related health concerns.  She shares her experience as patient and being denied her hormones at her last visit even after sharing the evidence with her provider. Heather L. Maurer, MA, CAE is the Chief Executive Officer for the National Association of Nurse Practitioners in Women's Health (NPWH), the national professional association for certified women's health nurse practitioners (WHNP-BCs) and other certified APRNs who provide women's and gender-related healthcare. Heather has over 25 years working in healthcare including nonprofit, association, and accreditation organization management.     Welcome back to our amazing listeners we have taken a little hiatus and are so excited to be back with you in 2024 for Season 4.  We have done some rebranding and have moved the just ask podcast into The HIVE collective which is a digital health platform built to empower women to make educated and informed decisions about their health.  I'm your co-host Dr. Heather Quaile a leading Women's and Gender-related Nurse Practitioner. We started in 2020 with myself and colleague doctor Tara Thompson, pharmacist and now welcome my new cohost and leading women's health expert and nurse practitioner, Jackie Piasta.  She is a huge women's health advocate on Instagram.  So without further ado I want you to meet Jackie- who you all might have remember from our Season 3 episode 7.  We wanted to discuss this rebrand and how much thought and advocacy has gone into this collective.
