Dr. Jose Luchsinger, Vice Chair for Clinical and Epidemiologic Research at Columbia University, is leading a phase 2 clinical trial of a prodrug Benfo that aims to raise the level of thiamine in the brain to improve memory and function in people with early Alzheimer's disease. This effort is based on research showing that Alzheimer's is associated with impaired thiamine processing in the brain, even if thiamine levels in the blood are normal. The phase 2 clinical trial for the drug is testing the ability to overcome the mishandling of thiamine in the brain by patients with mild cognitive impairment or mild dementia with evidence of amyloid.   Jose explains, "So this approach is based on decades of study by Dr. Gary Gibson, who's at the Burke Institute in New York and Cornell University in New York. What he has found, particularly in animal models of Alzheimer's disease, is that these animals have memory problems that are related to the ability to process thiamine in the brain, which is a key vitamin that is needed for proper function of the brain. The hypothesis that he came up with was that if you could somehow fix the misuse of the thiamine in the brain, that maybe you could improve memory symptoms and also functional problems in people who have Alzheimer's disease. He tested first in animals, a prodrug called benfotiamine that after ingestion it, increases thiamine levels a hundred times in the blood. And he did observe in animals that the thiamine handling deficiency in the brain was fixed, and that memory and other characteristics in these animals improved."   "One thing that I want to explain is that we're not saying that people with early Alzheimer's disease have low thiamine, which is vitamin B1 in the blood. What we're saying is that because of the Alzheimer's disease, their brains are not handling the thiamine that there is in the blood, even if it's at normal levels, in the correct way. 部落格圖文版 https://linshibi.com/?p=46969 更多藥物解說 https://linshibi.com/?cat=4347 許多朋友都說日本版比台灣版有效,但其中是否有台灣視為管制藥品的成分?到底可不可以從日本帶回呢? 日本大正百保能黃金A微粒(大正製藥パブロンゴールドA微粒) PABRON GOLD A〈GRANULES〉 1.這是複方的感冒藥物,其成份可舒緩感冒可能有的各種症狀(咳嗽、痰、喉嚨痛、打噴嚏、流鼻水、鼻塞、畏寒、發燒、頭痛、關節疼痛、肌肉痠痛) 。 2.主要有七種成分:1包(0.96g)中含有 Acetaminophen(乙醯胺酚)300mg:大家很熟悉的普拿疼,解熱鎮痛用。 Dihydrocodeine Phosphate(磷酸二氫可待因)8mg:鎮咳劑。 Guaifenesin(癒創木酚甘油醚) 60mg:袪痰劑 dl-Methylephedrine Hydrochloride(鹽酸甲基麻黃鹼)20mg:支氣管擴張(鎮咳)。 Chlorpheniramine Maleate(縮水蘋果酸氯芬尼拉明)2.5mg:抗組織胺,治鼻塞、鼻涕 Anhydrous caffeine(無水咖啡因)25mg:止痛藥EVE中也有。 Riboflavin (vitamin B2)(核黃素)4mg:維他命B2。 3.台灣版的大正百保能感冒顆粒則是1包(1.2g),有八種成分。主要不同的地方有: a.鎮咳劑改為Dextromethorphan hbr(右美沙芬)15mg。 二氫可待因是屬於類鴉片(opioid)藥物,在台灣屬於管制藥品,要由醫師開立處方,一般藥局無法取得,因此替換為較沒有成癮問題的右美沙芬。 b.抗組胺有改變。不過兩者都是第一代抗組胺,同樣都可能有嗜睡的副作用。因此仿單中有寫服用後請勿駕駛操作交通工具或機械類。 c.鹽酸甲基麻黃鹼從20mg降為10mg。我想是因為這類藥物可為製造安非他命類毒品之毒品先驅原料,因此故意降低其劑量。 d.多增加了Thiamine mononitrate維他命B1 8mg。 04b解讀: 1.感冒是病毒造成的上呼吸道感染的俗稱,可以有非常多症狀。病人來就醫時,醫師會依據您有什麼症狀針對性的給予單方藥物。所以我個人是不太建議大家使用這樣的複方大雜燴藥物,因為你可能會吃到不需要吃的藥物,而承受不必要的風險。 2.比方說如果你主要是發燒喉嚨痛,其他症狀都不明顯,你需要的是解熱鎮痛的藥物,而不是大正百保能。因為他解熱鎮痛的成分是最普通的乙醯胺酚罷了,沒有太大的效用!你可能還不如使用EVE效果會好些。 3.就成分上來看,日版顯然在止咳上效果會較佳,但風險是嗜睡的副作用有可能較明顯。另外主要就是在台灣屬於管制藥品的二氫可待因,可能會有噁心、嘔吐、便秘、鎮靜、嗜睡、皮膚癢等副作用,並隨劑量上升而增加。長期使用更可能產生成癮性,高劑量會有呼吸抑制風險。 4.另外一個日本很常見的新LuLuA感冒藥也同樣含有二氫可待因。日本近年在年輕族群出現了購買市售感冒藥物濫用的情形,因此各位常常到日本藥妝店目前會遇到每人限購一盒的狀況,這個之前已經和大家分享過了。 5.至於到底能不能帶回台灣?我的理解是侷限於自用,可以。而現在藥妝店普遍都有限購,我其實也沒有太擔心大家會帶太多。只是如果你帶的量會讓海關懷疑這超過了自用的量,可能會引起關注。 最後是老話一句:不要迷信日本成藥,不要把它當土產買來送人! 更多藥物解說 https://linshibi.com/?cat=4347

Today, we're going to take a look at vitamin B1, also known as thiamine. Vitamin B1 is intimately involved with the function of the mitochondria, so if you're deficient, low energy is one of the first symptoms. The more carbs and sugar you consume, the more B1 you need. This is because vitamin B1 is necessary to burn glucose. Even on a ketogenic diet, you'll still burn glucose for certain functions of the body. Vitamin B1 is water soluble, so it isn't stored in the body for long periods. This makes it easier to become deficient. B1 is also important for the myelin sheath—the coating on your nerves. B1 is crucial for the proper function of the autonomic nervous system and specifically affects the vagus nerve. The following conditions and symptoms are related to a deficiency in vitamin B1: •Gastroparesis (slow digestive system) •GERD •Peripheral neuropathy •Regurgitation of bile •Excessive sweating or no sweating at all •Excessive tears or no tears at all •Dizziness when standing up •Irritability •Low tolerance for stress •Trouble regulating body temperature •Increased pulse rate If you have a more severe B1 deficiency, you might experience the following: •Enlarged