In this episode, Dr. Ian Fields joins Dr. Mark Hoffman at the mic to discuss the role of the microbiome in obstetrical and gynecological conditions. --- SHOW NOTES Dr. Ian Fields completed a fellowship in Female Pelvic Medicine & Reconstructive Surgery and received a Master of Clinical Research degree from Oregon Health & Sciences University. His research focuses on changes in the urinary microbiome and their association with lower urinary tract symptoms. Dr. Fields begins the episode by describing the microbiome as “all of the things outside of our cells that make up the human body,” which includes bacteria, viruses, and fungi. He explains that the microbiome may play a role in how humans maintain states of health, in addition to having large impacts on the pathophysiology or development of disease states. Dr. Fields references the Human Microbiome Project, a United States National Institutes of Health research initiative that was aimed to improve the understanding of microbiota in relation to human health and diseases. The physicians then transition to cover the role of the microbiome, specifically in the field of obstetrics and gynecology. Dr. Fields explains that the most studied condition is bacterial vaginosis. From a urogynecology perspective, he expresses a particular interest in urinary tract disorders, such as urinary incontinence, recurrent urinary tract infections, and interstitial cystitis. Dr. Fields emphasizes to listeners that urine is not sterile. In addition, he highlights how the use of vaginal estrogen increases the presence of lactobacillus within the genital microbiome, and thus is beneficial in the settings of genitourinary syndrome of menopause and recurrent menopause. In this episode, Dr. Mark Hoffman invites Dr. Cheryl Iglesia to shed light on the topic of cosmetic gynecology. --- SHOW NOTES Dr. Iglesia is the Director of the Section of Female Pelvic Medicine and Reconstructive Surgery at MedStar Washington Hospital Center, as well the current President of the Society of Gynecologic Surgeons. In addition to being involved with a consensus document regarding cosmetic gynecology, Dr. Iglesia has contributed numerous studies to the growing field and hopes to educate other providers on the topic. The episode begins with Dr. Iglesia sharing how she became passionate about cosmetic gynecology. Her experience initially began with a weekend course in California learning about topics such as “laser vaginal rejuvenation” and “designer laser vaginoplasty,” which were early marketing terms used for the field. After multiple years of training and education within a field filled with gray areas and limited evidence-based medicine, she later helped develop a consensus document about cosmetic gynecology procedures, which provides clarification for patients and opportunity for future research studies. Dr. Iglesia then describes the field of cosmetic gynecology, which includes the elective intervention to alter the aesthetic appearance of the external genitalia or modify the genital organs. These elective, functional procedures may be performed in the absence of any pathology (e.g., no incontinence, prolapse, etc.) with the goal of improving a person's quality of life (e.g., sexual function). She describes two pillars of the field, including cosmetic (e.g., labiaplasty) versus functional (e.g., surgical tightening of the vagina for vaginal laxity) procedures. The physicians then address the concerns of societal pressure, agreeing that the goal is to help patients make well-informed, ethical decisions, which requires discussing goals with patients. After discussing the field as a whole, Dr. Iglesia then highlights different procedures and technologies. For example, a fractionated laser may be used to stimulate tissue growth and may be utilized for diagnosis such as genitourinary syndrome (GSM) of menopause or lichen sclerosis. In addition, she briefly mentions aesthetic procedures, including a clitoral frenulum reduction (frenulectomy) or clitoral amplification with platelet rich plasma or the O-Shot. She addresses that a lot of the procedures and technologies are proprietary, have limited evidence, and are not risk-free. Today I speak with Cheryl B. Iglesia, MD, FACOG, FPMRS. She is Director of the Section of Female Pelvic Medicine and Reconstructive Surgery (FPMRS) at MedStar Washington Hospital Center and Director of the National Center for Advanced Pelvic Surgery (NCAPS) at MedStar Health, an internationally renowned center that combines urogynecology and minimally invasive gynecologic surgery. She is also Professor of Obstetrics/Gynecology and Urology at Georgetown University School of Medicine. As a double board-certified physician in Ob/Gyn and female pelvic medicine and reconstructive surgery with more than 30 years in the field, Dr. Iglesia specializes in diagnosing and treating pelvic floor disorders, including urinary incontinence, fecal incontinence, overactive bladder, and pelvic organ prolapse. She also specializes in