Teresa Chan is the Director of the Master's in Insurance Management program at Columbia University. She launched the program in 2019 after 27 years of insurance industry experience in legal, product, and business development. Prior to joining Columbia, Teresa was an advisor on the formation of MGU start-ups funded by Willis Group that leverage emerging technological capabilities to underwrite specialized markets on digital platforms and a consultant for energy performance warranty insurance. Teresa has a J.D. from Fordham University and a B.S. in Operations Research from the School of Engineering and Applied Science at Columbia University. She has been recognized by Business Insurance as one of “25 Women to Watch” and by ReActions Magazine as one of the “Top Insurance Women.” In today's episode of the In The Know podcast, Chris Hampshire and Teresa take a look at the value of giving back through insurance education. This included a self-assessment by scholars on their current professional social media engagement, a wonderful precis of the various social media platforms by Patricia Tran, and group work on social media dilemmas for health professions educators. The podcast conversation explored these themes, with a focus on the different roles in which that health professions educators might be using social media and online engagement – as teachers, as institutional leaders, and as faculty developers. Tales from the Front Line: A Pandemic Podcast The Novel Coronavirus 19 pandemic is an event to be remembered in the history books, the epidemiology texts, and healthcare seminars. The global impact of the COVID-19 pandemic has been discussed endlessly since early 2020. Frontline workers have been reported on extensively but what about their own stories? Since the pandemic began, frontline workers including emergency physicians have worked to save their communities from COVID-19. Their stories deserve to be heard Tales from the Frontline brings us back to the dawning of the epidemic through the lens of three emergency physicians – Dr. Teresa Chan, Dr. Kevin Dong and Dr. Jonathan Sherbino. The podcast explores the origins of the pandemic and initial reactions by providers to the news. The uncertainty is palpable, fear is brewing, and no one knows how long this will go on for. In this stand-alone special, we take you back to March 2020. The pandemic has arrived, and with it questions around transmission and severity abound. There is no playbook and policies change every day in the emergency room. As the cases grow, emergency physicians become increasingly aware of their role in fighting this virus as frontline healthcare workers. In this podcast Vic speaks with Teresa Chan about ‘digital presence' - our online personas – and about the opportunities to disseminate our scholarship via online platforms. We recorded the conversation live at the 2022 Harvard Macy program - Transforming your Teaching using Technology. In a metacognitive move, we hoped our podcast conversation would illustrate some of the points we made in the conversation – “the medium is the message”! We started with Teresa's website, and reflected on the choices she made in establishing this as her online ‘homebase,' referencing Michael Hyatt's conceptual framework and book Platform. We talked about the process of curating such as website as a digital portfolio, and the practicalities, such as hosting on Wix or Squarespace. Teresa talked about how she got interested in online presence and the encouragement of mentors like Jonathan Sherbino from KeyLime podcast. Our conversation turned to Teresa's article Social Media and the 21st Century scholar (one of the pre-readings for the Transforming your Teaching using Technology course). In this piece, Teresa and her co-authors offer 2 personal case studies of how to use social media for education, scholarship and health advocacy. In our further conversation Teresa talked about the ‘moral imperative of getting great science out there,' and we walked through an example of how she might disseminate the messages in an article accepted for publication. Twitter features heavily in Teresa's dissemination strategies, but we also considered the opportunities of other platforms - Snapchat, Instagram, Facebook and LinkedIn. We finished with a deeper dive on podcasting, including Teresa's own MacPFD Spark podcast, and our Harvard Macy Institute podcast. Our ‘live audience' on Zoom joined the conversation with questions from the chat, and we talked about podcast length, audio recording, and how to listen… Thanks to the faculty team and scholars who joined our conversation, and to Teresa Chan for sharing her wealth of experience.

COVID-19 disrupted the medical licensing examination system in Canada.  During the pandemic,  exams delivered by The College of Family Physicians of  Canada  (CFPC) and the Royal College of Physicians and Surgeons of Canada (RCPSC) were delayed, canceled or adapted, disrupting the lives of hundreds of physicians.However, those challenges prompted many to rethink the historical approach to medical licensure in Canada. In this episode, Drs. Brent Thoma and Teresa Chan discuss their proposal to shift away from all-or-nothing examinations and towards a system of graduated licensure. They are two authors of a recent CMAJ commentary entitled Replacing high-stakes summative examinations with graduated medical licensure in Canada.Dr. Thoma is an emergency and trauma physician in Saskatoon and an associate professor of emergency medicine at the University of Saskatchewan.  Dr. Chan is an associate professor of medicine at McMaster University, where she is the Associate Dean of Continuing Professional Development.