heart •Edema •Confusion •Memory loss •Nystagmus •Ataxia •Hearing loss •Sleep apnea •Trouble breathing Avoid synthetic B1, which is sometimes made with hydrochloric acid, acetone, ammonia, and coal tar. Allithiamine and benfotiamine are the two best options for vitamin B1 supplements. Drugs can also affect the absorption of vitamin B1. Metformin, diuretics, and certain antibiotics can lead to vitamin B1 deficiency. Low magnesium can affect vitamin B1 absorption as well. For the first day of the new year, we dive into what is considered the first proposed vitamin, vitamin B1 also called thiamine.This episode goes in-depth on the critical role thiamine plays in our bodies, and its necessary presence in adequate amounts for producing ATP, our cellular energy currency. In this episode, I go over the often-overlooked connection between carbohydrate intake and thiamine needs, and the consequences that can follow when this vitamin is in short supply.When thiamine levels drop, it's not just our energy that's at risk – our brain health also might hang in the balance. I dive into the neurological consequences of thiamine deficiency (and even a sustained state of inadequacy...) like the development of Wernicke-Korsakoff syndrome, a two-part disorder that is usually, although not exclusively associated with chronic alcohol dependency. I will also consider thiamine's [or lack thereof] potential links to Alzheimer's and Parkinson's diseases. Other brain responses to a marginal thiamine shortage, include adaptations that could lead to reduced appetite and anorexic behavior. The theoretical rationale will be provided in this episode.As I round out the episode, I go over the groups most vulnerable to this nutrient shortfall. Lastly, and perhaps most importantly, I will confront the silent epidemic of thiamine deficiency that has been suggested by many to be hiding behind very common symptoms or metabolic disorders. Culinary Medicine: Food Cons & Food Conversations

The term vitamin was coined by the chemist Casimir Funk as he revealed the Thiamine structure to the world. So while you may think – isn't vitamin A (it is the alphabet) the first vitamin – we take our poetic license here to point out that the first compound to be called a vitamin was Thiamine, now known as vitamin B1. But the story of that vitamin was made possible by two individuals – but the most noteworthy was Takaki Kanehiro.Our podcast today discusses the education of Kanehiro and how that influenced his view. But let's work backward a bit.Vitamin B1 and BeriberiBeriberi is caused by a lack of vitamin B1 (thiamin) in the diet. This vitamin is essential in the biochemical process of utilizing fats, carbohydrates, and proteins for energy. The nerves are particularly affected because of their metabolic pathways – hence, a wide range of symptoms from difficulty with walking (motor function) then sensory function of the legs, and finally, it affects the conduction in the heart, which leads to death.White Rice and BeriberiWhite polished rice was the fad food not only in the cities of Japan but also in China. To polish the rice, the husk and germ are removed, but this is also where the vitamins are. When people eat just one food item, they are likely to develop a deficiency disease, and this was the case with sailors and white polished rice.The officers would not only eat rice but were given a variety of foods and thus avoided beriberi. But the crew typically only ate white rice.Beriberi in Mid-1800's JapanIn Japan in the mid-1800s, especially in the city, polished white rice was the calorie source of choice. White rice is easy to store, looks clean, is faster to cook, and eating it is less chewy. White rice was considered what the wealthy people ate – it was beautifully displayed, and unlike brown rice, did not go rancid. What they didn't know was the processing of the rice led to stripping the rice of the vitamin-rich portion. In the rural areas of Japan, there was little beriberi as they mostly ate brown rice (barley rice). This led to a major problem in their Navy, where beriberi was endemic. White polished rice was the caloric source for the Navy, and often one third of the crew would develop beriberi and be unable to carry out their duties.There were many thoughts as to the cause of beriberi, but none of them had panned out well.Eastern Traditional Cures for BeriberiOn occasion, Eastern healers would recommend various teas and herbs. But, some would recommend barley rice. Some of these cures are retrospective, meaning once the traditional Japanese medical