This week on the Disaster Podcast Dr. Joe Holley comes on the show for a clinical update episode on Heat-Related Medical Emergencies. As we head into the summer months here in the Northern Hemisphere, the likelihood of encountering patients with heat emergencies increases. For disaster responders in the summertime, the risks are even greater as they wear gear and carry equipment that adds to their load while working in hot weather. Co-Hosts Sam Bradley and Jamie Davis join Joe on the discussion. Types of heat emergencies Heat Cramps – The symptoms of heat cramps include painful involuntary muscle spasms brought on by loss of sodium through excessive sweating. Most often seen in people exerting themselves in extreme heat situations without rehydrating themselves or taking adequate breaks to allow the body cool down. Heat Exhaustion – Next on the spectrum of heat emergencies is heat exhaustion which progresses from heat cramps as the body starts to lose the ability to compensate for lack of adequate cooling. Symptoms include early signs of shock like dizziness, nausea, cool, pale, and clammy skin. Heat Stroke – The most severe level of heat emergency is heat stroke which is a true medical emergency that may result in permanent disability or death. Hot, dry skin, loss of consciousness, and complete failure of the body’s cooling mechanisms are key signs. Heat Illness Spectrum and Prevention Joe explained the spectrum of heat illnesses, from heat cramps (caused by heavy sweating and salt loss leading to muscle cramps) to heat exhaustion (when cooling mechanisms fail and body temperature rises, causing symptoms like nausea, dizziness, and clammy skin) to heat stroke (when internal temperature exceeds 104°F with severe neurological and cardiovascular effects). Jamie asked about factors affecting the progression from milder to severe heat illness, to which Joe responded that various factors including physical condition, medical conditions, medications, ambient conditions, and physical activity all influence how quickly someone might progress through the heat illness spectrum. Joe emphasized the importance of prevention through proper education, preparation, and having readily available cooling methods, noting that work cycles may need to be limited in extreme heat conditions. Heat Management in Disaster Situations Joe explained that preparing for heat management in disaster situations is crucial, especially for first responders working in austere environments with limited support systems. He highlighted the increased physical and mental demands of search and rescue operations in debris-filled areas following adverse weather conditions. When asked about immediate actions for heat stroke emergencies, Joe indicated that it is a life-threatening situation requiring immediate attention, though the specific response was not fully detailed in the transcript. Heat Stroke Treatment Techniques Joe shared his experience with a 16-year-old who suffered heat stroke during football practice, explaining the importance of cooling techniques like exposing skin, using water and fans, and eventually ice baths for treatment. A new study shows OpenAI's chatbot called ChatGPT Health is underestimating the seriousness of certain medical emergencies. For the study published in the journal Nature Medicine, ChatGPT Health was given 60 medical situations, and the responses were compared to the responses of three doctors. Researchers concluded that a little over half of emergency cases, ChatGPT recommended seeing a doctor within 48 hours in contrast to the opinion of doctors saying ChatGPT Health should have advised the emergency room instead. SUCKED!!! -ALL the 007 Movies on Netflix!!   Weerd:   Daniel: -S&W Night Guard 396 and more .44 Specials   David:   Drink Segment: Backwoods Bastard   Food Segment:  Leg of lamb Ingredients 7-8 lb leg of lamb 1 Tbs Black pepper 2 Tsp Salt ½ Tsp Crushed rosemary 2-3 cloves of Garlic Olive oil Recipe Rinse and pat dry the leg of lamb, and place on a rack in a roasting dish, meaty side up. Mix salt, pepper, and rosemary, then coat the surface of the lamb with about half the mixture. Sliver the garlic and coat it with the remaining spice mix. Poke holes in the roast and insert slivers of garlic. Coat with olive oil. Preheat the oven to 450F. Put the roasting pan in and immediately lower the temperature to 325F. Cook until the internal temperature reaches 125–130°F for medium-rare. Take out of the oven, and let rest for 15–20 minutes.   Chef John's Tzatziki Sauce   Main Topic:   Planning Layout: Doors, window, cameras Equipment: First aid, AED, phones, flashlights, door wedges, etc Build a team with clearly defined and assigned roles Connect with local law enforcement agencies Communication plans Training: classes, exercises, and drills. Evacuation plan, Bunker plan, Medical Emergency plan, Fire plan, etc. Mental preparation: security team members need to give themselves permission to do bad things to bad people. If anyone can't, they should be relegated to a support role. There is no shame in this. Build good mental habits. Primary and secondary members assigned to provide information to Law Enforcement/911 Name and location of incident Physical description of shooter(s) Location of the shooter. Number of shooters, if more than one. Number and type of weapons held by the shooter(s) IE: handgun, shotgun, rifle? Number of potential victims at the location. How many people are present? Identifiable entrances and exits. Are the doors numbered? Law Enforcement Response - what to expect They are not there to treat wounded, they are there to end the threat. The initially responding officers will shout commands and may physically move or restrain people other than the shooters as needed. Once the threat is ended, additional first responders will enter to provide medical care. How to Survive the Incident Run, hide, fight Once they arrive, follow LEO instructions, keep hands visible, avoid quick movements. OODA loop: Observe, Orient, Decide, Act Legal defence: If force was used (up to and including deadly force) protecting members, there's a good chance that person or persons will be taken into custody. The family of the attacker may sue the defenders and victims. After an event: If LEOs haven't arrived, clear the scene Once LEOs arrive, see above Try to avoid disturbing the crime scene within reasonable limits Medical care. As with a depressurized airplane, check yourself first, then others Media: Primary and secondary security team personnel assigned to interact with media and run interference between them and traumatized/injured members.
