In this episode, we speak with Dr. Mary Lynn, an obgyn at Loyola University Medical Center, about the often overlooked topic of female sexual wellness and dysfunction. From low libido and pain during sex to emotional barriers and hormonal shifts, we explore the physical, mental, and emotional aspects that can impact sexual health. Our goal is to foster understanding, break down stigma, and provide practical insights. Dr. Mary Lynn is an associate professor in the Department of Obstetrics and Gynecology at Loyola University Medical Center. She is the co-director for the Loyola Sexual Wellness Program and recently published on outcomes of this multidisciplinary program in the Journal of Sexual Medicine. She is active in research and mentored medical students through the Star program as well as research honors. Dr. Lynn created the women's sexual health education module which is part of the curriculum for 3rd year medical students and she lectures to them regularly on this topic. Currently, she serves on the International Society for the Study of Women's Sexual Health education committee and as well as the Faculty Advisory Board for the Medical Student Forum on Female Sexual Medicine. Additionally, she contributes to the Sexual Medicine Society of North America women's sexual health committee and acts as an advisor for the Medical Student Forum on Female Sexual Health. She is an oral board examiner for the American Osteopathic Board of Obstetrics and Gynecologists and serves on the recertification committee. She is a PCM1 Advisor and active on the Loyola IRB committee. Her current research focuses on Women's Sexual Health Medical Education and Sexual Health in Pregnancy. Research Nerd Alert! This is an overview of our current understanding of what central sensitisation (aka nociplastic pain) is and how it impacts pelvic pain syndromes and of course...what we are goibg to do about it! Some great research around this topic has emerged over the past couple of years, with a growing understanding of why just focusing on the issues in the tissues is not enough and how we can incorporate lifestyle strategies such as sleep, nutrition and physical activity into our anti-inflammatory treatment toolbox So although this review is very evidence based for the first part, I do explore why this means when we are working with real life people - why and how to screen for CS and what steps we can take to get better outcomes - ultimately to help women with persistent pelvic pain live well! We delve into the critical and often under-discussed topic of female sexual health issues related to chronic genital graft-versus-host disease (GVHD), a condition affecting many women post bone marrow transplant.Dr. Goje begins by highlighting the prevalence of sexual dysfunction among female survivors of malignancies, particularly those who have undergone bone marrow transplants. Astonishingly, 80% of female survivors report no significant improvement in their sexual function even five years post-transplant. This issue, often underdiagnosed and unspoken about, affects their quality of life despite their physical recovery.She explains that the persistent sexual dysfunction is multifactorial. Factors include the systemic effects of chronic GVHD, which can cause fatigue and changes in body appearance, making patients feel less attractive. Chronic genital GVHD specifically affects the vulva and vagina, causing changes like thickening, rawness, and atrophy, leading to pain with intimacy, burning, and even bleeding. Additionally, medications can alter libido, and the overall psychosocial burden of their diagnosis and treatment exacerbates these issues.Dr. Goje emphasizes the importance of a multidisciplinary approach to manage these problems. She details the symptoms of genital GVHD and the necessity of individualized treatment plans. Hormone replacement therapy is essential for those with premature ovarian insufficiency or menopause, which can be accelerated by the transplant. Vaginal estrogen or other moisturizers and lubricants are often required, along with immunosuppressants like topical clobetasol to manage inflammation.Communication emerges as a vital theme throughout our conversation. Dr. Goje stresses that healthcare providers need to proactively ask about sexual health issues. Patients, often so grateful for their survival, do not prioritize these concerns. Utilizing validated questionnaires can help initiate these crucial discussions and overcome hesitation to begin the conversation. For patients, Dr. Goji advises regular consultations with a gynecologist and open discussions about their medications and any sexual health issues they experience, even before diagnosis.Addressing the specific needs of younger women, Dr. Goje discusses options like egg freezing before treatment to preserve fertility. She also notes that many foundations and insurance plans provide financial support for these procedures.Dr. Goje highlights the various treatments available for managing sexual dysfunction, including the use of silicone dilators, surgical interventions, and laser therapy. She encourages couples to communicate openly about their sexual health and consider sex therapy or couples therapy to address issues together. For patients experiencing chronic vaginal pain or infections, she recommends appropriate medical treatments, including potential surgery or laser treatment. She emphasizes the need for accurate diagnosis and management.Finally, Dr. Goje shares poignant stories from her practice, illustrating her deep commitment to her patients' holistic health. Christine DeLozier, a food expert, research scientist and licensed acupuncturist, combined all of her talents to produce a book entitled, “Diet for Great Sex: Food for Male and Female Sexual Health.” You can have great sex if you eat certain foods and reduce consumption of other foods, her research shows. During my Smart Sex, Smart Love podcast, DeLozier talks about how great sex is much more than being with the right person and being in the right mood. Throw in the right foods and watch your sexual pleasure skyrocket, she finds. Sex is better when we have good blood flow, which generates strong and rapid nerve conduction to our genitals, her research reveals. Our goal is to awaken our sexual health pathways by achieving the trifecta, DeLozier calls it. Improved blood flow, hormonal balance and nerve condition are the great sex trifecta, she has learned. In this episode we talk about the bias and stigma around women and sexual health and particular what his looks like for Muslim women. We talk about religious, culture and family views as well as societal taboos around sexual health.Dr. Rahman is a board-certified