Most People Don't… But YOU Do! with Erik MeltzerBart sits down face to face with Erik Meltzer in a community room in Old Town Alexandria for a conversation that starts with an unusual career path and keeps opening into something deeper. Erik traces his journey from a kid who talked his way into covering President Clinton at age 14, through journalism school and TV news, to writing newscasts for a dissident Chinese station that was literally saving lives, to a dozen years training TV stations around the world, and finally into entrepreneurship: blockchain-backed real estate through Plutus Properties and a heart-centered networking group called The Salon Circle. The thread running through all of it is curiosity, and Erik is refreshingly clear about where his comes from. A hard childhood led him at fifteen to find a free meditation practice he has now kept for 26 years, one that reshaped how he handles adversity. That sets up the heart of the episode: a shared belief that every hardship, good or bad, is raw material for becoming better.Bart and Erik trade stories on mindset, the mentors and friends they both learn from, why most people never ask a single question about anyone else, and what it actually takes to be a connector who gives without keeping score. It is a warm, honest conversation about curiosity, humility, and the quiet power of making other people feel seen. What you'll take away from this episode:Curiosity is a muscle, not a personality trait. Being interested in others is intentional. Left on autopilot, everyone defaults to "me, me, me."Disarming beats impressive. Erik gets access to remarkable people by being non-threatening, sincere, and genuinely uninterested in extracting anything. People feel safe, so they open up.Every hardship is raw material. Gold gets forged through heat. The skill isn't avoiding hard times, it's shrinking them, taking it from minutes to moments."What do you do" is a weak opening. "Why do you do it" is the real one. The upgrade isn't avoiding the question, it's going one layer deeper.Connecting is the strategy. Business and meaning both flow out of relationships, not the other way around.Moments worth remembering:"Being curious is an intentional behavior. If I am not being intentionally interested, then I will default back to me, me, me.""People find me not terribly threatening, and I'm a pretty good listener. I don't really have anything that I'm trying to gain from other people.""Everything that happens in life, whether it be good or bad, is a good thing, because it can lead to self-improvement.""It's not 'will I never be upset?' It's, can I turn it from minutes to moments?""It's okay to know what people do for a living. Where it gets real is, tell me more about why you do what you do."Connect with Erik Meltzer:Instagram: @theerikmeltzerLinkedIn: Erik MeltzerThe Salon Circle: joinsaloncircle.comConnect with Bart and the show:If this episode resonated with you, do three things. What if wilderness therapy has been helping people heal for a long time—but the field never fully understood why it works? In this episode Will sits down with Rob Meltzer, founder of the new Wilderness Therapy Institute. Rob explains why he believes wilderness therapy may function as a form of "metabolic health treatment" and shares groundbreaking efforts to study how sleep, movement, sunlight, nutrition, circadian rhythm, inflammation, and extended time in nature impact mental health. The conversation explores emerging research in metabolic psychiatry, biological mechanisms of healing, and why wilderness experiences may create profound psychological and physiological change. At the same time, this episode does not avoid the field's controversies. Rob and Will examine critical questions surrounding authority, transport practices, ethical intervention, trauma, and the evolving identity of wilderness therapy in a post-"Hell Camp" era. Rob argues that the field must improve without losing its core foundations: extended wilderness immersion, primitive living, deep human connection, and nature-based healing. This episode will resonate with therapists, researchers, parents, former students, outdoor professionals, and anyone questioning both the promise and the problems of wilderness therapy today. Wilderness Therapy Institute Webpage: Rob was featured in pervious episodes: Episode 5: Rob Meltzer, educational consultant and founder of the wilderness therapy symposium 201: Celebrating 20 Years of Wilderness Therapy Symposium This podcast is supported by White Mountain Adventure Institute (wmai.org), offering adventure inspired retreats and coaching for men and facilitated by Will White.
