For people looking for a relationship, what do they need to know in order to be successful in dating? Damona said that dating is a pretty and repeatable process. If you are already getting frustrated with dating, learn about these five areas to know what you could have been doing wrong. How do you handle sexual concerns as well as various loaded topics during dating? Sex is a part of any relationship, but COVID changed the dating scenario such that there are added health concerns that needed to be addressed first before you even begin to see each other in person. For example, instead of talking about STI testing and the like, you now talk about getting tested for COVID and quarantining. But COVID aside, having a difficult conversation such as sex, politics, or race in the early phases of dating ultimately boils down to listening and understanding. There should be a time when we set aside our beliefs and focus on what really matters like communication, conflict resolution, values, and goals. Interracial dating and racial bias in dating Damona talks about the hot topic of race and how it comes into play in dating. Does excluding a particular race from your dating preferences just simply a matter of preference or racial bias? Learn how to ask the five “why's” to unpack biases and beliefs and how to turn your differences from your partner into a very rich and positive aspect of the relationship. How important is sexual compatibility during dating? Can this still change over time? People toss away a perfectly good relationship because in the early phase, the sex isn't as fulfilling as prior relationships that they compare it to. Likewise, don't pretend to be satisfied with a casual encounter if you actually want something serious. Communicate your boundaries. Ask your sexual partner if there is anything they would like for you to know. Ask your sexual partner for what you need. Give them what they need if it is within your power to do so. Value your sexual partner's pleasure as much as your own. Create a sexual experience together, as opposed to using each other for your separate enjoyment. Pay attention to your sexual partner's comfort level and check in with them frequently. If they ever seem uncomfortable, stop what you're doing and ask if they're okay. If you are uncomfortable, tell them you need to stop. You are always allowed to change your mind. After having sex, see to your sexual partner's needs. Are they hungry? Tired? Dehydrated? Sore? They don't cease to matter once you've gotten off. Everyone can benefit from a glass of water. Don't make promises you don't intend to keep. In 2019, there were 468,342 new sexually transmitted infection (STI) diagnoses made at sexual health services in England. Of these, the most commonly diagnosed STIs were chlamydia (229,411; 49% of all new STI diagnoses), gonorrhoea (70,936; 15%) and genital warts (51,274; 11%). Between 2012 and 2019, rates of new diagnoses increased for: chlamydia: a 3.5% increase, from 394 to 408 per 100,000 population gonorrhoea: a 151% increase, from 50 to 126 per 100,000 population syphilis: a 152% increase, from 5.6 to 14.2 per 100,000 population. Over the same time period, there was a decline in new diagnoses of genital warts (a 36% decrease, from 142 to 91 per 100,000 population), and the rate of genital herpes diagnoses remained constant at 61 per 100,000 population. Episode 68: Prevention - Aspirin, STIs, and Diabetes. Updates on aspirin use for preeclampsia prevention, updated STIs screening guidelines, and new age to start screening for diabetes.  Introduction:  COVID-19 Booster Shots.  Every week there is a lot of information to cover about COVID-19. I'm sure you are aware of some of this information, but here you have it again for historical purposes. Pfizer and BioNtech announced on September 20, 2021, that their COVID-19 vaccine is protective in pediatric patients between 5 and 11 years of age. Let's remember that this vaccine is being used for patients older than 12, but so far none of the vaccines have been authorized for younger patients. A submission to FDA has been sent, but no approval has been given yet.Recently, we mentioned to you that a booster shot for the mRNA COVID-19 vaccines were likely to be authorized by the FDA around September 20. Indeed, an authorization for a booster was given on September 22, 2021. This authorization was given to the Pfizer/BioNtech vaccine only, and it can be given at least 6 months after the completion of the primary series.The patients who are authorized to receive the booster shot are: Patients who are 65 