Today, Dr. Kristin Brogaard is joining me today to talk about the Path Fertility SpermQT test. I'm so excited to have her share about the test, because as you'll hear her say “lots of infertility goes undiagnosed because men go undiagnosed, and when men go undiagnosed, women go through procedures that will not work.” (Who else wants to see this on a bumper sticker?) While sperm were long considered to be just carriers of DNA, it turns out they are a lot more complex than that! Understanding the sperm side of the equation can help anyone trying to conceive better understand what's going on, and help them figure out the best plan to get pregnant. A few exciting highlights from our discussion: Male and sperm epigenetics play a big role in getting pregnant, and having a successful live birth. The SpermQT test can tell someone if their sperm is excellent, average, or poor quality. - Knowing this information can help you and your doctor determine the most efficient and appropriate next steps in your treatment or protocols. Understanding the quality of the sperm can lead to better outcomes. If it is “excellent,” there is an increased likelihood of IUI working, even in cases where doctors thought sperm might be the issue with getting pregnant. If sperm quality is “poor,” you may want to try making lifestyle changes to improve sperm quality, and retest. - Poor sperm quality often correlates with other longer term health issues for men, as sperm acts as a “canary in the coal mine.” (an early warning system) Dr. Kristin Brogaard has a PhD from Northwestern University where she developed a novel epigenetic technology that allowed for a more accurate understanding of gene expression in health and disease cells. She is now the chief scientific officer of Path Fertility, whose goal is to raise the standard in male reproductive health, starting with male infertility. Thank you for joining us, Kristin! When a patient talks about their sexual dysfunction, I always ask: what's going on with the person they're having sex with? You'd be surprised how many providers don't.Most people don't think to ask what's happening on both sides of a sexual relationship. But sexual dysfunction doesn't happen in a vacuum—it's a two-way street. In this episode, I'm joined by Dr. Jessica Yih, a urologist who treats both men and women, and we get real about the overlap of issues that show up in the bedroom. We dive into Peyronie's disease and why it's been overlooked for so long, how injections can break down plaques, and why those curves aren't just uncomfortable—they can lead to serious problems for both partners. We also break down erectile dysfunction beyond the “just pop a blue pill” myth. Spoiler: Viagra doesn't work for everyone.Jessica brings a powerful perspective to the conversation, especially around what it's like treating both members of a couple, sometimes in back-to-back appointments. She's also helping train the next generation of sexual health urologists through her fellowship at UC Irvine.Sex is more than individual dysfunction—it's communication, treatment access, aging, and understanding both bodies in the room. If you're a provider, a patient, or just curious about how these dynamics work together, this episode is for you.Highlights:The truth about Peyronie's disease, painful sex, and treatment optionsWhy Viagra isn't a one-size-fits-all fix for erectile dysfunctionHow injectable medications and implants are changing the gameDyadic sexuality and the importance of treating both partnersJessica Yih's unique path to sexual medicine and her work training new fellowsIf this episode got you thinking, please hit that follow button, leave a comment, and give us a review on Apple Podcasts or Spotify. All too often when talking about infertility, we only tell one side of the story. Traditionally in the public discourse, it has very much fallen into the category of a ‘women's issue'. However, as our guest this week Reece Conca can all too painfully attest, 40% of infertility issues are actually male factor related. Reece Conca is a former Australian Rules Footballer, who played over 150 games for Richmond and Freemantle. When Reece and his partner Bell were finding it difficult to conceive, it took them far too long to discover that Reece has a condition called Azoospermia, which effects 1% of men - a condition where in Reece had no sperm in his semen. In this Vulnerabiltea House, Reece speaks openly and honestly about the physical and emotional toll of a fertility journey he is very much still on, the hormone injections, the surgeries and much, much more. Dr. Ben Bikman discusses the intricate relationship between metabolic health and male infertility. While metabolic health is often associated with conditions like obesity and diabetes, Dr. Bikman emphasizes its relevance to less obvious issues like male infertility. He notes that while the processes of reproduction differ between the sexes, they share a common metabolic core.Ben highlights that infertility affects approximately 15% of couples trying to conceive, with men contributing to around 30% of cases. He explains the role of hormones like follicle-stimulating hormone (FSH) and luteinizing hormone (LH) in male reproductive function, detailing their influence on spermatogenesis and testosterone production. The classroom lecture delves into how poor metabolic health, particularly insulin resistance, can disrupt testosterone production and lead to issues like erectile dysfunction and reduced sperm quality. Insulin resistance affects testosterone synthesis directly in the testes and indirectly by promoting aromatization, the conversion of testosterone into estrogen, leading to a vicious cycle of reduced testosterone and increased insulin resistance.Dr. Bikman discusses various strategies to improve metabolic health and potentially alleviate male infertility, including dietary changes, medication such as insulin-sensitizing drugs like metformin, and exercise, particularly resistance training. He stresses the importance of addressing underlying metabolic issues to improve reproductive outcomes and suggests that prioritizing metabolic health before attempting reproduction is crucial. In conclusion, Dr. Bikman underscores the interconnectedness of metabolic health and reproductive function, advocating for a holistic approach to addressing male infertility that focuses on improving insulin sensitivity and overall metabolic well-being.00:00 - Introduction to metabolic health's relevance in male infertility01:18 - Infertility statistics and men's contribution to the issue02:37 - Hormonal role in male reproductive function: FSH and LH03:53 - FSH and LH stimulation of spermatogenesis and testosterone05:57 - Testosterone's functions in male fertility and maturation08:04 - Nitric oxide's role in erectile function and its insulin connection11:28 - How insulin resistance impacts testosterone production and erectile dysfunction14:02 - Insulin resistance's effects on testosterone synthesis and aromatization18:55 - Strategies to improve metabolic health and alleviate male infertility26:38 - Conclusion: The link between metabolic health and reproductive function#MaleInfertility #FertilityHealth #Metabolism #ReproductiveHealth #InfertilityAwareness #MenHealth #SpermHealth #HormonalHealth #NutritionForFertility #HealthTalk #MaleHealth #FertilityJourney #HealthyLiving #HolisticHealthhttps://www.insuliniq.com Hosted on Acast. I'm delighted to have Dr. Ramy Ghayda join me today to talk about sperm, and the importance of sperm DNA Fragmentation testing and how to improve sperm health. When trying to conceive it takes both an egg and sperm cell. And, as you'll hear Dr. Ghayda explain, we now have access to more tests for male factor infertility, and they are more affordable and accessible than ever. He and I both believe it's very important for people, and fertility patients (everyone, really!) to know their options and to be educated about what they can do to improve sperm quality, to test sperm quality. This can help make the whole journey the best it can be, and be one that hopefully leads to good outcomes. Dr. Ghayda is a reproductive urologist.