Hosted by Michael Tetreault | Editor-in-Chief, Concierge Medicine Today Episode Overview In one of the most comprehensive episodes in DocPreneur Leadership Podcast history, host Michael Tetreault takes an honest, evidence-based, and encouraging look at the cash-pay and subscription-based primary care landscape — who it serves, how it works, where it's heading, and what every physician and advanced practice clinician needs to understand before making a career-defining decision. This episode doesn't take sides. It takes a clear-eyed look at the full picture — including the parts that don't always make it into the conference keynote. What's Covered in This Episode The Foundation Not all subscription-based primary care models are the same. Two models operating in this space share surface-level similarities but are structurally distinct businesses with different economic logic, different patient populations, and different long-term trajectories. Understanding which one you're considering — and why — changes everything about how you plan. A Lesson From Healthcare History Before committing to any practice model, it helps to understand what happened to the movements that came before it. This episode traces three instructive parallels: the micropractice and ideal medical practice movement of the early 2000s; the decades-long fight for healthcare price transparency and what happened when physicians finally got it; and the rise and reality check of retail health — what scaled, what didn't, and why. The common thread in every model that has achieved durable scale in American healthcare is the same: structural fit with the economic environment, not ideological purity. Two Pathways, One Brand Name The episode walks through both economic models in the cash-pay primary care space — the purist, cash-only, no-insurance model and the employer-integrated model — explaining how each works, who each serves, and what the financial picture actually looks like for physicians considering either path. The revenue math is done out loud. The sustainability data from peer-reviewed research is cited. The patient demographic fit for each model is examined honestly and specifically. Who Each Model Serves — and Where Other Models Fit Better A detailed breakdown of the patient populations each model genuinely serves well — and an honest, evidence-based look at the patient populations where other models may be a better structural fit. Including Medicare-eligible patients, patients with complex chronic disease, lower-income households, and employees of small and mid-sized businesses. The Overlooked Opportunity — NPs, PAs, and Advanced Practice Clinicians One of the most significant and underexplored opportunities in subscription-based healthcare delivery today is the direct-care model as a pathway for nurse practitioners, physician assistants, and other advanced practice clinicians. The evidence on NP and PA-led primary care outcomes is strong and peer-reviewed. The physician shortage projections make the need urgent. And the organizational infrastructure for advanced practice clinician-led direct-care practices is largely unbuilt — which means the opportunity belongs to whoever moves first. The Organizational Landscape An honest look at what the multiplicity of organizations, coalitions, and alliances in the cash-pay primary care space tells us — and what research on professional association dynamics says about the long-term implications of organizational fragmentation for legislative effectiveness and individual practice planning. One Brand, Two Directions Drawing on four documented historical parallels from the history of American medicine — the AMA and managed care, osteopathic medicine's identity divide, family medicine's emergence as a separate specialty, and the micropractice movement — the episode makes the case that two communities with genuinely different economic interests and regulatory priorities currently sharing a brand name may, consistent with historical precedent, find their own distinct professional homes over time. This is presented as pattern recognition grounded in verified historical evidence — and as practical planning context for physicians building practices today. The Tax and Structuring Update A clear, practical summary of the 2025 "One Big Beautiful Bill" Act changes — effective January 2026 — and what they mean for HSA eligibility of cash-pay membership fees. What qualifies, what doesn't, and why legal counsel is essential before making any representations to patients about tax-advantaged payment options. Eight Questions Before You Commit A practical pre-decision checklist — eight specific questions every physician or advanced practice clinician should be able to answer clearly before committing to any cash-pay practice pathway. Key Takeaways Cash-pay primary care and concierge medicine are not the same model, do not serve the same patient populations, and should not be evaluated as interchangeable alternatives. The purist cash-pay model has grown from approximately 100 practices in 2009 to over 2,100 by 2023 — real and meaningful growth. The financial sustainability data, however, reflects consistent challenges that peer-reviewed research has documented specifically in lower-income markets and solo practice settings. The employer-integrated pathway has stronger structural sustainability — multiple revenue streams, embedded benefit relationships, and documented employer cost reductions of 12 to 20 percent over three to five years. A December 2025 Johns Hopkins study found concierge and cash-pay primary care practices combined grew 83.1 percent between 2018 and 2023. The employer-integrated model is the primary driver of that growth trajectory. Concierge