When a patient talks about their sexual dysfunction, I always ask: what's going on with the person they're having sex with? You'd be surprised how many providers don't.Most people don't think to ask what's happening on both sides of a sexual relationship. But sexual dysfunction doesn't happen in a vacuum—it's a two-way street. In this episode, I'm joined by Dr. Jessica Yih, a urologist who treats both men and women, and we get real about the overlap of issues that show up in the bedroom. We dive into Peyronie's disease and why it's been overlooked for so long, how injections can break down plaques, and why those curves aren't just uncomfortable—they can lead to serious problems for both partners. We also break down erectile dysfunction beyond the “just pop a blue pill” myth. Spoiler: Viagra doesn't work for everyone.Jessica brings a powerful perspective to the conversation, especially around what it's like treating both members of a couple, sometimes in back-to-back appointments. She's also helping train the next generation of sexual health urologists through her fellowship at UC Irvine.Sex is more than individual dysfunction—it's communication, treatment access, aging, and understanding both bodies in the room. If you're a provider, a patient, or just curious about how these dynamics work together, this episode is for you.Highlights:The truth about Peyronie's disease, painful sex, and treatment optionsWhy Viagra isn't a one-size-fits-all fix for erectile dysfunctionHow injectable medications and implants are changing the gameDyadic sexuality and the importance of treating both partnersJessica Yih's unique path to sexual medicine and her work training new fellowsIf this episode got you thinking, please hit that follow button, leave a comment, and give us a review on Apple Podcasts or Spotify. On this episode of #mensexpleasure, I chat with Melissa Hadley Barrett. Melissa is a Nurse Practitioner and Sexologist from Western Australia with over 20 years of experience. She is the Principal, Director and lead clinician of Restoring Men's Intimate Health (RMIH) Pty and Restorative Sexual Health Clinic (RSHC) Pty Ltd. Melissa is an advocate for sexual health and regularly engages in public speaking and education regarding sexual health, particularly prostate cancer for both health professions and community members. Melissa has co-founded the Penis Project Podcast, which shares real-life stories to support learning and offer support, gaining a worldwide audience. Key points: Melissa's background and expertise Melissa introduced herself as a nurse practitioner and sexologist who runs the Restorative Health Clinic. She specializes in helping couples and men navigate intimacy, relationships, and sexual function, taking a holistic approach that addresses both the physical and psychological aspects. Treating dysfunction in cancer patients Melissa explained that about two-thirds of her clients are prostate or bladder cancer patients, as the treatments for these cancers can significantly impact sexual function. She described how the nerves that control erections can be damaged, leading to issues like erectile dysfunction and Peyronie's disease. Melissa emphasized the importance of rehabilitation to maintain penile health and function during this recovery period. Overcoming stigma and shame around sexual health Melissa discussed how many cancer patients feel guilty or ashamed about wanting to maintain their sex life, believing they should just be grateful to be alive. She works to reframe this mindset, emphasizing that quality of life and intimacy are important, and that positive outcomes can come from adversity. Melissa also highlighted the need to involve partners in these conversations to address misconceptions and provide support. Educating healthcare providers Melissa shared how she works to upskill other healthcare providers, like GPs and urologists, to have open conversations about sexual health with their patients. Even if they don't have the expertise to provide comprehensive treatment, she encourages them to at least ask about it and provide referrals to appropriate specialists. Melissa's resources and programs Melissa described the various online programs and resources she offers, including rehabilitation programs for issues like erectile dysfunction, Peyronie's disease, and premature ejaculation. She also highlighted her podcast, The Penis Project, which features real stories from clients about their experiences. Let's cut right to the chase: Ally is looking for a certain kind of man to satisfy her needs, and she's not finding him. This isn't your typical female ask for a life partner or a long-term relationship. It's more a recognition that she's had difficulty finding someone who can get the job done for her. Which brings up an interesting difficulty that women can face: It's actually difficult to find out, early in a relationship, how a guy is going to perform in bed. Or, indeed, if he'll be able to perform at all. And, I suspect dick pix are no help, as they're often taken in a flaccid state, or taken from an angle that just doesn't do justice to the man's true stature. Along the way, we discuss Peyronie's disease, some listener feedback on the podcast and listener questions. And, Mike informs Ally that none other than Bill Clinton, according to a sexual harassment lawsuit by Paula Jones, suffers from Peyronie's Disease. It's an epidemic! The penis doesn't always do what you want it to do. For example, sometimes you might want an