OB/GYN who has tailored her practice to meet the needs of women for gynecologic and cosmetic services. After years of practicing obstetrics and gynecology in a variety of large academic settings and locations, Dr. Rahman started her own practice to focus on her special areas of interest to ensure patients have their best quality of life. Through her research and dedication, she is one of the few physicians nationally to receive the designation of ISSWSH (International Society for the Study of Women's Sexual Health ) Fellow (IF) and is actively involved with cutting edge approaches to sexual pain and sexual dysfunction. She also is an active member of the IPPS, International Pelvic Pain Society, and specializes in a multidisciplinary approach to chronic pelvic pain. For midlife women, Dr. Rahman is also a specialist in menopause management with the designation of Certified Menopause Physician through the North American Menopause Society (NCMP).Dr. Rahman was raised in the south in North Carolina and as you will see from her rapport, she understands what good ole southern hospitality means. She attended Duke University for her B.S. degree in Biology where she also specialized in Women's Studies. She then continued in her home state at the University of North Carolina at Chapel Hill for her M.D. As women's health care and women's issues have always been on the forefront of concerns for her, she moved up north for her residency in Massachusetts in Obstetrics and Gynecology at the University of Massachusetts. After completing her residency, she made her way out west to Los Angeles, California where she spent four years on the faculty at the University of Southern California, Keck School of Medicine . In L.A., she spent a great deal of her time on resident and medical student education and simulation training in obstetrics and surgery. She also spent her personal time performing medical missions internationally with a variety of international women's organizations as well as working with at the WHO in Switzerland.She has taken extra time to explore areas of aesthetic medicine with the American Board of Aesthetic Medicine, including anti-aging and laser technology and improving sexual health through additional work in aesthetic vaginal procedures in southern California.Currently, Dr. Rahman is a Clinical Assistant Professor of OB/GYN at the Northwestern Feinberg School of Medicine in downtown Chicago. She previously worked at Northshore University Health Systems and was on the faculty at the University of Chicago. She is a member of the American Congress of Obstetricians and Gynecologists and American College of Aesthetic and Cosmetic Physicians and a Fellow of The American College of Obstetrics and Gynecology.With her experience and expertise, she decided to venture on her own opening the Center for Gynecology and Cosmetics and tailors her practice to your needs!In her spare time, she enjoys spending time with her children and family, running outdoors no matter the season, and traveling with her children in tow. Our guest this time is Dr. Pebble Kranz from the Rochester Center for Sexual Wellness. She is a Family Medicine doctor with a background in theater who now specializes in sexual medicine. Mentored by a beloved sex therapist, the late Gina Ogden, she has been trained in sexual medicine through the European Society of Sexual Medicine and became a Fellow of the European Committee on Sexual Medicine (FECSM) in 2018. Because one training program is never enough, she also has trained with the International Society for the Study of Women's Sexual Health (ISSWSH), the American Association of Sexuality Counselors Educators and Therapists (AASECT), and the World Professional Association on Transgender Health (WPATH) where she is pursuing certification in gender-affirming medical care. Dr. Kranz founded the Rochester Center for Sexual Wellness with her husband Dan Rosen, a sex therapist and supervisor in 2017. They see patients and clients of all genders and also do training for therapists interested in becoming sex therapists and sexuality counselors. Since 2021, she has been honored to serve on the board of the Scientific Network on Female Sexual Health and Cancer. Pebble talks with Sabitha and Jane about seeing her patients as whole people, exploring their life stories, asking what sex means to them in their own bodies, and enjoys teaching about so many areas of sexual health and wellness. One of the most-widespread complaints Christine receives from women is they often have low libido and difficulty achieving orgasm. What they don't realize is that the food they are eating affects each of those things. Leafy greens are great for great sex. They increase nerve conduction, blood flow, reduce cortisol, and optimize testosterone. In addition, antioxidants show stronger nerve impulses, protect against damage, and increase clitoral sensitivity. Eating a salad is more about being healthy than losing weight. Chomp on This Before Going on a Date [15:57] In Christine's book, Diet for Great Sex: Food for Male and Female Sexual Health, she includes a Date Night Sex Menu. She describes what greasy foods do to the libido and how refined sugars disrupt hormones and taint our testosterone. Plus, to stay confident and comfortable throughout a date, eat plenty of vitamin C and potassium before leaving the house on a date. Oysters are known as libido-raising food due to their high zinc content. How to Achieve Great Sex Based on Chinese Medicine [26:11] Traditional Chinese medicine teaches us that hormonal balance is the balance of Yin/Yang in the body. Yin is the feminine essence and the physical attributes of the body. Yang is male energy, think nerve conduction, and the heart beating. Balance is key. The traditional Chinese view on sex is that when we have strong Chi we have good sex. And, the essence of life is believed to be housed in the kidneys. She is also Chief of Clinical service for OB/GYN at Emory University Hospital. Gina's clinical areas of interest include geriatric gynecology, neuromodulation, pelvic organ prolapse, urinary incontinence, and overactive bladder. She currently participates in several clinical trials and translational research projects investigating mechanisms of female pelvic floor disorders. Prior to joining the faculty at Emory, Dr. Northington was an assistant professor at the University of Pennsylvania where she also completed her fellowship training. In addition to her administrative positions at Emory, Gina is also fellowship program director for FPMRS at Emory and was responsible for Emory receiving ACGME Accreditation for the Fellowship program the first year it was offered by the ACGME in 2013.