folks learned that barley rice was a cure for beriberi, they stated that they had prescribed it for years. Barley rice does have vitamin B1 (1 cup contains a day's supply). But during the time, the cures from these village doctors were rare and not enough for the Japanese Navy, in spite of losing more people to beriberi than to war – would adopt. Also, note that barley rice is not as transportable as white polished rice and has a shorter shelf life. During this time, Japan was becoming westernized, and they were working on importing medical ideas from Germany, although they had a fair bit of influence from the British method. Wernicke's encephalopathy (WE) is a neurological syndrome secondary to severe, short-term B1 deficiency. In the past, but less frequently nowadays, it was more commonly associated with alcohol use disorder. However, today we know that any condition that decreases dietary intake and increases thiamine use, or its elimination, puts patients at risk of developing this encephalopathy. Causes:Chronic alcoholism is the most important cause of WE. Around 70-80% of the causes are associated with chronic alcohol consumption. Non-alcoholic WE may be caused by Decreased intake: Some types of WE may be caused by a psychiatric illness that decreases the dietary intake of B1, such as anorexia nervosa, schizophrenia, or dementia. Arreaza: Also, prolonged fasting or starvation.Lack of absorption of B1: Other causes might be related to malabsorptive syndromes, bariatric surgeries, or hyperemesis gravidarumIncreased use of B1: Any disease that increases the use of B1 and, therefore, low levels of thiamine, such as cancer, thyrotoxicosis, and systemic illnesses like infections. High-carb diets are associated with high thiamine use. Also, patients who receive IV glucose w/o supplements are at risk of developing Wernicke's encephalopathy.Increased elimination of B1: Other causes are related to increased elimination of B1, such as dialysis.Immunodeficiencies: Immunodeficiency syndromes and transplantation also cause WE.Why is thiamine important?Thiamine is one of the main cofactors in three key enzymes for energy metabolism: alpha-ketoglutarate dehydrogenase, pyruvate dehydrogenase, and transketolase. If we go back to biochemistry in med school, we can remember these enzymes play a significant role in the Krebs cycle and pentose phosphate pathways. Thiamine uses Mg+2 as a cofactor, so a magnesium deficiency can mimic WE.Pathophysiology.B1 deficiency causes lactic acid accumulation due to anaerobic glycolysis, leading to neuronal cytotoxic edema and vasogenic edema with petechial hemorrhages. MRI of the brain shows symmetrical hyperintensities, most commonly in the thalamus, mammillary bodies, cerebellum, and the periaqueductal area surrounding the third and fourth ventricles. The diagnosis of WE is made clinically, even though the MRI is a useful complementary tool to the clinical diagnosis. Diagnosis.WE presentation has always been described as the classic triad of ophthalmoplegia (or nystagmus), encephalopathy (confusion or memory impairment), and gait ataxia. However, this presentation is present only in less than 20% of patients, and most of the patients present with a neurologic syndrome that includes 2 out of the classic triad plus nonspecific symptoms such as hallucinations, hypothermia, hypotension, indifference or inattentiveness, seizures, behavioral disturbances, and bilateral lower extremity weakness. In 1997, Caine et al. suggested that a diagnosis of WE can be made if 2 out of 4 of these criteria were present in a patient with ophthalmoplegia/nystagmus + ataxia + memory impairment or confusion and clinical evidence of malnutrition or from laboratory data. Thiamine levels can be normal in patients with WE, so thiamine level is not a requirement for diagnosis.Almost 80% of WE cases are diagnosed on autopsy, which means this disease goes undiagnosed most of the time. The diagnosis is clinical, and MRI can assist in cases that are uncertain. Treatment.Thiamine supplementation is inexpensive, accessible, and easy to administer, so if we have a patient with a suspicious neurologic syndrome that could be WE, B1 must be given as soon as possible. Treatment should not be delayed while waiting for MRI results, The treatment consists of IV thiamine 500mg TID for 2-3 days, followed by 250 IM or IV for additional 5 days, in combination with other B vitamins. Because GI absorption of thiamine is impaired in alcoholics and malnourished patients, oral administration is contraindicated during initial