The RunRunLive 4.0 Podcast Episode 4-400 – Matt makes it stick (Audio: link) audio:http://www.RunRunLive.com/PodcastEpisodes/epi4401.mp3] Link MarathonBQ – How to Qualify for the Boston Marathon in 14 Weeks - Hello, my friends and welcome to the RunRunLive Podcast episode 4-401.  Nice to be back in the saddle from the holidays.  I feel like I’ve had a good couple months.  I took an extra week on my cadence for this one because It was the holidays and, frankly, I had a lot going on.  Plus I was sick.  I got whatever cold is going around.  Not the flu.  Not pneumonia. Just a cough that hung around for the better part of 2 weeks.  I managed to hold the line on my diet through the holidays.  I did not lose a bunch of weight, but I did eat relatively cleanly and avoided most of the sweets and adult beverages of the season.  This was my plan, so well done me.  This puts me in a healthy spot to hit my Boston training hard and lose 10-15 pounds. Because I think that’s what I’m going to need to do to have any shot of requalifying.  People think ‘Hey, Boston is in the spring! You’ve got plenty of time!” But that’s not really true.  When you turn the corner to a new year on January 1st you are only 3 and ½ months away, which in broad strokes means 14 weeks.  A typical hard training cycle for me is 12 weeks with a 2 week taper, so it’s game on.  I’m not starting from scratch and I’m healthy and I’m mentally enthusiastic.  The dark times will come, but as of now I’m in a good spot.  Today we chat with Matt who is a team mate of mine.  He’s got a great story.  And it’s a timely story as we ease into the ‘resolution season’.  Listen to what it took for Matt to find the courage to switch the momentum and begin his transformation.  In section 1 I’ll give you a wrap up on the Groton Marathon, which, against the odds we managed to pull off successfully last Sunday.  Thinking about it, if I include that in my week, I’m over 50 miles for week one of my training!  Feel pretty good for that. In section 2 I’ll perambulate about goals and stuff, because, hey, it’s that time of year. … When I was out running the Groton Marathon with my buddies I told them I had two topics that I wanted to discuss.  The first was the ending of the movie Road House and the second was the Boston Marathon of Sex.   Now, this being a family friendly show I’ll reserve the latter explanation for my members feed. I’m writing that as we speak and I crack myself up. But, I was watching, or more to the point being forced to watch the movie Road House with Patrick Swazey, may he rest in piece, and like so many of the movies from that era, the ending didn’t make any sense.  So he rips the one bad guy’s throat out.  Then he goes after the big bad guy.  Kills 4 of his henchmen and then has the final fight with the big bad guy.  In the end he decides not to rip that guy’s throat out but the friendly villagers kill the bad guy with shotguns.  Shortly after the police show up.  Now we have been told that the police are in the back pocket of the bad guy.  The police ask, ‘What happened?’ and everyone says “We didn’t see anything…”  And that’s it.  Cut to the final seen of Swazey skinny dipping with the love interest.  So, let me get this straight… There’s 6 dead bodies, one of which has Patrick Swazey’s knife sticking out of his chest, and the police just shrug their shoulders and say, ‘Well, I guess since no one saw anything…it’s all good… nothin to see here…”? I’ve seen enough episodes of Law and Order to know they’re not getting off that easy! It’s ok, I ripped his throat out in self-defense… Plus, they shot this guy with old shotguns.  He probably wouldn’t be dead yet when the police come in.  If it was bird shot he’d just be uncomfortable.   Maybe he had a heart attack from the shock.  But think about all the movies you’ve seen where the end is a big pile of dead guys and it’s ok.  That will give you something to talk about on your next long run. On with the show. … I’ll remind you that the RunRunLive podcast is ad free and listener supported.  What does that mean? It means you don’t have to listen to me trying to sound sincere about Stamps.com or Audible.. (although, fyi, my MarathonBQ book is on audible) We do have a membership option where you can become a member and as a special thank you, you will get access to member’s only audio. There are book reviews, odd philosophical thoughts, zombie stories and I curate old episodes for you to listen to.  I recently added that guy who cut off is foot so he could keep training and my first call with Geoff Galloway.   “Curated” means I add some introductory comments and edit them up a bit.  So anyhow – become a member so I can keep paying my bills. Because I think that's what I'm going to need to do to have any shot of requalifying.  