As many as six in ten men have experienced erection problems over the course of their life. And that proportion has been on the rise in the last fifteen years, according to a study released by IFOP in April 2019. It's also known as impotence, and is a big health taboo among men. Erectile dysfunction is often linked to cardiovascular risk factors, so it's important to consult a doctor if affected. Furthermore, it can impact relationships, sometimes due to misunderstandings about the cause of the problem. The good news is it can be addressed with lifestyle changes, as we'll discuss in a minute. How do erections actually work? So why might that mechanism not work in some people? What about psychological or emotional factors? First broadcast: 07/02/2022 Learn more about your ad choices. In this powerful and deeply honest episode of Please Me!, host Eve welcomes Brent Dowlen, relationship and personal development coach, podcast host, and creator behind multiple shows including Driven To Thrive Broadcast and Dad Hat Shenanigans. Together, they unpack the rarely discussed realities of men's sexual health, including performance anxiety, shame, stress, and identity — and how these factors deeply impact intimacy, relationships, and mental health. Brent shares candid personal experiences with erectile dysfunction caused by stress and exhaustion, shedding light on how strongly many men tie sexual performance to masculinity and self-worth. The conversation explores how fear, shame, and societal expectations can quickly turn sex from a source of connection into a source of anxiety — and what men (and their partners) can do to interrupt that cycle. This episode also dives into foundational habits for long-term sexual health, including sleep, nutrition, hydration, exercise, stress management, and honest self-assessment. Eve and Brent address the men's mental health and loneliness epidemic, the outdated “playbook” men are still handed, and why vulnerability is often encouraged in theory but punished in practice. The episode concludes with a realistic and educational role-play conversation, modeling how couples can communicate desires, boundaries, fantasies, and consent — proving that healthy sexual communication can be awkward, playful, respectful, and deeply connecting. Permission at 70: Sex, Silence, and Starting Again

You're not past it. You may even be approaching your peak.A psychiatrist just told me that women tend to reach their highest sexual satisfaction at 64. I'm 64. And I cannot tell you how many times I've heard the opposite message, that desire fades, that this is the time to wind down, that the body just stops cooperating.Dr. Shila Patel has been listening to patients talk about sex, relationships, and desire for 25 years. She grew up in Kenya, went to medical school in England, and spent the bulk of her career practicing psychiatry in the American South before retiring at 51. When the pandemic hit and the Me Too movement was at its peak, she started writing. Two books later, she's on a mission to reach people that one-on-one clinical work never could. She's also nearly 70, still sexually active, and completely unapologetic about both. What struck me most in our conversation wasn't just the 64 statistic - which I found astounding (I hadn't ever heard before!), it was the context. Because while women are approaching their sexual peak, men are often stepping back. Erectile dysfunction, reduced interest, withdrawal. Dr. Patel saw this pattern constantly in clinical practice, and she sees it in her own life and I've certainly seen it in my own. Women in her condominium complex, all roughly her age, laugh and joke about it. It's a real thing. And most women, she says, just go quiet and accept it.That acceptance is partly cultural. Dr. Patel grew up in Indian culture, where sex was not discussed. Full stop. Not by parents, not between siblings, not even now. At 70, she and her sister have never once discussed whether either has had an orgasm. She described watching a preview of a Bollywood film where a group of women, talking about an upcoming wedding, couldn't form the word for orgasm. They landed on “intense pleasure.” She said it with affection and exasperation in equal measure. But she's quick to say this isn't an Indian problem, or an Asian problem. It's a human problem. Americans of her generation were raised the same way. Her 91-year-old mother still tenses slightly when hugged. The silence around sex, pleasure, and desire crosses every culture she encountered in 25 years of clinical work. What can be done about it? Quite a lot, actually.Dr. Patel was frank about her own experience with vaginal atrophy and vaginismus after a hysterectomy and the way the body can, as she put it, just close up. Her gynaecologist prescribed estradiol cream, and it changed everything. Lubrication, libido, comfort. It also dramatically reduces UTIs and vaginal infections, something most women are never told. I have my own version of this story: I found out about vaginal estrogen from a friend, not a single healthcare professional. When I complained about painful sex, she asked if I was on vaginal estrogen. I