treatment for WE. Thiamine 100 mg PO should be continued after the completion of parenteral treatment and after discharge from the hospital until patients are no longer considered at risk. Magnesium and other vitamins are replaced as well, along with other nutritional deficits if present.B1 blood levels or erythrocyte transketolase activity (which is a way to evaluate thiamine deficiency) are measured before and after thiamine pyrophosphate supplementation. A low transketolase and a more than 25 percent stimulation are positive for thiamine deficiency. This test is often not readily available, especially at the ED. High-quality liquid chromatography can also measure serum thiamine or TPP level in serum or whole blood.For practical purposes, and given high undiagnosed rates and mortality, IV thiamine should be given to all patients with malnutrition admitted to the hospital or seen in clinic. __________________________Conclusion: Now we conclude episode number 117 “Wernicke's Encephalopathy.” Dr. Malave explained that a low thiamine level is not required for the diagnosis of Wernicke's encephalopathy. Start supplementation if you have a high suspicion of thiamine deficiency, especially before giving IV glucose to a malnourished patient. We were reminded that GI absorption of thiamine is impaired in patients with chronic alcohol use, so make sure you give thiamine IV or IM. This week we thank Hector Arreaza and Maria Fernanda Malave. There are eight B-vitamins that benefits overall health, including keeping your nerves healthy. But three of these vitamins – Thiamine (B1), Pyridoxine (B6), and Cobalamin (B12) are mainly known to support nervous system function. Continue reading to find out how these three B-complex vitamins help support your nervous system and keep it in tip-top shape. Thiamine (B1) Vitamin B1 has a huge benefit to your central nervous system (CNS). This vitamin aids the synthesis of acetylcholine, which is one of the most abundant neurotransmitters in your CNS. This chemical helps to relay messages from your brain to your body muscles, thereby influencing movement and behavior. Acetylcholine also helps memory and cognitive function. More so, it plays a role in regulation of hormones, learning, sleep cycles, and pain sensation. Now, imagine if your body is not producing enough of acetylcholine because you're having vitamin B1 deficiency. You'll not only experience fatigue, but you'll suffer poor short-term memory and confusion. Your feet and hands may become numb and your movement hampered. So ensure you're getting enough of vitamin B1 in your diet. Some thiamine-rich foods you can include in your diet include beans, nuts and seeds, beef, yeast, liver, oranges, eggs, pork, peas, legumes, and oranges. Pyridoxine (B6) While thiamine is essential for the synthesis of acetylcholine, pyridoxine is needed in the synthesis of other neurotransmitters like norepinephrine and serotonin. Norepinephrine is the stress hormone helpful in triggering a fight-or-flight response. This neurotransmitter helps increase the amount of oxygen in your brain, helping you to think and make decisions faster. Serotonin is an import neurotransmitter responsible for regulating your appetite, mood, and sexual desire. It also helps to enhance your memory and maintain your sleep cycle. Vitamin B6 also plays an important role in myelin formation – the insulating sheath around your nerves. This helps in quick and efficient transmission of electrical impulses along your nerves. When you're deficient in vitamin B6, you tend to feel more tired, irritable, nervous, and depression. This vitamin deficiency is also linked with conditions such as neuritis, neuralgia, and carpal tunnel syndrome. So it is important to add pyridoxine-rich foods to your diet. Some good options including meat, legumes, bananas, avocados, fish, tofu, soybean, and potatoes. Cobalamin (B12) Both vitamin B6 and B12 are needed for maintaining your myelin. Cobalamin helps in the synthesis of myelin that protects the nerves. If myelin becomes damaged, it can lead to dysfunctional nerves, which leads to problems such as peripheral neuropathy. Pregnant women who do not eat foods rich in cobalamin puts their babies at risk of nervous system disorders like seizures, brain atrophy, microencephaly, and even blindness. Some good food options that are rich in vitamin B12 include read meat, fish, poultry, eggs, lamb, liver, fortified yeast products, and milk. Looking at these benefits of the B-complex vitamins for your nervous health, it is important that you get enough of