People think ‘Hey, Boston is in the spring! You've got plenty of time!” But that's not really true.  When you turn the corner to a new year on January 1st you are only 3 and ½ months away, which in broad strokes means 14 weeks.  A typical hard training cycle for me is 12 weeks with a 2 week taper, so it's game on.  I'm not starting from scratch and I'm healthy and I'm mentally enthusiastic.  The dark times will come, but as of now I'm in a good spot.  Today we chat with Matt who is a team mate of mine.  He's got a great story.  And it's a timely story as we ease into the ‘resolution season'.  Listen to what it took for Matt to find the courage to switch the momentum and begin his transformation.  In section 1 I'll give you a wrap up on the Groton Marathon, which, against the odds we managed to pull off successfully last Sunday.  Thinking about it, if I include that in my week, I'm over 50 miles for week one of my training!  Feel pretty good for that. In section 2 I'll perambulate about goals and stuff, because, hey, it's that time of year. … When I was out running the Groton Marathon with my buddies I told them I had two topics that I wanted to discuss.  The first was the ending of the movie Road House and the second was the Boston Marathon of Sex.   Now, this being a family friendly show I'll reserve the latter explanation for my members feed. I'm writing that as we speak and I crack myself up. But, I was watching, or more to the point being forced to watch the movie Road House with Patrick Swazey, may he rest in piece, and like so many of the movies from that era, the ending didn't make any sense.  So he rips the one bad guy's throat out.  Then he goes after the big bad guy.  Kills 4 of his henchmen and then has the final fight with the big bad guy.  In the end he decides not to rip that guy's throat out but the friendly villagers kill the bad guy with shotguns.  Shortly after the police show up.  Now we have been told that the police are in the back pocket of the bad guy.  The police ask, ‘What happened?' and everyone says “We didn't see anything…”  And that's it.  Cut to the final seen of Swazey skinny dipping with the love interest.  So, let me get this straight… There's 6 dead bodies, one of which has Patrick Swazey's knife sticking out of his chest, and the police just shrug their shoulders and say, ‘Well, I guess since no one saw anything…it's all good… nothin to see here…”? I've seen enough episodes of Law and Order to know they're not getting off that easy! It's ok, I ripped his throat out in self-defense… Plus, they shot this guy with old shotguns.  He probably wouldn't be dead yet when the police come in.  If it was bird shot he'd just be uncomfortable.   Maybe he had a heart attack from the shock.  But think about all the movies you've seen where the end is a big pile of dead guys and it's ok.  That will give you something to talk about on your next long run. On with the show. … I'll remind you that the RunRunLive podcast is ad free and listener supported.  What does that mean? It means you don't have to listen to me trying to sound sincere about Stamps.com or Audible.. Comic book genius Stan Lee, Spider-Man creator, dies at 95

LOS ANGELES (AP) — Stan Lee, the creative dynamo who revolutionized the comic book and helped make billions for Hollywood by introducing human frailties in superheroes such as Spider-Man, the Fantastic Four and the Incredible Hulk, died Monday. He was 95. Lee was declared dead at Cedars-Sinai Medical Center in Los Angeles, according to Kirk Schenck, an attorney for Lee’s daughter, J.C. Lee. As the top writer at Marvel Comics and later as its publisher, Lee was widely considered the architect of the contemporary comic book. He revived the industry in the 1960s by offering the costumes and action craved by younger readers while insisting on sophisticated plots, college-level dialogue, satire, science fiction, even philosophy. Millions responded to the unlikely mix of realistic fantasy, and many of his characters, including Spider-Man, the Hulk and X-Men went on to become stars of blockbuster films. Recent projects he helped make possible range from the films “Black Panther” and “Doctor Strange” to such TV series as “Agents of S.H.I.E.L.D” and “Guardians of the Galaxy.” Lee was recognizable to his fans — he had cameos in Marvel films and TV projects, his hair gray and his glasses tinted. “I think everybody loves things that are bigger than life. … I think of them as fairy tales for grown-ups,” he told The Associated Press in a 2006 interview. “We all grew up with giants and ogres and witches. Well, you get a little bit older and you’re too old to read fairy tales. But I don’t think