She grew up in Kenya, went to medical school in England, and spent the bulk of her career practicing psychiatry in the American South before retiring at 51. When the pandemic hit and the Me Too movement was at its peak, she started writing. Two books later, she's on a mission to reach people that one-on-one clinical work never could.She's also nearly 70, still sexually active, and completely unapologetic about both.What struck me most in our conversation wasn't just the 64 statistic - which I found astounding (I hadn't ever heard before!), it was the context. Because while women are approaching their sexual peak, men are often stepping back. Erectile dysfunction, reduced interest, withdrawal. Dr. Patel saw this pattern constantly in clinical practice, and she sees it in her own life and I've certainly seen it in my own. Women in her condominium complex, all roughly her age, laugh and joke about it. It's a real thing. And most women, she says, just go quiet and accept it.That acceptance is partly cultural. Dr. Patel grew up in Indian culture, where sex was not discussed. Full stop. Not by parents, not between siblings, not even now. At 70, she and her sister have never once discussed whether either has had an orgasm. She described watching a preview of a Bollywood film where a group of women, talking about an upcoming wedding, couldn't form the word for orgasm. They landed on “intense pleasure.” She said it with affection and exasperation in equal measure.But she's quick to say this isn't an Indian problem, or an Asian problem. It's a human problem. Americans of her generation were raised the same way. Her 91-year-old mother still tenses slightly when hugged. The silence around sex, pleasure, and desire crosses every culture she encountered in 25 years of clinical work.What can be done about it? Quite a lot, actually.Dr. Patel was frank about her own experience with vaginal atrophy and vaginismus after a hysterectomy and the way the body can, as she put it, just close up. Her gynaecologist prescribed estradiol cream, and it changed everything. Lubrication, libido, comfort. It also dramatically reduces UTIs and vaginal infections, something most women are never told. I have my own version of this story: I found out about vaginal estrogen from a friend, not a single healthcare professional. When I complained about painful sex, she asked if I was on vaginal estrogen. I was angry that no doctor had mentioned it.If you're avoiding sex because it hurts, or because desire has gone quiet, it's worth asking about estradiol cream/pessaries or ring. It has certainly changed my life.Dr. Patel's bigger message is about permission. Permission to still want sex at 70. Permission to tell your partner what you need — including that if they've stepped back, you might need to look elsewhere for that connection. Permission to adapt: different positions, more patience, a lot of laughter, and no more hanging off the chandeliers.She said it plainly: we only go through this life once. Real Life Pharmacology - Pharmacology Education for Health Care Professionals
Vilazodone (brand name Viibryd) is an antidepressant with a unique pharmacologic profile compared to most other agents in the SSRI class. While not a first-line choice for every patient, understanding its mechanism, adverse effects, and interaction profile is essential for optimizing therapy and preventing downstream prescribing problems. Mechanism of ActionVilazodone is classified as a selective serotonin reuptake inhibitor (SSRI) and a partial agonist at the 5-HT1A receptor. The SSRI activity increases synaptic serotonin by blocking the serotonin transporter, while partial agonism at 5-HT1A receptors may contribute to antidepressant effects and potentially reduce certain SSRI-associated adverse effects (though clinical evidence for this benefit is mixed). Adverse Effects GI effects – diarrhea, nausea, and vomiting are frequent early in therapy. Taking the medication with food can help minimize these. Insomnia – often dose-related; morning dosing may help. Sexual dysfunction – may be slightly lower than with some SSRIs but still present. Serotonin syndrome – rare but serious, particularly if combined with other serotonergic drugs. Discontinuation syndrome – abrupt cessation can lead to dizziness, irritability, and flu-like symptoms. Drug InteractionsVilazodone is primarily metabolized by CYP3A4. This means: CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin, ritonavir) can increase vilazodone concentrations, potentially worsening side effects—dose reductions may be required. CYP3A4 inducers (e.g., carbamazepine, rifampin, St. John's Wort) can lower drug levels, reducing effectiveness. Other serotonergic agents (e.g., triptans, SNRIs, MAOIs, tramadol, linezolid) increase the risk of serotonin syndrome. Antiplatelets and anticoagulants – SSRIs can impair platelet aggregation, increasing bleeding risk when combined with aspirin, NSAIDs, or warfarin. Prescribing Cascade ExamplesVilazodone's adverse effects can easily lead to unnecessary prescriptions if side effects aren't recognized: GI upset → Acid suppression therapy – Diarrhea or nausea prompts the addition of proton pump inhibitors or antiemetics, instead of adjusting vilazodone dose or timing. Insomnia → Hypnotic initiation – Trouble sleeping results in adding zolpidem or trazodone, without reassessing morning dosing or vilazodone's role. Sexual dysfunction → PDE5 inhibitor prescription – Erectile dysfunction leads to sildenafil use, when the root cause is vilazodone's serotonergic activity. Vilazodone's combination of SSRI and 5-HT1A partial agonist activity makes it somewhat distinct, but its side effect profile and interactions require the same careful monitoring as other antidepressants. Healthcare professionals can play a key role in catching early signs of adverse effects, preventing prescribing cascades, and ensuring drug–drug interactions are managed appropriately.

He described it not just as a sexual problem, but as one of the earliest "check engine lights" for heart disease. The blood vessels leading to the penis are much smaller than those in the heart. As we age, plaque can build up in these vessels. This process, known as atherosclerosis, affects the smaller vessels first.This means if you're experiencing persistent issues with erections, it's a strong indicator that you may have vascular problems that could soon affect your heart. As Dr. Kohler put it, "If the penis is failing, you should assume you're going to have problems with your heart soon until proven otherwise." The good news is that anything good for your heart—like exercise and a healthy diet—is also good for your penis.The "Use-It-or-Lose-It" OrganThe conversation also tackled the surprising phenomenon of penile shortening. When the penis goes into a "garage" for an extended period—meaning there is a consistent lack of erections—it can lose length. Dr. Kohler shared that a consistent lack of erections for just three months can lead to a loss of 1-2 centimeters in length due to the formation of scar tissue. This is why he calls the penis a "use-it-or-lose-it" organ. The key to maintaining penile health is to exercise the penis through erections, and tools like a vacuum erection device can be used for penile rehabilitation.Muscle Mass, Testosterone, and Sexual FunctionFor many men, this is where the conversation gets personal.
