In the latest episode of Veteran Oversight Now, a VA OIG healthcare inspection hotline director discusses findings at the Raymond G. Murphy VA Medical Center in Albuquerque, New Mexico, where there was no documentation of required reusable medical device reprocessing, which put patients at risk for infection if the reusable medical devices used during subsequent procedures were, in fact, not cleaned per requirements. The VA OIG also found that high-level disinfection documentation was missing and made seven recommendations related to oversight of the medical center's Sterile Processing Service. Related Report: Failure to Follow a Consult Process Resulting in Undocumented Patient Care at the Chillicothe VA Medical Center in OhioThe VA OIG conducted a healthcare inspection for 10 allegations related to the quality and management of patient care and the availability of resources within the Urgent Care Center at the Chillicothe VA Medical Center in Ohio.One allegation involved an urgent care provider sending a patient with a T12 vertebrae compression fracture to have chiropractic care at the Complementary and Alternative Medicine (CAM) clinic. The patient returned a week later with a T12 burst fracture and rib fractures.The OIG found that an urgent care provider verbally referred a patient for pain management and not for chiropractic care. However, the OIG found that the urgent care provider did not enter a CAM consult until eight days after seeing the patient. Due to this delay, the chiropractor and clinical massage therapist failed to review the consult prior to seeing the patient. Additionally, the chiropractor and massage therapist could not link documentation to the consult and had no other process to complete the documentation resulting in the failure to document care provided within the medical record.The patient returned to the Urgent Care Center eight days later where a computerized tomography scan showed an acute burst fracture and acute rib fractures. Because of the lack of documentation and provider recall, the OIG could not conclusively determine the relationship between the actions taken by the chiropractor and clinical massage therapist and the patient's bone fractures.The OIG found the nine additional allegations to be unsupported and lacked merit.The OIG made two recommendations to the Facility Director related to education of providers, chiropractors, and clinical massage therapists on the use of consults and timely documentation, and conducting an internal review of the CAM program processes related to patient care, reviewing consults, scheduling appointments, checking-in patients, and documentation.

He will work closely with all departments to enhance outreach methods to maximize growth of the VAMC.  Buzz is very passionate about serving our nation's heroes and looks forward to increasing the VA footprint in our communities. The opioid crisis is truly an epidemic that doesn't discriminate. Homefront host Craig McKee sat down with two veterans with very different addiction journeys. Neither of them thought of the VA Medical Center as an option to find sobriety. The Cincinnati VAMC is increasing overdose response training for its employees and increasing the availability of overdose reversal drugs to help save lives. There is also an effort to increase awareness of the benefits veterans can receive.

Dr. Jay Lynch received his BA from the University of Virginia and MD from Eastern VA Medical School in 1984. In 1991 he returned to the UFCOM and has served in multiple roles during this tenure including, course director in Oncology (20 years), program director for hematology/oncology (13 years), section chief of hematology/oncology at the VAMC and now serves as the Assistant Dean for Admissions. In 2006 was honored by the University of Florida as one of 5 Distinguished Teaching Scholars. He was co-founder with his wife of the Christian Study Center at the University of Florida and serves as its board president. He and his wife Laura, have 4 children and 3 grandchildren.

What do abusers see when they look at a victim walking across the street that tells them they can abuse the individual? Why do people commit suicide? Why do people have road rage? What does childhood abuse look like in adulthood? How does a PTSD victim describe what is going on inside of them? What is the human element that would turn a trauma into a life experience? My name is Lady and I have been diagnosed PTSD. I had a flashback in 2003 of a violent military rape that I had suppressed for over 40 years. I was also sexually abused as a child. Counseling for over 40 years, I never dreamed I would have to walk the same path I led my clients on. If I could have had something in my hand to act as a lifeline, it would have gone a long way toward giving me hope. My strength, my therapist and my friends were what helped me to go back to the original pain and connect the dots in my life to the present. To have a lifeline for others is the reason I wrote about the pain of my dysfunctional life. "My Journey to Peace with PTSD" describes how anger quietly sets in at any age and creates a trauma victim who becomes a candidate for PTSD; road rage; failed and successful suicides; how and why some people become abuse victims and why others become abusers; and more. As a PTSD victim, the one element that affected me the most was the feeling of isolation, which can be as terrifying as the trauma. The Foreword to the book is written by Ruth Crawford, LCSW, VAMC, with additional praises from other health care professionals.