was angry that no doctor had mentioned it.If you're avoiding sex because it hurts, or because desire has gone quiet, it's worth asking about estradiol cream/pessaries or ring. It has certainly changed my life.Dr. Patel's bigger message is about permission. Permission to still want sex at 70. Permission to tell your partner what you need — including that if they've stepped back, you might need to look elsewhere for that connection. Permission to adapt: different positions, more patience, a lot of laughter, and no more hanging off the chandeliers. She said it plainly: we only go through this life once. Around the world, we're seeing declining sperm counts, rising infertility, more erectile dysfunction, and lower testosterone—even in younger men. These trends aren't just affecting family planning; they're pointing to something much bigger happening in men's health. Joining me today on The Dr. Hyman Show is Dr. Michael Eisenberg, a Stanford researcher and leading voice in male reproductive health. We talk about why sperm quality matters far beyond fertility—and how everyday health choices quietly shape it. Watch the full conversation on YouTube or listen wherever you get your podcasts. [YOUTUBE THUMBNAIL] We cover: • Why sperm quality may be one of the strongest early indicators of overall health and longevity • How metabolic dysfunction, belly fat, and low testosterone impact fertility and sexual function • The role environmental toxins and endocrine disruptors play in declining sperm counts • What men can do right now to protect fertility and long-term vitality When we look at fertility through the lens of whole-body health, it changes how we think about prevention, risk, and longevity. Truth is, these are medical issues, and understanding them can make a significant difference. Today, we're discussing it openly on the latest episode of the podcast. If you've been quietly wondering, “Is this just part of getting older?”—you're not alone. Men's sexual and urinary health issues are common, treatable, and nothing to be embarrassed about. On Episode 6 of our show, Dr. Hilden sits down with urologist Dr. Travis Pagliara for a candid, stigma-free conversation about men's sexual and urinary health. We'll cover what is normal, what's not, how common these issues are, how common they are, who they affect, and the wide range of treatment options available. Your fertility might be the canary in your body's coal mine. Urologist Dr. Justin Houman explains why this matters way beyond making babies.Full show notes and resources can be found here: jordanharbinger.com/1254What We Discuss with Dr. Justin Houman:Male fertility is a diagnostic window into overall health. Low sperm counts correlate with cardiovascular disease, metabolic dysfunction, hormonal imbalances, and even earlier mortality. Your reproductive system is essentially sending you a biological memo about the state of your entire body.Varicoceles (varicose veins of the testicles) are one of the most underdiagnosed yet treatable causes of male infertility, low testosterone, and testicular pain. They cause blood to pool and overheat the testicles, and most men never know they have them because general practitioners rarely check for them during routine physicals.TRT (testosterone replacement therapy) shuts down natural testosterone and sperm production — and once you start, you essentially can't stop. Before jumping on the TRT train, explore alternatives like addressing vitamin deficiencies, improving sleep, and using medications that stimulate natural production instead.Erectile dysfunction isn't just a bedroom problem — it's often the first warning sign of cardiovascular disease. The same arterial issues that cause heart attacks show up in penile blood flow first, making your erection quality a surprisingly accurate early warning system for your heart health. The foundations of long-term hormonal and sexual health are remarkably simple: consistent strength training, quality sleep, stress management, and whole foods. Starting in your twenties and thirties, these habits build the infrastructure that keeps your systems running smoothly for decades to come. And much more...And if you're still game to support us, please leave a review here — even one sentence helps! Dr Leah Hechtman, one of Australia's most respected naturopathic educators, joins fx Medicine by BioCeuticals ambassador Emma Sutherland to discuss the often-overlooked stage of men's health: andropause. More than simply “low testosterone,” andropause reflects age-related changes including Leydig cell decline, metabolic changes, vascular shifts, and neurological alterations that shape male wellbeing across the lifespan. Leah outlines how andropause may present in practice, from body composition changes and hair greying to mood shifts, sleep disruption, prostate dysfunction, and reduced cognitive performance. She highlights the importance of comprehensive testing — extending beyond testosterone — to include oestrogen, progesterone, follicle-stimulating hormone (FSH), luteinising hormone (LH), insulin, leptin, insulin-like growth factor-1 (IGF-1), and DUTCH hormone analysis. These insights allow clinicians to identify early signs and develop a more complete picture of male ageing. The discussion also explores strategies that may support resilience, including optimising sleep, managing stress, and addressing metabolic health. Useful complementary medicines include nutritional considerations such as nicotinamide riboside, bioflavonoids, and zinc, as well as herbal medicines including ginkgo, grape seed extract, ashwagandha, and tribulus. Real Life Pharmacology - Pharmacology Education for Health Care Professionals