medicine — particularly the PCM model — is not retreating. The global concierge medicine market is projected to surpass $34 billion by 2032 and is growing at a compound annual rate that outpaces most healthcare market segments. The National Academy of Medicine's 2021 Future of Nursing report, AAMC physician shortage projections, and peer-reviewed NP/PA outcomes research collectively point to advanced practice clinician-led direct-care models as one of the most significant underexplored opportunities in subscription-based healthcare delivery. Pattern recognition from healthcare history — price transparency, retail health, the micropractice movement — consistently shows that the distance between a compelling healthcare idea and durable scaled impact is longer and more complicated than early advocacy suggests. Models that have achieved durable scale in American primary care share one characteristic: structural fit with the economic environment, not independence from it. "I'm not suggesting that repression has lost its place as a fundamental defense mechanism. Repression remains central, coherent, and fundamental to the founding of the unconscious. It is what makes certain contents inaccessible to consciousness, and what we access as psychoanalysts through dreams, play, symptoms, and associations. That remains true. What I was observing, and I'm still observing more now, is something different. When I see children and adolescents that are more capable to work on a task while doing homework, and at the same time listening to music, and at the same time texting with somebody - I don't think that they are real. This is my point. I don't think they are real. This multitasking way of living is part of life today… The clinical question for us becomes this - when does this multiplicity become a symptom? When does it interfere with the capacity for depth, for intimacy, for a sustained emotional contact? I think that this is what we need to see, to study and to differentiate in our consulting room." Episode Description: We consider how changes in our culture may impact the individual's intrapsychic space and from that the nature of the psychoanalytic encounter. Virginia comments on the diminishing of the paternal symbolizing function and with that a change in the 'rites of passage' that adolescents traverse - now the rituals are "created by the young people themselves" as contrasted with those passed down by their elders. This, she feels, has resulted in "intimacy becoming spectacle" and for many, the analytic session is where "the construction of intimacy may begin." She shares clinical material with us from 40 years ago and contrasts the nature of her interventions with her contemporary treatments. Now, "I appreciate the mystery in the process and that we create meaning with the patient." Virginia closes with seeing analytic treatment as "an invitation to a process of thinking that, to remain alive, must be rethought." Our Guests: Virginia Ungar M.D., training analyst at the Buenos Aires Psychoanalytic Association (APdeBA). She specializes in child and adolescent analysis, was the Chair of the IPA's Child and Adolescent Psychoanalysis Committee (COCAP) and of the IPA Committee for Integrated Training. She was awarded a Konex of Platinum in 2016. She is the former President of the International Psychoanalytic Association (2017-2021). Recommended Readings: Etchegoyen, H. Ted Houghton, Tomas Antona, Dave Giffen and Tom Meltzer recruited drummer Stephen Moses and quickly found a substantial audience at CBGB. Guitarist Michael Jung soon replaced Meltzer. The band's first commercial release was the Donut Comes Alive album, released in 1988 on Jello Biafra's Alternative Tentacles label, followed in 1989 with Bucketfulls of Sickness and Horror in an Otherwise Meaningless Life.
Things got a little wonky this week, so we are re-releasing an episode that can keep with the theme of Women in Cinematography. So, this week, we are revisiting Chantal Akerman's 1975 film, "Jeanne Dielman, 23, quai du Commerce, 1080 Bruxelles" with guest Sam Meltzer. This film was shot by cinematographer Babette Mangolte. The film is truly an exemplary feminist cinematic work. With long lingering shots, “Jeanne Dielman” brilliantly frames a woman's ever-shrinking existence. The production design keeps Jeanne's world relatively stark. But it is Mangolte's cinematography that plays with this idea of a stifling, sometimes suffocating, existence. When Sam was on, we discussed Jeanne Dielman through Chantal Akerman's direction, but it also makes sense this film had a woman as the director of photography. In lesser hands, Jeanne could have easily been reduced to her age, her profession, or her proximity to men. For a film that has minimal dialogue, the cinematography is critical to articulating so much about Jeanne, her life, and her inner workings. So, while we may not explicitly discuss Mangolte's work, it certainly permeates the conversation. Enjoy revisiting "Jeanne Dielman."You can follow SamLetterboxdYou can follow Female Gaze: The Film ClubInstagramBlueSkyWebsite

The most ridiculous and delusional wrestling awards are back, and despite being panned by almost everyone with a shred of common sense, Meltzer had the audacity to say these were the “best received awards” ever. Obviously this guy doesn't look beyond his own bubble of braindead unwashed virgin subscribers - who he leads towards voting for what he wants anyway - but he also has the warped sense of self-importance to present these awards as some type of official barometer for the year that was— despite the fact that 90% of wrestling fans won't even be familiar with either the “wrestler of the year” or the “promotion of the year.”

