Tranexamic acid (TXA) is a synthetic lysine derivative that binds with the lysine site on plasminogen, inhibits fibrinolysis and stabilizes clot. While it has been around since the 1960's, its popularity for major trauma has gained a lot of steam in recent years. Bu yazı, ALiEM’deki “How I work smarter?” başlığında geçen yazı dizisinin benzerlerinden birisi aslında, dolayısıyla yazacaklarım fazlasıyla bana özel ve subjektif olacaktır. If you're involved in the care of critically unwell patients then you will frequently encounter patients who are shocked. The European Society of Intensive Care Medicine defines shock as; 'Life-threatening, generalized form of acute circulatory failure associated with inadequate oxygen utilization by the cells. Look for ophthalmoplegia, ataxia and confusion in patients that have risk factors for thiamine deficiency. Don't think that it can't be Wernicke's because the triad isn't complete; any two of the components (dietary deficiency, oculomotor abnormalities, cerebellar dysfunction or altered mental status) makes the diagnosis. Treat Wernicke's with an initial dose of 500 mg of thiamine IV and admit for continued parenteral therapy. If they're small and on the extremeties, it's reasonable to leave them unpacked. Not all patients need MRSA coverage for cellulitis. Most cellulitis without abscess is strep. Necrotizing Fasciitis can be tough to pick up. The LRINEC scoring system is one method to help. Most patients will be toxic but look for pain that's out of proportion to the examination. In this first ever episode of the Journal Jam podcast, a collaboration between EM Cases, Academic Life in EM and The Annals of Emergency Medicine's Global Emergency Medicine Journal Club, Teresa Chan and I, along with Jeff Kline, Jonathan Kirschner, Anand Swaminathan, Salim Rezaie and Sam Shaikh from ALiEM, discuss the potential for Age Adjusted D-dimer to rule out pulmonary embolism in low risk patients over the age of 50. We discuss 4 key questions about the ADJUST-PE Study from JAMA in March 2014 including: Would you order a CTPA on a 60 year old woman with an age adjusted D-dimer of 590 ng/L? The problem until now has been that the older the patient, the more likely the D-dimer is to be positive whether they have a PE or not, so many of us have thrown the D-dimer out the window in older patients and go straight to CTPA, even in low risk patients.