Vilazodone (brand name Viibryd) is an antidepressant with a unique pharmacologic profile compared to most other agents in the SSRI class. While not a first-line choice for every patient, understanding its mechanism, adverse effects, and interaction profile is essential for optimizing therapy and preventing downstream prescribing problems. Mechanism of ActionVilazodone is classified as a selective serotonin reuptake inhibitor (SSRI) and a partial agonist at the 5-HT1A receptor. The SSRI activity increases synaptic serotonin by blocking the serotonin transporter, while partial agonism at 5-HT1A receptors may contribute to antidepressant effects and potentially reduce certain SSRI-associated adverse effects (though clinical evidence for this benefit is mixed). Adverse Effects GI effects – diarrhea, nausea, and vomiting are frequent early in therapy. Taking the medication with food can help minimize these. Insomnia – often dose-related; morning dosing may help. Sexual dysfunction – may be slightly lower than with some SSRIs but still present. Serotonin syndrome – rare but serious, particularly if combined with other serotonergic drugs. Discontinuation syndrome – abrupt cessation can lead to dizziness, irritability, and flu-like symptoms. Drug InteractionsVilazodone is primarily metabolized by CYP3A4. This means: CYP3A4 inhibitors (e.g., ketoconazole, clarithromycin, ritonavir) can increase vilazodone concentrations, potentially worsening side effects—dose reductions may be required. CYP3A4 inducers (e.g., carbamazepine, rifampin, St. John's Wort) can lower drug levels, reducing effectiveness. Other serotonergic agents (e.g., triptans, SNRIs, MAOIs, tramadol, linezolid) increase the risk of serotonin syndrome. Antiplatelets and anticoagulants – SSRIs can impair platelet aggregation, increasing bleeding risk when combined with aspirin, NSAIDs, or warfarin. Prescribing Cascade ExamplesVilazodone's adverse effects can easily lead to unnecessary prescriptions if side effects aren't recognized: GI upset → Acid suppression therapy – Diarrhea or nausea prompts the addition of proton pump inhibitors or antiemetics, instead of adjusting vilazodone dose or timing. Insomnia → Hypnotic initiation – Trouble sleeping results in adding zolpidem or trazodone, without reassessing morning dosing or vilazodone's role. Sexual dysfunction → PDE5 inhibitor prescription – Erectile dysfunction leads to sildenafil use, when the root cause is vilazodone's serotonergic activity. Vilazodone's combination of SSRI and 5-HT1A partial agonist activity makes it somewhat distinct, but its side effect profile and interactions require the same careful monitoring as other antidepressants. Healthcare professionals can play a key role in catching early signs of adverse effects, preventing prescribing cascades, and ensuring drug–drug interactions are managed appropriately.

Erectile dysfunction is one of the most common sexual difficulties that men encounter, and it can be very distressing. But while ED is highly treatable, we really need to understand what causes it in the first place in order to treat it most effectively. In this groundbreaking episode, Dr. Gabrielle Lyon sits down with Dr. Tobias Kohler, a professor of urology at the Mayo Clinic, to discuss a topic most people are afraid to talk about: men's sexual health.Dr. Kohler shares eye-opening insights, backed by over two decades of clinical experience and research, on the powerful link between foundational health habits and male sexual function. They reveal why erectile dysfunction is a critical "check engine light" for heart disease and how building muscle and exercising can improve not just your erections, but your overall longevity. This conversation goes beyond simple fixes, providing actionable, science-based information that every man needs to hear. This episode is brought to you by: Puori - Get 20% off sitewide with code DRLYON → https://puori.com/DRLYON Cozy Earth - Use code DRLYON for up to 40% off - https://cozyearth.comBONCHARGE - Use code DRLYON for 15% off your entire order - https://boncharge.com/DRLYONOneSkin - Get 15% off with the code DRLYON – https://www.oneskin.coChapters0:00 - Link between muscle mass & erections0:52 - Erectile function1:13 - The truth about penis size & penile implants2:32 - The foundational pillars of men's health8:48 - Erectile dysfunction is an early sign of heart disease16:14 - The role of anxiety, alcohol & cannabis on ED24:29 - The "use-it-or-lose-it" organ29:31 - The truth about PDE5 inhibitors (Viagra & Cialis)46:46 - The "C"s of urinary frequency54:43 - The connection between muscle, testosterone & sexual function1:02:07 - Is there a point of no return for penile health?Erections: A “Check Engine Light” for Your HealthOne of the most striking points from the discussion was Dr. Kohler's perspective on erectile dysfunction (ED). He described it not just as a sexual problem, but as one of the earliest "check engine lights" for heart disease. The blood vessels leading to the penis are much smaller than those in the heart. As we age, plaque can build up in these vessels. This process, known as atherosclerosis, affects the smaller vessels first. This means if you're experiencing persistent issues with erections, it's a strong indicator that you may have vascular problems that could soon affect your heart. As Dr. Kohler put it, "If the penis is failing, you should assume you're going to have problems with your heart soon until proven otherwise." The good news is that anything good for your heart—like exercise and a healthy diet—is also good for your penis. The "Use-It-or-Lose-It" OrganThe conversation also tackled the surprising phenomenon of penile shortening. When the penis goes into a "garage" for an extended period—meaning there is a consistent lack of erections—it can lose length. Dr. Kohler shared that a consistent lack of erections for just three months can lead to a loss of 1-2 centimeters in length due to the formation of scar tissue. This is why he calls the penis a "use-it-or-lose-it" organ. The key to maintaining penile health is to exercise the penis through erections, and tools like a vacuum erection device can be used for penile rehabilitation.Muscle Mass, Testosterone, and Sexual FunctionFor many men, this is where the conversation gets personal. Dr. Kohler highlighted the direct correlation between muscle mass and erectile quality. Studies show that men with more muscle mass have better erections and higher testosterone levels. The reverse is also...

Top 5 Questions Men Ask About ED and Performance Anxiety

Erectile dysfunction (ED) and performance anxiety affect countless men, yet many suffer in silence—feeling ashamed, confused, or alone. As a certified sex therapist, I frequently hear the same core concerns: “Is this normal?”, “Is it in my head or my body?”, “Can I talk to my partner about this?”, and “Will I ever feel like myself again?”   The truth is, ED is incredibly common and often caused by a mix of physical and psychological factors. Even when a doctor says everything looks fine physically, stress, shame, and fear can disrupt sexual function. That's where therapy comes in—not just to talk, but to understand the root causes and offer real, evidence-based tools for healing. Opening up to a partner can be tough, but silence only adds to the tension. Therapy can also help with communication strategies that bring couples closer together. The good news: ED is treatable. With the right support, men can rebuild confidence, restore intimacy, and redefine what "normal" means in their sex lives. Do you have vertical ridges on your fingernails? Don't treat the symptom without determining the root cause! In this video, we'll discuss some potential causes of nail ridges to help you address the problem instead of covering it up. Vertical ridges on nails are typically caused by one of 3 nutrient deficiencies:1. Iron deficiency2. Zinc deficiency 3. Biotin deficiencyTo determine which nutrient deficiency is causing the vertical ridges on your nails, first, look at your other symptoms. If you have an iron deficiency, you may also experience heavy menstruation and symptoms of anemia. Some people have a genetic problem that causes too much iron. Even with too much iron, you can become anemic because the iron in your body is dysfunctional. Liver problems and blood tests pinpointing a problem with iron could mean your nail ridges are caused by an iron deficiency. Cravings for ice or dirt and feeling cold with cold feet and hands can also signify iron deficiency. If zinc deficiency is behind your nail ridges, you may notice frequent illnesses, or it may take you a while to recover after getting sick. Other zinc deficiency symptoms include: •Low testosterone•Libido issues•Erectile dysfunction•Loss of muscle mass•Night blindness•Alopecia •Inflammation of the skinMost of your biotin is made by your microbes, so a history of antibiotic use can contribute to biotin deficiency. Biotin deficiency symptoms include brittle hair, thin nails, neurological problems, and peripheral neuropathy. Biotin, zinc, and iron control collagen. A deficiency in either of these nutrients can cause vertical lines on the nails. Once you identify which nutrient you are most likely deficient in, you can increase your intake of certain foods or supplements to fix the problem. Grass-fed liver pills or spleen extract pills are the best option for an iron deficiency. To increase zinc, consume more shellfish, oysters, and grass-fed beef. If you choose to take a supplement for zinc, take 50 grams in a blend with other trace minerals, including copper. If you have a biotin deficiency, try increasing your biotin with a supplement. Take a good probiotic to support gut health, which will naturally help increase biotin.