In this episode, we talk to Fred Patterson, aka Phast Phreddie, about his work at the ARChive of Contemporary Music, plus his own magazine Back Door Man and his band Thee Precisions. Beginning with his early musical life teaching classes with Don Waller at UCLA's Experimental College, we hear about how he named Back Door Man after a Howlin Wolf song and wanted to cover "hard core rock n roll". Phreddie remembers how different it was working with R. Meltzer compared to Lester Bangs, who both contributed to the mag. Next, we hear about where Thee Precisions got their name and how they were intended as a garage rhythm 'n' blues combo that might fit into south central L.A.'s rockabilly scene without doing rockabilly. After telling us about moving across the country to help set up a record shop in Albany, Phreddie then takes us back to the ARChive of Contemporary Music's original mission statement to collect two of every record and explains how they operate. Three clips from a 1977 Mary Wilson audio interview prompt Barney to wonder whether the Supremes are the greatest girl group of all time. To wrap things up, we pay tribute to Sly Dunbar of Sly & Robbie before Mark and Jasper talk us out with highlights from pieces including interviews with Isaac Hayes and De La Soul. Many thanks to special guest Phast Phreddie Patterson. Find out more about the ARC at arcmusic.org. Pieces discussed: Articles, interviews and reviews from Phast Phreddie Patterson, The L.A. Welcome to Episode 164 of Wrestling Tonight, powered by G FUEL and Dick Lazers. Use code TAVERN for 20 percent off. Acefield Retro and Chad are back, and this week is built around the road to Royal Rumble: Riyadh, while also zooming out to examine how WWE, AEW, TNA, AAA, NJPW, Stardom — and the global wrestling economy itself — are all adjusting direction at the same time. We open with a full preview of Royal Rumble: Riyadh, taking place January 31, 2026 at the King Abdullah Financial District as part of Riyadh Season. This marks the first traditional Royal Rumble ever held outside North America, the first Royal Rumble in Saudi Arabia, and the first of WWE's "Big Five" events to take place in the Kingdom. We break down why this Rumble matters more than usual, with both the men's and women's winners earning world championship matches at WrestleMania 42 in Las Vegas, the event's return to its traditional January slot, and the shift in distribution with ESPN streaming the Rumble in the U.S. for the first time while Netflix carries most international markets. From there, we dig into what's already locked in for Riyadh: the Men's and Women's Royal Rumble matches, Drew McIntyre defending the Undisputed WWE Championship against the winner of the Saturday Night's Main Event four-way, and Gunther vs. AJ Styles in a career-threatening match where Styles must retire if he loses. We also run through the early betting odds, where Roman Reigns and Bron Breakker sit at the top of the board, followed by Sami Zayn, Gunther, and Cody Rhodes, and discuss what those numbers tell us about WWE's short-term and long-term thinking. We cover the growing list of announced entrants on both the men's and women's sides and how the field is already beginning to take shape. We then pivot into Saturday Night's Main Event fallout from Montreal, breaking down Cody Rhodes vs. Jacob Fatu, AJ Styles vs. Shinsuke Nakamura, the Women's Tag Team Championship defense with Rhea Ripley & IYO SKY against Liv Morgan & Roxanne Perez, and the massive four-way number one contender's match featuring Damian Priest, Randy Orton, Sami Zayn, and Trick Williams, with the winner stepping directly into McIntyre's title picture at the Royal Rumble. From there, we widen the lens to WrestleMania season planning. We discuss WWE's current reluctance to turn Cody Rhodes heel, how WrestleMania 42 plans remain fluid, and why AJ Lee vs. Becky Lynch is shaping up as one of the most locked-in matches on the card. We also look ahead to WrestleMania 43 in Saudi Arabia, with The Rock openly addressing his excitement for the event and internal speculation swirling about possible appearances from Rock and Stone Cold Steve Austin as WWE enters its busiest stretch of the year. We then shift fully into the global business of wrestling, as Dave Meltzer reports that Místico has headlined 13 consecutive sellouts at Arena Mexico in 2026, a venue that holds approximately 16,000 fans. Meltzer called the run unprecedented, noting that sustained, high-frequency sellouts in the same building represent a business pattern rarely seen in wrestling history. We contextualize the streak against past drawing eras, the legacy of El Santo, and why comparisons to Japan or North American touring models often miss the structural realities of how wrestling draws actually function. We also discuss the growing push for Místico to capture a major championship currently held by talent from All Elite Wrestling, and what that would signal about CMLL's place in the current power structure. Outside WWE, AEW remains in a moment of transition. We break down Powerhouse Hobbs officially signing with WWE, the company parting ways with longtime executive Nik Sobic, and what those exits say about AEW's current phase. We also cover Will Ospreay's next step toward a return as he undergoes a medical evaluation, Hikaru Shida being backstage in Orlando as she edges closer to U.S. competition, and clarity emerging on AEW's streaming future as reports indicate the promotion is expected to remain aligned with Warner-backed platforms rather than moving to Netflix. TNA continues to operate within a shared ecosystem, as we revisit the fallout from Genesis and Impact's AMC debut amid lingering visa issues, the accidental TNA+ audio leak revealing No Surrender plans, Trey Miguel's emotional return, and the Knockouts Tag Team picture coming into focus with ZaRuca crowned as new number one contenders. We also hit international headlines, including AAA's FOX era officially beginning, Dominik Mysterio's accidental AEW belt graphic during a promo, El Hijo del Vikingo emerging as number one contender for the Mega Championship, NJPW seeing TMDK lose the NEVER Six-Man titles at Korakuen Hall, and Stardom keeping its options open following an alleged intellectual property violation involving trading cards. Episode 164 is a full-scale snapshot of the wrestling industry at a turning point — Royal Rumble season underway, WrestleMania plans coming into focus, media rights shifting, talent moving, and promotions everywhere adjusting direction rather than delivering final outcomes as the calendar heats up.