treating complex pelvic floor complications after prior surgery, including urinary and bowel fistulas; complications after pregnancy and childbirth, including pain from episiotomies and lacerations, as well as accidental bowel and bladder leakage; and complications after cancer treatment, such as sexual health issues and vaginal dryness. The co-hosts then cover pitfalls commonly identified in normal, open surgery (e.g. excessive neck flexion, truncal deviation, arm abduction) and laparoscopic surgery (e.g. body posture, monitor height). They transition to highlight possible resolutions to these pitfalls, including anti-fatigue mats, appropriate footwear, and proper posturing; ultimately addressing a need for further research in this aspect of medicine. In addition to managing her own very busy caseload, she mentors novice pelvic health physiotherapists and is a clinical preceptor for family medicine residents and midwifery students from McMaster University. Sinéad in an invited member on several committees for various organizations including the Society of Obstetricians and Gynecologists of Canada and the International Continence Society. She performs reconstructive urologic surgery for both women and men, and treats all urologic and urogynecologic issues, including pelvic organ prolapse, urinary and fecal incontinence, pelvic floor disorders, urethral strictures, fistulas, hematuria and recurrent UTIs. Dr. Polland has extensive training in minimally-invasive surgical techniques, including vaginal, laparoscopic, and robotic-assisted surgery. Dr. Polland has a strong research background with a special interest in women's sexual health. She has received grants from the Sexual Medicine Society of North America and the International Society for the Study of Women's Sexual Health. She has given lectures in this area of expertise at the Johns Hopkins Hospital and the Mount Sinai Hospital. The American Urological Association (AUA), in partnership with the Society of Urodynamics, Female Pelvic Medicine and Urogenital Reconstruction (SUFU) released the 2021 clinical guideline for the diagnosis, evaluation, treatment and follow-up of Neurogenic Lower Urinary Tract Dysfunction (NLUTD) in adults.NLUTD is a term used to refer to the abnormal function of either the bladder, bladder neck and/or its sphincters related to a neurologic disorder. Prior terminology commonly used "neurogenic bladder" to describe this condition. With the understanding this is not an issue confined to the bladder, NLUTD is now the preferred way to describe the various voiding issues seen in patients with a neurologic disorder.The focus of this guideline is on the evaluation, treatment and follow up of care for adults with NLUTD. A total of 60 recommendations, including those associated with initial evaluation, autonomic dysreflexia, urinary tract infections, non-surgical and surgical treatment options, as well as follow up and post treatment care are reflected within this new clinical guidance.Highlights include:An algorithm that allows for risk stratification of patients with NLUTD (low-, moderate- or high-risk), helping guide appropriate follow-up and surveillance.Recommendations on which patients need to be monitored for autonomic dysreflexia (AD) during procedures and how to intervene if AD occurs.UTI recommendations focusing on issues such as not treating asymptomatic bacteruria, not obtaining screening urinalysis or culture in asymptomatic patients and the consideration of daily antibiotic prophylaxis for NLUTD patients who manage their bladder with clean intermittent catheterization and have recurrent UTI.Treatment options to improve bladder storage parameters such as oral therapies (antimuscarinics and/or beta-3 agonists) or, if oral therapies fail, onabotulinumtoxinA to improve bladder storage parameters, decrease episodes of incontinence, and improve quality of life measures.David A. Olivia O. Cardenas-Trowers, M.D., is a fellowship-trained Urogynecologist at the Mayo Clinic in Jacksonville, Florida who specializes in treating female pelvic floor disorders. Dr. Cardenas-Trowers is a Senior Associate Consultant in the Department of Medical and Surgical Gynecology and an Assistant Professor of Obstetrics and Gynecology at the Mayo Clinic Alix School of Medicine. Dr. Cardenas-Trowers earned her medical degree from Meharry Medical College in Nashville, Tennessee. She completed her residency in Obstetrics and Gynecology at the University of Arizona in Tucson, Arizona, followed by a fellowship in Female Pelvic Medicine and Reconstructive Surgery at the University of Louisville in Louisville, Kentucky. In addition to her clinical practice, Dr. Cardenas-Trowers is active in research and education. She has been awarded research funding and has authored more than 20 peer-reviewed publications and textbook chapters. She is a national speaker and holds positions on several national committees including the American Urogynecologic Society (AUGS) and Society of Gynecologic Surgeons (SGS).
