Christine Clawley, MA, LPC is an NDE experiencer, counselor, and life coach who has worked in a variety of capacities, including providing counseling and support at community mental health centers, hospitals, prisons, inpatient eating disorder clinics, homeless shelters, and crisis support. She currently works in private practice and has a Jungian and Depth-oriented practice that helps clients heal from traumatic experiences, as well as supports individuals who have also had near-death experiences or other spiritually-transformative experiences. After overcoming the life-threatening Necrotizing Fasciitis at age 24, she embarked on a journey of healing and self-understanding through exploring various holistic techniques, including a mindfulness practice, meditation, yoga, self-hypnosis, and indigenous healing modalities. Prior to contracting this illness, she received many dreams and messages that foreshadowed her illness. While spending nearly a month in a medically-induced coma she had many dreams that mirrored what was happening to her on a physical, emotional, and spiritual level. Upon awaking from the coma, she experienced increased intuition, empathy, and an increased frequency of lucid and precognitive dreams. Join our Emergency General Surgery team as they discuss Necrotizing Soft-Tissue Infections. Hosted by Drs. Jordan Nantais, Ashlie Nadler, Stephanie Mason and Graham Skelhorne-Gross. Necrotizing Soft-Tissue Infections: - Also known as “flesh eating disease”, gas gangrene, necrotizing fasciitis/myositis, Fournier's gangrene.  - Early findings are non-specific - Rapidly fatal - diagnostic delay can lead to tremendous additional morbidity and mortality Classification: - Type 1 - polymicrobial category (most common) found in immunosuppressed or elderly - Type 2 - monomicrobial infection [Group A Streptococcus > Methicillin-resistant Staphylococcus aureus (MRSA)] - Type 3 - monomicrobial infection (Vibrio or Clostridium) - Type 4 - fungal (rare) in immunocompromised or after penetration or trauma from candida or Zygomycetes. Initial Workup - History: (comorbidities, immunosuppression, recent infections or trauma) - Exam: swelling, open lesions, drainage, erythema, crepitus, and pain out of proportion      - Most common: swelling, pain, erythema      - Bullae, skin necrosis, crepitus are less common - Labs: Hb, wbc, Na, Creat, glucose, and CRP - Imaging: CT, MRI *sensitive and specific but may not change management - Cut-down: bedside vs in OR - Gm stain  Management - Initially: two large bore IVs, foley catheter, aggressive fluid resuscitation, broad spectrum antibiotics, vasopressors PRN - Abx choices: carbopenem or piperacllin-tazobactam or cefotaxime plus metronidazole. Clindamycin (antitoxin effect) and vancomycin (MRSA) should be considered. - OR: must debride all dead/infected tissue, involve other surgical specialties as needed      - Mark edge of cellulitis and use as initial debridement      - Healthy dermis – pearly and white      - Healthy fat – pale, yellow, glistening      - Healthy fascia – should bleed, doesn't easily separate from muscle      - Healthy muscle – contract with cautery      - Dressing: betadine-soaked gauze on the wound      - Most patients will need at least 3 ORs (second OR generally 8-12 hours after the first)      - No VAC or stoma at first OR References:  1.    Pelletier J, Gottlieb M, Long B, Perkins JC Jr. Necrotizing Soft Tissue Infections (NSTI): Pearls and Pitfalls for the Emergency Clinician. Today I had the pleasure of speaking with Blake Haxton, one of the most inspiring United States Paralympians. At a very early age, he went through a massive amount of trauma after a lot of things happened to him, but then he carries an incredible shift of mindset and has the right constraints. This week we visit with Chris DT Gordon of Michigan. Chris was an outstanding guest and I really enjoyed chatting with him and listening to his testimony on how he became a scar bearer in his battle with Necrotizing Fasciitis. Chris contracted this flesh eating bacteria back in 2015 and spent more than 2 months in the hospital and 6 months of Physical Therapy. He is a husband and father of 3 and is an online special education middle school teacher. Chris is a motivational speaker and has since started his own podcast, "Scar Bearers" along with a YouTube channel and continues to stay active even during a crazy quarantine. He has set or tied 4 PRs since being in the hospital and has completed 3 Ragnars and since recording this podcast achieved his black belt in Tae Kwon Do....so watch your back if you don't listen in to this week's episode. It was an entertaining visit and I look forward to having Chris back on in the future.

My name is Joshua Middleton and I was diagnosed HIV positive on June 5, 2012. I had survived death twice already from Necrotizing Fasciitis and Septic shock while being hospitalized over 30 + times in a two year period. I was diagnosed when I was already in a very stressful moment of my life. I was in a relationship with a girl in Tijuana Mexico that I thought would eventually end in the perfect white picket fence life, including marriage and a baby. I was wrong… She cheated on me twice while we were attempting to have a baby together. Not knowing how to deal with the relationship breakup I started to seek out love in all the wrong places including having multiple one night stands unprotected and even visiting brothels in Northern Tijuana in an effort to want to feel what it was like to have a woman's touch again. I had put myself in many high risk situations, I wasn't sure if my ex had used protection, and we had both gotten tattoos just month prior. I got tested as I normally did out of routine after getting symptoms of pink eye. I expected a negative result as I always had received but this time it was not so good news , I came out positive. I was forced to make the decision of where my life would go from there on out. After being inspired by several HIV activist who have now become good friends, I was determined to not let this virus defeat me. I started on treatment a week after being diagnosed and decided to take the optimistic route to this virus. I became an HIV activist, letting everyone know about my story, assuring people this is certainly not just a gay mans disease. I am more than a virus and have big dreams/goals for my life that this virus will not stop me from accomplishing. This is my story a California boy who does not fall into the average "stigmatized statistic" that people think of when it comes to HIV however this is my life, I am HIV + and there is no shame in me knowing my status.ory. Meet Melinda Minton; a mother, a successful business woman, a world explorer, an author, a woman overflowing with laughter and a bi-lateral amputee.  Listen to her story of flu-like symptoms that was found to be Necrotizing Fasciitis, a Strep A germ that kills more often than not. Her journey that includes: "death", a coma,  listening to the doctors tell her she would lose part of her lips, part of her tongue, and her legs, the painful rehabilitation, the fitting of prosthesis, learning to crawl and walk again.