The show kicks off with Rams hype — “1, 2, 3… RAMS!” — plus a fun conversation about Tim’s brief life as a model, stadium anthems, and what song truly represents the Rams, sparked by Fernando Mendoza singing Fernando at the stadium. Things turn intense with breaking news as a jewelry store robbery in Anaheim Hills leads to a high-speed pursuit that ends in two separate collisions, raising questions about public safety and police chases. Send us a textOn the release of his latest thriller, The Viper: A Zig and Nola Novel, Brad Meltzer joins Joe for an in-depth conversation on writing, curiosity, service, and the often-hidden moments that shape a life. From bestselling thrillers to children's books, Meltzer reflects on how stories—both real and imagined—help us make sense of who we are, what we've lived through, and the paths we choose moving forward.Drawing on his own unlikely origin story, Meltzer shares how a single teacher's encouragement set him on the path to becoming a writer, why falling in love with the process matters more than chasing outcomes, and how curiosity has been the throughline of his career. They explore how paying attention—to people, details, and quiet acts of kindness—can open doors we didn't even know existed. The conversation also dives into Meltzer's deep connection to the military community, from his work with the USO to the research behind his Zig and Nola thriller series set at Dover Air Force Base. Together, Joe and Brad discuss service, sacrifice, grief, and why storytelling can help destigmatize mental health struggles—especially for those transitioning out of uniform. In this episode, Brad Meltzer and Joe also explore:How one teacher's belief can change the trajectory of a lifeWhy curiosity is a more powerful tool than talentFalling in love with the process—not the outcome—of creative workWhat writing thrillers has taught Meltzer about human natureWhy Dover Air Force Base became the heart of his Zig and Nola seriesHow small acts of kindness ripple outward in unexpected waysThe challenge of transitioning from a life of constant motion to stillnessWhy seeking help is a sign of strength, not weaknessHow reading builds empathy, critical thinking, and resilienceWhy transformation is the hardest, and most important, kind of changeWhether you're a writer, a leader, a veteran, or someone navigating a transition, this conversation is a reminder that paying attention, staying curious, and honoring the quiet work of becoming can lead to a life richer than any plan you could have written in advance.Watch the full episode on Youtube! We also consider how their origins might cause friction within a team-up. When you read The Viper, the new Zig and Nola novel from Brad Meltzer, it doesn't take long to see how comics inform the author's creativity. As he says during our conversation, “It's Superman and Batman. It's Spider-Man and Wolverine.” Meltzer grew up a comic book kid, and the ideas he found in Marvel Comics and DC Comics remain at the forefront of his imagination. It's been a minute since he's played in our favorite medium, having written stellar runs on Green Arrow and Justice League of America, and we can never forget the monumental event that was Identity Crisis. However, he's never been far from comics, and it's evident to anyone who's experienced his Ordinary People Change the World series with Chris Eliopoulos and his novels. The Viper, released this week from William Morrow, is Meltzer's return to the thriller. It's a zippy, twisty narrative about a dying man who delivers his suit to the funeral home tasked with handling his body, shortly before he's murdered, advancing his earthly exit. The death quickly reunites Meltzer's heroes from The Lightning Rod and The Escape Artist, Zig and Nola, his Superman and Batman, and plunges them into a conspiracy with ties to Nola's past. We got a lot from The Viper, and we bring it all to Brad Melzer on this week's podcast. We discuss his life as a comic book kid, how he can't escape comics, and what he might have on the horizon in terms of the four-color medium. Yes, his Ghost Machine collaboration with Geoff Johns and Gene Ha is coming, but there seems to be something else, too. We try to tease the news out of him. Migration Routes and Genetic Groups 2. Professor Meltzer details the migration routes of ancestral Native Americans, explaining that while they crossed the land bridge during maximum glaciation, they were initially trapped in Alaska by two vast continental ice sheets. The "ice free corridor" along the Rockies was not a viable route until around 13,000 years ago, so the likely path was down the Pacific coast, which may have cleared as early as 16,500 years ago. Meltzer introduces the concept of a "genetic ghost," which is evidence in the genomic record of a contributing population for which no physical sample has yet been found.