years of age and older; patients between 18 and 64 years of age at high risk of severe COVID-19; and individuals 18 through 64 years of age with frequent occupational exposure to COVID-19.The Moderna vaccine has not been authorized for a booster shot.Let's remember that both Pfizer and Moderna have been authorized for a third dose in patients who are immunocompromised. The third dose can be given 4 weeks after completing he initial 2 doses of these vaccines. They have released several recommendations in the last few months. Aspirin and preeclampsia: On September 28, 2021, the USPSTF released their recommendation about the use of aspirin to prevent preeclampsia in pregnant persons at high risk. This recommendation is consistent with the previous recommendation given in 2014. New evidence has reinforced that aspirin is effective at reducing risk of perinatal mortality when used properly.The recommendation states: “The USPSTF recommends the use of low-dose aspirin (81 mg/day) as preventive medication after 12 weeks of gestation in persons who are at high risk for preeclampsia.” This is a grade B recommendation. A grade B recommendation means the net benefit of this preventive intervention is moderate to substantial.Who is at risk for preeclampsia? You can classify the risk as High, Moderate, and Low.High: Preeclampsia during previous pregnancies (especially if you had an adverse outcome), multifetal gestation, chronic hypertension, type 1 or 2 diabetes before pregnancy, kidney disease, autoimmune disease, or a combination of multiple moderate-risk factors. Recommend aspirin if a woman has 1 or more of those high-risk factors. Moderate: Nulliparity, obesity, history of preeclampsia in mother or sister, black persons, low income, age 35 years or older, personal history factors (e.g. low birth weight or small for gestational age, previous adverse pregnancy outcome, >10-year pregnancy interval, and in vitro conception. Recommend aspirin if patient has 2 or more of these moderate risk factors. You may recommend aspirin even to women with 1 of these risk factors. Low: Do not recommend aspirin to pregnant women who have low risk for preeclampsia. A patient is considered low risk if she had a previous uncomplicated term delivery and has none of the risk factors mentioned above.As a side note, given the current movement for diversity, equality and inclusion, the article also states that “black persons have higher rates of preeclampsia and are at increased risk for serious complications due to various societal and health inequities,” not due to biological propensities.When do you stop aspirin in pregnancy?The decision to continue aspirin in the presence of obstetric bleeding (or bleeding risk) should be considered on a case-by-case basis. You can decide to stop at 36 weeks or continue until delivery based on your clinical judgement or local protocol.Bottomline: Recommend low-dose aspirin to pregnant women who are at increased risk for preeclampsia after 12 weeks of gestation.Chlamydia and gonorrhea screening: On September 14, 2021, the USPSTF recommended screening women younger than 24 years old who are sexually active for BOTH chlamydia and gonorrhea infection. Also, screen all women 25 years and older who are at increased risk. Increased risk means: a previous or coexisting STI, history of incarceration, and any kind of sexual intercourse out of a mutually monogamous relationship (new partner, more than one partner, partner who has sex with other partners, partner with an STI, history of exchanging sex for money or drugs).  The screening for GC/Chlamydia in women is a grade B recommendation. In this recommendation, the term “women” refers to persons born with female genitalia and does not apply to persons who identify as women but have male genitalia.  This recommendation also includes pregnant persons and adolescents. The evidence is insufficient (Grade I) to assess the balance and benefits and harms of screening for chlamydia and gonorrhea in asymptomatic men. Remember, a grade I recommendation is not a recommendation for or against a preventive intervention. To make it easy to remember “I stands for I don't know, more research is needed”. Recommendations about the age to start screening or the frequency of screening is not given explicitly, but the population with the highest incidence is women between 15-24 years old. Use your clinical judgement to determine when to start and how often. Prediabetes and diabetes screening: On August 24, 2021, the USPSTF updated their recommendation for prediabetes and diabetes screening. The