Additionally, she serves as the Social Media Lead for the McMaster Department of Medicine. Dr. Chan is also interested in developing medical education innovations (and evaluating them!). Altered Mental Status in Children

How do you approach the child who may be altered?   Altered mental status in children can be subtle.  Look for age-specific behaviors that range from irritability to anger to sleepiness to decreased interaction. In the altered child, anchoring bias is your biggest enemy.  Keep your mind open to the possibilities, and be ready to change it, when new information becomes available. For altered adults, use AEIOU TIPS (Alcohol-Epilepsy-Insulin-Overdose-Uremia-Trauma-Infection-Psychosis-Stroke). Try this for altered children: remember that they need their VITAMINS! V – Vascular (e.g. arteriovenous malformation, systemic vasculitis) I – Infection (e.g. meningoencephalitis, overwhelming alternate source of sepsis) T – Toxins (e.g. environmental, medications, contaminated breast milk) A – Accident/abuse (e.g. non-accidental trauma, sequelae of previous trauma) M – Metabolic (e.g. hypoglycemia, DKA, thyroid disorders) I – Intussusception (e.g. the somnolent variant of intussusception, with lethargy) N – Neoplasm (e.g. sludge phenomenon, secondary sepsis, hypoglycemia from supply-demand mismatch) S – Seizure (e.g. seizure and its variable presentation, especially subclinical status epilepticus)   Case One: Sleepy Toddler 16-month-old who chewed on his grandmother's clonidine patch Clonidine is an alpha-2 agonist with many therapeutic indications including hypertension, alcohol withdrawal, smoking cessation, perimenopausal symptoms.  In children specifically, clonidine is prescribed for attention deficit hyperactivity disorder, spasticity due to cerebral palsy and other neurologic disorders, and Tourette’s syndrome. The classic clonidine toxidrome is altered mental status, miosis, hypotension, bradycardia, and bradypnea.  Clonidine is on the infamous list of “one pill can kill”. Treatment is primarily supportive, with careful serial examinations of the airway, and strict hemodynamic monitoring. Naloxone can partially counteract the endogenous opioids that are released with clonidine's pharmacodynamics. Start with the usual naloxone dose of 0.01 mg/kg, up to the typical adult starting dose is 0.4 mg. In clonidine overdose, however, you may need to increase the naloxone dose (incomplete and variable activity) up to 0.1 mg/kg.  Titrate to hemodynamic stability and spontaneous respirations, not full reversal of all CNS effects.   Case Two: In Bed All Day A 7-year-old with fever, vomiting, body aches, sick contacts.  Altered on exam. Should you get a CT before LP? If you were going to perform CT regardless, then do it. Adult guidelines: age over 60, immunocompromised state, history of central nervous system disease, seizure within one week before presentation, abnormal level of consciousness, an inability to answer two consecutive questions correctly or to follow two consecutive commands, gaze palsy, abnormal visual fields, facial palsy, arm drift, leg drift, and abnormal language. Children: if altered, and your differential diagnosis is broad (especially if you may suspect tumor, bleed, obvious abscess). Influenza is often overlooked as a potential cause of altered mental status.  Many authors report a broad array of neurological manifestations associated with influenza, such as altered mental status, seizures, cranial nerve abnormalities, hallucinations, abnormal behavior, and persistent irritability.  All of this is due to a hypercytokinemic state, not a primary CNS infection.   Case Three: Terrible Teenager 14-year-old brought in for "not listening" and "acting crazy"; non-complaint on medications for systemic lupus erythematosus (SLE). SLE is rare in children under 5. When school-age children present with SLE, they typically have more systemic signs and symptoms.  Teenagers present like adults.  All young people have a larger disease burden with lupus, since they have many more years to develop complications. Lupus cerebritis: high-dose corticosteroids, and possibly IV immunoglobulin.  Many will need therapeutic plasma exchange (TPE), a type of plasmapheresis.   Summary In altered mental status, keep your differential diagnosis open Pursue multiple possibilities until you are able to discard them Be ready to change your mind completely with new information Make sure your altered child gets his VITAMINS (Vascular, Infectious, Toxins, Accident/Abuse, Metabolic, Intussusception, Neoplasm, Stroke)   References Beckman HB, Frankel RM. It makes sense that the treatment of primary spnontaneous pneumothorax would lend itself well to outpatient management, since patients are usually young and otherwise healthy, and the mortality and morbidity from these air leaks are really very low. Most patients would rather be managed as an outpatient rather than admitted to hospital and sending these patients home would probably end up saving the system resources and money. In this first ever episode of the Journal Jam podcast, a collaboration between EM Cases, Academic Life in EM and The Annals of Emergency Medicine's Global Emergency Medicine Journal Club, Teresa Chan and I, along with Jeff Kline, Jonathan Kirschner, Anand Swaminathan, Salim Rezaie and Sam Shaikh from ALiEM, discuss the potential for Age Adjusted D-dimer to rule out pulmonary embolism in low risk patients over the age of 50. We discuss 4 key questions about the ADJUST-PE Study from JAMA in March 2014 including: Would you order a CTPA on a 60 year old woman with an age adjusted D-dimer of 590 ng/L? The problem until now has been that the older the patient, the more likely the D-dimer is to be positive whether they have a PE or not, so many of us have thrown the D-dimer out the window in older patients and go straight to CTPA, even in low risk patients. In this first ever episode of the Journal Jam podcast, a collaboration between EM Cases, Academic Life in EM and The Annals of Emergency Medicine's Global Emergency Medicine Journal Club, Teresa Chan and I, along with Jeff Kline, Jonathan Kirschner, Anand Swaminathan, Salim Rezaie and Sam Shaikh from ALiEM, discuss the potential for Age Adjusted D-dimer to rule out pulmonary embolism in low risk patients over the age of 50. We discuss 4 key questions about the ADJUST-PE Study from JAMA in March 2014 including: Would you order a CTPA on a 60 year old woman with an age adjusted D-dimer of 590 ng/L? The problem until now has been that the older the patient, the more likely the D-dimer is to be positive whether they have a PE or not, so many of us have thrown the D-dimer out the window in older patients and go straight to CTPA, even in low risk patients.