Anne M. Suskind, MD, FACS, MS is an Associate Professor of Urology; Obstetrics, Gynecology, and Reproductive Sciences; the Associate Chair of Faculty Affairs and Diversity, Equity and Inclusion; and the Chief of Neurourology, Female Pelvic Medicine & Reconstructive Surgery at the University of California San Francisco. She is an accomplished surgeon scientist and a wonderful human being. Dr. Anat Zelmanovich is board certified Obstetrician Gynecologist who specializes in Female Pelvic Floor and Reconstructive Medicine. She completed her undergraduate degree in Psychobiology from the University of Binghamton, after which she worked in Ovarian Cancer research at New York University. Being originally from Israel, Dr. Zelmanovich decided to get her medical training by returning to her homeland, she attended the Sackler School of Medicine at Tel Aviv University where her interest in women's health expanded. She decided she wanted to devote her career to service of women of all ages by specializing in the field of Obstetrics and Gynecology at Albert Einstein College of Medicine in New York. After an intense four years of residency, Dr. Zelmanovich worked as an OBGYN in Manhattan's upper east side at Lenox Hill hospital, even though she enjoyed Obstetrics fully, felt that she needed more surgical expertise. She made the decision to move on to a Fellowship in Female Pelvic Medicine and Reconstructive Surgery at Hackensack University Medical Center. Since then Dr. Zelmanovich has been honored to serve her patients with care, compassion, and surgical expertise that focus on minimally invasive, vaginal, and robotic approaches to treatment of a vast array of conditions. Throughout her medical training Dr. Zelmanovich has fallen in love with the art of learning and teaching. Dr. Zelmanovich loves to teach not only her patients but her peers and colleagues and as such she is currently serving as the Associate Program Director for the OBGYN Residency Program at Jamaica Hospital Medical Center.

She was recently awarded a grant from the Michael Smith Foundation for Health Research, to build a website and initiate a social media campaign about the female pelvic floor. Since many pelvic floor issues start in pregnancy, the initial focus of the campaign was on disseminating evidence-based information on pregnancy and childbirth, and how these two life events impact pelvic health. In this episode we discuss a novel treatment for BPH called iTind. Unlike some other treatment options for BPH, the iTind procedure is performed in the clinic setting. It involves inserting a tiny device through the penis with a flexible camera. The device is then removed in the clinic about a week later. iTind is one of the least invasive procedures with BPH, is rapidly performed, and requires minimal if any recovery time. But is the procedure safe? How effective is it? How does it compare with more traditional BPH procedures like TURP, HOLEP and minimally invasive treatments like Rezum and UroLift? In order to answer these questions, we turned to an expert on the treatment of BPH. Dr Dean Elterman is associate professor of urology at the University of Toronro. Dr. Elterman completed his medical degree followed by residency in urologic surgery at the University of Toronto. He became a fellow of the Royal College of Physicians and Surgeons of Canada in 2011. Dr. Elterman completed a two-year fellowship in Voiding Dysfunction, Neuro-Urology, Female Urology and Pelvic Reconstruction at Memorial Sloan-Kettering Cancer Center and New York Presbyterian Hospital/Weill Cornell Medical College in New York City. Dr. Elterman is a member of several professional societies including the Society of Urodynamics, Female Pelvic Medicine and Urogenital Reconstruction, the Society of Genitourinary Reconstructive Surgeons, the International Neuromodulation Society, and the International Society of Men's Health.

Guest Bio: Jocelyn J. Fitzgerald, MD is an Assistant Professor of Obstetrics and Gynecology in the division of Urogynecology and Reconstructive Pelvic Surgery at the University of Pittsburgh School of Medicine. She completed her Female Pelvic Medicine and Reconstructive Surgery Fellowship at Georgetown University/MedStar Health with the National Center for Advanced Pelvic Surgery and her Gynecology and Obstetrics residency at the Johns Hopkins Hospital. She attended medical school at the University of Pittsburgh and graduated from the Physician Scientist Training Program. Before medical school, Dr. Fitzgerald graduated from Penn State in 2008 with a BS in Neurobiology from the Eberly College of Science and a BS in Women's Studies with honors and an emphasis in Women's Health from the College of the Liberal Arts. Her research has focused on mechanisms of female chronic pelvic and bladder pain, patient-centered outcomes of gynecologic surgery, and predicting how women seek care for pelvic floor disorders with a particular interested in the role of social media in medicine. Maria Uloko is a urologist, Inclusive Sexual Medicine, prosthetic/reconstruction, transgender health and Male Infertility Fellow at San Diego Sexual Medicine. She is currently Associate Professor and Director of the Division of Female Pelvic Medicine and Reconstructive Surgery (FPMRS) in the Department of Gynecology and Obstetrics. She is also Chief of Clinical service for OB/GYN at Emory University Hospital. Gina's clinical areas of interest include geriatric gynecology, neuromodulation, pelvic organ prolapse, urinary incontinence, and overactive bladder. She currently participates in several clinical trials and translational research projects investigating mechanisms of female pelvic floor disorders. Prior to joining the faculty at Emory, Dr. Northington was an assistant professor