recommendations states: “The USPSTF recommends screening for prediabetes and type 2 diabetes in adults aged 35 to 70 years who have overweight or obesity (BMI above 25). Clinicians should offer or refer patients with prediabetes to effective preventive interventions. This is a Grade B recommendation. The age to start screening is now 35 years old (instead of the previous recommended age of 40). This is an update from the recommendation given in 2015. We should consider screening at a younger age in:Persons from groups with high incidence and prevalence. These groups are American Indian/Alaska Native, Asian American, Black, Hispanic/Latino, or Native Hawaiian/Pacific Islander persons. Persons with family history of diabetes, history of gestational diabetes, or a history of polycystic ovarian syndrome.Screen Asian Americans with a BMI of 23 or greater after age 35.How to screen for prediabetes and diabetes? You have three options: Fasting glucose (normal below 100, prediabetes below 125, diabetes above 126)Hemoglobin A1C (normal Below 5.6, prediabetes below 6.4%, diabetes above 6.5%). Do not use point of care A1C for screening, use a venous sample instead.Oral glucose tolerance test in the morning (fasting); measure glucose 2 hours after ingesting 75 g of oral glucose (normal below 140, prediabetes below 200, diabetes above 200). The diagnosis of prediabetes or type 2 diabetes should be confirmed with repeat testing before starting intervention.Random glucose above 200 is highly suggestive of diabetes.      The diagnosis of diabetes should be confirmed before starting interventions.Summary: Aspirin for preeclampsia prevention, screen for gonorrhea and chlamydia all women younger than 24, and screen for diabetes everyone older than 35 with overweight or obesity.______________________HIV Series Part II: HIV TransmissibilityBy Robert Dunn, MS3, Ross University School of Medicine.People infected with HIV are often thought to be contagious even by touch; though the reality is transmission is primarily transmitted via sexual contact, bodily fluids, from mother to baby during pregnancy, shared needles, or accidental needle sticks in the medical workplace. And when it comes to sex, it is common that a person is afraid to engage in any sexual contact with a HIV positive person, even though the patient may have their infection controlled with medicine.Per the CDC, the most common ways of contracting HIV are through sex without protection, shared needles, and perinatal transmission from mother to child.Sexual transmission:With anal sex, the receptive partner, or bottom, is at higher risk of contracting HIV because the rectal mucosa is thin, more prone to micro-abrasions and create an opportunity to contract HIV. The insertive partner, or top is also at risk of infection via the opening of the urethra, the foreskin of an uncircumcised penis, or any cuts, scratches or open sores on the penis. With vaginal sex, the woman can be infected via the mucus membranes that line the vagina and cervix. And the man can become infected from the vaginal fluid or blood that may carry HIV. Needlesticks:Sharing needles is high risk for contracting HIV. If one person has HIV and uses a needle, the blood of that person is carried on the needle and can inject the virus directly into anyone else who uses that needle. This can occur if people are sharing injected drugs, medications, or even in a needle stick accident that may occur when treating patients in the hospital with HIV. Vertical transmission:Perinatal transmission occurs when the mother infected with HIV passes the infection to her newborn. It is now recommended to test every pregnant woman for HIV and treat as needed. This can occur while the fetus is in the womb or upon delivery. It is recommended that the mother be placed on HIV medication immediately to reduce the risk of infecting the baby. These are ways how HIV is NOT transmitted: kissing on the cheeks, hugging, holding hands, sharing silverware, talking to someone, mosquitos, or sharing toilet.___________________________Now we conclude our episode number 68 “Prevention – Aspirin, STIs, and Diabetes.” Dr Arreaza and Dr Civelli explained the most recent updates by the USPSTF regarding use of aspirin to prevent preeclampsia, screening for gonorrhea and chlamydia in women, and screening for diabetes in patients older than 35. Robert continued with his HIV series and explained how HIV is mostly transmitted, a good reminder for all of us that the most common way of transmission continues to be sexual transmission. Kay is our youngest guest yet, and she lets us in on how her generation is exploring sex differently because they're the first with easy access to p*rn. She has dated people of multiple genders, and realized she loves all bodies. Despite her young age, Kay has already learned hard lessons about toxic relationships, trust, and consent.   