erection but can't get one, or maybe you’ll reach orgasm faster than you’d like. These things happen. But when does a penis problem cross the line into becoming a sexual difficulty in need of treatment? That's what we're going to be exploring today. We're going to talk about common male sexual health issues, including how they are clinically defined and treated, as well as tips for maintaining penile health as you age. I am joined once again by Melissa Hadley Barrett. Melissa is a nurse practitioner and sexologist, and the founder of Restorative Sexual Health Clinic and Restore Men's Intimate Health. She is also co-founder of The Penis Project Podcast. Some of the specific questions we answer in this episode include: What is Peyronie’s disease, and how would someone know if they had it? What “counts” as having premature ejaculation? How can men last longer in bed? What’s the difference between a sexual dysfunction and a sexual disappointment? What are some different ways of dealing with erectile dysfunction? You can check out Melissa’s website and YouTube channel to learn more. Got a sex question? Thank you for joining us for our 2nd Cabral HouseCall of the weekend! I'm looking forward to sharing with you some of our community's questions that have come in over the past few weeks… Darren: Good day Dr Cabral. I appreciate all that you're doing. But ts making a difference in so many people's lives. Like really huge differences. My question has to do with Bluetooth air buds or wireless headphones. I know you harp on the dangers of using airbuds due to EMFs that may pass through brain, but is the dangers still present if someone uses only one side while leaving the other bud in the case? That's what I've been doing since you talked about it, thinking it'll minimise the risk cause I didn't just wanna throw them out but if you say that's also harmful, I would now. Keep up great work. Anonymous: Hi Dr. Cabral, I have been dealing with some double vision issues for a few years now. My recent MRI shows no obvious link. My ophthalmologist is blaming hormones/thyroid even though my numbers haven't changed in years. I did notice that when I consistently used CBD/THC in microdose, the double vision would almost resolve. Currently I am pregnant so I can't use this. The double vision is not pregnancy related and happens when I am and am not pregnant. Do you have any ideas on what could be the cause for the double vision? Or what could help this symptom in the meantime? Thank you for sharing all of your knowledge and caring enough to spread it!! Anonymous: Hi Dr Cabral, Not sure this is something you would want to broadcast, but I wanted to ask if there's anything you can recommend for peyronie's, or curvature due to fibrosis due to an injury. I've been taking Serrapeptase off and on, but it doesnt seem to help. My Urologist prescribed me a medication that increases blood flow, but I would rather explore options outside of pharmaceuticals. Thanks Mina: Hi Dr. Cabral, I am curious to hear more about the relation between Candida overgrowth and digestive issues. I've always had digestive issues - heavy bloating and pain after eating, and noticed you mentioning that it could be related to a Candida overgrowth. I always test positive for Candida (albicans), have so for years, but my gynaecologist has always said that this is very common and most people have it. Thank you so much! Amy: Hi dr! My hrv (Oura ring). Is in the 150's to 200's. Do you think this is a problem and what do you attribute this to. If it goes women to the 9 0's is that a good thing or bad..so confused. Thank you!! This week on the BackTable Urology Podcast, Dr. Jose Silva discusses sexual health with Dr. Arthur “Bud” Burnett, professor of urology at John Hopkins University, with an emphasis on the importance of preventative medicine in improving sexual health and different treatments for erectile dysfunction. --- SHOW NOTES First, Bud and Jose explore the various treatments available for patients with erectile dysfunction (ED). They not only discuss how oral drugs can be used to treat the majority of patients, but also how to manage a young patient with a single episode of ED. Furthermore, they evaluate the safety of these oral drugs and the alternatives, such as injections, rings, and vacuum erection device therapy. Next, they note the important role of testosterone replacement therapy (TRT) in improving sexual health for prostate cancer survivors. They also delineate various modalities of testosterone replacement therapy such as gels, injectables, and pellets, and their safety profiles. They both agree that it is important to follow guidelines and monitor patients who are on TRT. Additionally, they note the prevalence of Peyronie's Disease, and the psychological implications the condition can have on men. The doctors then move onto the topic of priapism, a condition where a man experiences an erection for a prolonged period of time. They explore the potential causes of the disorder, the treatments available, and the potential therapies that may help prevent it from occurring. Finally, Dr. Burnett also shares his journey to writing his book, The Manhood Rx, in order to provide men with a comprehensive resource to improve their sexual health and overall wellness. --- RESOURCES The Manhood Rx: Every Man's Guide to Improving Sexual Health and Overall Wellness by Arthur Burnett https://www.amazon.com/Manhood-Rx-Improving-Overall-Wellness/dp/1538166593 WellPrept https://wellprept.com/