these vitamins from your diet or take supplements (as approved by your doctor). In the previous article, we've described what B vitamins are, the eight types, their functions and food sources. In this article, let's proceed to talk about the common symptoms associated with a deficiency in each of the eight B-group vitamins. B vitamins are essential for maintaining cell health and keeping you energized, and you must keep taking them in adequate amount since they are water-soluble and are not stored in the body. This means excess amount is given out through urine, and you have to load up on these vitamins daily to stay healthy. Some foods are high in several B vitamins, while certain foods are particularly high in a specific B vitamin. However, eating a balanced diet is key to getting all these vitamins, at least in amounts that your body needs. How much of B vitamins should you be taking? The recommended daily intakes are: Vitamin B1: 1-1 – 1.2 mg Vitamin B2: 1.1 -1.3 mg Vitamin B3: 14 -16 mg Vitamin B5: 5 mg Vitamin B6: 1.3 mg Vitamin B7: 30 mcg Vitamin B9: 400 mcg Vitamin B12: 2.4 mcg A normal healthy and balanced diet should supply you with all these vitamins in adequate amounts, but some common symptoms are associated with a deficiency in any of these vitamins. However, if you have a deficiency, it is something that must be determined by your doctor. But unfortunately, some of us suffer vitamin B deficiencies. The high-risk group includes pregnant women, people with certain conditions such as Celiac disease, Crohn's disease, and HIV. What are the symptoms of Vitamin B deficiency? Symptoms of vitamin B deficiency vary based on the particular B vitamin you're deficient in. Here's a quick rundown of the symptoms of deficiency of each B vitamin Vitamin B1 and vitamin B2 deficiency is very rare because many foods including whole-grain cereals and milk are fortified with these vitamins. Vitamin B3 deficiency is also very rare but severe deficiency of this vitamin can result in a condition known as “pellagra,” which is characterized by symptoms such as: a bright red tongue a rough skin that turns red or brown in the sun constipation and diarrhea vomiting aggressive, paranoid, or suicidal behavior fatigue hallucinations Vitamin B5 deficiency is extremely rare because it is found in such a variety of foods, including kidneys, milk, egg, meats, legumes, and many more. Vitamin B6 deficiency is also rare, but people women on contraceptive pills, people who drink excessive amounts of alcohol, people with thyroid disease and the elderly are more at risk. Vitamin B7 is required in very small amounts and it is widely distributed in foods, so its deficiency is very rare. However, over-consumption of raw egg whites for several months can induce deficiency. This is because a protein in egg white inhibits the absorption of biotin. Bodybuilders should keep this in mind. Vitamin B9 deficiency is also very rare, but low levels of this vitamin can result in the following: trouble concentrating fatigue megaloblastic anemia, which cause weakness heart palpitations shortness of breath headache irritability change in hair, skin, or fingernail color open sores in the mouth Pregnant women who are deficient in vitamin B9 or folate could end up having babies born with neural tube defects. Vitamin B12 deficiency can lead to disruption in the circulatory and nervous system. Also, a deficiency in this vitamin can lead to megaloblastic anemia, a condition characterized by the production of large abnormally shaped red blood cells by the bone marrow. The red blood cells do not function properly. Some common symptoms associated with vitamin B12 deficiency includes: weakness tiredness or fatigue confusion weight loss constipation poor memory soreness of the mouth balance problems loss of appetite If you suspect you might be deficient in a B vitamin, you can contact your doctor for physical examination and blood testing. To prevent a deficiency in the B vitamins, however, you should always eat a varied diet of vegetables, fruits, lean meats, and grains. This way, you'll be getting all the nutrients you need. Contributor: Aaron Lessen, MD Educational Pearls: Prior, smaller studies showed intravenous Vitamin C given to patients with sepsis significantly improved patient mortality and additional outcomes.  A recently published, randomized control trial with >800 ICU patients who received up to 4 days of IV Vit C or placebo concluded that the end points of death or persistent organ dysfunction at 28 days were unaffected by Vitamin C administration.