Kay is an 18-year-old cisgender female. She describes herself as Black, pansexual, monogamous, and in a relationship. She grew up Catholic and describes her body as full.   Bookmark moments: 5:34 - Kay shares her first memory of sexual pleasure, when her peers in 6th grade were watching porn. It was her closest thing to a learning experience before trying things like handjobs and blowjobs. 9:55 - Kay “sort of” came out as bi in 7th grade, and then discovered she liked trans people too. She went to a Catholic school with genders separated and describes girls kissing in class. As they get older, school culture turns anti-gay. 14:40 - Sex Ed in school was brief biology, everything she already knew. Her first kiss was a dare on IG live with her best friend. It becomes her first relationship, which has its struggles. 18:00 - Kay talks about suicidal thoughts that both she and her girlfriend worked through. 25:30 - Kay lives with her mom; her mom found out about her dating girls after seeing her phone but didn't take issue with it. As if one new play opening wasn't enough pressure Olivier award-winning playwright, Morgan Lloyd Malcolm, has two this month. ‘Typical Girls', set in a women's prison, opened this week at the Crucible Theatre in Sheffield, and tomorrow ‘Mum' begins previewing at the Theatre Royal Plymouth. A psychological thriller, ‘Mum' explores the pressures and complex emotions many women experience when they have a baby. She joins Emma. Rates of sexually transmitted infections such as chlamydia, gonorrhoea and herpes have been on an upward trend for the past decade, according to the latest figures from Public Health England. But despite being more common, the stigma attached to contracting an STI still runs deep. One Woman's Hour listener got in touch to share her story of catching herpes in her 50s and the devastating effect this has had on her life and confidence. Dr Liz Foley, a consultant in genito-urinary medicine for the Solent NHS Trust, and Marian Nicholson, Director of the Herpes Viruses Association discuss the facts about herpes in the UK and how to break down the stigma that comes with a diagnosis. A listener wanted to share her experience as a 'late life lesbian' who realised she was gay at the age of 44. She is about to publish a book of lesbian erotica under the pen name Flick Bayliss and explains to Emma why. The University of Leicester turns 100 this year and as part of their celebrations, they are launching a programme called Our 100 – commemorating their‘ hidden heroes'. It is national recovery month, so we're going back to out home base - Sobriety and Recovery! We're talking to Hayley Waters, the Recovery Baddie. This interview was such a good time and we're so excited to have you sit in on this powerful conversation! Hayley is one badass sober babe and we are HERE FOR IT!!!! In this episode we discuss:- Identifying as an alcoholic/addict vs. being alcohol-free (labels) -Shame associated with being an alcoholic/ addict as a mother-Gratitude"My name is Hayley. I'm 35, from Washington, D.C., and my sobriety date is August 28th, 2020.  I grew up in an American military family. Looking back, I started displaying signs of addiction at a very early age. Early in childhood, my parents divorced; I grew up fast, experimented young, and struggled to find my identity and purpose in life.By 16, I began partying often. When my peers stopped or moved on, I was looking for the next “scene” and the next group of friends that accepted my lifestyle without judgment or questions. By nothing short of a miracle, I made it to adulthood. I fell in love, had a beautiful child, and shortly after his birth, I was diagnosed with Stage II Lymphoma. After 6 months of chemotherapy, I came out on the other side. Nevertheless, I struggled with an endless search to reveal why I survived, what incredible thing was I destined to do that granted this second chance? Shame for disappointing the universe and my perception of higher purpose reverted me to the only thing I knew to cope with- substance abuse.I retreated into my disease quickly. I lost relationships, engaged in risky behavior over and over, and continued to down a shame spiral perpetuated by my addiction. At the height of it, I was drinking from the moment I woke up; to the moment I went to bed and stopped loving myself.   