at the University of Pennsylvania where she also completed her fellowship training. In addition to her administrative positions at Emory, Gina is also fellowship program director for FPMRS at Emory and was responsible for Emory receiving ACGME Accreditation for the Fellowship program the first year it was offered by the ACGME in 2013. Dr. Lamia Gabal is a Urologist with a sub-specialty in Female Pelvic Medicine and Reconstructive Surgery. She talks about all kinds of concerns women bring to a Urologist, information about the treatment options, and how to go about it. Sexual issues that bring women to urologists Women come to urologists for various kinds of sexual concerns. While traditionally urologists were thought to be “Male gynecologists”, Dr Lamia says urologists deal with much more than that. Women come with issues of Urinary Incontinence, Urinary Tract Infection, orgasmic dysfunction, female sexual dysfunction, libido issues, and more. Many women who come with these concerns back away from having sex with their partners because of the embarrassment they feel around it. Sometimes fixing their medical problems also helps them with their sex lives. While male concerns around this subject are already well understood and treated, female sexual dysfunction has only received a “trash can diagnosis”, says Dr. Lamia. There are several types of female sexual dysfunctions and each needs to be treated accordingly. Urologists also deal with hormonal changes and core dysfunction. Thinning of vaginal tissue as women age can also lead to sexual dysfunction. Pelvic organs prolapse after childbirth can also lead to sexual dysfunction and can be painful. Medical concerns that drive women away from having sex There's an overlap of urologists and gynecologists in the sub-specialty of female pelvic medicine and reconstructive surgery, with each performing their roles. However, not all urologists or gynecologists specialize in the field Dr Lamia does. She categorizes the kinds of concerns patients come in by their age. While most of her patients are post-childbirth age, she also treats young women who come in with issues of painful sex which could be pelvic floor dysfunction or dyspareunia and is usually associated with sexual trauma or PTSD. Women who are of child-bearing age often come in with recurring Urinary Tract infections. Women who are getting older and are past having kids struggle with pelvic organ prolapse, urinary incontinence, and fecal incontinence. All the concerns Dr. Lamia mentioned can drive women away from having sex or from having a satisfying sex life. According to Dr. Lamia's advice, women who experience Urinary Tract Infections that have constipation, pelvic prolapse, and vaginal atrophy (thinning of vaginal tissues) should consult a urologist. Where does sex intersect in terms of conversations with patients? As a doctor, there's no training you get in medical school that prepares you to have these conversations about sex with your patients. It's often one's interest to seek out more information and awareness that leads to these conversations with patients. Dr Lamia says it's important to talk about sex with their patients to provide better health care, it aids in understanding how it affects their body and to make important decisions. However, most doctors don't have these conversations for reasons of not having enough time or not being comfortable enough. Sometimes because of the assumption that an older patient might not be sexually active, which should not be done. Pelvic Organ Prolapse – treatment options Pelvic Organ Prolapse commonly occurs after childbirth and is more common with vaginal deliveries. All of these factors put pressure on organs making them lean into the vaginal wall. It increases the risk of urinary incontinence, fecal incontinence, and UTIs, and the most severe case can cause kidney dysfunction. It can be treated by “Pessary” which delays or prevents the need for surgery. Surgeries like cystocele repair, rectocele repair, and slings for incontinence are also an option, but patients can expect them to be redone after 15-20 years. It can impede sexual intercourse when the patient is constipated or something else. The surgery fixes vaginal laxity caused by this which can benefit sexual intercourse for both partners. Treatment options for Urinary Tract Infections To treat Urinary Tract Infection, all the other causes of the infection has to be ruled out through either a physical exam, an ultrasound of the kidneys, or a cystoscopy. Dr. Gabal explains some treatments that could help such as maintaining good sexual hygiene, treating constipation, emptying bowels regularly, consuming fiber and a lot of water, peeing before and after sex, using plenty of lubrication that isn't “warmed or flavored” can help to reduce the infection. Using antibiotics after intercourse and using probiotics to normalize vaginal bacteria can also help. She suggests supplements like cranberry and D-mannose prevent certain types of UTIs. Conditions That Can Cause Sexual Pain Dr. Lamia talks about pelvic floor dysfunction as the most common cause of dyspareunia or painful sex. It causes mild pelvic floor muscle spasm to vaginismus where the vagina doesn't open and causes pain. To treat this, she suggests soaking in a tub, putting heat on the area, or taking muscle relaxant drugs prescribed by the doctor. Pelvic floor physical therapy is the most effective of all and is done by specially trained pelvic floor physical therapists. Post-menopausal vaginal atrophy could also be the cause. She talks about birth control as an understated cause that causes thinning of vaginal tissue and a change in PH levels causing painful sex. She recommends putting topical testosterone mixed with estrogen to treat it.