you ever outgrow your love for those kinds of things, things that are bigger than life and magical and very imaginative.” Lee considered the comic-book medium an art form and he was prolific: By some accounts, he came up with a new comic book every day for 10 years. “I wrote so many I don’t even know. I wrote either hundreds or thousands of them,” he told the AP in 2006. He hit his stride in the 1960s when he brought the Fantastic Four, the Hulk, Spider-Man, Iron Man and numerous others to life. “It was like there was something in the air. I couldn’t do anything wrong,” he recalled. His heroes, meanwhile, were a far cry from virtuous do-gooders such as rival DC Comics’ Superman. The Fantastic Four fought with each other. Spider-Man was goaded into superhero work by his alter ego, Peter Parker, who suffered from unrequited crushes, money problems, and dandruff. The Silver Surfer, an alien doomed to wander Earth’s atmosphere, waxed about the woeful nature of man. The Hulk was marked by self-loathing. Daredevil was blind and Iron Man had a weak heart. “The beauty of Stan Lee’s characters is that they were characters first and superheroes next,” Jeff Kline, executive producer of the “Men in Black” animated television series, told The Blade of Toledo, Ohio, in 1998. Some of Lee’s creations became symbols of social change — the inner turmoil of Spider-Man represented ’60s America, for example, while The Black Panther and The Savage She-Hulk mirrored the travails of minorities and women. Lee scripted most of Marvel’s superhero comics himself during the ’60s, including the Avengers and the X-Men, two of the most enduring. In 1972, he became Marvel’s publisher and editorial director; four years later, 72 million copies of Spider-Man were sold. “He’s become our Mickey Mouse,” he once said of the masked, web-crawling crusader. Lee also published several books, including “The Superhero Women” in 1977 and “How to Draw Comics the Marvel Way” the following year, when he was named publisher of the year by the Periodical and Book Association of America. CBS turned the Hulk into a successful TV series, with Bill Bixby and Lou Ferrigno portraying the doomed scientist from 1978-82. A Spider-Man series ran briefly in 1978. Both characters were featured in animated TV series as well. The first big-budget movie based on Lee’s characters, “X-Men,” was a smash in 2000, earning more than $130 million at North American theaters. “Spider-Man” did even better, ...

Jeff Kline talks about PEs in older adults. What's different and what's the same for older vs younger patients? Find out here! See gempodcast.com/2016/06/02/pulmonary-emboli-in-older-adults/ for a full description, links, and to leave comments! Not a day goes by that an Emergency Physician doesn’t at least consider PE in a patient who presents with chest pain, dyspnea, or syncope. We have become familiar with using risk stratification tools like the Wells Score and the PERC criteria. But what do you do in older adults? All of them will automatically NOT be PERC negative because of their age. In this episode, with PE guru Jeff Kline, we discuss the presentation of PE in older adults, including the demographics, diagnosis, and how treatment may differ from younger adults in small, sub-massive, and massive PEs. In this first ever episode of the Journal Jam podcast, a collaboration between EM Cases, Academic Life in EM and The Annals of Emergency Medicine's Global Emergency Medicine Journal Club, Teresa Chan and I, along with Jeff Kline, Jonathan Kirschner, Anand Swaminathan, Salim Rezaie and Sam Shaikh from ALiEM, discuss the potential for Age Adjusted D-dimer to rule out pulmonary embolism in low risk patients over the age of 50. We discuss 4 key questions about the ADJUST-PE Study from JAMA in March 2014 including: Would you order a CTPA on a 60 year old woman with an age adjusted D-dimer of 590 ng/L? The problem until now has been that the older the patient, the more likely the D-dimer is to be positive whether they have a PE or not, so many of us have thrown the D-dimer out the window in older patients and go straight to CTPA, even in low risk patients. If you are a risk averse doc, this strategy will lead to over-utilization of resources, huge costs, length of stay, radiation effects etc; and if you're not so risk averse, then you might decide not to work up the low risk older patient at all and miss clinically important PEs. expert peer reviewFor all the questions discussed on this podcast, the original Google Hangout interview from which this podcast was based, and the crowd sourced opinions from around world, visit the ALiEM website. Many thanks to all the talented people who made this podcast possible. Together, we're smarter!