In August 2020, I checked myself into detox and I've been in recovery ever since. Through meetings, my program, and my sober support network, I have a life I never thought possible, and that I finally believe I deserve. I love myself more than I ever thought possible. You may have noticed that we've got a new look and sound. I'm so excited to share my new vision for the podcast with you as we continue on this journey together. If there's an elephant in the room, I'm gonna talk about it. Are you ready to ask the hard questions with me?   I am so excited to be starting off this new season of the podcast with Dr. Celeste Holbrook. She has been a guest several times (her episode is one of the most downloaded ever!) and this time we're diving into sex ed. Something I've realized recently is that a lot of us, myself included, didn't get the comprehensive sex ed we should have in school. So let's fix that together!   Let's Talk About It There's this idea in Christian culture that if we talk about sex or consent or how to put on a condom… that everyone will start having sex.   “The more information a person has about sex and sexuality, the more likely they are to make healthy decisions about sex.” - Dr. Celeste Holbrook     Where Is The G Spot? Do all women have one? The backside of the clitoris (the clitoris is larger than you think!) If you were to put a finger inside, you'd feel it about an inch up on the front side of your vaginal canal. Everyone has a G Spot, but not everyone likes it being stimulated.   What Is Squirting? It's not pee, but it does have small amounts of urine in it. It's actually mostly interstitial fluid, which is held in the G Spot.   What's The Difference Between STIs and STDs? STI: Sexually Transmitted Infection // STD: Sexually Transmitted Disease For the most part, doctors use the term STI because conditions that are sexually transmitted do not always develop into STDs (and it carries less stigma). Dr. Celeste and I chat about how to talk to the person you're dating about any STIs you may have.   To lube or not to lube? When you get wet, that is your body creating a sort of natural lubricant to make the rubbing and friction of skin glide more smoothly. Lube assists in making sex more comfortable and pleasurable. There are many reasons why you may not get wet even though you are aroused— it's always best to have lube around because you'll never know when you need the extra help.   What Type Of Lube Should You Use?? Silicone: Cannot be absorbed and act as a pillow in your vaginal canal. (For this reason it is also a bit messy.) Best for vaginal pain during sex. Do not use this when using a silicone toy.  Water-based: Will feel the most like your natural lubricant and will be absorbed into your skin so you may have to reapply. More viscous than slippery. Hybrid (water and silicone): Has a more natural feel while still feeling very slick. NEVER use a lubricant that has any sort of sugar as it will mess with your natural pH. Always read the ingredients. Need some lube suggestions: Uberlube + Jo.   Should You Go To The OBGYN? Even if you're not sexually active? From Hopkins Medicine: Women should start Pap smear screening at age 21. Between the ages of 21-29, women whose Pap smears are normal only need it repeated every three years. Women ages 30 and over should have testing for the human papillomavirus (HPV) with their Pap smear.   Are Blue Balls A Thing?   “The term ‘blue balls' in and of itself is coercive.” -Dr. Celeste Holbrook   It's uncomfortable, sure. But not serious or dire.  This term should never be used to coerce someone into having sex. Masturbation is always a solution. Also did you know women experience blue vulva?   Does Sex Get Better With Time And Practice? Or are some people just incompatible?   “The biggest lie we are told is that sex is natural. Sex is biological. We are built for pleasure. We are built for reproduction. Sex is not natural— it is a skill we need to learn.” - Dr. Celeste Holbrook   As you practice sex and communication more, it can absolutely get better. Work with your partner to explore how to make it feel better for both of you.   “We are not modeled normative sexual behavior. All sexual behavior we see is performed sexual behavior, either in porn or in media.” - Dr. Celeste Holbrook     Interested in learning more? Last week we learned that South Carolina topped the entire country for COVID case rates per capita, multiple hospitals were reporting their ICUs were full, and some even called in the National Guard for assistance.
